A Case of Complete Resection of a Solid Pseudopapillary Tumor with Hepatic Metastasis

  • Solid pseudopapillary tumor (SPT) of the pancreas is a rare neoplasm with low malignant potential, which has a good prognosis with more than 95% survival at 5 years. Only approximately 10% to 15% cases of SPTs are malignant. This report presents a case of a 38-year-old woman who developed malignant SPT of the pancreas with synchronous multiple hepatic metastases. She underwent a successfully complete surgical resection for multiple hepatic metastatic tumors in addition to primary tumor.

  • Tracheal neurilemoma, an extremely rare benign tracheal tumor that there has been only one case reported in 1996 throughout the nation, is a slowly progressing disease that obliterates the upper airway, delays diagnosis for its symptom similarity to asthma, and makes intubation for operation difficult. Bronchoscopic is therefore needed for diagnosis. There are two options for the treatment methods, a bronchoscopic resection or open surgical resection; however if intubation is difficult, then the bronchoscopic resection is used first to keep the airway open for the surgical resection. In this case, the severe tracheal stenosis impeding intubation made the surgical resection of the primary tracheal neurilemoma with extratracheal mass impossible; therefore, bronchoscopic laser resection was applied first to optain the airway passage for endotracheal intubation, followed by a successful open surgical resection.

Surgical Strategy for Primary Colorectal Carcinoma and Synchronous Pulmonary Metastasis Resection

  • Background: The surgical strategy for single-stage resection of primary colorectal cancer (CRC) and synchronous pulmonary metastases remains a matter of debate. Methods: Perioperative data of patients who underwent single-stage resection of primary CRC and synchronous pulmonary metastases were compared to those of patients who underwent 2-stage resections. The demographic data, number of metastases, type of pulmonary and colorectal resections, operation time, blood loss, postoperative complications, morbidities, mortality, medical costs, and length of hospital stay were analyzed. Results: Twenty-two patients underwent single-stage resection of primary CRC and pulmonary metastases, while 27 patients underwent 2-stage resection. Tumor size and the number of pulmonary metastases were not significantly different between the 2 groups. The extent of pulmonary metastasectomy and abdominal procedures were similar in both groups, as was the thoracic surgical approach (video-assisted thoracic surgery vs. thoracotomy). However, open laparotomy was performed more frequently in the 2-stage group than in the single-stage group (p=0.045), which also had a longer total anesthetic time (p=0.013). The operation time, medical costs, estimated blood loss, complication rates, and severity were similar in both groups, but the length of hospital stay was shorter in the single-stage group (p<0.001). Conclusion: Single-stage colorectal and pulmonary resection shortened the overall hospital stay, with no significant changes in operation time, medical costs, hospital mortality, and morbidity. Therefore, single-stage resection could be a good surgical strategy in selected patients.

흉골에 발생한 연골육종: 수술 치험 1례 (Chondrosarcoma of the Sternum - One Case Report -)

  • Primary neoplasms of the ribs and sternum are rare. Most primary bony chest wall neoplasms are malignant, and chondrosarcoma is the most common malignancy in this location The etiology of chondrosarcoma is unknown. Definitive diagnosis of chondrosarcoma can only be made pathologically. The natural history of chest wall chondrosarcoma is one of slow growth and local recurrence. Most tumors of the sternum require wide resection and reconstruction procedures, with potentially serious postoperative problems. Advances in chest wall reconstruction primarily through refinement in muscle transposition and clarification of the functional anatomy and blood supply of trunk muscles, has resulted in a more aggressive resection of the these tumors . Recently we experienced a case with chondrosarcoma of the sternum. A 56 year-old man was admitted to our hospital due to painless, slowly enlarging mass at the left sternoclavicular junctional area. The chest radiograph strongly suggested an underlying cartilaginous neoplasm owing to the appearance of typical flocculent and curvilinear calcifications within the lesion. On CT of the chest, the tumor exhibited a scalloped or lobulated contour, hypodensity of the nonmineralized component in comparison to adjacent muscle, and characteristic stippled cartilaginous matrix mineralization, also typical for cartilaginous neoplasm. The patient underwent wide resection of the chest wall tumor include with a 2-3cm margin of normal tissue on all sides and the thoracic skeletal defect was reconstructed with polytetrafluoroethylene [Gore-Tex] soft-tissue patch. Soft tissue reconstructive procedure was done with the pectoralis major muscle transposition. The patient had an uneventful postoperative course and discharged without adjuvant treatment such as radiation and chemotherapy.

