Cancer Patients Are at High Risk of Mortality if Presenting with Sepsis at an Emergency Department

  • Background: Sepsis is an emergency condition with high mortality and morbidity rate. There are limited data on the association of cancer as a risk factor for mortality in sepsis patients in the emergency department (ED). Materials and Methods: This retrospective study was conducted at the ED, Faculty of Medicine, Ramathibodi Hospital, Bangkok, Thailand. The study period was between January 1st and December $31^{st}$, 2014. The inclusion criteria were as follows: adult patients over 15 years of age who presented at the ED with suspicion of sepsis, received treatment at the ED, and whose blood culture was found to be positive. Clinical data were recorded from medical records including the Mortality in Emergency Department Sepsis score (MEDS score). The primary outcome of this study was mortality at one month. Multivariate logistic regression analysis was used to identify independent factors associated with death. Results: During the study period, there were 775 eligible patients. The two most common pathogens identified from blood cultures were Staphylococcus aureus (193 patients; 24.9%) and Escherichia coli (158 patients; 20.4%). At one month after presenting at the ED, 110 patients (14.2%) had died. There were four significant factors for death, having cancer, being on an endotracheal tube, initial diagnosis of bacteremia, and high MED scores. Having cancer had an adjusted OR of 2.12 (95% CI of 1.29, 3.47). Conclusions: Cancer patients have double the risk of mortality if presenting with sepsis at the ED.

Treatment for Patients with Acute Ischemic Stroke Presenting beyond Six Hours of Ischemic Symptom Onset : Effectiveness of Intravenous Direct Thrombin Inhibitor, Argatroban

  • Objective : The objectives of this study were to analyze the outcome and hemorrhagic risk of intravenous (IV) argatroban in patients with acute ischemic stroke presenting beyond six hours of ischemic symptom onset. Methods : Eighty patients with acute ischemic stroke who were admitted to the hospital beyond six hours from ischemic symptom onset were retrospectively analyzed. We could not perform IV thrombolysis or intra-arterial thrombolysis because of limited time window. So, IV argatroban was performed to prevent recurrent thrombosis and progression of infarcted area. The outcome was assessed by the National Institute of Health Stroke Scale (NIHSS) score and related hemorrhagic risk was analyzed. Also, each outcome was analyzed according to the initial stroke severity, subtype, and location. Results : The median NIHSS was 8.0 at admission, 4.1 upon discharge, and 3.3 after three months. A good outcome was achieved in 81% of patients upon discharge and 88% after three months. Symptomatic hemorrhage occurred in only two patients (3%). IV argatroban was effective regardless of initial stroke severity, subtype, and location. Conclusion : IV argatroban may be an effective and safe treatment modality for acute ischemic stroke presenting beyond six hours of ischemic symptom onset.

  • 평소 건강하게 지내다가 갑자기 시작된 반복적인 구토와 보채는 증상으로 5개월 여아가 병원에 내원하였고, 단순 흉부 방사선 검사에서 비정상적 음영이 좌측 흉부에서 관찰되었다. 이어 시행된 흉부 전산화 단층촬영에서 선천성 횡경막 탈장으로 진단되어, 흉부외과에서 수술적 치료 시행하였으며 수술 결과 횡경막 좌측후외방의 Bochdalek 형식의 선천성 횡격막 탈장으로 확인되었고, 수술적 치료 후 증상의 호전을 경험하였기에 문헌고찰과 함께 보고하는 바이다.

Efficient Generation of BLCL Expressing Foreign Antigen as Antigen-presenting Cells with Recombinant Retroviruses

  • Epstein-Barr Virus(EBV)-transformed lymphoblastoid B cell lines, BLCL which expresse antigens, are potential antigen-presenting cells(APCs) for the induction of CTL in vitro. However transfection of BLCLs with subsequent selection by antibiotics is notoriously difficult because plating efficiencies of BLCLsare reported to be 1% or less. To generated stable transfectants of BLCLs we produced high titers of retroviruess encoding pp 65 antigen of human cytomegalovirus of foreign antigens and trans-duced them of BLCLs. The pp 65 gene was cloned into the retroviral vector pLXSN. The recombinant retroviral vector was transfected to ecotropic packaging cell line, CP&E86, and this polyclonal recom-binant retrovirus was transduced to PA317 that is amphotropic pakaging cell line. The titers of colned PA317 amphotropic retroviruses ranged from 5 to $\times$10$^{6}$ colony forming units (CFU)per ml (CFU/ml) We performed three rounds of consecutive transductions to BLCLs in order to improve the clon-ing effieiencies. The expression of recombinant HCMV-pp65 antigen was more than 20% after the final transduction. THe third-transduced BLCLs were easily selected in optimal concentration of G418. BLCLs expressing foreign antigens could be used as target cells for CTL assay and/or as APCs for induction of in vitro CTL responses specific for viral and tumor antigens.

