Sarcopenia: Nutrition and Related Diseases

  • "Sarcopenia", sarcopenia is an old age syndrome, and used to describe the reduction of skeletal muscle. Initially, it was thought that sarcopenia was only a senile disease characterized by degeneration of muscle tissue. However, its cause is widely regarded as multifactorial, with neurological decline, hormonal changes, inflammatory pathway activation, declines in activity, chronic illness, fatty infiltration, and poor nutrition, all shown to be contributing factors. Skeletal muscle mass can be measured by a variety of methods, currently, the commonly used methods are dual-energy X-ray scanning (DXA), computer tomography (CT), magnetic resonance imaging (MRI), etc. Muscular skeletal disorders can also be assessed by measuring appendicular skeletal muscle (ASM), particularly muscle tissue content. At the same time, sarcopenia refers to skeletal muscle cell denervation, mitochondrial dysfunction, inflammation, hormone synthesis and secretion changes and a series of consequences caused by the above process and is a progressive loss of skeletal muscle syndrome, which can lead to the decrease of muscle strength, physical and functional disorders, and increase the risk of death. Sarcopenia is mainly associated with the aging process, but also related to other causes such as severe malnutrition, neurodegenerative diseases, and disuse and endocrine diseases associated with muscular dystrophy, and it is the comprehensive results of multi-factors, so it is difficult to define that sarcopenia is caused by a specific disease. With the aging problem of the population, the incidence of this disease is increasingly common, and seriously affects the quality of the life of the elderly. This paper reviews the etiology and pathogenesis of myopathy, screening methods and diagnosis, the influence of eating habits, etc, and hopes to provide reference for the diagnosis and treatment of this disease. At present, adequate nutrition and targeted exercise remain the gold standard for the therapy of sarcopenia.

The study on development of easily chewable and swallowable foods for elderly

  • BACKGROUND/OBJECTS: When the functions involved in the ingestion of food occurs failure, not only loss of enjoyment of eating, it will be faced with protein-energy malnutrition. Dysmasesis and difficulty of swallowing occurs in various diseases, but it may be a major cause of aging, and elderly people with authoring and dysmasesis and difficulty of swallowing in the aging society is expected to increase rapidly. SUBJECTS/METHODS: In this study, we carried out a survey targeting nutritionists who work in elderly care facilities, and examined characteristics of offering of foods for elderly and the degree of demand of development of easily chewable and swallowable foods for the elderly who can crush foods and take that by their own tongues, and sometimes have difficulty in drinking water and tea. RESULTS: In elderly care facilities, it was found to provide a finely chopped food or ground food that was ground with water in a blender for elderly with dysmasesis. Elderly satisfaction of provided foods is appeared overall low. Results of investigating the applicability of foods for elderly and the reflection will of menus, were showed the highest response rate in a gelification method in molecular gastronomic science technics, and results of investigating the frequent food of the elderly; representative menu of beef, pork, white fish, anchovies and spinach, were showed Korean barbecue beef, hot pepper paste stir fried pork, pan fried white fish, stir fried anchovy, seasoned spinach were the highest offer frequency. CONCLUSIONS: This study will provide the fundamentals of the development of easily chewable and swallowable foods, gelification, for the elderly. The study will also illustrate that, in the elderly, food undergone gelification will reduce the risk of swallowing down to the wrong pipe and improve overall food preference.

Effect of tuberculosis treatment on leptin levels, weight gain, and percentage body fat in Indonesian children

  • Purpose: Tuberculosis (TB) remains a problem in the community. TB patients usually experience malnutrition, which is characterized by both decreased body weight (BW) and body fat percentage (BFP). Leptin, an important regulator of BW, also plays an important role in cellular immunity, which is integral to defense against Mycobacterium tuberculosis infection. We analyzed the effect of an anti-TB treatment regimen on the leptin level, BW, and BFP of children with TB. Methods: The design of this study was a group interrupted time series. The subjects were children with probable TB according to clinical criteria based on an Indonesian scoring system adopted from the Consensus of Expert Panel. BW; BFP; energy intake; fat and protein intake; and leptin levels before, 2 months after (intensive phase), and 6 months after (continuation phase) anti-TB treatment, were measured. About 40 children, aged 5-14 years, participated in this study. Results: The BW, BFP and leptin level increased from before treatment to after completion of the intensive phase and still showed an increased during the continuation phase: BW 18.65 kg, 19.75 kg, and 20.85 kg; BFP 18.3%, 19.5%, and 20.2%; and leptin level 1.9 mg/dL, 3.07 mg/dL, and 3.4 mg/dL, respectively (P<0.01). Conclusion: Leptin level, BW, and BFP increased throughout the course of anti-TB treatment, compared with pretreatment values. Further research is needed to compare the results with data for healthy children.

