재가노인의 건강문제와 보건ㆍ복지 서비스 지원 실태에 관한 연구 (Health Problems and Support from the Health & Welfare Service in the Elderly at Home)

  • Purpose: This study was conducted to identify health problems and support received from the health and welfare service using MDS-HC(Minimun Data Set for Home Care) in the aged living at home. Method: Eighty-one elderly persons were selected from those listed in community-welfare service centers in Seoul and Kyunggi Province between December 2002 and January 2003. Result: Eleven health problems per elderly person on average were identified, and the frequent care needs were in order: preventive health measure, health promotion, visual function, depression & anxiety, communication disorders, social function, pain, environmental assessment, oral health, cognition and falls. The number of health problems by the level of ADL was ‘ADL 1(Independence)’ 9.87, ‘ADL 2(Partial independence)’ 12.78, ‘ADL 3(Dependence)’ 13.73. Utilization of formal health & welfare services among the elderly was ‘meals on wheels’ 40.7%, ‘home helper’ 38.2%, ‘visiting of social welfare worker’ 21.0%, ‘physical therapy’ 19.6%, ‘day care center’ 12.3%, ‘volunteer's service’ 9.9%, ‘home visiting care’ 3.7%, ‘occupational therapy’ 3.7%, and ‘speech therapy’ 2.5%. Conclusion: The results suggest that using the MDS-HC 2.0 is applicable to help decide criteria for both health and welfare service supplied to the elderly.

건강보험청구 치면열구전색 치아수의 지역격차와 지역수준 관련요인 (Factors affecting regional disparities in the number of teeth sealed with pit and fissure sealants: information for the National Health Insurance)

  • Objectives: The purpose of this study was to interpret regional disparities in the number of teeth sealed with pit and fissure sealants, identify the factors that affect these disparities and find solutions for the same. Methods: Data were collected from the National Health Insurance Service and Korean statistical information service using metropolis-city-rural area dental health infrastructure variables, regional health behavior variables, and local finance-related variables. Results: In 2015, the number of teeth sealed with pit and fissure sealants per 100 people was higher in the metropolis or city than in the rural area. There was a positive correlation between the number of teeth sealed with pit and fissure sealants and the number of dentists, dental hygienists, dental institutions, standardization rate of subjective awareness of well-being, standardization rate of brushing after lunch, and the proportion of welfare budget in the general budget. There was a negative correlation with the annual standardization rate of health institution use, the standardization rate of unused medical services, and the local government's financial independence. According to the final model of the multiple regression analysis, while the impact of infrastructure on dentistry was not statistically significant, the statistical significance of standardization rate of brushing after lunch, the local government's financial independence, and the proportion of welfare budget in the general budget were maintained. Conclusions: To reduce regional disparities in the volume of use of pit and fissure sealants, it was concluded that it may be effective to select regions with a consideration of the level of regional economic power, implement separate and appropriate policies and projects, and improve the awareness in residents.

서비스업 종사자의 코로나 19에 대한 두려움과 감염위험인식이 우울에 미치는 영향 (The Impact of Fear and Perception of Infection Risk on Depression among Service Workers during Covid-19)

  • Purpose: This study aimed to examine the impact of fear and perception of infection risk on depression among service workers during prolonged Covid-19 service, and to provide basic data on the impact of COVID-19 prevention on the psychological health of service workers. Methods: Data were collected from workers nationwide from May 24 to 31, 2021. The data were analyzed using the t-test, ANOVA, Scheffé test, Pearson's correlation coefficient, and multiple regression analysis, using SPSS 28. Results: It was found that 44.3% of the participants experienced depression (PHQ-9 total scores ≥ 10) during the Covid-19 pandemic. The general characteristics of service workers that made a difference in the level of depression were health status, smoking, and perception of infection risk. Conclusion: Social support from institutions is needed to treat depression caused by Covid-19 among workers in the service sector.

