Purpose: This study was conducted to develop a mother-fetus interaction promotion program aimed at enhancing the sensitivity of primiparas, and to evaluate the effects of a mother-infant play interaction. Method: Participants were recruited from OB-GY clinic with postpartum take-care center (17 mother-infant dyads for intervention group and 17 dyads for control group). Data were collected from January 18 to August 5, 2005. For the intervention group, programmed education which focused on mother-fetus interaction in the 3rd trimester was given. For the two groups, home visiting or a postpartum care center was used for data collection of the mother-infant interaction which was conducted at postpartum 1 week and 1 month. Also mother-infant interactions during feeding were videotaped and two trained observers analyzed the tapes. Results: A significant difference was found in mother-infant interaction between the two groups (postpartum 1 week, t=6.10, P=0.000, 1 month t=6.69, p=.000). For variations in mother-infant interaction in the control group, a significant difference was found in between postpartum 1 week and 1 month (t=-2.564, p=.021). In subscale analysis, interactional behavior of the infant significantly increased in both groups. Conclusion: This study showed that the mother-fetus interaction promotion program aimed at promoting mother-infant interaction increase maternal sensitivity. Therefore, this study suggests that this nursing intervention to increase maternal sensitivity to the fetus should be broadly applied with primiparas, as it can be beneficial for formation of the mother-infant relationship, and for promotion of the social, emotional, and cognitive developments of the children.
Purpose: This study aimed to determine the actual state of childbirth in Korean women with natural childbirth and the degree of damage to the perineum during childbirth. Methods: This retrospective study analyzed the medical records of mothers who had natural childbirth at a hospital in Seoul, Korea in 2018. Data from 358 women with cephalic births at greater than 37 gestational weeks were analyzed. To determine natural childbirth characteristics and the degree of damage to the perineum, descriptive statistics were done. The difference in the degree of perineal injury according to obstetric characteristics was analyzed using independent t-test and one-way analysis of variance. Results: The mean age was 33.18±3.68 years, and 49.2% were primiparas, while 39% gave birth with a doula. The degree of perineal damage differed by age (F=9.15, p<.001), parity (t=19.13, p<.001), number of births in multiparity (F=3.68, p=.027), previous vaginal delivery in multiparity (F=3.00, p=.032) and birthing posture (F=7.44, p<.001). Having received therapeutic procedures (t=-4.62, p<.001), specifically fluid administration (t=-2.72, p=.007), oxygen supply (t=-2.76, p=.006) and epidural anesthesia (t=-2.77, p=.006) were statistically significant for perineal damage. There were no differences, however, by gestational period, doula use, water room use in labor, baby head circumference, or birth weight. Conclusion: Study findings suggest that support for older women, primiparas, and those who require therapeutic procedures may help to decrease the possibility of perineal damage during childbirth. As perineal damage was also associated with birthing posture, this should be considered when providing intrapartum nursing care.
Purpose: This study was conducted to evaluate the effectiveness of the maternal role education program for primiparas in mother-infant interaction, childrearing environment, and infant development. Method: A Non-equivalent control group time-series design was used. For the intervention group, programmed parenting education focusing on mother-infant interaction, home environment for infant development, and parent counseling and support was provided via home visits or telephone for twelve months. Result: Significant differences were found in the mother-infant interaction feeding scale at one and three months, but no differences were found in the teaching scale at six and twelve months between the intervention and control groups. Also, the difference in childrearing environment (HOME) between the two groups was significant at three, six, twelve months. In addition, the intervention group showed higher GQ in the Griffiths mental development scale at three and six months. In multiple regression analysis, 22.6% to 43.6% of infant development was explained by HOME, mother-infant interaction, and previous development. Conclusion: The maternal role education program proved to be effective in promoting mother-infant interaction, organizing the childrearing environment, and fostering infant development.
