A Study on the Preparation of Distribution Table for Quantitative Evaluation of Small-Scale Environmental Impact Assessment

  • Purpose: The small-scale environmental impact assessment has been in operation since its implementation in August 2000, and is a system that meets the purpose of sustainable development in consideration of the environment in areas requiring conservation. However, when preparing a small environmental impact assessment report, the contents of the reduction measures are too qualitative, or reports are prepared to simply list the compliance stipulated by individual laws, and the contents of consultations prepared by consultative agencies and review agencies are not much different. In addition, the direction of consultation on development projects for similar locations of the same project type is frequently changed by reflecting the subjective judgment of the consultative officer of the Ministry of Environment (Environment Agency). Therefore, this study attempted to improve the establishment of measures to reduce the existing simple listing of qualitative contents and the inconsistent presentation of review opinions by consultative agencies and review agencies. Research design, data and methodology: The research method extracted absolute evaluation items and relative evaluation items among small environmental impact assessment items, analyzed and presented detailed items, and prepared a distribution table for each section according to the details of the relative evaluation items, and presented them as a table. Results: This study was conducted to derive uniform results with objective indicators in the preparation and consultation process of a small-scale environmental impact assessment. Conclusions: Once a quantitative evaluation is established, the consultant can objectively determine and process the environmental impact.

  • Objectives: The aim of this trial is to evaluate the efficacy and safety of Yukwool-tang (Liuyu-tang) for the treatment of major depression in women by comparing the Yukwool-tang (Liuyu-tang)-treated group with the placebo-treated group and assessing the association of various biological factors related to depression through various outcome measures. Methods: This study is a single-center, randomized, double-blind, placebo-controlled, parallel-design clinical trial. The subjects to be selected are women between the ages of 19 and 65, and the registered subjects are to be randomly assigned to treatment with Yukwool-tang or the placebo control. The Yukwool-tang group will take 1 bottle of Yukwool-tang (30 mg) for 8 weeks, 3 times a day, before meals. The control group will take the placebo in the same way. The primary outcome to be examined will be the change between the total score after 8 weeks and the total score before the start of the study of the K-HDRS score. Secondary outcomes are assessed by the change in total score after 12 weeks of K-HDRS, K-HDRS remission rate, K-HDRS improvement rate, BDI-K, PITD, KSCL-95, ISI, STAI-K, EQ-5D, VAS, Emotional Stimulation Test, BDNF test, inflammatory cytokine and tumor necrosis factor test, intestinal microbiome test, dietary report and Beck's hopelessness scale. Results: This protocol has been approved by the IRB of Dunsan Korean Medicine Hospital of Daejeon University and is registered in the CRIS, and it is made public in advance to ensure transparency of the research process and conduct ethical clinical trials. Conclusions: Based on this protocol, when the trial is completed, its data can be used to access the validity and safety of Yukwool-tang for major depression in women, and it is also expected to be helpful in the study of the correlation between future treatment of Korean medicine for depression and related biological factors, and quality of life.

A Study on the Essential Information to Collect Disaster Sites for Effective Disaster Management : Focused on Jecheon Sports Center Fire case (효율적인 재난 상황관리를 위한 재난현장 필수정보 연구: 제천 스포츠 센터 화재사건을 중심으로)

  • Purpose: When a disaster occurs, information is collected and sent indiscriminately to the disaster safety situation room, making it difficult to make quick and correct decisions. Therefore, in this study, both the field response team and the situation room attempted to derive the essential information elements for disaster management so that they can identify and respond optimally. Method: In this study, the actual disaster response process was first identified based on the case of large-scale fire disasters in multi-dense facilities. Afterwards, essential information on disaster management was obtained by analyzing the standard manual for large-scale fire crisis management in multi-dense facilities and the fire situation report form prepared at the time of disaster. Result: The Essential information on disaster management was divided into four stages: situation reception·reporting·transmission, total response·regional emergency rescue control group·regional disaster safety task force, operation stage of central emergency rescue control group·central disaster safety task force, and recovery stage. Since then, the information to be collected at each stage and the information relevant to core tasks are presented. Conclusion: The Essential information on disaster management thus obtained can serve as a guide for the person who collects information on site to collect information efficiently. In addition, it is expected that the situation room in charge of disaster response decision-making will receive only the information necessary for decision making, so that it is possible to make a quick situation decision and more efficient disaster situation management.