범부처 공공보건의료인력 관리조직의 필요성과 과제 (Creation of a central public healthcare human resources management department to coordinate existing agencies)

  • Purpose : The aim of this study is to review the current status of healthcare provision and its human resources administrative management and propose a coordinated human resource management plan for the more efficient operations of healthcare organizations. Methods : We reviewed the literature and held discussions with officials from the United States Department of Health and Human Services to survey United States Public Health Service Commissioned Corps operations. In addition, we surveyed the literature to analyze the current structure and responsibilities of governing bodies involved in public healthcare in Korea. Results : In Korea, there are several administrative offices involved in public health: the Ministry of Health and Welfare, the Ministry of Defense, the Environment Ministry and others. Since these diverse agencies don't integrate their operations, it is difficult to grasp their management of both public healthcare services and their personnel. A potential model is the United States Public Health Service Commissioned Corps, a sub-group of the Department of Health and Human Services and an elite team of highly qualified, public health professionals, which coordinates and manages the overall work and personnel of diverse healthcare organizations. Conclusion : We suggest the establishment of a federal level, public health administrative department of human resource management to centralize and coordinate the existing, disparate healthcare administrative agencies.

Adjusting for Confounders in Outcome Studies Using the Korea National Health Insurance Claim Database: A Review of Methods and Applications

  • Objectives: Adjusting for potential confounders is crucial for producing valuable evidence in outcome studies. Although numerous studies have been published using the Korea National Health Insurance Claim Database, no study has critically reviewed the methods used to adjust for confounders. This study aimed to review these studies and suggest methods and applications to adjust for confounders. Methods: We conducted a literature search of electronic databases, including PubMed and Embase, from January 1, 2021 to December 31, 2022. In total, 278 studies were retrieved. Eligibility criteria were published in English and outcome studies. A literature search and article screening were independently performed by 2 authors and finally, 173 of 278 studies were included. Results: Thirty-nine studies used matching at the study design stage, and 171 adjusted for confounders using regression analysis or propensity scores at the analysis stage. Of these, 125 conducted regression analyses based on the study questions. Propensity score matching was the most common method involving propensity scores. A total of 171 studies included age and/or sex as confounders. Comorbidities and healthcare utilization, including medications and procedures, were used as confounders in 146 and 82 studies, respectively. Conclusions: This is the first review to address the methods and applications used to adjust for confounders in recently published studies. Our results indicate that all studies adjusted for confounders with appropriate study designs and statistical methodologies; however, a thorough understanding and careful application of confounding variables are required to avoid erroneous results.

Hebei Spirit Oil Spill Exposure and Subjective Symptoms in Residents Participating in Clean-Up Activities

  • Objectives: This study was conducted to examine the relationship between crude oil exposure and physical symptoms among residents participating in clean-up work associated with the Hebei Spirit oil spill, 2007 in Korea. Methods: A total of 288 residents responded to a questionnaire regarding subjective physical symptoms, sociodemographic characteristics and clean-up activities that occurred between two and eight weeks after the accident. Additionally, the urine of 154 of the respondents was analyzed for metabolites of volatile organic compounds (VOCs) and polycyclic aromatic hydrocarbons (PAHs) and heavy metals. To compare the urinary levels of exposure biomarkers, the urine of 39 inland residents who were not directly exposed to the oil spill were analyzed. Results: Residents exposed to oil remnants through clean-up work showed associations between physical symptoms and the exposure levels defined in various ways, including days of work, degree of skin contamination, and levels of some urinary exposure biomarkers of VOCs, metabolites and metals, although no major abnormalities in urinary exposure biomarkers were observed. Conclusions: This study provides evidence of a relationship between crude oil exposure and acute human health effects and suggests the need for follow-up to evaluate the exposure status and long-term health effects of clean-up participants.

