예방의학의 발전방향: 보건의료관리 분야 (Vision and Training Strategy for Health Management Specialist)

  • The identity crisis of preventive medicine appears to have been deepening. As a solution, it is insisted that preventive medicine should focus on clinical preventive medicine. However, in the field of heath policy and management, the better solution should be found in a serious search for visions and perspectives of its study on population and society. In this regard, the specialist who studies the field can be defined as a medical doctor majoring in public health. In this paper, I first forecasted major socioeconomic changes to occur in medical and public health arena and explored the role of those studying health policy and management. Secondly, I summarized their career paths and main activities in order to establish visions. Finally, I proposed curriculums on health policy and management for medical school undergraduates and for specialists majoring in preventive medicine, respectively.

  • Objectives: The purpose of this manuscript was to propose the policy and perspectives of prevention and management for hypertension and diabetes in Korea. Methods: Authors reviewed the chronic disease prevention and management projects and models were executed in Korea until now, and analyzed and evaluated their performances. Results: In the circumstances of Korea, the following several requisites should be improved ; Specific Korean strategy for development and pursuing of national level policy agenda for chronic disease management must be established. There are a need to establish several means of supplementing the weaknesses of the current chronic disease management policies and programs. Firstly, development and distribution of contents of guidelines on the systematic project execution regime (regarding systematization of local community, subjects and contents of the projects) with guarantee for the quality of chronic disease prevention and management are necessary. Secondly, there is a need for development of information system that can lead the chronic disease management programs currently being implemented. Thirdly, there is urgent need to develop resources such as cultivation of manpower and facilities for provision of education and consultation for the patients and holders of risk factors of chronic disease. Fourthly, there is a need for means of securing management system and financial resources for operation of policies and programs. Conclusions: The results can be able to use as a road map, models, and direction and strategies of policies for chronic disease prevention and management of Korea.

Control system modeling of stock management for civil infrastructure

  • Management of infrastructure stock is essential in sustainability of society, and its analysis and optimization are studied in the light of control system modeling in this paper. At the first part of the paper, cost of stock management is analyzed based on macroscopic statistics on infrastructure stock and economical growth. Stock management burden relative to economy is observed to become larger at low economic growth periods in developed economies. Then, control system modeling of stock management is introduced and by augmenting maintenance actions as control input, dynamic behavior of stock is simulated and compared with existing time history statistics. Assuming steady state conditions, applicability of the model to cross sectional data is also demonstrated. The proposed model is enhanced so that both preventive and corrective maintenance can be included as system inputs, i.e., feedforward and feedback control inputs. Optimal management strategy to achieve specified deteriorated stock level with minimal cost, expressed in terms of preventive and corrective maintenance actions, is derived based on estimated parameter values for corrosion of steel bridges. Relative cost effectiveness of preventive maintenance is shown when target deteriorated stock level is lower.

중소기업 종사자들의 안전분위기 인지도와 안전참여의 관계 및 안전동기의 매개효과 (The relationship between perceived safety climate and safety participation, and the mediating effect of safety motivation in small business employee)

  • This paper reviewed the relationship between safety climate(management commitment, safety education, preventive activity) and safety participation, and the mediating effect of safety motivation(intrinsic and extrinsic motivation) in small business employee. Based on the responses from 270 employees, the results of multiple regression analysis appeared as follow; 1) management commitment, safety education, and preventive activity effect positively on intrinsic/extrinsic safety motivation. 2) management commitment, safety education, and preventive activity effect positively on safety participation. 3) intrinsic and extrinsic safety motivation effect positively on safety participation. 4) intrinsic safety motivation mediates between 3 climate factors(management commitment, safety education, preventive activity) and safety participation. 5) extrinsic safety motivation mediates between only management commitment and safety participation.

