Bacteriology and Antibiotic Sensitivity for Diabetic Foot Ulcer (당뇨족 궤양의 세균 역학과 항생제 감수성)

  • Polymicrobial nature of diabetic foot infection has been well documented in the literature. Initial antibiotic therapy of diabetic foot infection is usually empiric until reliable culture data is shown. This study was carried out to determine the common bacteriological flora of diabetic foot infection and antimicrobial sensitivity pattern in order to enhance possible empiric treatment. The specimens were obtained from wounds of 207 cases of diabetic foot ulcer, and the bacteriological isolation, and antimicrobial susceptibility tests of the isolates were carried out by standard microbiological methods. Staphylococcus aureus was the most common isolate, with 46.2% of recover rate among total bacterial isolated cases. Among gram-negative organisms, Pseudomonas aeruginosa was most common. Gram-positive organisms showed significant susceptibility to clindamycin, trimethoprim/sulfamethoxazole, and levofloxacin, besides vancomycin. Cefoperazone, piperacillin/tazobactam, and amikacin in addition to imipenem were most effective agents compared to gram-negative organisms. Diabetic foot infection requires use of combined antimicrobial therapy for initial management. Our results indicate that the most effective antibiotic combination for diabetic foot infection of Korean patients is clindamycin plus cefoperazone.

Species and Antimicrobial Susceptibility of Nonfermentative Gram-Negative Bacilli Isolated from Clinical Materials (임상 검사물에서 분리된 비발효성 그람음성 간균의 균종과 항생제 감수성)

  • Species and antimicrobial susceptibility of nonfermenting gram-negative bacilli(NFB) isolated from clinical materials at Yonsei Medical Center during the period of September 1980 to August 1981 were analyzed and the following results were obtained. 1. NFB were isolated from 17.1% of sputum, 14.8% of pus, 5.0% of urine, 3.3% of throat and 1.4% of blood specimens. 2. Among the isolates 57.6% were Pseudomonas aeruginosa and 32.7% were Acinetobacter calcoaceticus. P. maltophilia and P. cepacia were 3.2% and 2.8% respectively. Other bacteria were rarely isolated. 3. The monthly isolation rates were high during the June to November period(8.8-12.0%), and low during the December to May period(4.2-8.4%). 4. Many of the isolates showed resistance to various antimicrobials. Although there were variations depending on the bacterial species, amikacin, colistin, gentamicin, tobramycin and co-trimoxazole showed more antibacterial activities than other antimicrobials.

Isolation of Hydrogen-producing Bacteria from Granular Sludge of an Upflow Anaerobic-Sludge Blanket Reactor

  • H$_2$-producing bacteria were isolated from anaerobic granular sludge. Out of 72 colonies (36 grown under aerobic conditions and 36 under anaerobic conditions) arbitrarily chosen from the agar plate cultures of a Suspended sludge, 34 colonies (15 under aerobic conditions and 19 under anaerobic conditions) produced H2 under anaerobic conditions. Based on various biochemical tests and microscopic observations, they were classified into 13 groups and tentatively identified as follows: From aerobic isolates, Aeromonar spp. (7 strains), Pseudomonas spp. (3 strains), and Vibrio spp. (5 strains); from anaerobic isolates, Actinomyces spp. (11 Strains), Clostridium 5pp. (7 strains). and Porphyromonas sp. When glucose was used as the carbon substrate, all isolates showed a similar cell density and a H$_2$ production yield in the batch cultivations after 12 h (2.24-2.74 OD at 600 nm and 1.02-1.22 mol H$_2$/mol glucose, respectively). The major fermentation by-products were ethanol and acetate for the aerobic isolates, and ethanol, acetate and propionate for the anaerobic isolates. This study demonstrated that several H$_2$ producers in an anaerobic granular sludge exist En large proportions and their performance in terms of H$_2$ production is quite similar.

Field Study on the Mycotoxin Binding Effects of Clay in Oreochromis niloticus Feeds and Their Impacts on the Performance as Well as the Health Status throughout the Culture Season

  • Total aflatoxin and ochratoxin were detected in 3 naturally contaminated fish feed samples using immune-affinity method. The results revealed that the average levels of aflatoxins in the 3 examined samples were (15, 22 and $12\;{\mu}g/kg$) respectively while the average levels of ochratoxins were (15, 6 and $6\;{\mu}g/kg$). The results of determination of the effects of clay as a mycotoxin binder on the health status and performance of Oreochromis niloticus in comparing with a control group revealed that the survival rate in control group was 81% after the end of the culture season. The results also revealed that the survival rate in group 2 which received clay treated feed was 86%. The results of regular parasitological examination revealed the identification of trichodina as external protozoa in Oreochromis niloticus from both ponds but without manifestation of disease signs. The results of bacteriological examination revealed the isolation and identification of Pseudomonas flouresence from some moribund Oreochromis niloticus. Higher performance parameters were recorded in group 2 that received feeds treated with clay which reflected in the total production which reaches 1646.47 kg while in the control pond, the total production was1308.36 kg.