A Case of Metal Fume Fever Associated with Copper Fume in a Welder (용접공에서 발생한 구리흄에 의한 금속열 1례)

  • Metal fume fever has been known as an occupational disease is induced by intense inhalation of fresh metal fume with a particle size smaller than $0.5{\mu}m\;to\;1{\mu}m$. The fumes originate from heating metals beyond their boiling point, as happens, for example, in welding operations. Oxidation usually accompanies this process. In most cases, this syndrome is due to exposure to zinc oxide fumes; however, other metals like copper, magnesium, cadmium, manganese, and antimony are also reported to produce such reactions. Authors report a case of metal fume fever suspected to be associated with copper fume inhalation. The patient was a 42-year-old male and was a smoker. He conducted inert gas tungsten arc welding on copper-coated materials without safety precautions such as a protective mask and adequate ventilation. Immediately after work, he felt metallic taste in his mouth. A few hours after welding, he developed headache, chilling sensation, and chest discomfort. He also complained of myalgia, arthralgia, feverish sensation, thirst, and general weakness. Symptoms worsened after repeated copper welding on the next day and subsided gradually following two weeks. Laboratory examination showed a transient increase of neutrophil count, eosinophilia, elevated erythrocyte sedimentation rate, and positive C-reactive proteinemia. Blood and urine copper level was also increased compared to his wife. Before this episode, he experienced above complaints several times after welding with copper materials but welding of other metals did not produce any symptoms. It was suggested that copper fume would have induced metal fume fever in this case. Further investigations are needed to clarify their pathogenic mechanisms.

A Study on Projection of Demand and Supply for Paramedic in the Emergency Medical Services Act (1급응급구조사의 수급에 관한 연구 -응급의료에관한법률을 중심으로-)

  • The purposes of this study which was conducted by applying three projection formulae to the data from admission quota for paramedic of the Ministry of Education & Human Resources Development the number of ambulances the number of emergency medical centers of the Ministry of Health & Welfare and rate of successful candidates of annual report of the National Health Personnel Licensing Examination Board were to find out demand and supply of paramedic from 2002 to 2045 and to expand scope of practice of paramedic in Korea. The conclusions from this study were summarized as follows; (1) The simple formulae derived from the projection formula of the Economic Planning Board were applied under the present Emergency Medical Services Act including qualifying over 3-years experienced EMT-Basic for paramedic examinee, stationing paramedic or EMT-Basic or physician or nurse per ambulance, stationing paramedic or EMT-Basic per emergency medical center and under the amended Emergency Medical Services Act including qualifying only paramedic graduate for paramedic examinee, stationing 4.5 paramedics per ambulance, stationing 10 to 2 paramedics per emergency medical center. (2) It was estimated that on the American basis of 5.6 EMTs per 10,000 in 1996, the number of paramedics under the present act will reach the basis before 2020, the number of paramedics under the amended act will reach the basis about 2040. (3) It was estimated that on the basis of 22,000 paramedics demanded from the number of ambulances, the number of emergency medical centers in 2001, the number of paramedics under the present act will reach the basis before 2015, the number of paramedics under the amended act will reach the basis about 2030. (4) There was relationship between requirements for emergency medical centers scope of practice of paramedic in the act and demand-supply of paramedic, this necessitates surveys, studies, amendment of the act, legalization for expanded scope of practice of paramedic including EMD, instructor, teacher of safety. (5) This study which includes only expanded scope of practice of paramedic and projection for paramedic in the act needs complementary studies such as decision-making process in health manpower policy and so on.

Scrutiny Made to SUPAC-IR Dealing with Postapproval Changes in Immediate Release Sold Oral Dosage Forms (경구용 속방성 성형제품의 허가 후 변경사항을 다루는 SUPAC-IR에 대한 검토)

  • The objective of this study was to provide a better understanding of SUPAC-IR and its application in handling postapproval changes to immediate release solid oral dosage forms. Originally, SUPAC-IR was aimed at reducing the regulator burdern of the industry when they were making postapproval changes, but still at maintaining the formulation quality and performance of a drug product. The postapproval changes that were covered under SUPAC-IR included variations in the components ad composition of formulation, the site of manufacturing, batch size, manufacturing equipment, and manufacturing process. The guidance defined levels of changes, based on the likelihood of risk ocurrence and potential impact of postapproval changes upon the safety and efficacy of a drug product I suggested what a type of fing report should be submitted to the FDA for each level of change. Chemist, manufacturing, and control tests to be executed were also recommended for each change level The important tests specified in the guidance included batch release, stability, in vitro dissolution, and in vivo bioequivalence tests. However, there have been strong demands on revising the current SUPAC-IR in order to resolve some issues and to improve its usefulness in evaluating postapproval changes to immediate release solid oral dosage forms. In particular, the rigorous requirement of case C dissolution test and the definition of batch size were challenged by both academia and the industry. A revision work was in progress to reflect these inputs and to expand the utility of SUPAC-IR. As a result of these concerted efforts, an updated 2nd version of SPAC-IR would be likely to be issued ver soon to the public.