20세 이상 저소득층 여성의 연령에 따른 비만 및 영양상태 평가 - 제4기(2007~2009) 국민건강영양조사 자료를 이용하여 - (Evaluation of Obesity and Nutritional Status by Age among Low-income Women aged over 20 - Using Data from the Fourth Korea National Health and Nutrition Examination Survey -)

  • The purpose of this study was to evaluate the prevalence of obesity, dietary habits, and nutritional status by age among low-income women, using data from the fourth Korea National Health and Nutrition Examination Survey (2007~2009). Subjects were 8,356 women aged 20 and over. The subjects were classified into four groups by age. Dietary data from 24-hr recall methods were used to analyze nutritional status. The prevalence of obesity in the 50~64 years age group was significantly higher than those of the other age groups. Among age groups, malnutrition was the highest in the 65-and-over age group. It appears that women in the 20~29 and 65-and-over age groups were the highest nutritional risk. The percentage of carbohydrates in total energy intake was higher and the percentages of protein and fat were lower in the 65-and-over age group than other groups. Frequency of skipping breakfast was lower in women aged 65-and-over, and moderate physical activity significantly decreased with increasing age. Awareness of dietary guidelines was higher in women aged 30~49 years than other groups, whereas it was lower in those aged 65-and-over years. Adherence to dietary guidelines of 'eating a variety of foods from each food group' was significantly lower in women aged 65-and-over years than those of other groups. However adherence to dietary guidelines of 'eating breakfast everyday with a pleasant mind' was significantly lower in women aged 20~29 years than those of other groups. Therefore, this study shows that low income women have various nutritional problems by age group, and we should support a tailored approach to improve their nutritional status.

입원 환아의 영양상태 평가 (Assessment of Nutritional Status in Hospitalized Pediatric Patients)

