Eosinophilic Gastrointestinal Disorder Presenting as Intractable Vomiting and Ascites in a Young Girl

  • Eosinophilic gastrointestinal disorder (EGID) is a rare disease in children that affects the bowel wall, with eosinophilic infiltration in the absence of any other causes for eosinophilia. The etiology remains unknown, but allergies and immunological imbalance are suspected triggers. We encountered a case of serosal EGID presenting as intractable vomiting and ascites in a 9-year-old girl, after influenza virus infection. Peripheral eosinophilia was not present. The diagnosis was confirmed by biopsy of the bowel wall through laparotomy and endoscopy, and controlled by 2 courses of steroid therapy due to recurring symptoms. Influenza virus infection was assumed to play a role in the onset of EGID through a Th2 response that stimulated eosinophilic infiltration in the GI tract. We therefore report this case along with a literature review.

  • The purpose of this study is to investigate the role and function of tokens to form a healthy blockchain-based ecosystem. Tokens must be constructed in a way that enhances their desired behavior to grow into a healthy token economy. The actions required of ecosystem participants in designing tokens should enable each individual to receive appropriate incentives (rewards) and encourage voluntary participation in taking this action. Also, all ecosystem participants must design to make the token ecosystem self-sustainable by generating profits. For example, in Bitcoin's proof-of-work method, mining is designed as a desirable behavior. Token-based services should be designed to induce multiple engagements, to design penalties for undesirable behavior, and to take into account evolutionary development potentials. Besides, the economic value of the entire token ecosystem will increase if the value that is designed and designed to take into account the revolutionary Innovation Possibility is greater than the reward amount paid to tokens. This study will contribute to presenting relevant service model by presenting how to design tokens and criteria when establishing blockchain-based service model. Future research is needed to discover new facts through a detailed comparative analysis between Tokennomics models.

  • In this paper, we propose the use of stochastic finite element method, that is popularly employed in mechanical structure analysis, for more practical designing purpose of RF device. The proposed method is formulated based on the vector finite element method cooperated by pertubation analysis. The method utilizes sensitivity analysis algorithm with covariance matrix of the random variables that represent for uncertain physical quantities such as length or various electrical constants to compute the probabilities of the measure of performance of the structure. For this computation one need to know the variance and covariance of the random variables that might be determined by practical experiences. The presenting algorithm has been verified by analyzing several device with different be determined by practical experiences. For the convenience of formulation, two dimensional analysis has been performed to apply it into waveguide with dielectric slab. In the problem the dielectric constant of the dielectric slab is considered as random variable. Another example is matched waveguide and cavity problem. In the problem, the dimension of them are assumed to be as random variables and the expectations and variances of quality factor have been computed.

International Comparison of Contents about Particle Concept in National Science Curricula (국가 수준 과학과 교육과정의 입자 관련 내용 국제 비교)

  • The purpose of this study was to find some suggestions for reorganization of contents about particle concept of matter in Korean science curriculum. For the purpose of this study, authors analyzed features of Korean science curriculum and compared science curricula of Korea, USA, UK, Japan and Finland. From the result of this study, authors find some features and important suggestions about reorganization of contents about particle in science curriculum. First, the sequence of contents about particle concepts in 2009 Revised National Curriculum was similar to that in the 6th National Science Curriculum. And the feature of 2009 Revised National Curriculum showed the articulation of contents about particle concept. If contents about particle concept is increased in elementary science curriculum, the total articulation would be increased. Second, the presenting sequence of atomic structure-first and laws about atom-later should be changed to laws about atomic-first and atomic structure-later. This presenting sequence is grounded by science curricula of other countries, history of science and developmental psychology. And science curriculum of Korea was required specific extended concept statement like science curricula of USA or UK. Also, Korean science curriculum could benchmark Finnish science curriculum if we want to develop some integrated learning activities such as those in STS or STEAM program.

  • The tiebar is used by joint for keeping from deformation by conserving tensile load between two structure. The tiebar, bolt joining type, is combined by tiebar and clevis like bolt and nut. By coupled, it maintains tensile load. While it goes through repeated load, it occurs to loose because joining force between clevis and nuts is reduced. So, continuos maintenance is needed such as making tighter periodically, repairing the broken part and so on. For that reason, this paper calculates necessary torque unfastening joint for conventional tiebar and presenting tiebar theoretically and then consider the reason that conventional bolt-typed tiebar unties. Also, through vibration untied test for two types of tiebar we confirm that presenting tiebar have a improvement of unfastening when we compare with the conventional one.

