간호대학생의 표준주의 수행에 미치는 영향요인 연구 (A Study on Factors Affecting Nursing Students' Standard Precautions Compliance)

  • 간호대학생의 표준주의 지식, 표준주의 태도, 사회적 지지 및 표준주의 수행의 관계를 파악하고, 표준주의 수행에 영향을 미치는 요인을 규명하기위해 시도된 서술적 조사연구이다. 연구대상자는 G시에 소재한 3개 대학교 간호대학생 170명을 대상으로 하였다. 자료분석은 SPSS 24.0 프로그램을 이용하였으며 multiple regression로 분석하였다. 연구결과 간호대학생의 표준주의 수행에 영향을 미치는 요인은 사회적 지지, 표준주의 지식 순으로 정적 영향을 미치는 것으로 나타났다. 따라서 간호대학생의 표준주의 수행을 높이기 위해 표준주의 지식의 함량을 증진시키고, 사회적 지지를 긍정적으로 인식하고, 강화시킬 수 있도록 구성된 체계적 교육전략과 프로그램을 개발해야 할 것이다.

  • Purpose: The purpose of this study was to investigate nursing students' awareness and performance on standard precautions and to provide meaningful information for nursing students' education regarding hospital infection control. Method: Four hundred forty seven nursing students at 6 universities in D-city participated in this study. Data collection was conducted from March to April 2011. Students' awareness and performance on standard precautions of infection control in hospitals were measured using the modified 2007 CDC standard precautions guidelines. Data analysis was performed using the SPSS WIN 18.0 program, descriptive statistics, t-test, and ANOVA. Results: The level of students' awareness in the standard precautions was higher than performance. The higher levels of students' awareness and performance on standard precautions included patient care equipment, safe injection practices, and worker safety. The lower levels of students' awareness and performance on standard precautions included hand hygiene and personal protective equipment. There was no difference in the students' awareness and performance of standard precautions according to their characteristics. Conclusions: Based on the findings of this study, hand hygiene and personal protective equipment need to be stressed more within the program to improve nursing students' infection control techniques.

임상 간호사의 혈액매개감염 주의지침에 대한 수행정도 (A Study on Performance Level for Universal Precautions on Blood-Borne Infections among Nurses in Hospitals)

  • Purpose: This study was conducted to investigate the cognition level and the performance level of universal precautions on blood-borne infections and to analyze related-factors on the performance level for universal precautions among the nurses. Method: A total of 166 nurses participated in the survey. We performed multiple linear regression to assess the related-factors on the performance level for universal precautions. Result: The mean scores of cognition and performance level for universal precautions were 65.61(${\pm}8.74$) and 53.98(${\pm}8.42$) respectively. The mean score of the cognition level was significantly lower than the performance level in all items. The cognition level was associated with the working field. The performance level was highest for nurses working over 36 years, nurses in university hospitals, and nurses in the operating room. The cognitive level for universal precautions, education level, exposure to blood-borne infection, family's income, and marital status significantly predicted the performance level for universal precautions in the multiple linear regression model (Adjusted $R^2=0.42$). Conclusion: The cognition level for universal precautions is the strongest predictor on the performance level for universal precautions. An integrated approach should incorporate training experiences to improve cognition and perception of risk on universal precautions for preventing blood-borne infections.

대학병원 의료종사자들의 병원감염에 대한 예방지침 실행수준과 관련요인 (Compliance Level of Universal Precautions to Hospital Infection and related factors of Health Care Workers in a University Hospital)

