A STUDY ON DIMENSIONAL STABILITY OF IMPRESSION MATERIALS FOLLOWING IMMERSION DISINFECTION (수종 인상재의 침지 소독이 경석고 모형의 크기 안정성에 미치는 영향에 관한 연구)

  • Dental practice can produce and spread some infectious diseases from patients to dentist, dental assistant, and dental labors. One possible method for preventing these cross-contamination is to immerse dental impression in chemical disinfectants. So for many investigators studied on the dimensional changes of dental impressions and on the surface qualities of stone casts made from impression following immersion in disinfectants. This study was proposed to evaluate some popular impression disinfectant combination from the point of dimensional stability. Impression was taken from dental arch-shaped metal model. Irreversible hydrocolloid and 3 elastomers(polyvinyl siloxane, polysulfide, polyether) were immersed in 3 disinfectants (2% glutaraldehyde, 1% povidone-iodine, 0.5% sodium hypochlorite) for 10 minutes and measured both cross-arch and anterior-posterior distance under stereo microscope to evaluate dimensional change. The results obtained were as follows: 1. Dimensional changes of irreversible hydrocolloid impression was statistically different in cross-arch and anterior-posterior distance when immersed in 2% glutaraldehyde solution and in anterior-posterior distance when immersed in 0.5% sodium hypochlorite solution from control group (p<0.05). 2. Dimensional changes of polyvinyl siloxane and polysulfide impression were not statistically different from control group (p>0.05). 3. Dimensional changes of polyether impression was statistically different in cross-arch distance when immersed in 0.5% sodium hypochlorite solution and in anterior-posterior distance when immersed in 1% povidone-iodine solution from control group (p<0.05). 4. In all cases, dimensional changes were less than 0.1% from the original dimension and concluded clinically acceptable.

Effects of Cotton Balls of Alcohol and Povidone for Disinfectoin of Skin in Newborns (신생아에서 알코올 솜 및 포비돈 솜의 피부 소독효과 비교)

  • Purpose : Despite of development of neonatal intensive care, infections, especially catheter-related infection, have been a major problem in newborns on intensive care. Catheter-related infection is closely related to skin colonization so that skin disinfection is crucial to protect newborns from the infection. Cotton balls of seventy percent alcohol made by hospital frequently open to air so that composition of alcohol may be easily evaporated. We hypothesized that 10% povidone would have better disinfection compared to 70% alcohol and normal saline. Methods : The effect of skin disinfection was evaluated in newborn infants who were admitted to the neonatal intensive care unit(NICU) of Hanyang University Kuri Hospital by a skin swab culture before and after disinfectant exposure. Before skin disinfective preparation, the peripheral intravenous catheter insertion sites were swabbed for culture with sterile cotton moistened with normal saline. Cotton balls of 70% alcohol(Alcohol group, n=20) or 10% povidone and 70% alcohol(povidone group, n=20) were randomly selected. Normal saline swabs were used for controls(n=10). After swabbing, the skin swab culture was done on same sites with same procedure. Results : Before skin disinfection, culture-positivity was shown in 9 of 20 leases of alcohol and povidone groups, respectively and in 8 of 10 cases of the controls. After skin disinfection, culture-positivity was still shown in 3 of 9 cases of the Alcohol group, in 2 of 9 of the Povidone group, and in 6 of 8 cases of the controls. Disinfective effect was significantly different among the 3 groups(p=0.0387). The povidone group had significantly better disinfection than the controls(p=0.0156), but the alcohol group was not significantly different from the controls. Conclusion : The cotton balls of povidone had similar effect of disinfection compared to those of alcohol, but better disinfection than the controls. A disinfective effect of the cotton balls of alcohol was not, however, significantly different from the controls. We suggest that purity of alcohol, quality control of cotton balls of alcohol, and disinfection method should be reevaluated.

Effect of saline irrigation used in combination with antimicrobial agents on salivary bacterial counts (생리식염수 세척이 구강 소독제의 타액 내 세균 수의 변화에 미치는 영향)

  • Introduction: The aim of the present study was to investigate the effect of mechanical irrigation in combination with mouthwash of antimicrobial agents on salivary bacterial counts. Materials and Methods: This study was performed with a randomized study employing a panel of 40 healthy volunteers (20 males and 20 females) between the age of 26 and 32 years. Volunteers were randomly put in one of four treatment groups. In the first group, 0.2 mL of non-stimulatory saliva was collected from every subjective person. Then, saliva was collected after rinsing with chlorhexidine (CHX) for 1 minute. In the second group, non-stimulatory saliva was collected, and then saliva was collected after rinsing with CHX and irrigation with saline. In the third and fourth groups, the same procedures as the first and second groups were performed with povidone iodine (PVI) instead of CHX. All of these samples were cultured for 48 hours aerobically. The reduction rates of colony-forming units (CFU) were calculated for each group. The reduction rate between each group was tested statistically using student t-test. Results: Using CHX in combination with saline irrigation showed a significant decrease of the salivary bacterial CFU when compared with only using CHX.(P<0.01) And using PVI with saline irrigation showed a little decrease of the CFU when compared with only using PVI, but there was no statistical significance.(P>0.01) Conclusion: It was concluded that the CHX or PVI used with saline irrigation made the salivary bacterial counts reduced more than when CHX or PVI was used alone as an oral antiseptic agent.