  • 목 적: 입원 환아의 영양결핍에 대한 통계가 국내에는 미비하여 영양평가의 지표를 삼기가 어렵다. 입원 환아의 영양상태를 평가하여 입원 환아 영양관리의 필요성을 강조하고 향후 영양평가의 기초자료로 삼고자 본 연구를 하였다. 대상 및 방법: 1994년 2월부터 7월까지(200명)와 1999년 2월부터 7월까지(233명) 조선대학교 부속병원 소아과에 입원한 환아를 대상으로 연령에 대한 체중비, 연령에 대한 신장비, 신장에 대한 체중비를 구하여 급성 및 만성 단백질-에너지 영양결핍의 유병률과 질환별 및 연령군에 따른 유병률의 차이 그리고 영양결핍의 질환별 분포를 비교 분석하였다. 영양상태의 평가는 Waterlow 분류기준에 의해 분석하였고, 생화학적 지표로는 혈색소, 혈청알부민, 총 임파구 수를 조사하여 영양결핍과의 관련성을 알아보았다. 결 과: 1) 급성 PEM은 신장에 대한 체중비로 산출하였는데 1994년에는 중증 0.5%, 중등도 7%, 경도 18%로서 입원 환아 200명 중 총 51명이었고, 1999년에는 중증 2.0%, 중등도 3.6%, 경도 20.0%로서 입원환아 223명 중 총 58명으로 나타나 두 연도에서 영양결핍에 대한 높은 빈도의 유병률을 보였다. 만성PEM에서도 1994년에는 중증 5%, 중등도 5.5%, 경도 25.5%로서 총 72명이었고, 1999년에는 중증2.24%, 중등도 4.04%, 경도 22.87%로서 총 62명으로 나타나 두 연도간의 유의한 차이는 없으나 각각 높은 빈도의 PEM을 보이고 있다(p=0.287). 급성과 만성 PEM 양쪽에 해당되는 환아는 1994년에 25명, 1999년에는 20명이어서 두 연도에서 PEM의 유병률은 1994년에 49.0%, 1999년에는 44.8%로 나타났다. 2) 연령군별로 영양결핍의 정도를 볼 때 급성 영양결핍에서는 경도의 PEM이 전반적으로 높게 나타났으며, 심한 만성 PEM의 유병률은 2세 이전의 영아에서 10.8%로서 높게 나타나고 있으나 이들은 통계학적으로 의미가 없었다(p=0.511). 3) 급.만성 PEM의 질환별 분류를 볼 때 1994년에는 급성 혹은 만성 PEM을 보였던 환아 98명 중 호흡기 질환이 43명으로 52%를 차지하여 가장 많았고, 소화기, 일반 내과, 신경계, 순환기 질환 순이었으며, 1999년에는 99명 중 호흡기 질환이 38명, 소화기, 일반내과, 신경계, 내분비, 순환기 질환순 이었다. 4) 각 질환에 대해 급.만성 PEM의 유병률을 볼때 1994년에는 혈액종양, 내분비, 신경계, 호흡기, 순환기, 소화기, 비뇨기계통의 질환 순으로 높았으며, 1999년에는 내분비, 순환기, 소화기, 호흡기, 혈액종양, 신경계, 비뇨기계통의 질환 순이었다. 5) 1999년에 측정한 신체계측에서 PEM을 보인 환아에서 생화학적 기준에 따라 혈청 알부민치, 혈색소 및 총 임파구 수를 분류해 볼 때 혈색소 및 혈청 알부민 수치가 총임파구의 수치보다 낮게 나타났다. 결 론: 입원 환아 단백질-에너지 영양결핍의 빈도는 1994년에 49%, 1999년에는 44.8%로 높게 나타나고 있음을 알 수 있었다. 따라서 질병의 예방과 효과적인 치료 및 환아에게 적합한 영양처방을 결정하기 위해서는 영양관리의 중요성을 인식하고 포괄적인 영양평가와 함께 올바른 식 습관 형성에 대한 지도를 비롯한 적극적인 영양관리가 이루어져야 한다.

