Risk of Recrudescence of Lymphatic Filariasis after Post-MDA Surveillance in Brugia malayi Endemic Belitung District, Indonesia

  • Belitung district in Bangka-Belitung Province, Indonesia with a population of 0.27 million is endemic for Brugia malayi and 5 rounds of mass drug administration (MDA) were completed by 2010. Based on the results of 3 transmission assessment surveys (TAS), the district is declared as achieving elimination of lymphatic filariasis (LF) in 2017. The findings of an independent survey conducted by the National Institute of Health Research and Development (NIHRD) in the same year showed microfilaria (Mf) prevalence of 1.3% in this district. In 2019, NIHRD conducted microfilaria survey in 2 villages in Belitung district. Screening of 311 and 360 individuals in Lasar and Suak Gual villages showed Mf prevalence of 5.1% and 2.2% with mean Mf density of 120 and 354 mf/ml in the respective villages. Mf prevalence was significantly higher among farmers and fishermen compared to others and the gender specific difference was not significant. The results of a questionnaire based interview showed that 62.4% of the respondents reported to have participated in MDA in Lasar while it was 57.7% in Suak Gual village. About 42% of the Mf positive cases did not participate in MDA. Environmental surveys identified many swampy areas supporting the breeding of Mansonia vector species. Persistence of infection is evident and in the event of successful TAS3 it is necessary to monitor the situation and plan for focal MDA. Appropriate surveillance strategies including xenomonitoring in post-MDA situations need to be developed to prevent resurgence of infection. Possible role of animal reservoirs is discussed.

Initiating Smokeless Tobacco Use across Reproductive Stages

  • Background: The use of smokeless tobacco (SLT) among women is increasing in India, especially among those with limited education and resources. Preventing the initiation of SLT among women is critical since it has known negative consequences for oral and reproductive health. Most research on tobacco initiation in India focuses on adolescents. This paper addresses the unrecognized issues of post marital initiation among women of reproductive age, highlighting the importance of reproductive stages in women's tobacco initiation. The objective is to examine the correlates of SLT initiation among low income women in Mumbai from pre-marriage through early marriage, first pregnancy and beyond, using case examples to illustrate initiation during each of these stages. Materials and Methods: In 2011-2012, cross-sectional community level survey data were collected from a representative sample of 409 daily SLT-using married women aged 18-40 years in a low income community in Mumbai. Information on socio-demographics, initiation by reproductive stage, types of tobacco use, childhood exposure to tobacco, learning to use, and initiation influences and reasons were collected through a researcher-administered survey. Univariate and bivariate analysis assessed factors influencing initiation of SLT use by reproductive stage. In addition 42 narratives of tobacco use were collected from a purposive sample of pregnant and non-pregnant married women addressing the same questions in detail. Narratives were transcribed, translated, and coded for key concepts including initiation of tobacco use. Results: Thirty-two percent of women initiated SLT use before marriage, 44% initiated after marriage but before pregnancy, 18.1% initiated during their first pregnancy and the remainder started after their first pregnancy. Mean age of marriage among women in this study was 16 years. Younger women (i.e. age at time of the interview of less than 30 years) were 0.47 [95% CI (0.32, 0.87)] percent less likely to initiate after marriage than women aged more than 30 years. Women who got married before 18 years of age were 2.34 [95% CI (1.40, 3.93)] times more likely to initiate after marriage than their counterparts. Childhood exposure was a predictor for initiating SLT use prior to marriage but not after. Women reporting tooth and gum pain were 1.85 times more likely to initiate after marriage than their counterparts. Husband and neighbours were the most significant influences on post-marital initiation. Narratives highlighted differences in processes of initiation pre and post marriage and during pregnancy. Conclusions: Most tobacco prevention interventions are directed to adolescents in school. This study suggests that especially for low literate or illiterate women, school based interventions are ineffective. To be effective strategies to prevent SLT initiation must reach women in urban areas at or immediately after marriage and during their first pregnancy. Messages must negate culturally rooted beliefs about the health benefits of SLT in order to prevent initiation and onset of daily use.

