Fractured tracheostomy tube presenting as foreign body in the tracheobronchial tree is very rare. Authors experienced a case of broken tracheostomy tube presenting as a foreign body in the trachea, which was removed with the help of flexible bronchoscope. A few causes could be suggested for the tube break: aging and deterioration of the tube with repeated use, a flaw of the tube in the manufacturing process, and a wrong sterilization.method causing weakness of the tube. We report this case with brief literature review.

Alimentary tract duplication cysts are rare congenital anomalies, most commonly located in the ileum, but may present anywhere from mouth to anus.Clinically, they may be asymptomatic, incidentally diagnosed or may present with obstruction, volvulus, intussusception or gastrointestinal bleed. Here we report a case of a one year old male child presenting in gasping state and shock. Despite the initial strong suspicion of Meckel's diverticulum and tubercular abdomen, the final diagnosis remained elusive till exploratory laparotomy was performed which revealed a duplication cyst of ileum with perforation into the umbilicus. Duplication cyst should always be kept as a differential diagnosis so that early intervention can help in better management.

In this work, free vibration characteristics of functionally graded piezoelectric (FGP) nanobeams based on third order parabolic shear deformation beam theory are studied by presenting a Navier type solution as the first attempt. Electro-mechanical properties of FGP nanobeam are supposed to change continuously throughout the thickness based on power-law model. To capture the small size effects, Eringen's nonlocal elasticity theory is adopted. Using Hamilton's principle, the nonlocal governing equations for third order shear deformable piezoelectric FG nanobeams are obtained and they are solved applying analytical solution. By presenting some numerical results, it is demonstrated that the suggested model presents accurate frequency results of the FGP nanobeams. The influences of several parameters including, external electric voltage, power-law exponent, nonlocal parameter and mode number on the natural frequencies of the size-dependent FGP nanobeams is discussed in detail.

Metformin which is an oral hypoglycemic agents, acts by enhancing insulin sensitivity, decreasing hepatic glucose production and increasing peripheral utilization of glucose. Deliberate self poisoning with oral hypoglycemic agents is rare. The lactic acidosis associated with metformin toxicity is well described in the medical literature. Metformin overdose even in otherwise healthy patients may produce a profound and life threatening lactic acidosis. We report a case of massive metformin ingestion(75g) in a patient presenting with lactic acidosis and hypotension. She died 24h after presenting to our emergency department despite bicarbonate treatment and hemofiltration therapy.

Ellagitannins have been reported to enhance the immune system. In this study, the effects of pedunculagin on langerhans cells were examined. Pedunculagin, an ellagitannin from Alnus hirsuta var. microphylla. Betulaceae, is a novel immunomodulator. Langerhans cell are known as the potent antigen presenting cell and elicit the Contact Hypersensitivity (CHS) response by presenting Ag to trafficking Ag-specific T cells within the skin. For determining the effects af pedunculagin on murine langerhans cell, the expression of TNF-$\alpha$ mRNA was examined by RT-PCR. As a result, the expression of TNF-$\alpha$ mRNA was upregulated by pedunculagin. These results suggest that pedunculagin enhances TNF-$\alpha$ and could be used as an immunomodulator in skin immune system.

Bickerstaff's brainstem encephalitis (BBE) is an autoimmune central nervous system disorder. It can occur in more limited forms and may overlap with Guillain-Barr$\acute{e}$ syndrome (GBS). A 49-year-old female presented with rapidly progressive paralytic ileus, urinary retention, deep drowsiness, ophthalmoplegia, dysarthria, ataxia, quadriparesis and hyporeflexia after viral meningitis. She was diagnosed as BBE with GBS and treated with immunoglobulin. She was completely recovered after 1 month. It is a rare case of BBE overlapping with GBS presenting with severe paralytic ileus.

Korea Gas Corp. has developed the design technology of the LNG storage tank. The membrane to be applied inside of LNG storage tank is provided with corrugations to absorb thermal contraction and expansion caused by LNG temperature. We constructed strain measurement system by using strain gage. In this paper, some problems which should be considered when measuring strain at $-162^{\circ}C$, are discussed by presenting test results on the characteristics of strain gages, Temperature sensor, adhesive and lead wire. And presenting the procedure of the constructing strain measurement system.

This research shows that as the use of IT spreads, the Investments of IT must be accompanied by the innovation of the business process of an organization to increase productivity. It examines closely to the relations between these by presenting a theoretical formula, with related-theories at the basis, that shows the characteristics of IT Infrastructure and heightens the results of organization. In previous researches, various results co-exist in presenting the direct relativeness between IT Infrastructure and results of organization. But in this research, a parameter called business process has been included making it possible to analyze even the indirect effects to the results of organization. Also, the difference and relativity between the IT Infrastructure and business process groups according to the company environment factor has been proved.