Impact of Interatrial Septal Reconstruction on Atrial Tachyarrhythmia after Surgical Resection of Myxoma

  • Background: Complete surgical excision is the only curative treatment for primary cardiac tumors. For wide excision, interatrial septal reconstruction (ISR) is commonly performed. We hypothesized that ISR may increase the risk of postoperative atrial tachyarrhythmia (AT) after surgical resection of cardiac myxoma. Methods: After excluding patients with a history of cardiac surgery and concomitant procedures unrelated to tumor resection and those with AT or permanent pacemakers, we finally enrolled 272 adult patients who underwent benign cardiac tumor surgery from 1995 to 2021 at our institution. They were divided into the ISR (n=184) and non-ISR (n=88) groups. The primary outcome was postoperative new-onset AT. Results: The study cohort predominantly consisted of women (66.2%), with a mean age of 57.2±13.6 years. The incidence of postoperative new-onset AT was 15.4%. No 30-day mortality or recurrence was observed. The cardiopulmonary bypass time and aortic cross-clamping time were significantly longer in the ISR group than in the non-ISR group (p<0.001). The median duration of hospital stay of all patients was 6.0 days (interquartile range, 5.0-7.0 days), and no significant difference was observed between the 2 groups (p=0.329). ISR was not an independent predictor of new-onset AT (p=0.248). Male sex and hypertension were found to be independent predictors of new-onset AT. Conclusion: ISR was not a significant predictor of postoperative new-onset AT. ISR might be a feasible and safe procedure for surgical resection of cardiac myxoma and should be considered if needed.

Surgical Outcomes of Malignant Primary Cardiac Tumor: A 20-Year Study at a Single Center

  • Background: Malignant primary cardiac tumors are extremely rare, but have a poor prognosis. This study evaluated the surgical outcomes of patients with this disease. Methods: Forty patients who underwent surgery for malignant primary cardiac tumors between January 1998 and December 2018 were enrolled. Participants were divided into 3 groups based on resection margins (R0, 14 patients; R1, 11 patients; and R2, 11 patients) and their surgical outcomes were compared. Heart transplantation was performed in 4 patients with unresectable tumors. Results: Early mortality was reported in 2 cases (5%) due to postoperative bleeding and cerebral hemorrhage secondary to brain metastasis. The 1- and 2-year survival rates were 67.5% and 42.5%, respectively. The median survival time of the patients was 20.3 months (range, 9.2-37.6 months). The median survival time was 48.7, 20.3, and 4.8 months in patients with R0, R1, and R2 resections, respectively (p=0.023). Tumor recurrence occurred in 21 patients (61.7%), including 4 cases of local recurrence and 17 cases of distant metastasis. In patients who underwent heart transplantation, the median survival time was 29.5 months, with 3 cases of distant metastasis. Conclusion: Although surgery for malignant primary cardiac tumors has a poor prognosis, complete resection of the tumor may improve surgical outcomes.

흉선상피종의 치료 성적: 예후 인자 및 방사선치료 방법에 대한 연구 (Treatment Outcome of Thymic Epithelial Tumor: Prognostic Factors and Optimal Postoperative Radiation Therapy)

  • 목적 : 본 연구는 흉선상피종의 수술 및 수술 후 방사선치료의 치료 성적과 예후 인자로서 WHO세포형의 중요성에 대하여 알아보고, 방사선 조사 범위를 종양이 있던 부위로 국한하였을 때 재발 양상을 분석하여 조사 범위의 적절성을 평가하였다. 대상 및 방법 : 1994년 12월부터 2004년 6월까지 흉선상피종으로 진단 받고 수술을 시행 받은 160명을 대상으로 하였다. 수술 및 병리조직 소견 상 (1) 종양의 완전 절제가 의심되는 경우, (2) 병리 검사결과 절제연이 양성인 경우, (3) 조직병리가 WHO 세포형 B2이상인 경우, (4) Masaoka 병기 2기 이상인 경우에 수술 후 방사선치료를 추가하도록 권유하였으며, 실제 99명이 수술 후 방사선치료를 시행받았다. 방사선치료의 표적 용적은 종양 원발 부위에서 $1.5\~2$ cm 여유를 두고 결정하였으며, 매일 1.8 Gy또는 2 Gy씩, 주 5회 조사하는 통상분할조사법으로 목표선량은 54 Gy였다. 결과 : 전체 환자의 5년 생존율은 $87.3\%$였다. 단변량 분석결과 5년 생존율에 유의한 영향을 미치는 인자는 연령(60세 이상 $77.8\%$, 60세 미만 $91.3\%$: p=0.03), Masaoka 병기(1기 $92.2\%$, 2기 $95.4\%$, 3기 $82.1\%$, 4기 $67.5\%$: p=0.001), WHO세포형(A-B1 $96.0\%$, B2-C $82.4\%$: p=0.001), 절제 정도(완전절제 $92.3\%$, 부분절제 및 조직검사 $72.3\%$: p=0.001)였다. 다변량 분석 결과는 WHO 세포형만이 유의한 차이(p=0.03)를 보였다. 육안적 완전 절제(R0-1 절제) 후 보조적 방사선 치료를 종양 원발 부위에만 시행 받았던 Masaoka 병기 1-3기 환자 71명 중 총 5명에서 재발이 확인되었고, 재발 부위는 늑막강 파종이 2명, 심장막강 파종 및 폐 전이 1명, 폐 전이 1명이었고, 단 1명만이 종격동 림프절 재발이었다. 모든 환자에서 방사선 조사범위 안에서 재발은 없었다. 결론 : WHO 세포형은 Masaoka 병기, 완전 절제 여부, 연령과 함께 흉선상피종 환자의 생존율에 영향을 미치는 중요한 예후인자로 확인되었다. 또한 수술 후 보조적 방사선치료는 조사범위를 종양 원발 부위에 국한하여 시행하는 것이 재발 양상 및 부작용을 고려할 때 안전하고 유효한 방법으로 판단되며, 늑막강 및 심장막강 파종에 의한 재발을 막기 위해서는 효과적인 보조적 항암화학요법에 대한 연구가 진행되어야 할 것이다.