  • The purpose of this research is to suggest basic materials for the practical infection precaution program to protect health care workers from hospital infection by grasping their compliance level of Universal Precautions and examining the factors affecting them. The number of the health care workers we studied were 486, including the doctors, the nurses, and the lab technicians who were working in a university hospital. The period of this research was from Aug. 18th, 1997 to Aug. 30th, 1997. The method of the study was to measure the compliance level of Universal Precautions with the item of "Universal Precautions" established by CDC in 1987, and examine the questionnaire of 52 questions dividing related factors into socio-populational, individual, socio-psychological and organizational management ones. The data was analyzed by t-test. ANOVA, and chi-square test. The results were as follows : 1. An the compliance level of Universal Precautions, hand washing had the highest score(85.4%), and doctors(18.9%), nurses(44.0%), and lab technicians(7.6%), had a low compliance level in the safe handling of an injection syringe, and item not to handle patients and their samples when the subject suffered from dermatitis or injury had the lowest score of 17.1%. 23.3% of them said that they wear protection gown, goggles and mask. 2. Female's Compliance level of Universal Precautions Was higher than male. 3. The health care workers who had high recognition on Universal Precautions got significantly higher compliance level of Universal Precautions than those have low recognition on Universal Precautions(P<0.001). 4. The health care workers experienced a needle stick injury had a significantly higher compliance level of Universal Precautions than those who had not(P<0.000). 5. The health care workers who had infection protection education got a significantly higher compliance level of Universal Precautions than those who didn't(P<0.000). 6. The health care workers who had a firm belief in the effect of Universal Precautions got a higher compliance level of Universal Precautions than those who didn't. 7. The health care workers who had less conflicts between treating patient arid protecting them-selves got a higher compliance level of Universal Precautions than others with many conflicts. 8. The health care workers who had a high score in organizational management factors got a significantly higher compliance level of Universal Precautions than those with a low score(P<0.000). 9. Only 16.9 percent of the all respondents(82 in number) answered that they knew well or a little about the Universal Precautions, which is very low rate of recognition. 10. The variables which affected the score in organizational management factors were age, sex, education period, work experience, the kind of work, recognition on Universal Precautions, the experience of needle stick injury, revealing dangerous circumstance related to infection, and training on precaution again infection. According to the result above, compliance level of Universal Precautions showed high correlation with sex, the recognition on Universal Precautions, the experience of needle stick injury, training on precaution against infection, the belief in the effect of Universal Precautions, the recognition degree of conflicts and organizatinal management factors. These results could be used as the basic materials for the developing infection protection programs. Also, There should have a systematic training course to elevate a effective compliance level of Universal Precautions as well as the manageeent of infection protection programs.

공공의료기관 간호사의 감염 표준주의지침 수행도 영향요인 : 지식, 안전환경 중심으로 (Factors Affecting Nurses' Performance of Standard Precautions for Infection in Public Medical Institutions: Focusing on Knowledge and Safety Environment)

  • 본 연구는 공공의료기관에 근무하는 간호사의 감염 표준주의지침에 대한 지식, 안전환경, 수행도를 조사하고 표준주의지침에 대한 수행도에 미치는 영향 요인을 확인하기 위한 서술적 조사연구이다. 연구방법은 S시, N시의 2개의 공공의료기관 간호사 192명을 대상으로 설문지를 통해 자료를 수집하였고 SPSS WIN 22.0 통계프로그램을 사용하여 결과를 분석하였다. 연구결과 표준주의지침 지식은 근무경력에 따라 유의한 차이가 있었으며 표준주의지침 안전환경은 직위에 따라 유의한 차이가 있었다. 표준주의지침 지식 정도는 평균 21.98(25점 만점), 안전환경은 평균 5.68(7점 만점), 수행도는 평균 40.28(60점 만점)이었으며, 표준주의지침 안전환경과 표준주의지침 수행도(r=.21, p<0.01)는 유의한 양의 상관관계가 있었다. 표준주의지침 수행도에 미치는 영향요인은 표준주의지침 안전환경(𝛽=.22, p=.003)으로 나타났다. 따라서 공공의료기관에서 간호사의 감염관리를 위한 표준주의지침 수행도를 증진 시키기 위해서는 재정적 지원 및 지지적 안전환경을 제공하고 현 상황을 고려한 차별화된 감염관리 교육프로그램과 모니터링이 요구된다.

일개 종합병원 의료종사자 직종별 표준주의 인지도와 수행도 비교 (Awareness and Performance for Standard Precautions among Health Care Workers in a General Hospital)

  • Purpose: The purpose of this study was to explore health care workers awareness and performance of standard precautions. Methods: Participants were 296 health care workers including nurses, physicians, and medical technicians. Awareness and performance of standard precautions were measured with 4-point Likert scales. The data were analyzed with t-tests and one-way ANOVA by using SPSS 18.0. Results: The mean scores of awareness were 3.72 in nurses, 3.62 in physicians, and 3.47 in medical technicians. There was a significant difference of awareness by occupational classification (F=12.39, p<.001). The mean scores of performance of standard precautions were 3.45 in nurses, 3.19 in physicians, and 3.23 in medical technicians. There was a significant difference of performance by occupational classification (F=10.98, p<.001). In addition, the score of performance of standard precautions was significantly lower than that of awareness (t=11.89, p<.001). Conclusion: The results of this study indicated that awareness and performance of standard precautions were different by occupational classification. To improve performance of standard precautions in hospitals, it is necessary to provide a distinct infection control program by occupational classification.

