Perspectives of Preventive Medicine: Focused on Epidemiology (예방의학의 발전방향: 역학분야)

  • Epidemiology is the key element of public health and preventive medicine. Reversely, public health and social equity are the basic ground for epidemiologists. Current progress in the various fields of epidemiologic study in Korea calls for the increased participation of the trained epidemiologists. Expanding epidemiologic concepts to the wide spectrum of health and medical programs, active participation to the diversified health service fields and strengthening the role of epidemiology in the social and political decision making should be included in the perspectives of epidemiology in Korea. The future of epidemiology is certainly depend on the efforts of present epidemologists.

Diversity and its Characteristics of Life Phenomenon (생명현상의 다양성과 특징에 관한 고찰 - 보건의료에 미치는 영향을 중심으로 -)

  • This research was intended to delve into the diversity of life phenomenon and its characteristics. First of all, this research gave real examples to compare the differences in men's health, disease, and longevity in order to confirm the existence of diversity of life phenomenon. In addition, it also studied the process and mechanism of manifestation of life phenomenon, as well as the influence and problems of existing studies' results and implications. The results are as follow. 1. Differences in health, diseases, and longevity were very big and diverse in researches on different races, nations, ages, socioeconomic status, positions, and even (monozygotic) twins. 2. The basic foundation of all organisms is DNA, and environmental factors change DNA methylation and the structure of chromatin by constantly influencing DNA. Due to this, the manifestation, control, and phenotype of DNA change, resulting in diversified life phenomenon. Therefore, it is the environmental factors, not DNA, that has more influence on the diversity of life. 3. Looking at available studies, the most reasonable perspective on human requires focusing on the diversity of life phenomenon, holistic thinking, and reversible change instead of irreversible determinism. Considerable differences in life phenomenon between entities require a change in malformed perspective on life. Public health and medicine deals with live human beings, a more precise and accurate perspective on life is very important. Because management methods of health and disease, such as structure and approaches of medical research, prevention and cure, must be different by life perspectives.

The Trend of Published Articles to the Korean Journal of Oriental Preventive Medicine - From 1997 to 2010 - (대한예방한의학회지 게재논문의 경향성에 대한 연구 - 창간호(1997년)로부터 2010년까지 -)

  • Objective : The purpose of this study was to identify the trend of research in the Korean Journal of Oriental Preventive Medicine and to suggest future perspective for oriental preventive medicine research. Method : The contents of 344 articles published in this journal was reviewed from its beginning year 1997 to year 2010. Result : The number of articles was increased as times go on. An analysis of the research design showed, experimental research (in vivo or in vitro) was 36.9%, survey research was 26.5%, review was 20.1%. In the major classifications of topics published, health management 28.5%, oriental medicine effectiveness 25.3%, herbal safety and toxicity 13.1%, and environmental and occupational medicine 9.0% respectively. Conclusion : There has been a lack of health preservation(Yang-saeng) topic, epidemiology and health statistics topic. Further research need qualitative study and each subjects of oriental preventive medicine.

The relationship between empathic ability, communication competence, and attitudes toward the disabled among female dental hygiene students (일부 치위생과 여학생들의 공감능력 및 의사소통능력과 장애인에 대한 태도와의 연관성)

  • Background: To explore the empathic abilities, communication competence, and attitudes toward the disabled, and to identify the relationship between these variables among female dental hygiene students. Methods: A self-administered questionnaire was used to collect data from 190 female students in their second and third year of study at the dental hygiene department of a university located in Gyeongsangnam-do province, South Korea, from September 2-11, 2020. Data were analyzed using descriptive analyses, t-test, analysis of variance, Pearson's correlation, and multiple regression using SPSS 20.0. Results: The participants' scores for empathic abilities, communication competence, and attitudes toward the disabled were 3.63, 3.69, and 3.54 points, respectively. A significant positive correlation was observed between empathic abilities and attitudes toward the disabled. Regarding the sub-elements of empathic abilities, the participants' attitudes toward the disabled had a significant positive correlation with the scales measuring their perspective-taking abilities and personal distress (p<.05). A positive correlation (p<.05) was observed between communication competence and attitudes toward the disabled. The students' attitudes toward the disabled were significantly influenced by the scale measuring their perspective-taking abilities (B=.210); those with a higher perspective-taking ability displayed more positive attitudes toward the disabled. Conclusion: Female dental hygiene students showed empathic abilities, which positively influenced their attitudes toward the disabled. Positive attitudes toward the disabled can contribute to improving the quality of medical services.