A Case of Cecal Volvulus Presenting with Chronic Constipation in Lissencephaly

  • Cecal volvulus is uncommon in pediatric patients and there are few reports of cecal volvulus with cerebral palsy. Here, we report the case of a 19-year-old male patient who presented with abdominal distension, a history of cerebral palsy, refractory epilepsy due to lissencephaly, and chronic constipation. An abdominal x-ray and computed tomography without contrast enhancement showed fixed dilated bowel intensity in the right lower abdomen. Despite decompression with gastric and rectal tube insertion, symptoms did not improve. The patient underwent an exploratory laparotomy that revealed cecal volvulus. Cecal volvulus usually occurs following intestinal malrotation or previous surgery. In this patient, however, intestinal distension accompanying mental disability and chronic constipation resulted in the development of cecal volvulus. We suggest that cecal and proximal large bowel volvulus should be considered in patients presenting with progressive abdominal distension combined with a history of neuro-developmental delay and constipation.

  • Toxic shock syndrome(TSS) is a multisystemic disease presenting with high fever, sunburn like rash that subsequently desquamates, and hypotension mainly caused by toxin producing strains of Staphylococcus aureus. It was first reported in 1978 by Todd et al, thereafter many patients have been reported. In children, TSS is rare and must be differentiated from other erythematous febrile diseases such as Kawasaki disease, scarlet fever, drug eruption etc. We experienced a case of TSS associated with staphylococcal cellulitis in 26-month old boy, who was presenting similar symptoms to Kawasaki disease at initial stage of illness. As time passed, the patient represented more typical symptoms of TSS and Staphylococcus aureus was isolated from cellulitis of the right elbow area. Therefore, we report this case with a brief review of related literatures.

  • Pulmonary aspergillosis is a rare disease, mostly commonly presenting as secondary invasion of pre-existing cavitary disease. In severance hospital, 9 patients have recognized as having this disorders in the 16 years from 1960 to April, 1976. The diagnosis of the aspergillosis has been made by examination of tissues removed at surgery in severance hospital, all the cases of aspergillosis except one (a Lt upper lobectomy due to bronchiectasis, performed 20 years ago) the case had been diagnosed as pulmonary tuberculosis and treated with anti-tuberculosis drugs for periods ranging from 2 to 14 years the most common presenting symptoms were hemoptysis, blood-tinged sputum and cough for a long time. All of the cases were confirmed by pathological examination to be aspergillosis superimposed on underlying tuberculosis and/or bronchiectasis three cases were suspected clinically by fungus cultures, seven cases were confirmed as aspergillosis, after surgery. One patient died following surgery because of bleeding and pulmonary insufficiency.

  • Chordomas are slowly growing and rare malignant tumors arising from the remnants of the notochord. Furthermore, intrathoracic chordomas presenting as a posterior mediastinal tumor account for only 1∼2% of all reported chordomas. Incomplete resection of these tumors can lead to local recurrence, distant metastasis and result in a poor outcome, but complete remission can be expected with complete resection and adjuvant radiotherapy. We report a case of thoracic chordoma presenting as a posterior mediastinal tumor which was successfully treated with complete resection and adjuvant radiotherapy without recurrence and distant metastasis for 2 years.

Surgical Treatment of Pulmonary Aspergillosis (폐 Aspergillosis의 외과적 치료)

  • Pulmonary aspergillosis is relatively rare disease, most commonly presenting pre-existing cavitary disease. This investigation is designed to illustrate the clinical features, preoperative diagnosis and surgical role in the management of this disease. In retrospective review of opeative cases since Jan. 1988, total 16 cases were analysis. Peak incidence of age lies in the 3rd & 5th decade[81.3%].The common presenting symptoms were hemoptysis & blood tinged sputum[90.9%] & all cases had a history of treatment with antituberculous drugs, but the tuberculosis was revealed in only 3 cases in the pathologic studies on specimen after resection. The locations of lesion were upper lobes in 13 cases, lower lobe in 2 cases, and left whole lobe in 1 case. The operative procedures for treatment of aspergillosis were performed. The lobectomy was performed in 7 cases, lobectomy & segmentectomy in 2 cases, lobectomy & decortication in 2 cases, segmentectomy in 3 cases, bilobectomy & segmentectomy in 1 case and pneumonectomy in 1 case. There was one death due to respiratory insufficiency 21 days later,postoperatively, and empyemas & residual space problems were developed in 2 & 3 cases,respectively. The empyemas were treated with open thoracostomy and the dead spaces was filled with granulation tissues within 6 months. During follow up, there was no recurrence. In conclusion, surgical resection is the treatment of choice for symptomatic aspergillosis and needed in asymptomatic patient to prevent possible fatal sequelae in the future.
