The Effect of Resilience on Posttraumatic Stress Disorder Symptoms and Comorbid Symptoms in Firefighters (강원지역 소방관에서 외상후 스트레스 증상 및 동반증상에 미치는 리질리언스의 영향)

  • Objective : This study investigated the relationship between the resilience and posttraumatic stress symptoms, as well as comorbid symptoms in firefighters. Methods : We collected 764 firefighters, who worked at six fire department stations in Gangwon-do. We investigated the impact of event scale-revised (IES-R), the life events checklists (LEC), Connor-Davidson resilience scale (CD-RISC), Beck depression inventory (BDI), state trait anxiety inventory (STAI) and alcohol use disorder identification test (AUDIT). Full PTSD groups, partial PTSD groups and non-PTSD groups, which were classified by IES-R scores, were compared in the LEC, CD-RISC, BDI, STAI and AUDIT, ; multiple linear regression analyses were done for independent predictors of variables. Results : Of the 764 firefighters, there were significant differences in LEC (p<0.001), CD-RISC (p<0.001), BDI (p<0.001), and AUDIT (p=0.001) among the full PTSD groups, partial PTSD groups and non-PTSD groups. However, STAI did not show significant difference among three groups. In multiple regression analysis, CD-RISC (${\beta}=-0.168$, p<0.001), LEC (${\beta}=0.211$, p<0.001) and AUDIT (${\beta}=0.115$, p=0.001) were significant predictors for IES-R. Conclusion : The results of the present study suggested that resilience might be a protective factor in PTSD and comorbid symptoms of PTSD.

Associations of Overlapped Job Stress Components with Depressive Symptoms and Suicidal Idea (직무스트레스 영역의 중복과 우울 및 자살사고 사이의 관계)

  • Objective : Several recent studies determined the associations of job stress with depression and suicidal idea, but the association between the number of job stress components and depression remains unclear. In the present study, we investigated the associations of the number of components of job stress with depression and suicidal idea. Methods : We studied 194,226 participants who attended employee health screenings from January to December, 2014, and completely answered all the questionnaires, including the short form of Korean Occupational Stress Scale (KOSS), the Center for Epidemiologic Study-Depression (CES-D) and suicidal idea. The presence of clinical depressive symptoms was defined as a CES-D score of ${\geq}21$. The subjects in the highest quartile of each subscale of KOSS were considered as suffering from each component of job stress. Chisquare tests, t-tests and logistic regressions were performed to compare study outcomes between groups. Results : When we included all seven components of job stress, there was no significant association of the number of the components with depressive symptoms, or suicidal idea. However, the prevalence of depressive symptoms and suicidal idea was increased only in three occupational stress subscales including job demand, job insecurity and occupational climate. When we analyzed only these 3 components of job stress, the number of job stress subscales was significantly associated with depressive symptoms and suicidal idea, even after adjustment for sex and age. Conclusion : The number of components of occupational stress was associated with depressive symptoms and suicidal idea, only in the related components.

Fatigue Symptoms and Its Related Factors among General Hospital Nurses (종합병원 간호사들의 피로수준 및 관련요인)

  • The purpose of the present study was intended to assess fatigue symptoms of nurses working at special part(operating room, intensive care unit, emergency room) and ward in general hospitals, and to reveal its related factors. The self-administered questionnaires were given to 393 nurses employed in 3 participating hospitals located in Daejeon City. As a results, the special part workers had higher scores of fatigue symptoms than the ward workers. Multiple stepwise analysis revealed that the factors of influence on fatigue symptoms in the special part workers included subjective health status, age and job satisfaction, and in the ward workers, the factors of influence on fatigue symptoms included subjective health status, sleep hour, shift work and job satisfaction. In conclusion, the fatigue symptoms were higher in the special part workers than the ward workers, and its related factors were different from two worker groups.

  • Background and Purpose : The aim of the present study was to investigate the prodromes of stroke in already attacked patients and to prove the traditional hypothesis that some symptoms were to be prodromes of stroke in the oriental medicine. Methods : The questionnaire which was based on symptoms of traditional hypothesis was distributed to subacute stroke patients who were confirmed by Brain CT or MRI in Oriental Medical Hospital, Kyung Hee University, except patients with indistinct onset, symptoms proved any other disease. Results : Eighty-three patients(82.2%) felt some prodromal symptoms and fifty-nine patients (58.4%) underwent a change in their life within one week before onset. Most common prodromal symptoms was the weakness or numbness of limbs, single or unilateral. Conclusion : Our results suggest that the prodromal symptoms before stroke can be regarded as predicting sign. And we think that these research may contribute to preventing stroke and relapse.

  • Objectives: TuRo, defined as a dance therapy applying the meridian Qi system into the dancing movements, may have effects on ameliorating the psychological symptoms that often appear in adolescent students in a stressful situation. The Objective of the present study was to examine whether TuRo training relieves the psychological symptoms in adolescent female students. Methods: Symptom Check List-90-Revision (SCL-90-R), a 5 point Likert scale, consisted of 90 items that represent psychological symptoms, were used to measure the psychological health status in healthy adolescent female. Twenty two adolescent female students were trained TuRo dancing for two months while 29 in control group were trained mimicked movements without the concept of Qi for the same period. Changes in Somatization, Obsessive-Compulsive behaviors, Interpersonal Sensitivity, Depression, Anxiety, Hostility, Phobic Anxiety, Parinoid Ideation and Psychotic symptoms by the two months of training were compared between TuRo and control groups using SCL-9Q-R. Prognostic significance of non-chest pain symptoms in patients with non-ST-segment elevation myocardial infarction

