Objectives: The purpose of this study is to identify the difference between the awareness and reuser rate of infection control t for disposable dental care supplies (DDCS) according to general characteristics and infection management-related characteristics. Methods: A questionnaire was used for 277 dental hygienists to check the general characteristics, infection management-related characteristics, awareness of infection control disposable dental care products, syringe needle, prophylaxis cup, prophylaxis brush, plastic saliva ejector, orthodontic bracket, and gloves reuse rate. Results: The awareness of infection control for DDCS differed according to 'hospital type', 'average number of patients per day', 'presence or absence of infection control guidelines', and 'experience in infection management training in the last two years' (p<0.05). Reuser rates of disposable dental care products differed according to 'hospital type', 'average number of patients per day', 'presence or absence of infection control guidelines', and 'experience in infection management training in the last two years' (p<0.05). Conclusions: In order to manage infection of DDCS, the level of infection control system in the workplace is improved and support for related education is needed. In addition, guidelines and regulations on prohibition of reuse and classification criteria for various DDCS should be prepared.
A 2-year-old, castrated male, Scottish fold cat was referred to Veterinary Medical Teaching Hospital of Seoul National University (VMTH-SNU) for evaluation of acute bilateral blindness after general anesthesia. For dental prophylaxis in local animal hospital, general anesthesia had been induced with intravenous acepromazine and ketamine, and maintained with isoflurane after intubation. At VMTH-SNU on next day, complete blood count, electrolytes and serum chemistry values were within normal ranges. On neurologic examination, visual placing and postural reactions like as hopping, hemiwalking and wheelborrowing were reduced on right hindlimb. On ophthalmic examination, menace responses were absent on both eyes and pupillary light reflex (PLR) reduced on right eye, but other reflex and fundus were normal. Prednisolone (2 mg/kg sid for 3 days) was administrated orally and tapered. Visual placing was possible on 2nd day, and postural reactions were recovered on 4th day after dental prophylaxis. Based on the process and recovery, this case was considered as postoperative visual loss (POVL) after general anesthesia.
Purpose: Clotting factor X deficiency is one of the most uncommon coagulation disorders. The authors describe a case of cleft palate in a patient with a congenital clotting factor X deficiency. Methods: In pediatric patients with a cleft palate, the coagulation problem is more worrisome, because they are more sensitive to blood than adults, and because postoperative bleeding can cause blood ingestion with subsequent vomiting, aspiration, and airway obstruction. To prevent hemorrhagic complications in the described case, fresh frozen plasma (FFP) was administered every 24 hrs from the day before surgery to the second postoperative day. Results: Good hemostasis, normal healing, and no complications was shown postoperatively. Conclusion: The replacement of fresh frozen plasma was useful in the case of congenital clotting factor deficiency for bleeding prophylaxis in cleft palate operation.
Purpose: The purpose of this study was to evaluate the effectiveness of a ventilator-associated pneumonia (VAP) bundle. Methods: This was a retrospective study that was carried out between June 2010 and October 2015. In this study, 3,224 intubated patients were included. The VAP bundle which was applied to Group 1 patients (n=470) included head-of-bed elevation to 30 degrees, cuff pressure monitorization, prophylaxis of peptic ulcer, and prophylaxis of deep vein thrombosis. The VAP bundle for Group 2 patients (n=1,914) included all the elements of the VAP bundle for Group 1 patients and one additional element which was oral care with 0.12% chlorhexidine. The VAP bundle for Group 3 patients (n=870) added sedative interruption and assessment of readiness to extubate to the VAP bundle for Group 2. Results: The numbers and incidences of VAP were significantly different among the three groups. Moreover, there were significant differences among groups in ICU length of stay and mortality. Conclusion: Three different VAP prevention bundles made different effects in patient outcomes.
