Allied Health Professionals and Work-Related Musculoskeletal Disorders: A Systematic Review

  • Work-related musculoskeletal injuries and disorders (WMSD) are a significant issue in the health care sector. Allied Health professionals (AHP) in this sector are exposed to physical and psychosocial factors associated with increased risk of developing a WMSD. Clarification of relevant hazard and risk factors for AHP is needed to improve understanding and inform WMSD risk management. A systematic analysis of the literature was undertaken to determine prevalence and risk factors for WMSD in AHP. Databases of Ovid MEDLINE, CINAHL (EBSCO), EMBASE and the Cochrane Database of Systematic Reviews were reviewed. This quality of articles was low. Outcome measures were varied, with prevalence rates of WMSD reported from 28% to 96% over a one-year time period. The lower back was the most commonly affected body part. Relevant factors identified with the development of WMSD included inexperience in the role and area of employment. Future research needs to focus on undertaking high quality prospective studies to determine the factors associated with WMSD development in AHP.

  • This study was prospectively conducted to investigate any relationship of nutritional status at the time of admission to length of hospital stay and mortality. All patients admitted to the Asan Medical Center between October 13 and November 12, 1997 who met the study criteria were included in the study. Patients were classified as Not-at-risk, At-risk Ⅰ or At-risk Ⅱ based on the levels of serum albumin and total lymphocyte count in a computerized nutrition screening program. Sixty three percent of the patients were classified as Not-at-risk Group, 29% as At-risk Group Ⅰ and 8% as At-risk Group Ⅱ. Significant correlation was observed between nutritional status and LOS (P<0.01) as well as mortality rate (P<0.05). The more the patient had the nutritional risk factors, the longer the LOS and the higher the mortality rate were. Further studies have to be done in order to demonstrate cost-effectiveness of medical therapy for the malnourished hospitalized patients.

Restoration of Mandibular Edentulous Patient By Dental Implant: Case Report

  • The completely edentulous patient has few treatment options in conventional dentistry. When implants are considered, treatment plans range from a 2-implant overdenture to a completely implant-supported prosthesis. Fixed prosthesis is often the preferred selection of the edentulous patient. fixed full-arch cert amo-metal restorations can be a predictable implant treatment modality for the edentulous patient. Implant-supported fixed prosthesis has several advantages: predictability, fixedness, retrievability, improved function, lower maintenance of prosthesis, long-term published success. Edentulous patients with a severely resorbed mandible often experience problems with their dentures. Treatment concepts involving two to four implants for the support of an overdenture have been proposed. There seems to be no need to insert more than two endosteal implants to support an overdenture, however, long-term prospective studies are needed to support this notion. Using short endosseous implants and an overdenture in the extremely resorbed mandible is a justified treatment option because of the relative simplicity and low morbidity of this treatment strategy. Implant-supported overdenture has several advantages: Cost, retrievability, hygiene access, profile and contour control, increased retention and stability, implant installed in a predicted region(ant. mandible).

당뇨병성 족부궤양의 재발과 연관된 위험인자: 심리사회적 위험인자를 포함한 후향적 연구 (The Risk Factors Associated with Foot Re-Ulceration in Diabetes: A Retrospective Study Including Psychosocial Risk Factors)

  • Purpose: To evaluate several risk factors related to re-ulceration of diabetic foot including psychosocial aspects such as familial support and degree of independence of patients' activity. Materials and Methods: We reviewed medical records and performed telephone interview with eighty-five patients who had a history of hospitalization in our hospital due to diabetic foot ulceration from year 2002 to 2010. Based on the collected data, we analyzed several factors such as age, gender, prevalence duration, accompanying diseases, HbA1c level, degree of independence and familial support. Results: The mean age was 61.4 years and most common in the 4th decade. There were 57 cases (67%) of recurrence, predominance of male. Eleven patients with recurrent diabetic foot ulceration had undergone major amputations. Psychosocial problems such as depression, insufficient familial support and mortality were more frequently observed in recurrent group. Conclusion: This study shows that psychosocial factor such as familial support for patient with diabetic foot could be important to reduce the recurrence rate of diabetic foot ulceration. Therefore, we should pay attention to strategic plans for prevention, screening, treatment, and aftercare through the prospective studies including psychosocial risk factor in diabetic foot ulceration.

초등예비교사 교육을 위한 수학적 과제 설계: 기하 판 위의 정삼각형이 가능한가? (Developing Mathematical Task for Pre-Service Primary Teachers: Equilateral Triangle on Dotty Grids)

  • 본 연구는 초등 예비교사에게 필요한 수학지식을 가르치는 효과적인 수단으로서 수학적 과제의 특징을 논의하고 그에 맞게 과제를 설계하고 교사교육에 실행한 사례를 제시하였다. 그 결과 예비교사들은 '격자점 정삼각형 만들기' 라는 수학적 과제를 해결하면서, 주어진 상황을 수학적 문제로 변환하고 기존 지식과의 연결을 통해 문제를 변형하고 해결하며, 기존의 수학적 개념을 새로운 관점에서 보는 경험을 할 수 있었다. 이러한 경험이 예비교사의 교육에 대한 관점의 변화와 어떻게 연결되는지 그리고 교사교육에 적합한 수학적 과제 설계를 위한 조건에 대해 논의하였다.