대전광역시 지역사회 통합돌봄 체계내에서 공공보건의료 연계 모델 개발 (Development of a public health care linkage model within the community care system in Daejeon City)

  • Objectives: This study aimed to establish a linkage model involving regional responsible medical institutions after analyzing the existing conditions and deriving problems through qualitative analysis within the community care system. Methods: A total of 14 participants of this study were selected through the snowball sampling method, including 7 community care service providers and 7 service users. As for the research data, primary data were collected through interviews, and as a result of analyzing according to Aday&Anderson' model, a total of 5 catergories, 8 topics, and 22 sub theme were derived. Results: The problem derived from the interview is that division services are provided for each institution due to the absence of a key central institution of community care system, and users' commercial institutions is unclear. The second is the inconsistency between the needs and supply for community care, resulting in a possibility of delay in returning to the community after discharge. Based on these problems, it is necessary to unify it as an community care window of the Dong-community center. In addition, there is a need for public health centers to play an active role, and to establish a public-private joint system with the Health and Living Support Center to establish a model that can play a certain role. Conclusions: Therefore, based on the results of this study, it can be used as basic data when constructing community care model and applying it as an expanded model in the future.

마을보건 요원의 활동력 평가 (Evaluation of Village Health Worker's Activities)

  • In order to develop some indicies for the evaluation of village health worker's activities and to find out personal characteristics and other factors that affect the activities, an interview survey was conducted to thirty seven village health workers (VHW) in Sunwon, Naega and Bulun townships in Kangwha county, where the Community Health Project of Yonsei University, College of Medicine, has been implemented for the past ten years. In addition, daily activity records of the VHW's were also analyzed. The results are summarized below: First, meeting attendance rates, number of regular family visits, number of antenatal care visits and number of family planning visits were identified as the most meaningful criterion for the evaluation of the village health worker's activities. Second, personal factors that significantly affect the village health worker's activities were identified as age, educational background, living with in-laws, religion, presence of preschool child in the family, holding leader's position of village woman's association concurrently, and duration serving as a VHW. The more aged and the more educated VHWs were the more effective. Those VHWs who were living with in-laws, without preschool child in the family, holding the leader's position of the village woman's association, and the longer service duration were the more effective. Other factors that affect the VHW's activities were the number of households in the target village and the number of natural villages in the target villages. It showed that the smaller the size of number of households and natural villages, the higher the degrees of the effectiveness of the VHW.

치위생과 학생의 관찰-수행실습 빈도와 만족도 (Satisfaction and the frequency of observation and clinical practice in dental hygiene students)

  • The frequency of clinical observation and clinical practice and satisfaction among dental hygiene students were investigated during the clinical practice process. The results were analyzed to come up with a more effective clinical practice program and provide basic data that would aid in actual operation of clinical practice. The tool used in this study was a survey composed of 11 areas with a total of 55 questions. It was distributed to 471 graduating dental hygiene students at 5 different universities. The Cronbach's Alpha coefficient was .975. The data were analyzed using SPSS 10.0 for Windows. Significance was determined at ${\alpha}$=0.05. The results were as follows. 1. The results of analyzing the frequency of clinical observation according to the different clinical areas showed that the score was the highest in the area of basic diagnosis, followed by prosthodontics, operative dentistry, hospital service management, oral surgery, oral radiology, periodontics, preventive dentistry, orthodontics, pediatric dentistry, and oral medicine. 2. The results of analyzing the frequency of clinical performance according to different clinical areas showed that the score was the highest in the area of basic diagnosis, followed by operative dentistry, oral surgery, prosthodontics, oral radiology, pediatric dentistry, orthodontics, oral medicine, preventive dentistry, endodontics and hospital service management 3. The area that showed most significance difference between the frequency of clinical observation and the frequency of clinical performance was the area of hospital service management, followed by prosthodontics, periodontics, preventive dentistry, oral radiology, oral medicine/oral surgery, orthodontics, operative dentistry, and pediatric dentistry. 4. When satisfaction in dental hygiene students was analysed according to different clinical areas, they were most satisfied practicing in basic diagnosis, followed by operative dentistry, prosthodontics, oral surgery, oral radiology, pediatric dentistry, orthodontics, preventive dentistry, hospital service management, periodontics, and oral medicine.
