Association of Pre- and Perinatal Risk Factors With Tourette Syndrome or Chronic Tic Disorders in a Korean School-Age Population

  • Objectives: Tic disorders are highly heritable; however, growing evidence suggests that environmental factors play a significant role in their pathogenesis. Studies on these factors have been inconsistent, with conflicting results. Therefore, this study aimed to examine the associations of pre- and perinatal exposure to Tourette syndrome (TS) or chronic tic disorders (CTD) in Korean school-aged children. Methods: This case-control study used data from a large prospective cohort study. The primary outcome was TS/CTD diagnosis according to the Diagnostic and Statistical Manual of Mental Disorders, 5th edition (DSM-5) criteria and Kiddie-Schedule for Affective Disorders and Schizophrenia-Present and Lifetime Version-Korean Version. Demographic, pre-, and perinatal information was obtained from the maternal questionnaires. Data between the TS/CTD and control groups were compared using the chi-squared or Student's t-test, as appropriate. Two-step logistic regression analyses were used to test the association between TS/CTD and pre- and perinatal risk factors. Results: We included of 223 children (78 with TS/CTD and 145 controls). Significant differences in the demographic data between the two groups were observed. The male sex ratio, mean parental age, parental final education level, and family history of tics were included as confounders. In the final adjusted multivariable model, TS/CTD was significantly associated with antiemetic exposure during pregnancy (odds ratio [OR]=16.61, 95% confidence interval [CI] 1.49-185.22, p=0.02) and medically assisted reproduction (OR=7.89, 95% CI 2.28-27.28, p=0.01). Conclusion: Antiemetic exposure and medically assisted reproduction are significantly associated with the risk of TS/CTD. These results should be replicated in future prospective and gene-by-environment studies.

An Ethnographic Research Study on Childbearing Process of Mother with Children in Korea (자녀를 둔 어머니의 출산과정 경험)

  • The childbearing process is a sociocultural phenomenon of a woman who gives birth to a child as well as a biological phenomenon. The purpose of this ethnographic research study was to explore the experience of childbearing process of mothers with children from pregnancy to the 3 months postpartum in Korea and to understand deeply the perspectives of childbearing women reflected on Korean sociocultural values. A convenient sample of 10 childbearing women were observed from January to October 2000 through field work in Seoul, Korea. Data analysis was accomplished under ongoing process. The results of this study were as follows : The mothers with children experienced self-reflection, family relation, and physical adaptation during pregnancy. In self-reflection, all mothers experienced universality and diversity in their self-discovering process. The universal experiences were maturation, life with family and priority on maternal value between being a mother and a woman. The diverse experiences were taking a dual role of working mother, emotional drift of a resigned mother, and disheartened life of a mother who has two daughters. In family relation, the foundation of the new marital relationship were attained during childbearing process and sexual life were changed for the benefit of a healthy mother and a healthy baby. All mothers established friendly relations with their mothers, but established friendly or conflicting or constraining relations with their mother-in-laws due to husband based family culture. In physical adaptation, the informants endured well the physical discomfort and recognized general appearance change. Also maternal-fetal interaction occurred and mothers realistically felt motherhood and accepted themselves as mother-to-be. The mothers prepared for the best delivery, look for a safe childbirth center, newborn goods, endorsed family coping during hospitalization and responded labor pain to make it more endurable, less painful, fast passed owing to labor recognition of the natural process to be a mother. After childbirth, they felt emancipation, satisfaction, accomplishment, more easiness, actually feeling as mother-to-be, emptiness, and showed response to the sex of newborn. Their Sanhujori practice was different according to the Sanhujori environment including provider, place, time in postpartum and reflected on Sanhubyung. The mothers felt actually mother-to-be and happiness during lactation regardless of feeding pattern. These mothers had a different maternal image about rearing subjecthood through their child-rearing experience. But all mothers felt need for family support and social support. The universal rearing response were actual feeling of mother-to-be, a strenuous experience, a pride on child-rearing, confusion, reflecting marital relationship, and wondering rivalry among children. In conclusion, mother of all with children went through self-discovery, self-reflection and made connections with the family as a mother and as a woman simultaneously during the childbearing process. Therefore it is suggested when harmony and balance between a mother and a woman is accomplished, the woman will lead a healthy and high quality of life. Also, this study sought to confirm the sociocultural factors affecting the childbearing process from the perspectives of the women with children. Therefore health care providers must understand deeply the childbearing women with children based on this finding of and try a integrative approach with new ideology of maternity with biocultural perspectives in a clinical setting.