Objective: To examine the prophylactic potential of 3 orthodontic bonding adhesives: Fuji Ortho SC, Illuminate, and Resilience. Methods: Thirty-six Wistar Wag rats were randomly divided into 4 groups consisting of 9 rats each. One of the groups received no treatment and was used as a control. In the other groups, individual bands coated with one of the 3 adhesives were cemented to the lower incisors. Enamel samples were obtained after 6 and 12 weeks and analyzed using scanning electron microscopy in combination with energy dispersive spectrometry. Results: Six weeks after band cementation, no fluoride was found in the enamel of the lower incisors. After 12 weeks, there was no fluoride in the enamel of teeth coated with the Resilience composite. However, in the case of the Illuminate composite and the resin-modified glass ionomer Fuji Ortho SC cement, the depth of fluoride penetration reached $2{\mu}m$ and $4.8-5.7{\mu}m$, respectively. Conclusions: Fluoride ions from orthodontic adhesives can be incorporated into the surface layer of the enamel. Orthodontists may apply orthodontic adhesives, such as the Fuji Ortho SC, to reduce the occurrence of caries during orthodontic treatment with fixed appliances.
Prophylactic antibiotics in acute nonperforated appendicitis have been used for preventing infection after appendectomy. However, duration of antibiotic administration for surgical prophylaxis in Korea has been noted to be longer than those recommended in other countries. The objective of this study was to identify appropriate duration of prophylactic antibiotics in acute nonperforated appendicitis by comparing two different antibiotic regimens for their wound infection rates. Eighty-four acute nonperforated appendicitis patients were enrolled in this prospective, randomized, open trial and were assigned to one of two antibiotic regimens: cefoxitin 1 g every 8 hours given intravenously for 24hours or cefoxitin 1 g every 8 hours given intravenously plus sisomicin 75 mg every 12 hours given jntramuscularly for 72 hours. First doses were given just prior to the induction of anesthesia. Postoperative wound infections were detected in $4.8\%$ of the 72-hour-treated group (n=42), whereas none occurred in the 24-hour-treated group (n=42). However, the difference in the rates of wound infections between two groups was not statistically significant. The most frequently isolated microorganism from appendiceal tissues was E coli. In conclusion, administration of cefoxitin alone for 24 hours is sufficient as surgical prophylaxis in nonperforated appendicitis.
Vertebroplasty and kyphoplasty are well-known, useful techniques for the treatment of painful vertebral compression fractures. Although the risk associated with these procedures is low, serious complications can occur. Of these complications, infection is even rarer, however, when it does occur, it is difficult to manage and can become life-threatening. We describe here a case of infectious spondylitis with epidural inflammation that occurred after performing kyphoplasty in a patient who had a thoracic vertebra compression fracture. We reviewed other case and literatures. Extreme asepsis of the operating theater, screening and treatment for systemic infection prior to the procedure, as well as the use of appropriate antibiotic prophylaxis are strongly recommended when conducting kyphoplasty to prevent infection.
목 적: 만성 B형 간염 산모로부터 분만된 신생아에게 주산기 예방조치를 실시하면 모유수유는 권장되어져야한다. 그러나 실지로 국내에서는 높은 비율에서 산모들은 모유수유를 꺼리고 있다. 설문조사를 통하여 이 같은 현상이 초래되는 요인을 찾아보았다. 방 법: 질병관리본부가 주관하는 'B형간염수직감염예방사업'에 참여하여 등록된 만성 B형 간염 산모 중 보건소에서 혈액 채취를 위해 보건소를 방문하고 동의서에 서명을 얻은 경우를 대상으로 하였다. 검사 결과를 고지할 때 이들의 모유수유에 대한 인식을 조사하기 위하여 반송봉투와 함께 설문지를 발송하였다. 결 과: 설문지를 발송한 839명 중 주소 이전 등의 이유로 반송된 114명을 제외한 725명이 설문지를 모두 받았다고 가정할 때 응답율은 17% (125명)이었다. 응답자의 수유방법은 모유 65명(52%), 분유 60명(48%)이었다. 모유수유를 한 경우 가장 영향을 미쳤던 요인은 본인 및 가족의 결정 75%, 산부인과 의사 17%이었고, 분유수유를 한 경우는 산부인과 의사 57%, 본인의 결정 28%이었다. 모유수유와 주산기 예방조치 실패간의 관련성에 대한 인식은 '관련이 있다' 45%, '관련이 없다' 50%이었고, 관련이 있다고 했을 때의 관련 정도는 '어느 정도 관련이 있다'가 55%이었다. 모유수유를 한 경우 추후에 모유수유를 하겠다는 답변은 91%이었고, 분유수유를 하였지만 '모유수유가 주산기 예방조치 실패와 직접적인 관련이 없다'는 사실이 소개된다면 추후에는 모유수유를 선택하겠다는 경우가 78%이었다. 결 론: 모유수유가 B형 간염 주산기 예방조치의 실패와 직접적인 관련이 없음에도 불구하고 많은 숫자에서 모유수유를 하지 않고 있는데, 이러한 결정을 하게 된 경우의 대부분이 의료인에 의해서였다. 교육을 통한 의료인들의 개념전환과 국민홍보가 이루어져야할 것이다.
