Effect of Red Ginseng on cytochrome P450 and P-glycoprotein activities in healthy volunteers

  • Background: We evaluated the drug interaction profile of Red Ginseng (RG) with respect to the activities of major cytochrome P450 (CYP) enzymes and the drug transporter P-glycoprotein (P-gp) in healthy Korean volunteers. Methods: This article describes an open-label, crossover study. CYP probe cocktail drugs, caffeine, losartan, dextromethorphan, omeprazole, midazolam, and fexofenadine were administered before and after RG supplementation for 2 wk. Plasma samples were collected, and tolerability was assessed. Pharmacokinetic parameters were calculated, and 90% confidence intervals (CIs) of the geometric mean ratios of the parameters were determined from logarithmically transformed data using analysis of variance after RG administration versus before RG administration. Results: Fourteen healthy male participants were evaluated, none of whom were genetically defined as poor CYP2C9, 2C19, and CYP2D6 metabolizers based on genotyping. Before and after RG administration, the geometric least-square mean metabolic ratio (90% CI) was 0.870 (0.805-0.940) for caffeine to paraxanthine (CYP1A2), 0.871 (0.800-0.947) for losartan (CYP2C9) to EXP3174, 1.027 (0.938-1.123) for omeprazole (CYP2C19) to 5-hydroxyomeprazole, 1.373 (0.864-2.180) for dextromethorphan to dextrorphan (CYP2D6), and 0.824 (0.658-1.032) for midazolam (CYP3A4) to 1-hydroxymidazolam. The geometric mean ratio of the area under the curve of the last sampling time ($AUC_{last}$) for fexofenadine (P-gp) was 0.963 (0.845-1.098). Administration of concentrated RG for 2 wk weakly inhibited CYP2C9 and CYP3A4 and weakly induced CYP2D6. However, no clinically significant drug interactions were observed between RG and CYP and P-gp probe substrates. Conclusion: RG has no relevant potential to cause CYP enzyme- or P-gp-related interactions.

A Frequency Domain Analysis of Corneal Deformation by Air Puff (Air puff에 의한 각막 변형의 주파수 영역 분석)

  • Intraocular pressure is measured after a cornea air puff by observing biomechanical properties such as thickness or displacement of the cornea. In this paper, we deal with a frequency domain analysis of corneal deformation in the air puff tonometry that is used to diagnose glaucoma or lasik. We distinguish the patient from the normal by measuring the oscillation frequency in the neighborhood of the central cornea section. A binary image was obtained from the video images, and cornea vertical oscillation profile was extracted from the difference between the vertical displacement data and the curve fitting. In terms of Fourier transform, a vibration frequency of 479.2Hz for the patient was obtained as well as more higher 702.8Hz for the normal due to stiffness. Hilbert-Huang transform's empirical mode decomposition generally describes local, nonlinear, and nonstationary data. After the data were decomposed into intrinsic mode functions, a spectrum and power were analysed. Finally, we confirm that the patient has 6 times more higher power ratio for the specific intrinsic mode function between the patient and the normal.

  • Shallow slope failures in residual soil during periods of prolonged infiltration are common over the world. Therefore, this study examines an approximate method to estimate the influence of infiltration on surficial stability of slopes by one-dimensional infiltration model. Modified GAML model based on the Green-Ampt model was extended to predict the infiltration behavior of two-layered slope. Then, the model has been considered to evaluate the likelihood of shallow slope failure which is induced by a particular rainfall event that accounts for the rainfall intensity and duration for various return periods in two-layered soil profile. The results obtained from the approximate method were compared with those obtained from numerical analyses. According to the results, with the use of properly estimated input parameters, the proposed method was found to give good results that agree reasonably well to those of the more rigorous finite element analyses.

  • Applicabilities of two numerical methods, the 2-dimensional and the 3-dimensional method, are evaluated to inverse test results obtained from the underwater SASW(Spectral -Analysis-of-Surface-Waves) method. As a result of this study, it has been found that the 2-dimensional method can supposed to be applicable for the cases where stiffness of soil layer increases gradually with depth, and the stiffness is relatively low. For the other cases, however, it has been concluded that the 3-dimensional method needs to be applied to determine realistic theoretical dispersion curves. An example is also shown that in situ soil profile underwater is estimated from experimental dispersion curves using the 3-dimensional method. As a results, it can be concluded that the underwater SASW method can be effectively applied to explore the underwater soil condition.