  • Background/Aims: Chest pain is an essential symptom in the diagnosis of acute coronary syndrome (ACS). One-third of patients with ACS present atypically, which can influence their receiving timely lifesaving therapy. Methods: A total of 617 NSTEMI patients from the Korea Acute MI Registry (KAMIR) and the Korea Working Group on MI (KorMI) databases were analyzed. The study population was divided into two groups by symptoms at presentation (typical symptoms group, 128; atypical symptoms groups, 128). Results: In this study population, 23% of patients presented without chest pain. After propensity score matching, the contact-to-device time ($2,618{\pm}381minutes$ vs. $1,739{\pm}241minutes$, p = 0.050), the symptoms-to-balloon time ($3,426{\pm}389minutes$ vs. $2,366{\pm}255minutes$, p = 0.024), and the door-to-balloon time ($2,339{\pm}380minutes$ vs. $1,544{\pm}244minutes$, p = 0.002) were significantly higher in the patients with atypical symptoms than in those with typical symptoms, respectively. Atypical symptoms were an independent predictor for 1-year mortality (hazard ratio, 2.820; 95% confidence interval, 1.058 to 7.515; p = 0.038). The Kaplan-Meier estimates showed higher risk for 12-month mortality in patients with atypical symptoms (p = 0.048) and no significant difference for 12-month major adverse cardiac events (p = 0.487). Conclusions: Acute myocardial infarction patients with atypical symptoms were not rare in clinical practice and showed a high risk of delayed reperfusion therapy. After imbalance between the groups was minimized by use of propensity score matching, patients who presented atypically had a high mortality rate.

The Relationship between Perceived Discrimination and Depression in Korean Canadian College Students: Moderating Effects of Coping Strategies, Cultural Identity and Bicultural Self-Efficacy (재캐나다 한국인 대학생의 지각된 차별감과 우울의 관계: 대처양식 및 문화 정체성과 이중문화 자기효능감의 조절효과)

  • The primary goal of the present study was to examine the moderating effects of coping strategies, cultural identity, and bicultural self-efficacy on the relationship between perceived discrimination and depressive symptoms of Korean Canadian college students. Data were collected from 214 Korean Canadian college students studying in Canada through an online and offline survey. A direct effect of perceived discrimination on depressive symptoms and the moderating effects of coping strategies on the relationship between perceived discrimination and depressive symptoms were examined by using hierarchical regression analysis. And three-way interaction of perceived discrimination, coping strategies, and cultural identity, and perceived discrimination, coping strategies, and bicultural self-efficacy on depressive symptoms were examined. Results from a hierarchical regression indicated that perceived discrimination significantly predicted depressive symptoms and this finding is in line with previous studies. Second, reflective, suppressive, and reactive coping strategies did not moderate the relationship between perceived discrimination and depressive symptoms. Third, the three-way interaction effect of perceived discrimination, suppressive coping strategy, and heritage identity predicted the depressive symptoms. Low heritage identity, high level of perceived discrimination and the frequent use of suppressive coping strategy increased the level of depressive symptoms. Fourth, the three-way interaction effect of perceived discrimination, coping strategies, and bicultural self-efficacy did not predict the depressive symptoms. Limitations and applied implications of the results are discussed.

  • Congenital lobar emphysema is an uncommon disease affecting newborns and infants with respiratory distress. This report describes a 1.9kg premature baby of 31weeks gestation who developed respiratory distress symptoms of congenital lobar emphysema involved left upper lobe.Left upper lobe lobectomy was carried out and good result was obtained. So, we present one case report with literature review.

Occurrence and Analysis of Apple Blotch-like Symptoms on Apple Leaves

  • Apple blotch-like symptoms (ABLS) were observed on 'Fuji' apple leaves in Cheongsong, Gunwi and Yeongcheon apple orchards located in Gyeongbuk Province during 2010-2014. Characteristics of ABLS were yellowing, brown spots on leaves, and defoliation, similar to apple blotch diseased (ABD) leaves, which are infected with Marssonina coronaria. It is difficult to differentiate by eye between ABLS and ABD, which has led to misdiagnosis and overuse of fungicides. The present study was conducted to investigate the cause of ABLS using stereomicroscopy, culture isolation, cross-sectional analysis of leaves, and PCR. No acervuli were found on the surface of ABLS leaves and no growth was observed on potato dextrose agar (PDA) plates in culture. Furthermore, cross-sectional analysis revealed similar results, and mycelia were absent in ABLS leaves. By contrast, all these characteristics were present in ABD leaves. Furthermore, no fungi or viruses were detected in ABLS leaves by PCR, suggesting that the disease is not caused by these agents. These findings suggest that ABLS might be a physiological disorder in plants that is distinct from ABD.

  • Traumatic ventricular septal defect (VSD) resulting from blunt chest injury is a very rare event. The mechanisms of traumatic VSD have been of little concern to dateuntil now, but two dominant theories have been described. In one, the rupture occurs due to acute compression of the heart; in the other, it is due to myocardial infarction of the septum. The clinical symptoms and timing of presentation are variable, so appropriate diagnosis can be difficult or delayed. Closure of traumatic VSD has been based on a combination of heart failure symptoms, hemodynamics, and defect size. Here, we present a case of a 4-year-old boy who presented with a traumatic VSD following a car accident. He showed normal cardiac structure at the time of injury, but after 8 days, his repeated echocardiography revealed a VSD. He was successfully treated by surgical closure of the VSD, and has been doing well up to the present. This report suggests that the clinician should pay great close attention to the patients injured by blunt chest trauma, keeping in mind the possibility of cardiac injury.