목적: Indomethacin은 미숙아 동맥관 개존증의 예방적 용법 및 치료적 용법에 사용되어 왔다. 하지만 최근에는 indomethacin의 국내 보급이 중단됨에 ibuprofen이 사용되고 있다. Ibuprofen은 동맥관 폐쇄에 indomethacin만큼 효과적이며 부작용은 적은 것으로 알려져 있다. 그러나 ibuprofen의 예방적 용법에 관한 연구는 많지 않다. 본 연구에서는 미숙아에서 동맥관 개존증의 예방적 치료에 대한 ibuprofen의 효과와 안정성을 indomethacin과 비교하여 분석하고자 하였다. 방법: 2009년 1월부터 2009년 12월, 그리고 2010년 1월부터 2011년 2월까지 두 기간동안 3개의 참여 대학병원 신생아 중환자실에 입원한 34주 미만, 1,500 g 미만의 신생아 중에서 생후 24시간 이내에 indomethacin이나 ibuprofen이 예방적 목적으로 투여된 환자를 대상으로 하여 의무기록을 후향적으로 검토하였다. 두 군간의 동맥관 폐쇄에 대한 효과와 주산기 합병증의 발생 정도를 조사하였다. 결과: 두 군간에 환아의 성별, 재태 연령, 출생체중, 분만 방법, 1분 및 5분 Apgar 점수, 산모의 steroid 사용 여부, 폐 표면활성제 사용 여부 및 인공호흡기 사용여부 및 사용기간에 있어 유의한 차이는 없었다. 생후 7일에 시행한 심초음파 검사에서 indomethacin군은 17명 중 13명, ibuprofen군은 20명 중 19명이 동맥관의 폐쇄를 보였다. 총 입원 기간 외에 주산기 합병증의 빈도는 두 군간의 유의한 차이가 없었다. 결론: Ibuprofen의 예방적 사용은 동맥관 폐쇄에 효과적이며 주산기 합병증 측면에서 유의한 부정적인 차이가 없었다. 따라서 indomethacin을 대체하여 ibuprofen의 사용을 고려해 볼 수 있겠다.
강원지역 D 대학 치위생과 2, 3학년 학생들의 치면세마 실습시간에 내원한 환자중 20대 대학생 263명을 대상으로 구강건강 관련 스켈링 만족도 및 구강위생용품 사용 실태 조사 결과는 아래와 같다. 1. 학생들이 스켈링을 받은 경험이 있는지 살펴본 결과 스켈링을 받은 경험이 있는 학생이 61.2%로 그렇지 않은 학생 38.8%보다 많은 것으로 나타났다. 학생들이 스켈링을 받은 장소에 대해 살펴본 결과는 스켈링을 치과에서 받은 학생이 75.2%로 대부분을 차지하였으며 성별로는 남학생이 34.4%로 여학생 11.8%보다 스켈링을 학교에서 많이 받았고 여학생은 88.2%로 남학생 65.6%보다 치과에서 많이 받았으며, 성별에 따라 유의미한 차이를 보였다($\chi^2$=10.79, p<.01). 2. 스켈링에 대한 만족도에 대해 성별로 살펴본 결과 성별로는 남학생이 38.7%로 여학생 26.5%보다 "잘 모르겠다"에 높은 분포를 보였고, 여학생은 57.4%로 남학생 55.9%보다 스켈링을 받을 당시 아프지 않았다고 인식하였으며, 성별에 따라 유의미한 차이를 보였다($\chi^2$=6.38 p<.05). 3. 구강위생용품 사용 실태에 대하여 조사한 결과, 학생들이 치실을 사용한 경험이 없는 학생이 72.2%로 혀 세척기를 사용한 경험이 없는 학생이 86.3%로 대부분을 차지하였으며, 통계적으로 유의미한 차이가 없었다.
