  • Dental treatment of mentally challenged patients under general anesthesia is a series of challenging procedures not only for dental operators but also for dental anesthesiologists. Patients presenting with uncooperative behavior often resist the perioperative management for adesthestic administration. This case report suggests oral premedication as a conjuctive method for anestheitic induction. A 26-year-old male dental patient with autism was referred to dental treatment under general anesthesia. The patient refused to enter dental clinic office and was not able to receive preoperative assessment. In the day of operation, 15 mg of midazolam was given to the patient for oral premedication prior to anesthetic induction. Ater 20 minutes, the patient presented with drowziness and was transferred to the office. Anesthestic staff were able to achieve appropriate intravenous access and mask inhalation. The patient recieved 8 hrs long dental treatment and recovered in a noncomplicated way. Oral midazolam is commonly used to reduce anxiety for combative and irritated pateints. In this case, oral midazolam sedation was used as a preanesthetic management of a highly uncoopearive patient.

정형외과 환자의 수술 후 변비 발생과 영향 요인 (Constipation in Patients following Orthopedic Surgery: Incidence and Influencing Factors)

  • Purpose: The purpose of this study was to identify the occurrences and the factors contributing to constipation in the post surgical period following major orthopedic surgery. Methods: The sample included 133 patients who had surgery of the hip, knee, or spine. Patients were excluded from the study if there was a history of bowel surgery or constipation from chart review. Data were collected using questionnaires such as Korean version of Modified Barthel index (K-MBI), Hospital Anxiety and Depression Scale, and Constipation Assessment Scale (CAS) before surgery and on the third postoperative day. Results: Reports of constipation occurred in 77 instances (57.9%). The first reported defecation was within $4.0{\pm}2.2days$ following surgery. Mean days of first defecation of constipation group vs. non-constipation group was $5.5{\pm}1.7days$ vs. $2.1{\pm}1.0days$. There were statistically significant differences between patients who reported than those who did not in terms of age, length of NPO period, postoperative ADL, and the reported incidents of preoperative and postoperative depressive symptoms. Postoperative ADL, age, and NPO period were significant influencing factors of constipation and explained 52.4% of the variance. Conclusion: Constipation is a very common symptom for patients undergoing orthopedic surgery. These results indicate a need for improving patient's ADL after surgery to prevent constipation.

장애환자에서 과도한 감염치아 발치전 스트레스 감소법으로서 배농술의 효과 : 문헌적 고찰 및 증례보고 (EFFECT OF DRAINAGE AS A STRESS REDUCTION METHOD BEFORE EXTRACTION OF ADVANCED INFECTED TEETH IN DISABLED PATIENTS : REVIEW OF LITERATURE & REPORT OF CASES)

  • Dental extraction is potentially stress-inducing in many disabled patient. The body's response to dental stress involves the cardiovascular system(an increase in cardiovascular workload), the respiratory organ and the endocrine system(change in metabolism). To minimize the stress, the stress reduction method was established. The obtained contents were as follows: (1) Recognize the patient's degree of medical risk, (2) Complete medical consultation before dental therapy, (3) Schedule the patient's appointment in the morning, (4) Monitor and record preoperative and postoperative vital signs, (5) Use psychosedation during therapy, (6) Use adequate pain control during therapy, (7) Short length of appointment : do not exceed the patient's limits of tolerance, (8) Follow up with postoperative pain/anxiety control, (9) Telephone the risk patient later on the same day that treatment was given. Though the stress reduction method above was applied to the dental extraction in disabled patients with the advanced infected teeth, the complications(syncope, shock, bleeding & infection, etc.) may be occurred. For prevention of complications associated with the extraction, the authors treated the advanced infected teeth with endodontic drainage and incision & drainage before extraction. The final extraction and wound closure were then done after about 3 weeks.

To minimize the stress to the medical risk patient, the stress reduction protocol was established. This protocol is predicated on the belief that the prevention of or reduction of stress ought to begin before the start of an appointment, continue throughout treatment, and, if indicated, into the postoperative period. The authors used the stress reduction protocol in the care of local anesthesia infected teeth in medically compromised patients. The final prognosis was comfortable without any complications.