The purpose of this study is to investigate the effect of breast-feeding education on skills, attitude, and duration of breast-feeding among primipara mothers. The educational session was offered during the period of June through September at the T medical center in T city. One hundred thirteen primiparas were selected for this study, 61 who received breast-feeding education(lecture, demonstration, and additional telephone counselling) served as the experimental group and 52 who did not as the control. Data collection was done by self-reporting method. the reliability of the instrument was from .77 to .87. Duration of breast-feeding was classified into I through V according to the WHO classification at the time point of 4 weeks of postpartum. The results were as follows : 1. The score for the experimental group on the skills of breast-feeding techniques was significantly higher than those of the control(t=9.12, p=.000). 2. The score for the experimental group in the attitude toward breast-feeding was significantly higher than those of the control(t=6.12, p=.000). 3. Duration of breast-feeding for the experimental group was significantly longer than that of the control at the time of 4 weeks of postpartum($x^2$=20.79, p=.001). In conclusion, the breast-feeding practice can be promoted effectively through the breast-feeding education.
Objective: The aim of the paper was to compare the fit of data derived from daily automatic milking systems (AMS) and monthly test-day records with the use of lactation curves; data was analysed separately for primiparas and multiparas. Methods: The study was carried out on three Polish Holstein-Friesians (PHF) dairy herds. The farms were equipped with an automatic milking system which provided information on milking performance throughout lactation. Once a month cows were also subjected to test-day milkings (method A4). Most studies described in the literature are based on test-day data; therefore, we aimed to compare models based on both test-day and AMS data to determine which mathematical model (Wood or Wilmink) would be the better fit. Results: Results show that lactation curves constructed from data derived from the AMS were better adjusted to the actual milk yield (MY) data regardless of the lactation number and model. Also, we found that the Wilmink model may be a better fit for modelling the lactation curve of PHF cows milked by an AMS as it had the lowest values of Akaike information criterion, Bayesian information criterion, mean square error, the highest coefficient of determination values, and was more accurate in estimating MY than the Wood model. Although both models underestimated peak MY, mean, and total MY, the Wilmink model was closer to the real values. Conclusion: Models of lactation curves may have an economic impact and may be helpful in terms of herd management and decision-making as they assist in forecasting MY at any moment of lactation. Also, data obtained from modelling can help with monitoring milk performance of each cow, diet planning, as well as monitoring the health of the cow.
Objective: The purpose of this study was to examine the different effects on labor pain, length of delivery time, and hemoglobin for primipara women treated by San-Yin-Jiao(SP-6) acupressure of 15 minutes or 30 minutes. Methods: The design of this study was a randomized clinical trial with two experimental groups and one control group, the participants included primiparas who underwent vaginal delivery. Data were collected using a structured questionnaire consisting of general characteristics with a subjective labor pain scale with VA.S, and length of delivery time and hemoglobin were collected in the medical record. Data was analyzed using descriptive statistics, x2- test, t-test, and ANCOVA. Results: There was statistically significant lower labor pain in the group of 30 minutes SP6 acupressure than the control group(t=7.733, p=.007), and there was statistically significant short length of delivery time in the group of 15 minutes SP6 acupressure than the control group(t=-2.230, p=.030). Conclusions: Even though we could not found a consistent effect with the SP6 acupressure, It was found that there was effect on shortening length of delivery time by 15 minutes of SP6 acupressure or lessening labor pain by 30 minutes of SP6 acupressure without risk of blood loss. It is necessary to replicate the study with a larger number of participants to find a consistent result.
Purpose: This study was done to evaluate effects of a mother-fetus interaction promotion program on maternal and infant sleep/activity regulation, and examine the correlation between maternal and infant sleep/activity. Methods: Thirty-four mother-infant dyads were recruited from an OB-GY clinic with postpartum care center, and assigned to the intervention group (17) or control group (17). Data were collected from January 18 to August 5 2005. A programmed education focused on mother-fetus interaction was provided to the intervention group in the 3rd trimester. Maternal sleep/activity record was completed at 32-36 weeks gestation and infant sleep/activity record was completed for one month after birth. Results: During the 3rd trimester, there were no significant differences in maternal sleep/activity between the groups, except for frequency of mothers' night awakening and urination at night. There were no significant differences in infant sleep, but significant difference was found in feeding times between the groups. Correlation of maternal sleep in the 3rd trimester and infant waking times was significant. Conclusion: The results of this study indicate that maternal sleep/activity during pregnancy influences infant sleep/activity at 3-4 weeks, and that nursing interventions to regulate maternal sleep/activity provided to primiparas can be beneficial to the regularity of infant sleep/activity patterns.