Abrupt Rise of Humidifier Disinfectant Associated Health Problems since 2008: Was it chance or inevitable? (2008년부터 가습기 살균제 건강 피해 급증: 우연인가, 필연인가?)

  • Objective: The objectives of this study are to report the number of humidifier disinfectant (HD) associated health problems, including HD associated lung injury (HDLI), by year. This data was analyzed by the type of HD and HD brand. Methods: A total of 530 patients registered with the national program on HD through its third round were distributed based on the year when they developed their first health problem including HDLI (N=221). The distribution of health problems at diagnosis was clinically evaluated in order to examine the association between their lung injury and the use of HD. Results: The number of HD associated victims and HDLI patients was found to rise sharply from 2008 to 2011, with a peak in 2011. This trend was found not only for HD brands containing polyhexamethylene guanidine phosphate (PHMG), but also chloromethylisothiazolinone/methylisothiazolinone (CMIT/MIT). The number of patients who responded as developing health problems in the specific year was 35 for 2008, 51 for 2009, 108 for 2010 and 182 for 2011. Other types of HD brands and HD chemicals did not follow the trend of abrupt increase in HD associated patients since 2008. Conclusion: This study found the number of HD associated victims and HDLI patients who used HD brands containing PHMG sharply increased starting in 2008. A significant change in the process of manufacturing PHMG can be suspected with the abrupt rise in HD associated patients in specific years.

DEVELOPMENT OF NEW TAXONOMY OF INAPPROPRIATE COMMUNICATION AND ITS APPLICATION TO OPERATING TEAMS IN NUCLEAR POWER PLANTS

  • Inappropriate communications can cause a lack of necessary information exchange between operators and lead to serious consequences in large process systems such as nuclear power plants (NPPs). In this regard, various kinds of taxonomies of inappropriate communications have been developed to prevent inappropriate communications. However, there seems to be difficult to identify inappropriate communications from verbal protocol data between operators. Because the existing taxonomies were developed for use in report analysis, there is a problem of 'uncertainty'. In consequence, this paper proposes a new taxonomy of inappropriate communications and provides some insights to prevent inappropriate communications. In order to develop the taxonomy, existing taxonomies for four industries from 1980 to 2010 were collected and a new taxonomy is developed based on the simplified one-way communication model. In addition, the ratio of inappropriate communications from 8 samples of audio-visual format verbal protocol data recorded during emergency training sessions by operating teams is compared with performance scores calculated based on the task analysis. As a result, inappropriate communications can be easily identified from the verbal protocol data using the suggested taxonomy, and teams with a higher ratio of inappropriate communications tend to have a lower performance score.

Development of Clinical Protocol on the Correlation Between Disease Cause Pattern Identification and Pulse Wave Variables (병인변증과 요골동맥 맥상파의 특성 파악을 위한 탐색적 관찰 연구 : 임상시험 프로토콜 개발)

  • The purpose of this clinical study is to develop structured clinical trial protocol and guideline for improvement of safety, useful and effective of pulse diagnosis devices. As a first step, papers on pulse diagnosis and pulse diagnosis devices from 2001 and 2013 were systematically reviewed. In the next step, we have collected the opinions from the specialists, companies, and statistician in pulse diagnosis to evaluate the current condition, the state and problem of domestic clinical trial cases of pulse diagnosis device. And we have to created protocol and case report form (CRF) in regards to site condition and characteristics of pulse diagnosis devices, and showed the guideline of eligibility criteria, operation process, investigation items, evaluation items and so on. This clinical protocol will become a basic information for a researcher in designing or performing a clinical study of pulse diagnosis devices, and be used as a useful material during acquisition of good clinical data. Furthermore, we hope to enhance the invigoration of pulse diagnosis clinical trials and the performance improvement of pulse diagnosis devices.

Prevention Meteorological Database Information for the Assessment of Natural Disaster (자연재해 평가를 위한 방재기상 DB 정보)

  • In order to reduce the amount of damage from natural disasters and perform the natural disaster mitigation program, the prevention activities and forecasting based on meteorological parameters and disaster datas are required. In addition, it is necessary to process prevention meteorological information for prevention activities in advance. For this, we have analyzed four data, such as Statistical yearbook of calamities and Statistics Yearbook issued by the Ministry of Government Administration and Human affairs. And Annual Climatological Report issued by the Korea Meteorological Administration and Recently 10 years for natural disaster damage from the Central Disaster and Safety Countermeasures Headquarters. We analyzed the causes, elements, occurrence frequencies, and vulnerable areas of natural disaster, using the 4 disaster datas, but these datas was not consistent with their terminology and items. Through the analysis of a kind and damage of disaster, we have selected the disaster variables, such as causes and elements, the amount of damage, vulnerable areas of natural disaster, etc and made a database. This database will be used to assess the natural disasters and develop the risk model and natural disasters mitigation plan.