흉골과 늑골의 원발성 종양 3예 (Primary Malignant Tumors of Ribs and Sternum -Report of 3 Cases-)

  • This is a report of 3 cases of tumors, which primarily originated from ribs and the sternum. In the first case of multiple myeloma, the patient was 67 year old male with a tumor located on the middle sternum invading the manubrium and the body of the sternum manifesting symptoms after a contusion of the anterior sternum. The sternum was entirely resected and was replaced by tantalum plate to reconstruct the defective chest wall in order to prevent the paradoxical movement during respiration. In the second case of osteogenic sarcoma, the patient was 43 year old male with a tumor located on the costochondral junction of the left 5th rib for 6 months. The left 5th rib was resected between the middle part and sternochondral junction of it including tumor and adjacent soft tissues. In the third case of chondrosarcoma, the patient was 36 year old male with a tumor located near the posterior angles of the right 7th and 8th ribs manifesting back pain on the area where the tumor was located. Resection of right lower lobe was performed since direct invasion of tumor was seen in the superior segment of right lower lobe. This was followed by the resection of both 7th and 8th ribs at the area between the costovertebral junction and the portion 10 em apart from the tumor including the tumor and intercostal soft tissues. Diagnoses of 3 cases of tumors described above were confirmed by histopathologic examination postoperatively. The postoperative courses were uneventful.

Metachronous Ovarian Metastases Following Resection of the Primary Gastric Cancer

  • Purpose: We performed this study to evaluate the clinical presentation as well as the proper surgical intervention for ovarian metastasis from gastric cancers and these tumors were identified during postoperative follow-up. This will help establish the optimal strategy for improving the survival of patients with this entity. Materials and Methods: 22 patients (3.2%) with ovarian metastasis were noted when performing a retrospective chart review of (693) females patients who had undergone a resection for gastric cancer between 1981 and 2008. The covariates used for the survival analysis were the patient age at the time of ovarian relapse, the size of the tumor, the initial TNM stage of the gastric cancer, the interval to metastasis and the presence of gross residual disease after treatment for Krukenberg tumor. The cumulative survival curves for the patient groups were calculated with the Kaplan-Meier method and they were compared by means of the Log-Rank test. Results: The average age of the patients was 48.6 years (range: 24 to 78 years) and the average survival time of the 22 patients was 18.8 months (the estimated 3-year survival rate was 15.8%) with a range of 2 to 59 months after the diagnosis of Krukenberg tumor. The survival rate for patients without gross residual disease was longer than that of the patients with gross residual disease (P=0.0003). In contrast, patient age, the size of ovarian tumor, the initial stage of gastric adenocarcinoma, the interval to metastasis and adjuvant chemotherapy were not prognostic indicators for survival after the development of ovarian metastasis. Conclusions: Early diagnosis and complete resection are the only possible hope to improve survival. As the 3-year survival rate after resection of Krukenberg tumor is 15.8%, it seems worthwhile to consider performing tumorectomy as the second cytoreduction.

뇌교에 발생한 악성 핍지신경교종 1례 (A Case of Brain Stem Anaplastic Oligodendroglioma with Exophytic Growth)

  • Primary anaplastic oligodendroglioma in brain stem is extremely rare. The authors present a case of a anaplastic oligodendroglioma arising from pons. A 29 year-old male patient was admitted because of cranial nerve palsy and visual disturbance. Neurological examination revealed bilateral sixth and left seventh cranial nerve plasies. Near-total resection of tumor mass was performed through midline suboccipital appraoch. Tumor was not related with choroid plexus and major vessels but it was firmly attached to the fourth ventricle floor. Tumor was considered to be arised from the tegmental portion of pons, growing dorsally into the 4th ventricle. Hitopathological exmination revealed primary anaplastic oligodendroglioma. Postoperative course was uneventful. The authors believe that this type of tumor with dorsally growing pattern can be successfully resected without major neurological deficit.
