Analysis of prescription frequency of herbs in traditional Korean medicine hospital using electronic medical records

  • Objectives: To analyze the prescription frequency of various herbs as either individual or major herbs (in terms of dosage) and their usage patterns in the treatment of different diseases for standardization of traditional Korean medicine. Methods: We analyzed the prescription database of patients at the Pusan National University Korean Medicine Hospital from the date of establishment of the hospital to February 2013. The complete prescription data were extracted from the electronic medical records of patients, and the prescription frequencies of individual herbs, particularly, of major herbs, were analyzed in terms of gender, age, and international classification of diseases (ICD) code. Results: The prescription frequency of individual herbs based on age and gender showed a similar pattern. Herbal mixtures were also distributed in a similar manner. The use of some herbs differed according to age and gender (Table 1.). The herbs that were used at high frequencies for a given ICD code had similar usage patterns in different categories. However, some major herbs in the "Jun (King)" category were used uniquely for a given ICD code (Table 2.). There was significant difference between male and female on ICD code E and N, but the other ICD codes had small differences. The ratio of herbal medicine by gender showed different usage patterns in each gender. Conclusions: The findings of our study provide fundamental data that reflect the real clinical conditions in South Korea, and therefore, can contribute to the standardization of TKM.

지역약국 방문 성인의 일반의약품 복용실태 및 일반의약품과 처방의약품과의 상호작용 연구 (Patterns of Over-the-Counter Drug Use and Interactions between Over-the-Counter Drugs and Prescription Drugs in Adults Visiting a Community Pharmacy)

  • Objective: The purpose of this study was to evaluate the patterns of Over-the-Counter (OTC) drugs and their interactions with prescription drugs in adults visiting a community pharmacy. Method: The subjects were 151 adults aged over 20 years visiting a community pharmacy in Asan-si from December 16th 2011 to February 1st 2012. We used a survey questionnaire. The survey inquired about the prevalence and the details of any OTC drug use and the characteristics of the study subjects. The drug interaction classification system from Lexicomp's Lexi-interact data fields was used to identify OTC drugs likely to have clinically significant interactions with prescription drugs. Results: The patterns of OTC drug use were related to thirties (from 30 to 40 years old), female gender, higher education, non-smoking, sometimes use of alcohol, and self-perceived normal health status. The most commonly used OTC drug category was antipyretic-analgesics (n=104, 53.3%), and the most commonly used ingredient was acetaminophen (n=67, 64.4%). The biggest motivation for taking OTC drugs was suggestion by pharmacists, reported by 55.6%. After reviewing each patient's prescription drugs and OTC drugs, 14 patients (36.8%) of 38 patients using prescription drugs were taking drug combinations with potential for clinically significant interactions. The concomitant use of OTC drugs with prescription drugs may lead to increased potentially harmful interactions. Conclusion: It is suggested that health-care professionals should be more aware of the potential and possible interactions and take into better account their patients' OTC drug use.

油風의 病因, 病機 및 治療藥物에 關한 文獻的 考察 (A literatual studies on the yupung(油風).)

  • In the literatual studies on the yupung(油風), the results were as follows. 1. Yupung(油風) was called 'Quijidu(鬼지頭)', 'Quichedu(鬼剃頭)', 'Ballak(髮落)' etc. It was a localized loss of hair in round or oval ares without any subjective symptom 2. The etiology and pathogenesis of Yupung were disorder of vital energy& blood circulation caused by deficiency of blood and wind-dryness syndrome, energy-stagnation and blood stasis, impairment of the liver & kidney. The treatments of Yupung were invigorating the liver & kidney, clearing away heat-evil and cooling blood, nourishing the liver & kidney, activating blood circulation to dissipate blood stasis etc, 3. In the frequency of prescription, the most numerous prescription is Shineuingyangjindan(神應養眞丹) and the next are Tonggyuhwalhyultang(通竅活血湯) & Chilbomiyumdan(七寶美髥丹), 4. In the frequency of medicine, the most numerous medicine is Radix Angelicae Sinensis(當歸) and the next are Rhizoma Rehmanniae Praeparatae(熟地黃) & Rhizoma Ligusiici Chuanxiong(川芎). 5. In classification of drug action, medicines of clearing away wind-heat evil and invigorating yin used to be very busy. 6. In classification of four characters, the most parts are warm medicine. 7. In classification of five tastes, the most numerous tastes are sweet and bitter tastes. 8. In classification of toxicity, the most is non-toxic medicines. 9. In classification of the channel distribution, the most is the medicine that belongs to liver channel. 10. In acupuncture theraphy, it was used 'GV20(百會)', 'G20(風池)', 'GV16(風府)', 'GV14(大椎)', 'LI4(合谷)' etc. in body acupuncture, was used 'S25(天樞)', 'GV14(大椎)', 'B13(肺兪)', 'LI11(曲池)' etc. in moxibustion.