본 연구는 배우자지지가 초산모의 산후우울과 자기효능감에 미치는 융합적 효과를 알아보기 위해 37명의 정상만삭아를 분만한 초산모와 그들의 배우자를 대상으로 시행되었다. 배우자는 산후관리와 아기돌봄에 대한 교육자료와 함께 3회의 교육을 받은 후 산후 1주부터 6주까지 산후관리와 아기돌봄을 실천하였다. 산후 1주와 6주에 각각 수집된 자료는 SPSS 20.0으로 분석하였다. 산후 1주와 6주의 산후우울 유병률은 각각 24.3% 와 0% 였다. 배우자의 지지는 초산모의 산후우울을 감소시켰고 자기효능감을 증가시켰으며(p<.001), 산후우울에는 산후도우미, 월수입, 산전우울이, 산후 자기효능감에는 임신 중 입원치료가 영향을 미치는 요인으로 나타났다(p<.05). 본 연구결과는 대상자수를 확대하고 대조군을 설정한 후속연구 후 초산모의 산후우울감소와 자기효능감증진 프로그램개발에 활용될 수 있다.

Purpose: This study investigated self-food restriction during breastfeeding, reviewed the literature showing the effect of maternal diet on the health of breast-fed infants, and explored the validity of dietary restrictions. Methods: Questionnaire data were collected from breastfeeding Korean mothers who visited the pediatric clinic of Cheil General Hospital & Women's Healthcare Center from July 2015 through August 2015. The survey included items assessing maternal age, number of children, maternal educational attainment, household income, degree of difficulty with self-food restriction, types of self-restricted foods, dietary customs during breastfeeding, and sources of information about breastfeeding. Results: The questionnaire was completed by 145 mothers. More than a third (n=56, 39%) had discomfort from and usually avoided 4-5 types of food (mean, 4.92). Mothers younger than 40 years had more discomfort (odds ratio [OR], 12.762; P=0.017). Primiparas felt less discomfort than multiparas (OR, 0.436; P=0.036). Dietary practices were not influenced by maternal educational attainment or household income. The most common self-restricted foods were caffeine (n=131, 90.3%), spicy foods (n=124, 85.5%), raw foods (n=109, 75.2%), cold foods (n=100, 69%), and sikhye (traditional sweet Korean rice beverage) (n=100, 69%). Most mothers (n=122, 84.1%) avoided foods for vague reasons. Conclusion: Most mothers restricted certain foods unnecessarily. Literature review identified no foods that mothers should absolutely avoid during breastfeeding unless the infant reacts negatively to the food.
The purpose of this experiment was to investigate the effect of the diets supplemented with lard or prilled fat ($Carolac^{(R)}$) on lactation performance, plasma constituents and ruminal characteristics of Holstein cows under a warm climate. In trial 1, 18 Holstein cows, 14 primiparas at 43 DIM and 4 multiparas at 55 DIM, were randomly assigned into six $3{\times3}$ Latin squares, in which three dietary treatments were isoproteinous but varying in energy contents and three 21 d periods. The treatments were basal diet (Control), basal diet supplemented with 2.5% lard (LD), and basal diet supplemented with 2.5% commercial Prilled fat (PF). In trial 2, three rumen canulated pregnant nonlactating Holstein cows with 550 kg average body weight were allotted into a $3{\times}3$ Latin square design with diets same as in trial 1 were fed to the cows at the level of 1.5% body weight on dry matter (DM) basis. The results indicated that the DM intake did not differ among the treatments. Milk yield and 4% FCM yield were greater (p<0.05) in PF than in Control. LD and PF resulted in greater milk fat percentage. Protein, lactose and solid contents in milk were not different among the three dietary treatments. The concentration of nonesterified fatty acids (NEFA) in plasma was significantly greater in LD and PF than that in Control. However, the concentrations of triglycerides, urea nitrogen, and cholesterol in plasma were not significantly different among the three treatments. Although the ruminal molar percentage of isobutyrate in LD and PF was greater, no significant difference was observed in ruminal pH, NH3-N concentration and VFA production among the three treatments. Diet supplemented with fat can improve milk yield and milk fat percentage without resulting in disadvantages of ruminal characteristics in cows at early lactation and under warm climate.