Effects of Supplementing Brown Seaweed By-products in the Diet of Holstein Cows during Transition on Ruminal Fermentation, Growth Performance and Endocrine Responses

  • This study was conducted to examine the effects of supplementing brown seaweed by-products (BSB) in the diet of ruminants on ruminal fermentation characteristics, growth performance, endocrine response, and milk production in Holstein cows. In Experiment 1, the effects of different levels (0%, 2%, and 4% of basal diet as Control, 2% BSB, 4% BSB, respectively) of BSB were evaluated at 3, 6, 9, 12, and 24 h in vitro batch culture rumen fermentation. The pH tended to be higher for the higher level of BSB supplementation, with the pH at 12 h being significantly higher (p<0.05) than that of the control. The concentration of ammonia nitrogen was lower at 3, 9, 12, and 24 h incubation (p<0.05) compared with the control, and tended to be low at other incubation times. Volatile fatty acid concentration appeared to be minimally changed while lower values were observed with 4% BSB treatment at 24 h (p<0.05). In Experiment 2, effects of levels (0%, 2%, and 4%) of BSB on growth performance, endocrine responses and milk production were studied with Holstein dairy cows during transition. Dry matter intake, daily gain and feed efficiency were not affected by BSB supplementation. The concentration of plasma estrogen for the control, 2% BSB and 4% BSB after three months of pregnancy were 55.7, 94.1, and 72.3 pg/mL, respectively (p = 0.08). Although the differences of progesterone levels between BSB treatments and the control were minimal, the concentration in 4% BSB treatment increased to 157.7% compared with the initial level of the study. Triiodothyronine and thyroxine levels were also higher after both three months and eight months of pregnancy than the initial level at the beginning of the study. In addition, BSB treatments during one month after delivery did not affect daily milk yield and composition. In conclusion, the present results indicate that supplementation of BSB did not compromise ruminal fermentation, and animal performance at lower levels and hence may have potential to be used as a safe feed ingredient in dairy cows.

A Study on the Factors related to postpartum Depression in Postpartum Women (산욕기 산모의 산후 우울증에 영향을 미치는 요인)

  • The purpose of this study was to provide the basic, data for developing a program for effective prevention for Postpartum Depression (PPD) by investigating the level of PPD in postpartum 2 weeks women. The subjects were 384 women who visited obstetrical clinics for postnatal care. The data were collected from June 29. 1999 to April. 2000, using a 46-item questionnaire related to PPD, and analyzed by SAS program for t-test, ANOVA, Scheffe test, stepwise multiple regressions. The results were as follows : 1. The level of PPD according to general characteristics Women had mild PPD (Min score; 46.0, Max score; 124.0). The PPD levels were significantly differences according to religion and marital satisfaction (p<0.05). 2. The level of PPD according to obstetrical characteristics 1) Characteristics related to pregnancy The PPD levels were significantly differences according to mood change, confidence of body weight recovery, depression related to appearance change, husband's help to housework, and husband's emotional support (p<0.05). 2) Stressful events during pregnancy The PPD levels were significantly differences according to financial problem, conflict between partners, conflict between family, and husband's job change (p<0.05). 3) Characteristics related to delivery and post natal period The PPD levels were significantly differences according to baby's health state, parenting confidence, and difficulties related to postpartum care (p<0.05). 3. The variables to predict postpartum depression in postpartum women are depression related to appearance change (10.4%), parenting confidence (8.8%), husband's help to housework (2.7%), confidence of body weight recovery (2.4%), husband's job change (1.9%), baby's health state (1.9%), difficulties related to postpartum care (1.6%), mood change (1.2%), conflict between partners (0.6%), marital satisfaction (0.5%), financial problem (0.4%). The sum total of all the above variables can account for 32.4% of postpartum depression. 4. The level of PPD according to PPD factors. Women had the highest degree of PPD in biophysiological phenomena-disturbance of physical functioning factor. The factors of relationship to baby-negative feeling and cognitive phenomena-self concept disturbance were showed the lowest degree of PPD. As a result of the above findings, a systemic and individualized program is strongly recommended for PPD prevention, diagnosis, and care for PPD in postpartum women. In near future, this study should be expanded to investigate the coping skills according to the PPD levels in postpartum women.

Effects of Self Efficacy, Body Image and Family Support on Postpartum Depression in Early Postpartum Mothers (산욕초기 산모의 산후우울에 자기효능감, 신체상, 가족지지가 미치는 영향)

  • The purpose of this study was to determine the self efficacy, body image and family support affect on postpartum depression in early postpartum mothers. The data was collected 306 mothers on the first week after delivery from one hospital located in B city. The instruments were a survey of general characteristics, obstetrical characteristics, the Edinburgh Postnatal Depression Scale(EPDS), self-efficacy, body image and family support. Data was analysed using descriptive statistics, Pearson correlation coefficients, and stepwise multiple regression. The results revealed that the average item score of the EPDS was 6.09. The significant predictors of postpartum depression were body image, emotional state in pregnancy, and family support. These variables had a 38.4% explainability. The results indicate that the postpartum mothers should be screened for postpartum depression early in the postpartum period and it's necessary to implement nursing intervention focused on to enhance the body image and family support, especially the weak emotional state mothers in pregnancy.