Insulin-like growth factor가 소장 점막 세포 증식에 미치는 영향

  • Growth hormone (GH) plays a key role in regulating postnatal growth and can stimulate growth of animals by acting directly on specific receptors on the plasma membrane of tissues or indirectly through stimulating insulin-like growth factor (IGF)-I synthesis and secretion by the liver and other tissues. IGF-I and IGF-Ⅱ are polypeptides with structural similarity with proinsulin that stimulate cell proliferation by endocrine, paracrine and autocrine mechanisms. The initial event in the metabolic action of IGFs on target cells appears to be their binding to specific receptors on the plasma membrane. Current evidence indicates that the mitogenic actions of both IGFs are mediated primarily by binding to the type I IGF receptors, and that IGF action is also mediated by interactions with IGF-binding proteins (IGFBPs). Six distinct IGFBPs have been identified that are characterized by cell-specific interaction, transcriptional and post-translational regulation by many different effectors, and the ability to either potentiate or inhibit IGF actions. Nutritional deficiencies can have their devastating consequence during growth. Although IGF-I is the major mediator of GH's action on somatic growth, nutritional status of an organism is a critical regulator of IGF-I and IGFBPs. Various nutrient deficiencies result in decreased serum IGF-I levels and altered IGFBP levels, but the blood levels of GH are generally unchanged or elevated in malnutrition. Effects of protein, energy, vitamin C and D, and zinc on serum IGF and IGFBP levels and tissue mRNA levels were reviewed in the text. Multiple factors are involved in the regulation of intestinal epithelial cell growth and differentiation. Among these factors the nutritional status of individuals is the most important. The intestinal epithelium is an important site for mitogenic action of the IGFs in vivo, with exogenous IGF-I stimulating mucosal hyperplasia. Therefore, the IGF system appears to provide and important mechanism linking nutrition and the proliferation of intestinal epithelial cells. In order to study the detailed mechanisms by which intestinal mucosa is regulated, we have utilized IEC-6 cells, an intestinal epithelial cell line and Caco-2 cells, a human colon adenocarcinoma cell line. Like intestinal crypt cells analyzed in vivo or freshly isolated intestinal epithelial cells, IEC-6 cells and Caco-2 cells possess abundant quatities of both type Ⅰ and type Ⅱ IGF receptors. Exogenous IGFs stimulate, whereas addition of IGFBP-2 inhibits IEC-6 cell proliferation. To investigate whether endogenously secreted IGFBP-2 inhibit proliferation, IEC-6 cells were transfected with a full-length rat IGFBP-2 cDNA anti-sense expression construct. IEC-6 cells transfected with anti-sense IGFBP-2 protein in medium. These cells grew at a rate faster than the control cells indicating that endogenous IGFBP-2 inhibits proliferation of IEC-6 cells, probably by sequestering IGFs. IEC-6 cells express many characteristics of enterocyte, but do not undergo differentiation. On the other hand, Caco-2 cells undergo a spontaneous enterocyte differentiation. On the other hand, Caco-2 cells undergo a spontaneous enterocyte differentiation after reaching confluency. We have demonstrated that Caco-2 cells produce IGF-Ⅱ, IGFBP-2, IGFBP-3, and an as yet unidentified 31,000 Mr IGFBP, and that both mRNA and peptide secretion of IGFBP-2 and IGFBP-3 increased, but IGFBP-4 mRNA and protein secretion decreased after the cells reached confluency. These changes occurred in parallel to and were coincident with differentiation of the cells, as measured by expression of sucrase-isomaltase. In addition, Caco-2 cell clones forced to overexpress IGFBP-4 by transfection with a rat IGFBP-4 cDNA construct exhibited a significantly slower growth rate under serum-free conditions and had increased expression of sucrase-isomaltase compared with vector control cells. These results indicate that IGFBP-4 inhibits proliferation and stimulates differentiation of Caco-2 cells, probably by inhibiting the mitogenic actions of IGFs.

거제도(巨濟島) 주민(住民)의 영양실태조사(營養實態調査) (Nutrition Survey in Koje Island)