Bacterial Strains and Their Cellulase Activity from the Excavated Clothes at Daedeok-gu, Daejeon (대전 대덕구 출토 복식 유물에 부착된 세균의 종류 및 섬유소분해효소 활성)

  • This study was aimed to isolation and identification of attached bacteria on the clothes excavated from Deajeon area dating on the 16th century. From the observation with colony shape, 17 bacterial strains were isolated, and then 7 bacterial strains were identified with morphological and biochemical characteristics. Streptococcus sp., Alcaligenes faecalis, Gemella sp., Acinetobacter sp., Pseudomonas vesicularis, Aeromonas sain. salmonicida, Moraxella spp. In observation of the bacterial strains by the sort of textile, more bacterial strains were found in silk, cotton, and cotton batt than in ramie and hemp. It is suggest that hemp has antibacterial characteristics due to the presence of lignin. In the comparison washed samples with unwashed ones, there were more kinds of bacterial strains in washed samples. In the cellulase activity tests, all isolated bacteria had low level cellulase activity.

Isolation of Dibromosceptrin with Antimalarial Activity from the Unidentified Sponge, Agelas sp. (Agelas속의 미동정 해면으로부터 항말라리아 활성을 갖는 Dibromosceptrin의 분리)

  • In order to find some lead compounds for the treatment of opportunistic infections of malaria and pathogenic microbes, an undescribed Indonesian sponge Agelas sp. collected at Manado, Indonesian Waters, was suggested containing active compounds. Crude ethanolic extract of the sponge exhibited significant in vitro antimalarial and antimicrobial activity against Plasmodium falciparum (D6 colne) with $IC_{5O}$ values of $8\;{\mu}/ml$ and against pathogenic microbes such as Candida albicans $(150\;{\mu}/ml)$, Cryptococcus neoformans $(<20\;{\mu}/ml)$, Staphylococcus aureus $25\;{\mu}/ml$, methicillin-resistant Staphylococcus aureus $(<20\;{\mu}/ml)$, and Pseudomonas aeruginosa $(<20\;{\mu}/ml)$. Active compound (5.0 mg) was isolated from the ethanolic extracts of the sponge and purified by using silica gel and ODS column, successively. Active compound was elucidated as dibromosceptrin $(C_{22}H_{24}Br_2N_{10}O_2)$ by detailed analysis of FTESI-MS and comparison of $^1H,\;^{13}C$, DEPT and HMQC NMR spectral data with those reported.

Causes and surgical management of postthoracotomy empyema (개흉술후 발생한 농흉의 유인 및 외과적 치료)

  • Thoracic empyema is defined as purulent pleural effusion or effusion with positive bacteriology. Recently, the empyema has markedly decreased by developement of antibiotics, but empyemas following thoracotomy were occasionaly reported. During the period of January, 1985 to May,1991, 18 patients with postthoracotomy empyema have been treated in the Department of Thoracic and Cardiovascular Surgery of Chonbuk National University Hospital.There were 17 males and 1 female ranged from 18 years to 67 years of age. The underlying diseases of empyema were tuberculosis [50%], lung cancer [33.3%], esophageal cancer [11.1%],and aspergillosis with tuberculosis[5.6%]. In surgical procedures causing the empyema, there were lobectomy[38.9%], pneumonectomy[22.2%], decortication[16.7%], decortication & lobectomy[11.1%], and esophagectomy [11.1%]. Etiologic organisms in the pleural fluid were Pseudomonas [27.7%], S.aureus [16.7%], mixed infection [16.7%], K.pneumonia [5.6%], M.tuberculosis [5.6%], and no isolation [27.7%]. In 6 cases with BPF, completion pneumonectomy was performed in 1 case, and open thoracostomy in 5 cases. In 12 cases without BPF, closed thoracostomy was performed in 1 case, decortication in 2 cases, decortication & open thoracostomy in 2 cases, and open thoracostomy in 7 cases. In 6 cases with BPF, the fistulas were closed in 4 cases at follow up, the other 2 cases died from pulmonary insufficiency after completion pneumonectomy and open thoracostomy,respectively. In 12 cases without BPF, the empyema cavities were filled with expanded lungs and granulation tissues, except 1 case died from sepsis.