동의보감(東醫寶鑑) 외형편(外形篇) 권(卷)1 [두(頭)]부분 관련처방의 재해석 (A Study of Prescriptions of Head Part in DongEuiBoGam)

  • 許浚(Heo Jun), the best physician on his day, wrote 東醫寶鑑(DongEuiBoGam, 1613) based on his clinical experience as a court doctor and life-long study of over 500 medical books. Now most clinicians consult 東醫寶鑑(DongEuiBoGam) for its practicality. And it is also considered as one of the most valuable medical books for basic medical studies. As 許浚(Heo Jun) mentioned on the 東醫寶鑑(DongEuiBoGam) preface. we should practice flexibility on the interpretation of the medical classics instead of binding ourselves to the precedents. But worship of 東醫寶鑑(DongEuiBoGam) may hinder the studying of new pathways. The treatment and its understandings of a disease should be based on objectivity of observation. Finding the common traits of the recipes in 東醫寶鑑(DongEuiBoGam) is important for the study of 東醫寶鑑(DongEuiBoGam). Accordingly, group synthesis of symptoms and resultant classification of recipes are vital. In this research, symptoms listed in the head part(頭門) of External Shape vol. I(外形篇 卷1) of 東醫寶鑑(DongEuiBoGam) are synthesized into 3 different parts: head-wind syndrome(頭風證), dizziness(眩暈), headache(頭痛). Classification of prescriptions according to these 3 parts is also suggested.

지역약국 약사의 처방중재 활동 평가 (Evaluation of Pharmacists' Intervention Activities in Community Pharmacy)

  • Background: Drug-related problems have the potential to threaten patient health, and pharmacists are in a position to prevent such problems through prescription reviews and patient counseling, actively engaging in pharmaceutical care activities. This study aims to categorize and analyze the intervention activities of pharmacists in community pharmacies concerning drug-related problems, following international criteria. Methods: Over a six-month period, prescription interventions completed in a community pharmacy in Seoul were selected as the research subjects. The causes of interventions were classified according to the Pharmaceutical Care Network Europe (PCNE) drug-related problems (DRPs) classification system and the types and frequencies of DRPs were identified. Results: Among a total of 49,334 prescriptions, 527 interventions were completed, constituting approximately 1.07% of the daily average filled prescriptions. Individuals over 60 years of age represented more than 50%. The primary cause of DRPs was prescribing and drug selection issues, comprising 256 cases (48.58%), with specific subcategories including 109 cases of drug selection, 79 cases of treatment duration errors, 47 cases of dose selection, and 21 cases of inappropriate dosage form selection. Patient-related issues accounted for 204 cases (38.71%). Conclusion: The study demonstrated that the pharmacists' intervention in community pharmacies contributes to the safe use of medication by patients.

당(唐), 천금방(千金方)에 기재(記載)된 중풍(中風)에 관한 고찰(考察) (Considerations of Jungpung(中風) in Chungumbang(千金方) at Tang(唐)‘s dynasty)

  • The study of Jungpung(中風)'s section in Chungumyobang (千金要方, 諸風門) by Sonsamak(孫思邈) showed the following results. 1. The cause of Jungpung(中風) in Jungpung(中風)'s section in Chungumyobang (千金要方, 諸風門) was quotated from the theory of Naekyung(內經) -Pung(風) evil from the external environment invade on Hu(虛) of human body(外來風邪, 內虛邪中). 2. At Jungpung(中風)'s section in Chungumyobang (千金要方, 諸風門), he devided into the four classes of Jungpung(中風)'s symptom-Pyungo, Pungbi, Pungyi, Pungbi(偏枯, 風?, 風懿, 風痺)-, and this classification had a great influence on the next generations. 3. At Jungpung(中風)'s section in Chungumyobang (千金要方, 諸風門), he understanded that changes of Jungpung(中風)'s pathology depended on the Eum-Yang-Han-Yul-Hu-Shil(陰陽寒熱虛實) of the patients. 4. At Jungpung(中風)'s section in Chungumyobang (千金要方, 諸風門), he gathered and rearranged various prescription which accumulated the clinical experiences during the period from Naekyung(內經), Sanghan(傷寒) times, to Tang(唐)'s dynasty. There were major Jungpung(中風)'s prescription -Sosokmyungtang(小續命湯), Daesokmyungtang(大續命湯), Jukryuktang(竹瀝湯), Jihwangjun(地黃煎), Dokhwalgisaengtang(獨活寄生湯) etc. And the principle of treatment was the remedy method of removing evil through sweating(發汗祛邪法). 5. Chungumyobang(千金要方) showed the development of quality, quantity in their prescription comparing the fomer ages. And the method of classification of Jungpung(中風)'s symptom had a great influence on the next generation in clinical aspect. But the basic theory in Chungumyobang(千金要方) coudn't get out of the limits of Naekyung(內經), Gumgueyoryak(金?要略).