  • Kojedo is the second largest island in Korea and a total population of 115,500 is living on the island of 394.69 sq. km. Under the direction of three nutrition professors, nutrition surveys in two villages in Kojedo, namely Siljun Ri in Hachung Myon and Soowol Ri in Shinhyun Myon, were carried by 30 college senior students majoring in nutrition from August to 20 August 1977. From a total of 176 households of the two villages, 67 households were randomly selected and 390 family members of the households were subjcets of the nutrition surveys. The precise weighing method was used in evaluating the kinds of foods and nutrient intakes of the subjects for three consecutive days. Thirty-seven pre-school children aged between 3 to 6 years and 27 fertile women were examined for biochemical findings and physical status. The main purposes of the surveys are to provide baseline data on nutrition in Kojedo Island for the Kojedo Community Development Project and to compare the nutritional status of the villages of Siljun Ri and Soowol Ri. Siljun Ri is located in the pilot project area of the Koiedo Community Health Project sponsored since December 1970 by the Christian Medical Commission of the World Council of Churches. While Soowol Ri is a control village for comparison. The results obtained are summarized as follows: Food Intake The average food intake per person per day in Siljun Ri, 1064 grams (91.7% in vegetable foods and 7.6% in animal foods) was 90 grams more than that of Soowol Ri, 974 grams (92.8% in vegetable foods and 5.9% in animal foods). However, the food intake per pre-school child in Siljun Ri, 485 grams (92.6% from vegetable foods and 6.4% from animal foods) was 21 grams lower than that of the Soowol Ri, 506 grams (88.5% from vegetable foods and 6.5% from animal foods). The average intake of beans was 16 grams(1.5% out of the total food intake) in Siljun Ri and 21 grams(2.2% of the total food intake)in Soowol Ri. The villagers should be guided for more consumption of soybeans to improve the quality of protein intake from vegetable foods. Nutrient Intake The adult intake in Siljun Ri and Soowol Ri were 2,529 kcal and 2,511 kcal respectively. The average energy intake of pre-school childen in Siljun Ri was 948 kcal and that for adult and 1,500 kcal for childen aged between 4 to 6 years-given by the Korea FAO Association, the diets in both villages were not adequate. Average daily protein intake of the subjected adult in Siljun Ri was 78.4 grams and that of Soowol Ri was 76.2 grams, while pre-school children took 30.7 grams in the former village and 31.7 grams in the latter village per child per day. The protein intake in both villages were lower than the recommended allowances, 80 grams for adult and 45 grams for $4{\sim}6$ years childen, and animal protein intake of the all subjects was very much lower than the RDA. The main charecter of the diet has been found low in quality of protein and high in carbohydrate. The calcium intakes of the pre-school children in both villages, 251.9 milligrams in Siljun Ri and 218.8 milligrams in Soowol Ri, were very much lower than the recommended allowance of 500 milligrams per day. It is apparent that the diet for children should be supplemented with calcium. Among the vitamin group, the daily average intakes of vitamin A and $B_{2}$(thiamine), $B_{2}$(riboflavin), C(ascorbic acid), and niacin were not adequate for the children in both villages. Especially the intake of riboflavin, 0.4 milligrams in both village children, was much lower than the RDA, 0.9 milligrams per day. Physical Characteristics Average height, weight, chest and head circumference of the pre-school children in both villages were similar to those of the Korean standard given by the Korean Paediatrics Association except that the average height of pre-school boys in Siljun Ri was 8 cm higher than the Korean standard of 105 cm. The mean values of upper arm circumference and skinfold thickness of pre-school boys in both villages were the same, 15.4 cm for upper arm circumference and 6.8 mm for skinfold thickness, but the mean values of those of the girls in Siljun Ri were higher than those of pre-school grils in Soowol Ri. Biochemical Findings Avera ge hemogobin value of boys and girls in both villages was the same, 11.1 grams per 100 ml of blood. The incidence of anemia (Hb value below 11g/100ml) was similar in both viltagesr 36.4% for boys and 50% for girls in Siljun Ri and 37.5% for boys and 50% for girls in Soowol Ri. Average hemoglobin values of fertile women were 10.7g% in Siljun Ri and 10.8% in Soowor Ri. The incidences of anemia(Hb valre brlow 12g/100ml) were 100% in Siljun Ri and 86.7% in Soowol Ri. The anemia of these subjects may be caused mainty low intake of good quality protein and iron intake from vegetable food. Recommendation In general, the nutritional status of a community health pilot village is not higher than that of control village due to the lack of nutrition improvement guldance services. Nutrition education should be delivered to the villagers as a main part of the health education artivities. The emphasis should be on building better health through bttter food habits and better food production as well as on preventing malnutrition and diseasrs. It can be an invaluable part of community developnent. Since nutrition is considered to be at least one-half of MCH care, no village or home visits should be made without careful provision for teaching and demoastrating something simple and practical on nutrition. The nurse, midwife, and village health worker should be the chief promoters of nutrition.

광주지역 저소득층 여자노인의 영양상태와 건강위험요인에 관한 연구 (Nutritional Status and Health Risks of Low Income Elderly Women in Gwangju Area)