Evaluation of Beam-Matching Accuracy for 8 MV Photon Beam between the Same Model Linear Accelerator (동일 기종 선형가속기간 8 MV 광자선에 대한 빔 매칭 정확도 평가)

  • This study aimed to assess of beam-matching accuracy for an 8 MV beam between the same model linear accelerators(Linac) commissioned over two years. Two models were got the customer acceptance procedure(CAP) criteria. For commissioning data for beam-matched linacs, the percentage depth doses(PDDs), beam profiles, output factors, multi-leaf collimator(MLC) leaf transmission factors, and the dosimetric leaf gap(DLG) were compared. In addition, the accuracy of beam matching was verified at phantom and patient levels. At phantom level, the point doses specified in TG-53 and TG-119 were compared to evaluate the accuracy of beam modelling. At patient level, the dose volume histogram(DVH) parameters and the delivery accuracy are evaluated on volumetric modulated arc therapy(VMAT) plan for 40 patients that included 20 lung and 20 brain cases. Ionization depth curve and dose profiles obtained in CAP showed a good level for beam matching between both Linacs. The variations in commissioning beam data, such as PDDs, beam profiles, output factors, TF, and DLG were all less than 1%. For the treatment plans of brain tumor and lung cancer, the average and maximum differences in evaluated DVH parameters for the planning target volume(PTV) and the organs at risk(OARs) were within 0.30% and 1.30%. Furthermore, all gamma passing rates for both beam-matched Linacs were higher than 98% for the 2%/2 mm criteria and 99% for the 2%/3 mm criteria. The overall variations in the beam data, as well as tests at phantom and patient levels remains all within the tolerance (1% difference) of clinical acceptability between beam-matched Linacs. Thus, we found an excellent dosimetric agreement to 8 MV beam characteristics for the same model Linacs.

Assessment of Yield Characteristics of Gas Pipeline Materials by Observing Surface-Local Deformation (미소 표면변형 관찰을 통한 가스배관 부재의 항복특성 평가)

  • A combination of the instrumented indentation and 3D morphology measurement has been tried in order to perform a real-time property measurement of degraded materials in gas pipelines; three-dimensional indent morphologies were recorded using a reflective laser scanner after a series of insturmented indentations on three metallic specimens. Dimensions of the permanent deformation zone and contact boundary were analyzed from the cross-sectional profile over an remnant indent and used for estimating yield strength and hardness, respectively. Estimated yield strength was comparable with that from uniaxial tensile test and actual hardness implying material pile-up effects was lower than the calculated value from indentation curve by $20{\sim}30%$. It means that this 3D image analysis can explain the material pile-up effects on the contact properties. Additionally, a combined system of indentation and laser sensor was newly designed by modifying a shape of the indentation loading fixture.

Pharmacokinetics of oxytetracycline in olive flounder (Paralichthys olivaceus) by dipping and oral administration (Oxytetracycline의 약욕 및 경구투여에 따른 넙치(Paralichthys olivaceus) 체내 약물동태학적 특성)

  • The pharmacokinetic properties of oxytetracycline (OTC) were studied after dipping and oral administration to cultured olive flounder, Paralichthys olivaceus (600 g). Plasma concentrations of OTC were determined after oral dosage (50, 100 and 200 mg/kg body weight) and dipping (50, 100 and 200 ppm, 1 h) in olive flounder (average 600 g, 23±1℃). Plasma samples were taken at 3, 5, 10, 15, 24, 32, 48, 72, 120, 168 and 240 h post-dose. In oral dosage of 50, 100 and 200 mg/kg, the peak plasma concentrations of OTC, which attained at 3 h post-dose, were 0.34, 0.44 and 1.18 ㎍/㎖, respectively. In dipping of 50, 100 and 200 ppm, those of OTC which also observed at 5 h post-dose, were 0.43, 0.38 and 0.64 ㎍/㎖, respectively. Plasma concentrations of OTC were not measurable at 240 h post-dose in all experiments. The kinetic profile of absorption, distribution and elimination of OTC in plasma were analyzed fitting to a one-compartment model by WinNonlin program. The following parameters were calculated for a single dosage of 50, 100 and 200 mg/kg body weight, respectively: AUC (the area under the concentration-time curve)=31.40, 28.07 and 32.97 ㎍∙h/㎖; T1/2 (half-life)􀆫0.89, 1.12 and 0.43 h; Tmax (time for maximum concentration)= 5.25, 3.70 and 7.30 h, Cmax (maximum concentration)=0.25, 0.38 and 0.61 ㎕/㎖. Following dipping at 50, 100 and 200 ppm, these parameters were AUC􀆫15.51, 14.63 and 19.72 ㎍∙h/㎖; T1/2= 0.75, 0.41 and 0.74 h; Tmax=4.90, 7.08 and 4.68 h, Cmax=0.40, 0.32 and 0.46 ㎕/㎖.