Analysis of Blood Serum and Epidermis Proteins by Exposure of Nature Lays on Treatments Bergamot af Grapefruit Essential Oil (Bergamot과 Grapefruit essential oil의 관리 시 자연광 노출에 따른 혈청 및 표피층 단백질 분석)

  • The result of the study for essential oil(Bergamot, Grapefruit essential oil)'s Anaiysis of Blood Serum Epidermis proteins by exposure of nature lays on treatments are as follows. 1. There is no significant difference between control group and the groups treated by essential oil in feed efficiency, but in water intake, there are significant differences between each group. Water intake of the groups treated by Bergamot and Grapefruit essential oil without treatment by sun cream is higher than that of any other group. It is presented that the effect of losing water of the surface of skin is related to the balance effect of water. 2. As a result of serum analysis without applied UV-rays, the total cholesterol in the groups treated by Bergamot essential oil is higher than that of the groups treated by Grapefruit essential oil. It is showed that the dissolution of cellulite of Grapefruit essential oil is stronger than that of Bergamot essential oil. After applied UV-rays, however, the change of total cholesterol of the groups treated by Bergamot essential oil is lower than that of the groups treated by Grapefruit essential oil, namely, the group treated by Bergamot essential oil drops is used to make sebum membrane on the surface of skin, so the quantity of influx into blood vessel was low. With the result that, atherosclerotic index(A.I) also was low in the group treated by Grapefruit essential oil. In the aspect of increasing cholesterol for obesity treatment, it was judged that the effect of treatment with Bergamot essential oil is somewhat slower than that with Grapefruit essential oil, but it is safer than the effect of treatment with Grapefruit essential oil in the aspect of health. 3. As a result of protein analysis, the groups applied UV-rays in the group of treated by Bergamot essential oil, the biggest change of protein's band was appeared under part of 20.0 1Kd in the change pattern of protein. As the trend of changing protein at the groups treated by SPF 15 and 30 was similar to that of the groups of the control, it was showed that the effect of sun cream can prevent the change of the composition of skin. In the groups treated by Grapefruit essential oil, after the groups treated by Grapefruit essential oil applied UV-rays, the biggest change of protein's bend was appeared the upper part of 43Kd in the change pattern of protein. It was also judged that it means the change of the composition of skin. However, the trend of changing protein at the groups treated by SPF 15 and 30 was also similar to that of the groups of the control. Eventually, it was also showed that it can prevent the change of the composition of skin by the effect of sun cream.

Possibility of Local Recurrence Caused by Surgical Instruments in the Mouse Skin Cancer Model (백서 모델에서 수술 기구를 통한 피부악성종양의 국소 재발 가능성)

  • Purpose: The goal of cancer surgery is complete removal of cancer tissue and prevention of recurrence. Surgeons can change the surgical instruments after total resection of the cancer mass. The purpose of this procedure is to prevent dissemination of the cancer cells attached to the surgical instruments. Authors hypothesize the possibility of local recurrence caused by the cancer cells attached to the surgical instruments in the skin cancer cases. Methods: Skin cancers were induced by using DMBA-TPA two-stage carcinogenesis model in 10 of Balb/c mice. In 2-weeks, skin cancer was developed in all 10 mice. cancer cell attached surgical instruments were made by pinching the removed cancer tissue using Adson tissue forcep 10, 20, 30 times each. To count number of cancer cells in each forcep with different number of pinching was done, the forceps were washed in 30 mL of the normal saline and Cytospin preparation was done. To make recurrence models from cancer cell attached surgical instrument, three incisions were made in normal skin of each mouse, and local seeding was done by pinching subcutaneous tissue in 10, 20, 30 times each by using Adson teeth forceps mentioned above as cancer cell attached surgical instrument. Results: All skin cancers were squamous cell carcinoma. Local recurrences were developed in 7 mice (3 in 10 times forceping site, 2 in 20 times forceping and 3 in 30 times forceping). In the cytospin test, the mean number of squamous cells in 100 microscope was 28.6 in 10 times, 47.2 in 20 times, 93.6 in 30 times, respectively. P value was 0.002 in Wilcoxon-Sign test. Conclusion: The number of cell count was significantly increased as number of pinching was increased. And these cells are able to induce local recurrence by local seeding. Considering this result, authors are able to confirm that the minimal handling in cancer surgery is important factor to prevent local recurrence.