예방의학 영역에서 심신의학적 접근의 적용 가능성: 스트레스 관리를 위한 이완과 명상 기법을 중심으로 (Mind-Body Approach in the Area of Preventive Medicine: Focusing on Relaxation and Meditation for Stress Management)

  • Objectives: Emotional support and a stress management program should be simultaneously provided to clients as effective preventive services for healthy behavioral change. This study was conducted to review various relaxation and meditation intervention methods and their applicability for a preventive service program. Methods and results: The author of this paper tried to find various relaxation and meditation programs through a literature review and program searching and to introduce them. The 'Relaxation Response' and 'Mindfulness Based Stress Reduction (MBSR)' are the most the widely used meditative programs in mainstream medical systems. Abdominal breathing, Progressive Musclular Relaxation (PMR), Relaxative Imagery, Autogenic Training (AT) and Biofeedback are other well-known techniques for relaxation and stress management. I have developed and implemented some programs using these methods. Relaxation and meditation classes for cancer patients and a meditation based stress coping workshop are examples of this program. Conclusions: Relaxation and meditation seem to be good and effective methods for primary, secondary and tertiary preventive service programs. Program development and standardization and further study are needed for more and wider use of the mindbody approach in the preventive service area of medicine.

A Health Management Service with Beacon-Based Identification for Preventive Elderly Care

  • Bluetooth low energy (BLE) beacon is an actively push-to-broadcast electronic signal and can be used for object identification. This paper uses such beacon-based identification and Internet of Things (IoT) technologies for the elder health management service system to simplify the user interfaces and steps for preventive elder care. In the proposed system, an elder's family member, caregiver, or medical worker can conveniently and quickly record daily health management information. Besides, through the statistics and analysis of the data on the back end of the system, it is helpful for the elderly to refer to the data of daily care management and future management trends. Similarly, it is also an essential reference data for system maintenance and the new preventive health care services development.

최소 신뢰도를 보장하는 비 주기적 예방보전 모형 개발 (Developing a Non-Periodic Preventive Maintenance Model Guaranteeing the Minimum Reliability)

  • Purpose: This paper proposes the non-periodic preventive maintenance policy based on the level of cumulative hazard intensity. We aim to construct a cost-effectiveness on the proposed model with relaxing the constraint on reliability. Methods: We use the level of cumulative hazard intensity as a condition variable, instead of reliability. Such a level of cumulative hazard intensity can derive the reliability which decreases as the frequency of preventive maintenance action increases. We also model the imperfect preventive maintenance action using the proportional age setback model. Conclusion: We provide a numerical example to illustrate the proposed model. We also analyze how the parameters of our model affect the optimal preventive maintenance policy. The results show that as long as high reliability is guaranteed, the inefficient preventive maintenance action is performed reducing the system operation time. Moreover, the optimal value of the proposed model is sensitive to changes in preventive maintenance cost and replacement cost.

한국기업의 설비관리정보시스템(CMMS) 구현실태 분석과 고도화에 관한 연구 (A Study on Analysis of the Implementation Status and the Advancement of Computerized Maintenance Management System (CMMS) in Korean Companies)

  • Purpose: This study is to develop an evaluation model for analysis of CMMS Implementation status and provide CMMS advancement methods to maximize implementation effect through the evaluation model. Methods: After extracting common modules from CMMS packages and establishing evaluation standard for each module, then the evaluation standard is applied to 33 Korean companies for evaluating their own current implementation status. Results: Preventive maintenance and analysis information modules were considered the most vulnerable in Korean companies which have introduced CMMS packages. And the reason why preventive maintenance is vulnerable is that there is poor build-up of their own preventive maintenance standards. Conclusion: Korean companies which will introduce CMMS need to make preventive maintenance standards, and data of the materials and the equipment to improve the effectiveness in advance.

예방의학의 발전 방향 (Preventive Medicine in Times of a Rapid Epidemiologic Transition in Korea)

  • Ever since the foundation of the Korean Society for Preventive Medicine in 1947, members of the Society had made remarkable contributions to the public health development and national health promotion. They had played key roles in establishing national health system, improving environmental hygiene, controlling infectious and chronic diseases, promoting family planning, improving industrial and environmental health, and developing health service management. However, the Society had less actively responded to the changes in health service needs of the population that were caused by a rapid epidemiologic transition in last a few decades. Early detection and treatment of chronic diseases including cancer and cardiovascular diseases and risk reduction by the life style modification are major approaches to the contemporary national health problems and they are the core contents of preventive medicine. The author proposed to develop the clinical preventive medicine specialist who will have additional training in clinical medicine for health screening and life style modification to the current preventive medicine training program and thus will be able to provide comprehensive preventive medical services. Another area that the Society may take the initiative is training preventive medicine specialist in the disaster, including bioterrorism, preparedness and management. The Society should be more active in proposing health policy and health service program and also participate collectively in a large scale health research project of the government. These approaches may not only contribute more effectively to the national health promotion but also improve the identity of the Society.