예비교사들의 수학교수지식(MKT) 측정 및 분석 연구 (Measuring and Analyzing Prospective Secondary Teachers' Mathematical Knowledge for Teaching [MKT])

  • 이 연구에서는 중등 예비교사들의 교수를 위한 수학적 지식(Mathematical Knowledge for Teaching, 이하 MKT) 수준을 검증할 수 있는 문항 개발을 확장하여서 이를 적용한 측정을 통해 중등 예비수학교사들의 MKT 수준이 어떠한지 알아보고자 한다. 이를 위해 중학교 수준의 수학 영역에 대한 검사 문항을 개발하여 예비교사 54명을 상대로 검사를 실시하여 자료를 수집하여 분석하였다. 자료 분석 결과 예비교사들이 얻은 점수를 백점 만점으로 환산 시 검사지 총점의 평균은 30.2점으로 나타났는데, 예비교사들은 대체로 문항에서 나타내는 학습 내용에 관한 지식을 정확히 이해하지 못하는 것으로 나타났으며 중등학생들이 어려워하는 부분과 이에 대한 정확한 설명 방법을 구체적으로 도입하지 못하는 것으로 나타났다. 교수 방법을 제시하고 설명하는 과정에서도 내용의 본질을 충분히 담지 못하며 학습자에 대해서도 일반적으로 관찰할 수 있는 특성을 기술하는 것으로 나타났다.

자살을 시도한 주요우울증 환자에서 혈청 콜레스테롤 농도의 저하 (Low Serum Cholesterol Level in Major Depression Patients with Suicidal Attempt)

  • Objective : Several studies hove suggested that reduction of cholesterol concentration and low cholesterol level increases suicide, homicide, and depression. The authors sought to determine whether low cholesterol is associated with a history of suicidal attempts among major depression patients. Method : The subjects were 105 patients with major depression, diagnosed according to the DSM-III-R criteria, and 105 age, sex matched healthy controls. Blood was token following overnight fast and serum cholesterol concentration were measured by a enzymatic method Results : The serum cholesterol level was significantly lower in the patients with major depression than in healthy controls($180.4{\pm}43.4mg/dl$ vs $199.7{\pm}35.8mg/dl$). And the serum cholesterol level was significantly lower in major depression patients with suicidal attempt than in major depression patients without suicidal attempt($156.7{\pm}38.2mg/dl$ vs $ 187.8{\pm}42.5mg/dl$) Conclusion : It is hypothesized that low cholesterol level is associated with depression by modifying the serotonin metabolism and the production of interleukin-2, low cholesterol concentration should be further investigated as a potential biological marker of suicidal risk in major depression. Prospective study with serial cholesterol determinations should be done.

  • Lateral epicondylitis is one of the most common causes of elbow pain and has been known to be caused by degeneration of the extensor carpi radialis brevis (ECRB). Nonoperative treatment should be tried first in all patients, because it has been deemed highly successful; however only few prospective studies suggest that symptoms frequently was completely resolved. Operative treatment is indicated for recalcitrant pain after failed conservative treatment, which involves excision of the pathologic portion of the ECRB and results in a high degree of subjective relief and functional restoration. We will review the pathology of the lateral epicondylitis and operative and nonoperative treatment of lateral epicondylitis.

폐절제술시 정량 폐관류스캔을 이용한 폐기능 변화 예견에 대한 평가 (Evaluation of Prospective Pulmonary Function Change for Pulmonary Resection Using Quantitative Perfusion Lung Scan)

  • Spirometry and regional function studies using 99m-Technetium were performed preoperatively to predict postoperative pulmonary function change in 34 patients who had various pulmonary resectional procedures at the Department of Thoracic and Cardiovascular Surgery, Seoul National University Hospital. Between two months and fourteen months postoperation all the patients were reinvestigated with spirometry and clinical examination to evaluate their functional respiratory status. The postoperative obtained values, especially forced vital capacity [FVC] and forced expiratory volume in one second [FEV1] among the other parameters were compared with the postoperative predicted values. Estimated values of FVC and FEV1 derived from preoperative spirometry and quantitative perfusion lung scan correlated well with the measured postoperative values. The linear regression line derived from correlation between postoperative estimated[X] and postoperative measured[Y] values of FVC and FEV1 in all patients are as follows; 1. Y=0.76x + 0.39 in correlation of FVC [r=0.91] 2. Y=0.88x + 0.17 in correlation of FEV1 [r=0.96],br> This method of estimation was one of the best methods of predicting postoperative pulmonary function change and valuable in determining the extent of safe resection and postoperative prognosis to a poor risk patient with chronic obstructive lung disease.

The Effect of Saphenous Vein Ablation on Combined Segmental Popliteal Vein Reflux

  • Background: This study examined the role of superficial vein surgery in patients with combined superficial venous reflux and segmental popliteal vein reflux. Methods: We retrospectively reviewed 42 limbs in 38 patients with combined superficial venous reflux and segmental popliteal vein reflux who underwent saphenous vein ablation between January 2014 and February 2017. Patients underwent outpatient follow-up duplex ultrasonography at 3, 6, and 12 months postoperatively. Resolution of deep vein reflux was defined as reversed blood flow in a popliteal segment for less than 1.0 second and a decrease in the reflux time of more than 20% of the preoperative reflux time. Results: The mean follow-up period was 9 months (range, 3-23 months). Saphenous vein ablations were performed by stripping in 24 limbs and radiofrequency ablation in 18 limbs. Preoperative segmental popliteal vein reflux resolved in 21 of the 42 limbs (50%). Conclusion: This study demonstrated that superficial venous surgery corrected segmental popliteal vein reflux in 50% of limbs with combined superficial venous reflux and segmental popliteal vein reflux. Other prospective studies are necessary to elucidate the etiology of the non-reversible cases.