Development of health education program model of young children for prevention and management of fine dust based on e-book using QR code (미세먼지 예방 및 관리를 위한 QR 코드 활용의 e-book 기반 유아건강교육 프로그램 모형 개발)

  • This study was to develop the health education program model of young children based on e-book using QR code to prevent and manage fine dust. We developed program to prevent and manage fine dust for young children' health through in-depth discussions with 2 young children's health experts and 3 young children education experts, and developed the e-book using QR code. The program model for fine dust's prevention education based on e-book using QR code was verified for suitability as a field adaptation and teaching medium for 5-year old. The results of this study are as follows: First, the program model for fine dust's prevention education based on e-book using QR code was developed through 3 stages of appreciation activity-central activity-finishing activity. Second, the e-book using QR code is a useful teaching medium applicable to fine dust's prevention education program. In conclusion, the e-book-based fine dust's prevention education program using QR code will be used as an appropriate education method for fine dust's prevention and health management activities in the field of education for young children.

The Various Effects of Xylitol as a Dietary Sugar Substitute on Improving Oral Health (식이 당 대체제인 자일리톨의 구강건강 증진에 미치는 다양한 효과)

  • A number of studies have been conducted to confirm the preventive effect of xylitol on dental caries as a whole or partial alternative to dietary sugars. This study reviewed the oral health effects of xylitol on the prevention mechanism of dental caries, the prevention of dental caries, the inhibition of mother-to-child transmission, and the oral health effects in the elderly based on existing studies on the oral health of xylitol. Carbohydrates and dietary sugars in food are fermented by acid-producing microorganisms in the mouth and produce dental plaque and acid, which cause dental caries. However, most dental decay-causing bacteria cannot produce acids by metabolizing xylitol. Xylitol, stored in cells as a non-metabolizable metabolite by Streptococcus mutans (S. mutans), affects bacterial glucose metabolism and inhibits bacterial growth. Xylitol consumption also reduces the amount of plaque and the population of S. mutans in both plaque and saliva. In addition, xylitol acts in the remineralization process. Xylitol has been confirmed to effectively prevent dental caries, inhibit mother-to-child transmission of MS, prevent dental caries, and increase salivary flow in the elderly. In conclusion, xylitol is an adequate sugar substitute for dental health, from infants to the elderly. For future studies, the researchers recommend reviewing the effects of xylitol on the oral and intestinal microbial environment and the side effects of excessive intake.

Factors related to the Experience of Violence Damage among Multi-cultural and General Adolescents in Korea: Focusing on health risk behavior (한국 다문화 청소년과 일반 청소년의 폭력피해경험 관련요인 : 건강위험행위를 중심으로)

  • This study was to identify factors related to the experience of violence damage, focusing on health risk behavior of multi-cultural and general adolescents in Korea. Data from the 15th online survey of youth health behavior (2019) was used to analyze 35,574 adolescents. The data were analyzed using complex sample analysis by using SPSS/Win 22.0. The multi-cultural adolescents had significantly higher experience of smoking and habitual drug use and violence damage than those ordinary adolescents. In both multi-cultural and general adolescents, the more they did not live with their family, the more they had smoking experience and habitual drug use experience, the higher they experienced violence. The experience of violence victimization of general youth was significantly related to gender, school, economic status, and drinking experience. To prevent the violence victimization of adolescents, social support for adolescents who do not live with their families is required, and careful attention is needed to adolescents who have smoking and drug use experience. In addition, since multi-cultural youth are more vulnerable to health risk behaviors and violence damage than general youth, educational programs are needed to induce multi-cultural youth to practice correct health behaviors and prevent experiences of violence.