HPLC Determination of Tolperisone in Human Plasma

  • A simple high performance liquid chromatographic (HPLC) method was developed for the determination of tolperisone in human plasma. Tolperisone and internal standard (chlorphenesin) were isolated from 1 mL of plasma using 8 mL of dichlormethane. The organic phase was collected and evaporated under nitrogen gas. The residue was then reconstituted with 300 mL aliquot of mobile phase and a 100 mL aliquot was injected onto the $C_{18}$ reverse-phased column. The mobile phase, $45\%$ methanol containing $1\%$ glacial acetic acid and $0.05\%$ 1-hexanesulfonic acid was run at a flow rate of 1 mL/min. The column effluent was monitored using UV detector at 260 nm. The retention times for tolperisone and the internal standard were approximately 7.1 and 8.4 min, respectively. The standard curve was linear with minimal intra-day and inter-day variability. The quantification limit of tolperisone in human plasma was 10 ng/ mL. The proposed method has been applied to the determination of pharmacokinetic profile of tolperisone in Koreans. The T max of tolperisone in Koreans $(0.94{\pm}0.42\;h)$ was not significantly differ from that reported in Europeans (0.5-1 h), but the mean half-life in Koreans $(1.14{\pm}0.27\;h)$ was shorter than that in Europeans $(2.56{\pm}0.2\;h)$. The proposed HPLC method is simple, accurate, reproducible and suitable for pharmacokinetic study of tolperisone.

Genetic alterations in Wnt family of genes and their putative association with head and neck squamous cell carcinoma

  • Head and neck squamous cell carcinoma (HNSCC) is the most frequent type of head and neck cancer that usually arises from the mucosal surfaces of several organs including nasal cavity, paranasal sinuses, oral cavity, tongue, pharynx, and larynx. The Wnt signaling pathway is a crucial mechanism for cellular maintenance and development. It regulates cell cycle progression, apoptosis, proliferation, migration, and differentiation. Dysregulation of this pathway correlates with oncogenesis in various tissues including breast, colon, pancreatic as well as head and neck cancers. The present study aims to assess the gene alterations in the Wnt family of genes so as to derive an association with HNSCC. Computational approaches have been utilized for the identification of gene alterations in the Wnt family of genes. Several databases such as cBioportal, STRING, and UALCAN were used for the purpose. The frequency of alteration was high in case of Wnt family member 11 (5%). Gene amplification, deep deletions, missense and truncating mutations were observed in HNSCC patients. There was a marked difference in the gene expression profile of WNT11 between grades as well as normal samples. The survival probability measured using the Kaplan-Meier curve also presented with a significant difference among male and female subjects experiencing a low/medium level expression. The female patients showed less survival probability when compared to the male subjects. This provides the prognostic significance of the WNT11 gene in HNSCC. Taken together, the present study provides clues on the possible association of WNT11 gene alterations with HNSCC, which has to be further validated using experimental approaches.

Seismic Fragility Evaluation of Cut-and-cover Tunnel (박스형 터널의 지진 취약도 평가)

  • In this study, the seismic response of cut-and-cover box tunnels is evaluated from pseudo-static analyses and the fragility curves are derived. A series of site profiles were used to evaluate the effect of soil conditions. A total of 20 ground motions were used. The fragility curves were developed as functions of peak ground acceleration for three damage states, which are minor, moderate, and extensive states. The damage indices, defined as the ratio of the elastic moment to the yield moment, correlated to three damage states, were used. The curves are shown to greatly depend on the site profile. The curves are further compared to those derived in previous studies. The widely used empirically derived curves are shown not to account for the site effects, and therefore underestimate the response for soft sites.