수지상세포의 항원제시 능력 및 항암활성에 미치는 Lipofectin의 영향 (Effect of Lipofectin on Antigen-presenting Function and Anti-tumor Activity of Dendritic Cells)

  • Background: Dendritic cells (DC) are professional antigen-presenting cells in the immune system and can induce T cell response against virus infections, microbial pathogens, and tumors. Therefore, immunization using DC loaded with tumor-associated antigens (TAAs) is a powerful method of inducing anti-tumor immunity. For induction of effective anti-tumor immunity, antigens should be efficiently introduced into DC and presented on MHC class I molecules at high levels to activate antigen-specific $CD8^+$ T cells. We have been exploring methods for loading exogenous antigens into APC with high efficiency of Ag presentation. In this study, we tested the effect of the cationic liposome (Lipofectin) for transferring and loading exogenous model antigen (OVA protein) into BM-DC. Methods: Bone marrow-derived DC (EM-DC) were incubated with OVA-Lipofectin complexes and then co-cultured with B3Z cells. B3Z activation, which is expressed as the amount of ${\beta}$-galactosidase induced by TCR stimulation, was determined by an enzymatic assay using ${\beta}$-gal assay system. C57BL/6 mice were immunized with OVA-pulsed DC to monitor the in vivo vaccination effect. After vaccination, mice were inoculated with EG7-OVA tumor cells. Results: BM-DC pulsed with OVA-Lipofectin complexes showed more efficient presentation of OVA-peptide on MHC class I molecules than soluble OVA-pulsed DC. OVA-Lipofectin complexes-pulsed DC pretreated with an inhibitor of MHC class I-mediated antigen presentation, brefeldin A, showed reduced ability in presenting OVA peptide on their surface MHC class I molecules. Finally, immunization of OVA-Lipofectin complexes-pulsed DC protected mice against subsequent tumor challenge. Conclusion: Our data provide evidence that antigen-loading into DC using Lipofectin can promote MHC class I- restricted antigen presentation. Therefore, antigen-loading into DC using Lipofectin can be one of several useful tools for achieving efficient induction of antigen-specific immunity in DC-based immunotherapy.

NCS 기반 역량 교육을 적용한 해군부사관과의 리더십 교육방안 연구 - 프로젝트와 프레젠테이션 교육을 중심으로 - (Studies of Leadership Education for Using NCS-Based Competency Curriculum)

  • 이 연구의 목적은 NCS 기반 역량 교육을 적용하여 해군부사관과에 적합한 리더십 교육방안을 고찰하는 것이다. NCS 기반 역량 교육을 적용한 리더십 교육은 지식과 경험을 통한 개별역량 통합적 활용을 키우는 것이다. 이에 따른 해군부사관과의 리더십 교육은 4단계로 구성된다. 구체적으로 보면, 첫째, 역량 진단. 둘째, 역량 진단을 통한 교육방향 설정과 프로젝트 구성. 셋째, 프로젝트를 통한 이론과 배경지식의 통합과 프레젠테이션을 통한 문제분석 및 해결방안의 의사소통 경험 제공. 넷째, 역량진단을 통한 학습 결과 제시이다. NCS 기반 역량 교육을 적용한 리더십 교육은 해군부사관과의 학생들이 프로젝트를 구성하고 프레젠테이션으로 발표함으로써 리더십 핵심역량을 향상시키는 방안이다. 마지막으로 이 연구의 한계와 향후 연구를 위한 제언을 하였다.

활성화된 단핵구 및 대식세포의 항원제시기능에 대한 Kaempferitrin의 조절 효과 (Modulatory Effect of Kaempferitrin, a 3,7-Diglycosylflavone, on the LPS-Mediated Up-regulation of Surface Co-stimulatory Molecules and CD29-Mediated Cell-cell Adhesion in Monocytic- and Macrophage-like Cells)

  • Kaempferitrin, isolated from Kenaf (Hibiscus cannabinus), was examined to evaluate its modulatory effects on antigen-presenting cell functions of macrophages/monocytes such as phagocytosis of foreign materials, up-regulation of costimulatory molecules (CD40, CD80 and CD86), adhesion molecule activation, and antigen processing and presentation. Kaempferitrin strongly blocked up-regulation of CD40, CD80 and CD86, but not pattern recognition receptor (PRR) (e.g., TLR2). It also suppressed functional activation of CD29 (${\beta}1$-integrins), as assessed by cell-cell adhesion assay, required for T cell-antigen-presenting cell (APC) interaction. Furthermore, this compound did not block a simple activation of CD29, as assessed by cell-fibronectin adhesion assay. However, the compound did not diminish phagocytic uptake, an initial step for antigen processing, and ROS generation in RAW264.7 cells. In particular, to understand molecular mechanism of kaempferitrin-mediated inhibition, the regulatory role of LPS-induced signaling events was examined using immunoblotting analysis. Interestingly, this compound dose dependently suppressed the phosphorylation of $I{\kappa}B{\alpha}$, Src, Akt and Syk, demonstrating that it can negatively modulate the activation of these signaling enzymes. Therefore, our data suggested that kaempferitrin may be involved in regulating APC function-relevant immune responses of macrophages and monocytes by regulating intracellular signaling.