Clinical Study on the Effect of Chuna Manupulation in Treating Postpartum Patients with Low Back Pain and Pelvic Girdle Pain (산후 요통 환자의 추나 수기 치료 효과에 대한 임상적 고찰)

  • Objectives: This study was designed to observe the effect of chuna manupulation in alleviating the symptoms of postpartum patients with low back pain and pelvic girdle pain. Methods: Targeted by outpatient 30 postpartum patients, A group was patients who were treated with acupuncture, traditional Korean medicine, physical therapy, B group was patients who were treated with acupuncture, traditional Korean medicine, physical therapy, and chuna manupulation. After 2-months, the progress of symptoms was evaluated by visual analogue scale(VAS) and Oswestry Disability Index(ODI) and Pain drawing. Results: 1. Group A and B were analyzed for general characteristics, with an average age of both groups, average weeks at admission after delivery, parity and method of delivery, weight gain and BMI during pregnancy were no significant differences in. 2. VNRS improvement in the rate of group A was $2.84{\pm}4.73$, VNRS improvement in the rate of group B was $4.85{\pm}1.49$. Between the two groups was statistically significant. 3. ODI improvement rate in group A was $10.14{\pm}4,39$, ODI improvement rate in Group B was $15.16{\pm}3.41$. Between the two groups was statistically significant. 4. Patients who received acupuncture, traditional Korean medicine, physical therapy, and chuna manupulation. in group B compared pubis, thighs, buttocks showing more effective in pain relief than group A, who received only acupuncture, traditional Korean medicine, physical therapy. Conclusions: In case of postpartum patients with low back pain and pelvic girdle pain, it is more effective to treat with Integrated traditional Korean therapy and chuna manupulation than to treat with only integrated traditional Korean therapy.

Inter-hospital Comparison of Cesarean Section Rates after Risk Adjustment (위험도 보정을 통한 병원간 제왕절개 분만율의 비교)

  • Objective : To determine the clinical risk factors associated with the mode of delivery decision and to compare cesarean section rates after adjusting for risk factors identified among Korean hospitals. Methods Data were collected from 9 general hospitals in two provincial regions by medical record abstraction during February 2000. A total of 3,467 cases were enrolled and analyzed by stepwise logistic regression. Performance of the risk-adjustment model (discrimination and calibration) was evaluated by the C statistic and the Hosmer-Lemeshow test. Crude rates, predicted rates with 95% confidence intervals, and adjusted rates of cesarean section were calculated and compared among the hospitals. Results : The average crude cesarean section rate was 53.2%, ranging from 39.4% to 65.7%. Several risk factors such as maternal age, previous history of cesarean section, placenta previa, placental abruption, malpresentation, amniotic fluid abnormality, gestational anemia, infant body weight, pregnancy-induced hypertension, and chorioamnionitis were found to have statistically significant effects on the mode of delivery. It was confirmed that information about most of these risk factors was able to be collected through the national health insurance claims database in Korea. Performance of the risk-adjustment model was good (c statistic=0.815, Hosmer-Lemeshow test=0.0621). Risk factor adjustment did lead to some change in the rank of hospital cesarean section rates. The crude rates of three hospitals were beyond 95% confidence intervals of the predicted rates. Conclusions : Considering that cesarean section rates in Korean hospitals are too high, it is apparent that some policy interventions need to be introduced. The concept and methodology of risk adjustment should be used in the process of health policy development to lower the cesarean section rate in Korea.

Estimation of Manganese Levels in Cord Blood of Pregnant Workers (임신 여성 근로자의 제대혈 중 망간 농도 추정)