3차원 동적집속모드 체외충격파 기기를 이용한 족저근막염 치료의 유용성 (The Efficacy of Three-Dimensional Sweeping Mode Extracorporeal Shockwave Treatment for Plantar Fasciitis)

  • Purpose: This was a pilot study to examine the clinical usefulness of the newly developed three-dimensional sweep mode extracorporeal shockwave treatment (ESWT) in patients with plantar fasciitis. Materials and Methods: Three-dimensional sweep mode ESWT was performed once a week for 5 weeks in patients with plantar fasciitis who showed no improvement with the conventional conservative treatment. A 100-mm visual analogue scale (VAS) reading for pain from walking and at rest after walking were collected before the treatment and 8 and 16 weeks after the initial treatment. In addition, the Foot and Ankle Outcome Score (FAOS) and EuroQol-5-dimension (EQ-5D) scores before and 16 weeks after the treatment were evaluated. Results: VAS for pain for walking improved from 50.60±8.38 to 19.80±15.61 at 8 weeks after the initial treatment (p=0.008) and 9.80±9.62 at 16 weeks after the treatment (p<0.001). VAS for pain at rest after walking improved from 36.60±19.55 to 11.80±12.95 at 8 weeks after the initial treatment (p=0.052) and 8.80±8.87 at 16 weeks after the treatment (p=0.024). Preoperative FAOS increased from an average of 74.80±9.73 before the treatment to an average of 81.00±8.86 at week 16 after the procedure (p=0.49) and compared to pre-treatment levels, there was a decrease of one level in the anxiety/depression domain of the EQ-5D, post-treatment. Conclusion: The results of this preliminary study confirmed that the newly developed EWST with the smart forging sweep mode was effective in improving pain and function in plantar fasciitis.

  • Backgrounds/Aims: Pancreatoduodenectomy (PD) is recommended in fit patients with a carcinoma (PDAC) of the pancreatic head, and a delayed resection may affect survival. This study aimed to correlate the time from staging to PD with long-term survival, and study the impact of preoperative investigations (if any) on the timing of surgery. Methods: Data were extracted from the Recurrence After Whipple's (RAW) study, a multicentre retrospective study of PD outcomes. Only PDAC patients who underwent an upfront resection were included. Patients who received neoadjuvant chemo-/radiotherapy were excluded. Group A (PD within 28 days of most recent preoperative computed tomography [CT]) was compared to group B (> 28 days). Results: A total of 595 patents were included. Compared to group A (median CT-PD time: 12.5 days, interquartile range: 6-21), group B (49 days, 39-64.5) had similar one-year survival (73% vs. 75%, p = 0.6), five-year survival (23% vs. 21%, p = 0.6) and median time-to-death (17 vs. 18 months, p = 0.8). Staging laparoscopy (43 vs. 29.5 days, p = 0.009) and preoperative biliary stenting (39 vs. 20 days, p < 0.001) were associated with a delay to PD, but magnetic resonance imaging (32 vs. 32 days, p = 0.5), positron emission tomography (40 vs. 31 days, p > 0.99) and endoscopic ultrasonography (28 vs. 32 days, p > 0.99) were not. Conclusions: Although a treatment delay may give rise to patient anxiety, our findings would suggest this does not correlate with worse survival. A delay may be necessary to obtain further information and minimize the number of PD patients diagnosed with early disease recurrence.

The obtained contents were as follows: (1) Recognize the patient's degree of medical risk (2) Complete medical consultation before dental therapy (3) Schedule the patient's appointment in the morning (4) Monitor and record preoperative and postoperative vital signs (5) Use psychosedation during therapy (6) Use adequate pain control during therapy (7) Short length of appointment : do not exceed the patient's limits of tolerance (8) Follow up with postoperative pain/anxiety control (9) Telephone the higher medical risk patient later on the same day that treatment was given Though the stress reduction protocol above was applied to the dental extraction in medically compromised patients with the advanced infected teeth, the final responsibility for the complications(syncope, bleeding & infection, etc.) in a patient rests with the dentist who ultimately treats him. For the prevention of postextraction complications & poor prognosis, the authors treated the advanced infected teeth with the pulp extirpation, opening drainage through the canal and complete occlusal reduction. The final extraction and wound closure were then done after $1{\sim}2$ weeks. The final prognosis was comfortable without common complications.