Yemen's Cholera Epidemic Is a One Health Issue

  • Yemen has been faced with the worst cholera epidemic of modern times, with more than 1 million suspected cases and 3000 deaths at the time of writing. This problem is largely due to the longstanding civil war between pro-government forces and the Houthi armed movement, which has severely damaged already vulnerable sanitation and healthcare facilities and systems in the country. It is further compounded by a dire lack of basic amenities, chronic malnutrition, and unfavourable weather conditions. Another contributory component may be aerial transfer by cholera-infected chironomid insects. To contain the spread of cholera in Yemen, a nation-wide armistice should be negotiated, and national and local committees must be convened to coordinate efforts on the ground. Community isolation facilities with proper sanitation, reliable disposal systems, and a clean water supply should be set up to isolate and treat sick patients. The continuity of vaccination programmes should be ensured. Public health campaigns to educate local communities about good hygiene practices and nutrition are also necessary. The One Health paradigm emphasizes a multi-sectoral and transdisciplinary understanding and approach to prevent and mitigate the threat of communicable diseases. This paradigm is highly applicable to the ongoing cholera crisis in Yemen, as it demands a holistic and whole-of-society approach at the local, regional, and national levels. The key stakeholders and warring parties in Yemen must work towards a lasting ceasefire during these trying times, especially given the extra burden from the mounting severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) outbreak worldwide.

Size Distribution and Concentration of Airborne Fungi in the Public Facilities (다중이용시설 내부에 분포하는 부유 진균의 입경별 농도 특성)

  • The aim of this study is to examine size-based concentration and genera of airborne fungi distributed in public facilities such as hospital, kindergarten, day-care center and postpartum nurse center and to provide fundamental data in order to prevent respiratory diseases caused by exposure to airborne fungi. Culturable total and respirable concentrations of airborne fungi averaged to $382\;cfu/m^3\;and\;292\;cfu/m^3$ in hospital, $536\;cfu/m^3\;and\;347\;cfu/m^3$ in kindergarten, $334\;cfu/m^3\;and\;266\;cfu/m^3$ in day-care center, and $371\;cfu/m^3\;and\;289\;cfu/m^3$ in postpartum nurse center, respectively. The ratio of respirable to total concentration of airborne fungi in the investigated public facilities was ranged from $55\%\;to\;70\%$ but there was no significant difference among them (p>0.05). The mean I/O ratio of culturable total and respirable concentrations were 0.56 and 0.64 in hospital, 0.72 and 0.91 in kindergarten, 0.33 and 0.45 in day-care center, and 0.63 and 0.73 in postpartum nurse center, respectively. Indoor concentration of airborne fungi did not correlated significantly with indoor temperature and relative humidity (p>0.05) but had a significant positive correlation with $CO_2$ concentration (p<0.01) and surrounding condition (p<0.05). Penicillium spp., Cladosporium spp., and Aspergillus spp. were estimated to over $95\%$ of total airborne fungi identified in the investigated public facilities.

Sensory Profiles of Koumiss with added Crude Ingredients extracted from Flaxseed (Linum usitatissimum L.)

  • Recently, the health-promoting effects of functional foods have been shown to prevent nutrition-related diseases, improve physical and mental well-being, satisfy hunger, and provide necessary nutrients for the human body. Koumiss is a common type of fermented mare's milk that has been shown to have beneficial therapeutic effects on cardiovascular disease, tuberculosis, and diarrhea as it can nourish vessels, relieve ill mood, and improve digestion. Hence, in this study, we aimed to prepare health-promoting koumiss using different concentrations of flaxseed (Linum usitatissimmum L.), which has the potential to control heart disease, hypertension, inflammation, and lung function. The pH was decreased to around 4.42-4.43, whereas the total anthocyanin (TA) content was increased to around 0.78~0.82% after fermentation of the koumiss premix. There were no significant differences in pH and TA among the three groups, including the control group. In sensory profile analysis, the taste, flavor, color, texture, and overall acceptability decreased in proportion to the added amount of flaxseed. These findings provided the first data describing the effects of flaxseed on koumiss fermentation, establishing a basis for commercial-scale production of koumiss containing fermented flax seed and for improving the health-promoting activity of koumiss.