  • 본 연구는 저소득층 노인을 대상으로 하여 식생활 실태 및 건강상태를 조사하여 노인의 건강과 영양상태의 상호연관성 및 이에 영향을 미치는 위험 요인을 파악하고자 실시되었다. 조사는 광주광역시에 거주하는 저소득층 여자노인 92명을 대상으로 하였으며, 조사대상자를 연령에 따라 세 그룹 ($65{\sim}74$세, $75{\sim}84$세, 85세 이상)으로 체위, 혈액, 영양상태 등을 비교하였으며, Nutritional Screening Initiative (NSI) 점수에 따라 $NSI{\leq}3$은 영양 양호군, NSI > 3은 영양 위험군으로 구분하여 영양위험정도에 따른 식생활 관련 건강요인을 비교하였으며 연구결과를 요약하면 다음과 같다. 1) 본 조사대상자의 평균 연령은 79.0세이었으며, 사회경제적 수준이 매우 낮았으며, 치아건강상태 (70.7%) 및 경제적인 이유 (76.1%)로 음식섭취가 부실하였으며 혼자거동하기 힘든 여자 노인의 비율이 48.9%인데, 63.0%의 노인이 혼자 식사한다고 응답하여 저소득층 여자 노인의 사회경제적 위험 요인이 영양상태를 더욱 악화시키는 것으로 조사되었다. 2) 본 조사 대상자가 앓고 있는 주요 질병으로는 고혈압(42.9%), 관절염 및 신경통(33.0%), 당뇨병 (15.4%), 심혈관계 질환 (8.8%), 소화기계 질환 (8.8%) 등의 순이었다. 전체 여자 노인의 44.6%가 비만하였으며, 고혈압, 당뇨병, 심혈관계 질환 중 1개 이상의 질병을 보유한 노인이 53.3%, 대사 증후군을 보유한 노인이 49.3%로서 여자 노인의 건강상태가 전반적으로 불량한 것 판단되었다. 3) 조사대상자의 평균 체중은 54.5, 신장은 147.1cm로서 한국인 영양권장량설정을 위한 체위 기준치와 비교할 때 체중은 더 많은 반면 신장은 낮은 것으로 조사되었으며, 연령증가에 따라 체중과 체단백질이 감소한 반면 체지방은 감소되지 않았고, 체지방 비율이 40.0%, 허리둘레가 87.7cm에 달하였으며, BMI가 25.1 $kg/m^2$으로 전체적으로 비만한 경향을 나타내었다. 4) 연령에 따라 혈압에 유의적인 차이가 없었으며, 혈청알부민을 제외하고는 혈액의 지질성분, 면역성분, 항산화 성분에서 연령별로 유의적인 차이가 없었다. 여자 노인의 평균 수축기 혈압 143.0 mmHg, 공복 혈당은 119.7mg/dl로서 노인의 중요한 건강 문제는 고혈압, 당뇨 등임을 확인할 수 있으며, TNF-${\alpha}$수준이 높아 노년기 염증 반응 증가를 시사하였다. 5) 조사대상자의 영양상태는 매우 불량하였으며, 연령증가에 따라 영양섭취 실태가 더 불량해지는 경향을 나타내고 있으나 연령에 따른 유의적인 차이는 없었다. NSI 점수를 기준으로 하여, 영양 위험군 (NSI>3)에 비해 영양 양호군 $(NSI{\leq}3)$으로 구분된 노인의 하루 3회 이상의 식사 비율이 높았으며, 건강 인지도가 더 좋고, 우울증이 적었으며 운동 실시율이 높고, 당뇨 유병률이 낮았다. 6) 영양섭취 실태와 면역, 염증반응, 항산화 능과의 상관관계를 살펴 본 결과, 혈청의 IL-6와 $TNF-{\alpha}$가 영양상태와 유의적인 상관관계를 나타내어 항 염증성 인자보다는 염증유발인자가 노인의 영양상태와 관련이 있었으며, IL-2, TAS, 공복 혈당 등이 체위와 유의적인 관계가 있어 노인의 영양상태나 체위가 염증반응을 일으켜 노인의 건강에 영향을 미칠 수 있는 것으로 생각된다. 결론적으로, 연령이 증가함에 따라 생리적, 심리적, 사회경제적 요인이 노인의 영양상태에 복합적으로 작용하여, 영양불균형을 초래하고, 만성질환이 있는 경우에는 영양상태가 더욱 불량해지는 등의 건강상의 악순환을 초래하고 있다. 노년기에 건강한 삶을 유지하기 위해서는 적절한 영양소 섭취, 운동, 정상체중 유지가 중요하며 영양문제를 초래할 수 있는 위험요인을 줄여야 될 것이다. 특히, 노인이 양질의 식사를 할 수 있는 방안이 필요하며, 노인의 영양개선 및 건강 증진을 위해서 우리나라 노인의 식습관 및 영양상태, 체위, 질병 등에 관한 종합적이고 체계적인 연구방법에 대한 타당성 연구가 선행되어야 할 것이며 타당성 있는 연구결과를 바탕으로 하여 노인의 영양균형 및 건강증진을 위한 적절한 연구와 지원대책이 필요한 것으로 사료된다.