중환자실과 응급실 간호사의 표준주의 수행에 영향을 미치는 요인 (Factors Influencing Compliance with Standard Precautions in Intensive Care Unit and Emergency Room Nurses)

  • Purpose: The purpose of this study was to investigate the level of knowledge, attitude and compliance with standard precautions and to identify predicting factors of compliance with standard precautions in ICU and ER nurses. Methods: The participants were 228 ICU and ER nurses working in one of three university hospitals and one general hospital. Collected data included general characteristics, features related to infection risk, and knowledge, attitude and compliance with standard precautions. For the final analysis, 218 questionnaires were used. Results: Within the previous one year, 103 (47.3%) nurses experienced pricking injury from syringe needles or other sharp materials, 111 (50.9%) nurses reported exposure to patients' blood and body fluid. In general, the scores for knowledge, attitude and compliance were all high. Compliance scores for nurses in intensive care units were significantly higher than those of nurses in emergency. There were significant correlations of knowledge, attitude, and compliance with standard precautions. Attitude and work place were significant factors predicting compliance with standard precautions. Conclusion: The results indicate that to increase nurse's compliance with standard precautions, continuous efforts to enhance positive attitudes and at the same time, improve work environments and use individualized approaches based on the work units are needed.

병원간호사의 감염예방 표준주의지침 지식, 인식, 안전환경 및 수행도에 관한 연구 (Knowledge, Perception, Safety Climate, and Compliance with Hospital Infection Standard Precautions among Hospital Nurses)

  • Purpose: The purpose of this study was to examine knowledge, perception, safety climate and compliance with hospital infection standard precautions and to identify the factors influencing nurse's compliance with standard precautions. Methods: Using the structured survey, data were collected from 292 nurses working at 4 general hospitals in the metropolitan city Seoul in October, 2008. Data were entered and analyzed with SPSS 12.0. Results: The majority of nurses were female with a mean age of 27.8. The mean score for knowledge of standard precautions was 21.2 out of 25. The nurses lacked knowledge on reusable gloves or gowns. Nurses perceived use of protective devices may not only increase time strain but also hinder development of therapeutic relationships with patients. Of safety climate factors, lack of time was the most frequently reported barrier to compliance with standard precautions. Knowledge, perception, and safety climate explained 16.1% variance of compliance with standard precautions. Conclusion: To improve nurses' compliance with standard precautions, provision of education and support for safety climate are necessary.

돌봄제공자의 감염관리를 위한 표준주의 이행도 영향요인 (Factors influencing Healthcare-associated Infection Control of Caregivers)

  • Purpose : The purposes of this study were to examine the relationships between knowledge, awareness, safe environment, and performance of standard precautions and identify factors associated with performance of standard precautions. Methods : This study was a descriptive research. A structured questionnaire on knowledge, awareness, safe environment, and performance of standard precautions was used for the survey with a convenience sample of 150 caregivers. Data were collected from July to August 2019 and were analyzed using descriptive statistics, independent t-test, one way ANOVA, Pearson's correlation coefficient, and multiple regression with SPSS/WIN 25.0 program. Results : The mean scores on knowledge, awareness, safe environment, and performance of standard precautions were 15.77±3.34, 7.35±1.91, 4.55±2.05, and 55.20±10.11 respectively. Performance of standard precautions showed a statistically significant positive correlation with knowledge (r=.54, p<.001), awareness (r=.54, p<.001), and safe environment (r=.50, p<.001). Awareness (β=.24, p=.025) and safe environment (β=.35, p<.001) were significantly associated with factors of performance of standard precautions. Also, education level (high school and above college), affiliated institution (private association), and importance of infection control education (moderate) were significantly associated with factors of performance of standard precautions. Conclusion : The results of the study indicate that factors influencing the performance of standard precautions of caregivers were awareness and safe environment. Therefore, to improve implementation of the standard precautions among caregivers, a safe environment within the hospital must be supported, and appropriate infection management education needs to be provided to caregivers to improve their knowledge and awareness of the standard precautions.

간호대학생의 표준주의 지식과 표준주의 수행 간의 관계: 주관적 규범 매개효과 (Relationship between Standard Precautions Knowledge and Standard Precautions Compliance of Nursing Students: The Meadiating Subjective Norms )

  • 본 연구는 간호대학생의 표준주의 지식과 표준주의 수행과의 관계에서 주관적 규범의 매개효과를 규명하기 위해 시행되었다. 연구의 대상자는 J도에 소재한 3개 대학교의 임상실습을 완료한 4학년 간호학과 재학생 195명이다. 수집된 자료로 SPSS 25.0 프로그램을 이용하여 t-test, ANOVA, Pearson's correlation 및 multiple regression을 실시하였다. 연구결과, 주관적 규범은 표준주의 지식과 표준주의 수행 사이에서 부분매개효과를 나타냈고, 표준주의 수행을 설명하는 설명력(R2)은 35%이었다. 따라서 간호대학생의 표준주의 수행을 촉진하기 위해서는 표준주의 지식 함양 및 이를 통해 주관적 규범을 유도할 수 있는 효과적 전략 마련이 필요하다.