<계축일기>에 나타난 갈등의 층위와 제시 방식 (The Layers of Conflicts and the Methods of Presenting Conflicts in Gyechugilgi)

  • 이 글에서는 <계축일기>의 성격에 대한 이해를 심화하기 위해 작품의 역사적 맥락과 작품에 제시된 갈등의 층위, 그리고 갈등의 제시 방식 등을 검토하였다. 작자는 내인들의 갈등과 수난을 상당한 비중을 두어 자세하게 다루고 있다. 이는 작자가 그것을 단순히 광해군과 인목대비의 갈등에서 파생된 부수적인 문제로 파악하고 있지 않다는 것이고, 따라서 광해군과 인목대비에 초점을 맞춰 이 작품을 이해하는 것이 작품의 실상과 거리가 있다는 것을 의미한다. 또한 이 작품에서는 대화를 빈번하게 교체 인용하여 갈등의 첨예함과 심각성을 효과적으로 드러내고, 갈등을 매개하는 인물을 형상화하여 사람의 욕망이 갈등을 어떻게 확대 심화시키는지 구체적으로 제시하고 있으며, 삽화를 나열하여 갈등의 상대방에 대한 비판과 자기들이 겪어야 하는 고통의 부당함을 강조하고 있다. 요컨대 작자는 갈등이 사람들 사이의 문제이고, 사람의 욕망이나 부정적 성격으로 인해 그것이 야기되고, 또 심화되기도 한다는 입장에서 광해군과 인목대비 양측의 갈등을 바라보고 있다고 할 수 있다. 흔히 광해군의 성격과 당시의 역사적 사건을 허구적으로 왜곡 과장한 점을 이 작품의 문제점, 혹은 이 작품을 소설로 보아야 하는 근거로 제시한다. 하지만 그것은 먼저 작자가 당시 상황을 정치나 역사의 관점이 아니라 사람에 초점을 맞춰 파악한 결과로 이해하는 것이 바람직하고, 이 작품의 문학적 성격과 가치도 그러한 점에서 찾아야 할 것으로 생각한다.

공공극장 기획공연의 프로그래밍 사례 연구 -예술의전당 오페라하우스 공연 기획의 성과와 과제- (A Case Study on the Programming of Public Theaters in Korea -Analysis of the Achievements and the Challenges of the Opera House of the Seoul Arts Center for the years from 1993 to 2011-)

  • The Seoul Arts Center is an representing Korean arts-complex consisting of the Opera House, the Concert Hall and the Museums. Since its opening in 1993, it has been established as a mecca of arts and the culture in Korea. Many local public theatres in Korea are benchmarking its operating system and policy. Unlike most European or American theatres, the Seoul Arts Center has no resident art companies. It is not a 'producing theatre' whose programming is mainly based on house productions with resident companies, but a 'presenting theatre' that is to be managed with rental and invitation programs without resident companies. The majority of the Korean public theatres are also presenting theatres. This study aims to research and analyze how the Seoul Arts Center has developed the programming of the Opera House for last 19 years and which challenges and issues it has been dealing with, and finally to propose what will be recommendable remedies for its successful future. The analysis is divided into four development stages: preparation period from 1981 to 1992, establishment period from 1993 to 1999, growth period from 2000 to 2006, and lastly crisis period from 2007 to 2011. Facing the 20th anniversary next year(2013), the Opera House of the Seoul Arts Center is going through recent critical situations: higher competitions among increased theatres, trends of commercial musical productions, and rapid declines in its house programs due to the low budget and unclear programming policy. I propose as a discussion to start a repositioning strategy as a Korea's representing arts-complex, utilizing all sources from inside and outside and having the policy direction.

전신 림프절 비대로 발현되어 림프종으로 오인된 톡소카라증: 증례 보고 (Toxocariasis Mimicking Lymphoma and Presenting as Multiple Lymphadenopathy: A Case Report)

  • 톡소카라증은 생간의 섭취 혹은 개의 분변으로 오염된 토양으로부터 인간에게 감염되어 호산구 증가증을 일으키는 기생충 질환으로 뇌, 눈, 폐, 간 등 다양한 장기에 증상을 일으킬 수 있다. 호산구 침윤이 일어난 장기에 따라 간 병변을 유발하거나 장벽 비후 및 복수의 증가로 나타나는 경우가 보고된 바 있으나 전신 림프절의 증대로 나타나는 것은 극히 드물다. 전신 림프절 증대로 나타난 톡소카라증은 현재까지 약 두 개의 증례 보고가 흉부와 경부, 서혜부에서 보고된 바 있다. 하지만 전신 림프절 증대로 인해 림프종으로 오인되었던 톡소카라증은 보고된 바 없어 이러한 드문 형태로 발현된 톡소카라증에 대해 증례 보고를 하고자 한다.