Preventive Maintenance Policy Following the Expiration of Extended Warranty Under Replacement-Repair Warranty (교체-수리보증 하에서 연장된 보증이 종료된 이후의 예방보전정책)

  • In this paper, we consider the periodic preventive maintenance model for a repairable system following the expiration of extended warranty under replacement-repair warranty. Under the replacement-repair warranty, the failed system is replaced or minimally repaired by the manufacturer at no cost to the user. Also, under extended warranty, the failed system is minimally repaired by the manufacturer at no cost to the user during the original extended warranty period. As a criterion of the optimality, we utilize the expected cost rate per unit time during the life cycle from the user's perspective. And then we determine the optimal preventive maintenance period and the optimal preventive maintenance number by minimizing the expected cost rate per unit time. Finally, the optimal periodic preventive maintenance policy is given for Weibull distribution case.

  • Social epidemiology is a sub-discipline of epidemiology explicitly investigating social determinants of population distributions of health, disease, and well-being. Persistent pattern of social inequalities in health in spite of the broad improvement in the physical environment over the last centuries necessitated the development of this field as an approach to understand disease etiology that incorporates social experiences as more direct determinant of health. Social epidemiology incorporates theories, measurement tools, and techniques from a wide variety of other social sciences. A population perspective, the social context of behavior, contextual multilevel analysis, a developmental and life-course perspective, and general susceptibility to disease are the most important guiding concepts in social epidemiology.

Characteristics of the COVID-19 Outbreak in Korea From the Mass Infection Perspective

  • On December 31, 2019, the Chinese government officially announced that the country had some cases of pneumonia with an unknown cause. By February 8, 2020, there were 24 confirmed cases in Korea, and the number of cases has steadily increased since then. On March 9, 2020, the cumulative number of confirmed cases in Korea was 7382, with 51 deaths. This study examines the characteristics of the coronavirus disease 2019 (COVID-19) outbreak from the perspective of the large-scale number of confirmed COVID-19 cases and deaths. This study is significant in that it emphasizes the precautionary principle in preventing and managing infectious diseases, and makes suggestions for urgently needed public health policies.

Cost-benefit Analysis of Mandatory Prescription in Korea (의약분업의 비용-편익 분석)

  • Objective : To evaluate the relative benefits and the costs associated with the introduction of the new pharmaceutical provision called 'Mandatory Prescription System' which separates the role of physicians from that of pharmacists with respect to the prescription and dispensation of from the perspective of consumers (i.e., patients). Methods : The costs of the system were measured by considering both direct and indirect costs. Direct costs included additional payments for ambulatory care and dispensing fees that occurred under the new system. indirect costs consisted of transportation expenses and costs related to time spent for physician consultation, waiting for the prescriptions to be filled, and extra traveling. Benefits identified in this study were the reduction of drug misuse and overuse, and the overall decrease in drug consumption among the Korean population. Sensitivity analysis was peformed for the inclusion of benefits for outpatients of hospitals, price elasticity, and increased fees for established patients. Results : The net benefit was estimated to be about minus 1,862 billion won and the benefit-cost ratio was 0.478. This indicates that the costs of 'Mandatory Prescription' outweigh its benefits, relative to the previous system. The sensitivity analysis results for all the variables considered in this study consistently showed the benefit-cost ratio to be less than 1. Conclusion : The results of this study suggest that implementing Mandatory Prescription System in Korea might be inefficient from the consumer's perspective. The results of this study do not coincide with the results of previous studies, presumably because of the differences in study design and in which items of costs and benefits were considered.

Analysis of Child protection system from a preventive Perspective : Focusing on the German case (예방적 관점에서 살펴 본 아동학대 대응체계 분석 - 독일 사례를 중심으로 -)

  • This study analyzed the child protection system in Germany from a preventive perspective and attempts to find the applicable implications to Korea. The research method was analyzed in terms of legal, policy and professionalism. The result is as follows. First, Child and Youth Support Act in Germany stipulated a preventive support system to restore the function of the family. Second, according to the Civil Act, it was stipulated that the family court could intervene early. Third, the federal Child Protection Act stipulated community cooperation for thd child protection system. Fourth, the Youth Agency as the general authority made it possible to provide preventive support and intervention at the same time. Firth, qualification standards were specified in the Child and Youth Support Act. Child protection specialists are granted to public officials who have worked for more than three years. The implications are as follows. First, the child protection system, which operates as a child abuse reporting system, should be expanded to a preventive support system. Second, it is necessary to expand monitoring by establishing an early warning system between networks in order to establish a support system for potentially at-risk children. Third, local governments should support children and parents flexibly and comprehensively for dysfunction caused by difficulties at home. Fourth, it is necessary to enact the Child Protection System Cooperation Act for a network cooperation system.