  • Objectives: This study aims to analyze manganese (Mn) concentrations in maternal and cord bloods at delivery and to estimate the Mn exposure risk for fetuses whose mothers were occupationally exposed to Mn. Materials and Methods: Forty-six pairs of maternal and cord blood samples were collected at delivery from mothers who were occupationally unexposed to Mn. Mn concentrations of blood were analyzed by graphite furnace atomic absorption spectrometer. Mn exposure levels for fetuses of female workers were estimated by simulating two working exposure scenarios. Results: The geometric mean concentration of Mn in maternal and cord blood were $27.0(1.34){\mu}g/L$, $46.6(1.25){\mu}g/L$, respectively. Transfer ratios of Mn from maternal to cord blood were $1.81{\pm}0.62$, which indicated that the Mn concentrations in cord blood were higher than those in maternal blood. Mn concentrations in cord blood for the worse or general scenarios were estimated to $22.3-1,881{\mu}g/L$ and $1.59-308{\mu}g/L$, respectively. The probabilities of exceeding $74{\mu}g/L$, which was adopted as a reference level reported in a previous study, were 95% and 44% for the two scenarios, respectively. Conclusions: Comparable levels of Mn exposure in maternal or cord blood to those in this study have shown various health effects in previous studies. This suggests that Mn exposure levels in mothers and fetuese in Korea need to be monitored and managed. In addition, female workers who are occupationally exposed to Mn should be protected from the exposure since their fetuses can be exposed to Mn at risky levels during their pregnancy.

The Trend of Nursing Research in Korea on Mother-Child Relationship (모아관계에 관한 국내 간호연구 동향)

  • Although nursing researchers have performed studies on parent-child relationship actively since early 1980s in Korea, nothing hasn't been tried on the analysis of the contents of researches. We researchers tried examining the trend of researches by investigating treaties on mother-child relationship conducted from 1981 to 1994. The purpose of this study was to analyze the trend of research on the mother-child relationship and to suggest direction for future study. The total numbers of the studies were 27 cases, These studies were analyzed for 1) time of publication 2) research design 3)main concept 4)thesis for a degree or nondegree 5) measurement tool The finding of the analysis were as follows. 1) In the chronological situation, the research on mother-child relationship began to perform in 1981 in the country and had been made most for five years to 1986 with 14 treaties and has decreased since 1990. 2) In the research plan, there are 21 survey(the most) ,5 quasi experimental(before 1986), 1 qualitative. 3) In the subject, the researches on mother-child are the most, 22 and of them 17 treaties on normal infant,4 comparative research on high risk infant and normal infant,5 treaties on mother, 4) The conception of mother-child relationship was used as various conceptions such as affection, union, bonding in early research of 1980s, but it hasn't been used as union bonding but studied mostly as the conception of 'mother-child interaction'since 1986. 5) The measurement tool of research was used variously, the Cropley's tool was used in early 1980s, but Walker's MIPS, Barnard's NCAST were used mostly in 1990. 6) Mother-child interaction influence on mother's self-conception, stress, care and feeding of child, frequency of delivery, planned pregnancy and delivery and health condition of child, birth weight, fetal period, period of mother-child life. Aspects of mother-child interaction composed on visual contact, looking at each other, skinship and caress with finger and fingertip. The leading action of mother-child interaction was mostly uttering action. 7) I suggest that the program for professional education is necessary for the establishment of the aspects of mother-child interaction.

  • Obstetrics and Gynecology include gynecology which is concerned with the treatment for the disease based on physiology and pathology of women, and obstetrics which is concerned with pregnancy delivery. These obstetics and gynecology can be said to date from the birth of human beings. This paper has carried on the studies about the generating and developing process of obstetics and gynecology. The results of this study are as follows : Distinction in Northern and Southern Dynasties were much more Obstetrics and Gynecology studying the Obstetrics and Gynecology books in that time, the study about "oncosis" (腫瘤) and forms of drug, pill, powder, pIaster were carried on. The highest distinction in Sui age were research and development on pathogenesis and cause of disease about Obstetrics and Gynecology disease and it was appeared representatively in "Zhubingyuanhoulun""諸病源候論" which was special book on cause of disease and pathogenesis and syndrome, And it was explained about abnormal vaginal discharge in this book. Obstetrics and Bynecology were developed on theory and clincis in Tang age when "Taiyshu"(太醫暑) was established. "Taiyshu"(太醫暑) taked complete charge of medical operation and education, published much excellent Obstetrics and Gynecology books than the past. Among them specially "Qianjinyaofang""千金要方" explained Obstetrics and Gynecology matter first in three volumes and "Qianjinyifang""千金翼方" explained this matter first in four volumes. This point indicate that Obstetrics and Gynecology's importance was established in Tang age, so Obstetrics and Gynecology's developmental history, Song age was the acme period. "Taiyju"(太醫局) was established in "xining""熙?" 9 year and Obstetrics belonged to one of nine departments under "Taiyun"(太醫局), so Obstetrics and Gynecology was separated from the other medical parts and it was prepared of it's progressing bases independently and appeared of changes in quality and increase in quantity about theory and prescription of Obstetrics and Gynecology, specially determination of treatment based on the differentiation of symptoms and sighs was taked a serious view. Besides four great physicians in Jing and Yuan dynasies diversified Obstetrics and Gynecology contents through their only medical theory and clinical treatment.