건강보험 청구자료를 이용한 알츠하이머 치매 치료제와 콜린알포세레이트의 처방 양상 분석 (An Analysis on Prescribing Patterns of Alzheimer's Dementia Treatment and Choline Alfoscerate using HIRA Claims Data)

  • Background: Alzheimer's dementia is the most common dementia. However, recently, choline alfoscerate is prescribed for treating Alzheimer's dementia, although it is not a treatment for this disease. Purpose: To analyze the prescription patterns of choline alfoscerate as a dementia treatment for patients with Alzheimer's disease and to analyze, as well as the factors affecting choline alfoscerate prescription. Method: The 2016 HIRA-NPS data was used in this study. The code of Alzheimer's dementia is F00 in the ICD-10 disease classification code. We analyzed the demographic, clinical, and regional characteristics associated with donepezil, rivastigmine, galantamine, memantine, and choline alfoscerate prescriptions. All statistical and data analyse were conducted by SAS 9.4 and Excel. Results: For patients with Alzheimer's disease, choline alfoscerate was the second most prescribed after donepezil. Analysis results showed that choline alfoscerate was more likely to be prescribed to men than to women, and more likely to be prescribed by local health centers than by medical institutions. Moreover, choline alfoscerate was highly likely to be prescribed at neurosurgical departments, among medical departments. Conclusions: This study confirmed that choline alfoscerate was prescribed considerably for patients with Alzheimer's dementia. Further studies valuating its clinical validity should be performed to clarify whether choline alfoscerate prescription is appropriate for treating Alzheimer's dementia.

다양한 색상 및 형태를 갖는 알약의 자동 분류 시스템 (Automatic Classification System of Tablets with Various Colors and Shapes)

  • The classification of the tablets recovered according to prescription changes is usually carried out manually by a number of pharmacists at the hospitals. Relatively high-wage pharmacists carry out the reclassification of the tablets, which results in a large loss of time and labor, and if the tablets are incorrectly classified, this can lead to medical accidents. In order to overcome these problems, a new automatic tablet classifying machine has been introduced. In the conventional automatic tablet classifying machine, tablets having various shapes, sizes, and colors are transferred to a classifying machine through the line feeder. Problems such as breakaway of the tablets from the line feeder, pilling of the tablets in the line feeder, and appearance contamination of the tablets occur. In this paper, we propose a system that automatically classifies the shape, size, and color of tablets through individual supply method by vacuum adsorption and image processing.

'폐의약품 수거사업'을 통해 지역약국으로 회수된 처방전의약품의 성분 및 약가 분석 (Analysis of Active Pharmaceutical Ingredients and Drug Cost of Prescription Medications Returned to Community Pharmacies through 'Drug-Take Back' Program)

  • Unused medication disposal is a burden due to the cost of disposing as well as the cost of the drugs. Investigating medication returns is expected to suggest areas of intervention to reduce unused medications. Purpose: The aim of this study was to examine types, quantity, costs, active pharmaceutical ingredients, and therapeutic category of the medications returned to community pharmacies. Method: From January 15, 2014 to February 28, 2014, the medications returned to the 17 community pharmacies in Gimhae, Jinju, and Incheon, Korea were examined. The pharmacists and student volunteers worked cooperatively to identify the medications and analyze drug cost of prescription pill medications returned to the pharmacies. Results: A total of 2,720 pills of prescription medication were analyzed and 91 active pharmaceutical ingredients were identified. According to the Anatomical Therapeutic Chemical (ATC) classification, the most predominant group was A (alimentary tract and metabolism) with 33.3%, followed by N (nervous system) with 15.0%. With regard to the drug cost of groups, group A was the highest with 26.6%, followed by J01 (antibacterials for systemic use) with 20.2% and N (nervous system) with 18.3%. The total cost of the oral pill prescription medications was 468,477 won. Conclusion: The result from this study implies that unused drugs impose a significant cost to the health care system in Korea. In this study, medicines used to treat gastrointestinal conditions were returned most frequently with the highest drug cost. Further research in nationwide level is necessary to establish strategies to reduce the wastage of unused medicines.

노인환자의 항정신병 약물 원외처방 내역에 미친 영향 요인 분석 (Analysis of Influencing Factors on the Outpatient Prescription of Antipsychotic Drugs in the Elderly Patients)

  • Background: Most antipsychotic drugs studies have been mainly conducted on side effects, randomized clinical trials, utilization rates, and trends. But there have been few studies on the influencing factors in elderly patients. The purpose of this study was to analyze the influencing factors on the outpatient prescription of antipsychotic drugs in the elderly patients. Methods: Active ingredients of antipsychotic drugs in Korea were selected according to the Korean Pharmaceutical Information Center (KPIC)'s classification. Data source was Korean Health Insurance Review and Assessment Service (HIRA) claims data in 2020 and target patient group was the elderly patient group. We extracted patients who have been prescribed one or more antipsychotic drugs and visited only one medical institution. Data were analyzed using descriptive statistics, chi-square, t-test, negative binomial regression. Results: A number of outpatients were 245,197 and prescriptions were 1,379,092. Most characteristics of patients were 75-85 year's old, female, health insurance type, no disease (dementia, schizophrenia), atypical drugs, cci score (>2) and characteristics of medical institution were neurology in specialty, rural region, general hospitals. Results of regression showed that patient's characteristics and medical center characteristics had significant effect on the outpatient prescription of antipsychotic drugs in the elderly patients. Conclusion: This study suggests that national policy of antipsychotic drugs in the elderly patients, with the consideration of the patients' and medical institutions' characteristics, is needed.