Microbiologic Pollution of Indoor Air in Industrial Work-Places (산업체 작업환경의 실내 공기에서 미생물 오염도)

  • This study was investigated to isolate identify the total bacteria and fungi from the indoor air of work-place of the shoes, paint, stainless steel, and plastic industries. The number of bacterial colonies on the nutrient agar plates were calculated by the open petridish method for 30 minutes in indoor air of work-places at the autumn and winter. The isolated bacteria were identified by Gram stain and biochemical test using API Staph and API 20E kits. The isolated fungal colonies were identified by gross appearance of the giant colonies and microscopic examination of their spore and hyphal characteristics on the slide culture method. The minimum inhibitory concentration (MIC) of several antibiotics against isolated bacteria was determined by the microdilution method with Mueller-Hinton broth. The 70-400 colonies in autumn and 54-236 colonies in winter were isolated from the indoor air of work-places of several industry. The isolation rates of Gram positive cocci, Gram positive bacilli, Gram negative bacilli, and Gram negative cocci were 46.3%, 19.8%, 17.3%, and 16.1%, respectively. In Gram positive cocci, the most strains were identified as Aerococcus spp, Micrococcus spp, and Staphylococcus spp. In Gram positive and negative bacilli, and Gram negative cocci were identified as Bacillus spp, Pseudomonas spp, and Neisseria spp, respectively. The frequently isolated fungi were Aspergillus spp, Penicillium spp and Rhizopus spp, respectively. The frequently isolated Aerococcus spp, Micrococcus spp, and Staphylococus spp were highly resistance against ampicillin, erythromycin, methicillin, and tetracycline. These results arouse our attention to microbiologic pollution in the indoor air of work-places of industries.

Isolation and Characterization of 4-(2,4-Dichlorophenoxy)Butyric Acid-Degrading Bacteria from Agricultural Soils

  • Eight numerically dominant 4-(2,4-dichlorophenoxy) butyric acid (2,4-DB)-degrading bacteria and three pairs of bacteria showing syntrophic metabolism of 2,4-DB were isolated from soils, and their phylogenetic and phenotypic characteristics were investigated. The isolates were able to utilize 2,4-DB as a sole source of carbon and energy, and their 2.4-DB degradative enzymes were induced by the presence of 2.4-DB. Analysis of 16S rDNA sequences indicated that the isolates were related to members of the genera, Variovorax, Sphingomonas, Bradyrhizobium, and Pseudomonas. The chromosomal DNA patterns of the isolates obtained by polymerase-chain-reaction (PCR) amplification of repetitive extragenic palindromic (REP) sequences were distinct from each other. Four of the isolates had plasmids, but only one strain, DB 1, rad a transmissible 2,4-D degradative plasmid. When analyzed with PCR using primers targeted to the tfdA, B, and C genes, only strains DB2 and DB9a produced DNA bands of the expected sizes with the tfdA and C primers, respectively. All of the isolates were able to degrade 2,4-D as well as 2,4-DB, suggesting that the degradation pathways of these compounds were closely related to each other, but respiratory activities of many isolates adapted to 2,4-DB metabolism were quite low with 2,4-D.

Isolation of causative agents from CMT-positive mastitic milk and antimicrobial susceptibility of isolates (CMT 양성 유즙에서 유방염 원인균 분리 및 분리균의 항균제 감수성)

  • This study was carried out to isolate of causative agents from CMT-positive and mean somatic cell count(SCC) $\geq$500,000 cells/ml mastitic milk, and evaluate to antimicrobial susceptibility of isolates in Iksan branch area from January to November, 1996. 1. The CMT-positivity(SCC 500,000 cells/ml) of 610 heads was 36.2% (221), and of 2,373 quarter milks was 16.1% (383). 2. The Gram-positive isolates were 153 strains which was Staphylococcus sp (115), Micrococcus sp (18), Streptococcus sp (10), Listeria monocytogenes (5) and Enterococcus faecalis(5). 3. The Gram-negative isolates were 66 strains including E coli(14), Yersinia sp (13), Shigella sp(8), Enterobacillus sp(8), Cedecea sp(5), Pseudomonas aeruginosa(5), Proteus sp(5), Klebsiella sp(4), Salmonella sp(2), kluyvera ascorbate(1) and Tatumella ptyseos (1). 4. The Gram positive strains of isolates were moderately susceptible to T/s, Cp, Fd, Imp, Aug, Rif, Cft and Va. And the Gram negative strains of Isolates were moderately susceptible to T/s, Cp, Imp, Pi and Ti, In order. 5. Multiple antimicrobial resistant patterns were encountered 62 and 36 from Gram positive and negative isolates, respectively.