Bactericidal Efficacy of a Disinfectant Solution Composed to Povidine-iodine Against Salmonella typhimurium and Brucella ovis

  • Salmonella spp. and Brucella spp. are associated with considerable diseases of both humans and animals. In addition, these microorganisms cause the economic loss in animal farming and food industry. In this study, the disinfection efficacy of a commercial disinfectant, composed to povidone-iodine was evaluated against S. typhimurium and B. ovis. A bactericidal efficacy test by broth dilution method was used to determine the lowest effective dilution of the disinfectant following exposure to test bacteria for 30 min at $4^{\circ}C$. The disinfectant and test bacteria were diluted with hard water (HW) or organic matter suspension (OM) according to treatment condition. On HW condition, the bactericidal activity of the disinfectant against S. typhimurium and B. ovis was 400 and 150 fold dilutions, respectively. On OM condition, the bactericidal activity of the disinfectant was 5 and 20 fold dilutions against S. typhimurium and B. ovis, respectively. As the disinfectant composed to povidine-iodine possesses bactericidal efficacy against animal pathogenic bacteria such as S. typhimurium and B. ovis, the disinfectant solution can be used to control the spread of bacterial diseases.

In Vitro Activities of 2,2'-Dipyridyl Against Trichomonas vaginalis, Candida albicans, and Gardnerella vaginalis

  • The in vitro activities of 2,2'-dipyridyl, an iron-chelator, against clinical isolates of Trichomonas vaginalis, Candida albicans, and Gardnerella vaginalis was evaluated and compared with those of four other vaginal suppositories, ornidazole, clotrimazole, povidone-iodine, and $Cenacert^{\circledR}$ (Methylbezethonium Chloride mixed with 9-aminoacrydine undecylenate and hydrochloric acid N-myristyl-3-hydroxy butyl amine). The 2,2'-dipyridyl killed T. vaginalis and G. vaginalis at concentrations of $410\;{\mu}g/ml$ and $205\;{\mu}g/ml$, respectively, however, ths agent was less active against C. albicans (80% of which was inhiited at $410\;{\mu}g/ml$). The inhibition of these three pathogens by 2,2'-dipyridyl was similar to clotrimazole. In addition, the effect of 2,2'-dipyridyl on the ultrastructure of T. vaginalis, C. albicans, an G. vaginalis was examined. Transmission electron microscopy indicated that 2,2'-dipyridyl induced modifications of the cellular contents and cell envolope concumitant with the degradation of the three pathogens. These results suggest that 2,2'-dipyridyl has an inhibitory effect on C. albicans and G. vaginalis, as well as T. vaginalis.

  • A 11-year-old gelding Thoroughbred horse was presented with a history of unilateral nasal discharge of right side. The horse was diagnosed as maxillary sinusitis based on blood examination and radiography. The horse was treated with bone flap of right maxillary sinuses. Abscess of maxillary sinus was drained and irrigated with antibiotic-saline and povidone iodine solution. After surgical procedure, nebulization and medication applied by the result of antibiotic sensitivity test. After treatment for two weeks, the patient was recovered completely.

Digital Squamous Cell Carcinoma in a Miniature Schnauzer (Miniature Schnauzer에서 발생한 발가락 편평세포암종)

  • An 11-year-old, female Miniature Schnauzer dog was presented with recurrent skin ulcer of the second metatarsal region in the right hindlimb following metatarsal resection. Physical examination revealed an ulcerated and bleeding lesion of the second metatarsal region in the right hindlimb. Impression smears of the ulcerative lesion confirmed numerous degenerated neutrophils and mixed bacterial infection. Initially, the dog was treated with antibiotics and povidone-iodine flushing for the control of deep pyoderma. Because the skin lesion had been deteriorated over time despite of topical and systemic treatments, skin biopsy was performed. Histopathologic examination indicated squamous cell carcinoma based on the features of multiple nests of squamous neoplastic cells and mitotic figures. Although amputation of the right hindlimb was performed, the dog was expired five months later because of tumor metastasis to the lung and the popliteal lymph node. This is the first case report describes malignant digital squamous cell carcinoma in Korea.

Cutaneous Myiasis Associated with Tick Infestations in a Dog (진드기에 감염된 개의 피부 구더기증 1예)

  • A 12-year-old intact male, Alaskan Malamute dog, which lives in the countryside, was presented with inflammation and pain around perineal areas. Thorough examination revealed maggots and punched-out round holes lesion around the perineal region. Complete blood counts (CBC) and serum biochemical examinations showed no remarkable findings except mild anemia and mild thrombocytosis. The diagnosis was easily done, based on clinical signs and maggots identification. Cleaning with chlorhexidine, povidone-iodine lavage and hair clipping away from the lesions were performed soon after presentation. SNAP 4Dx Test (IDEXX Laboratories, Westbrook, ME, USA) was performed to rule out other vector-borne diseases since the ticks were found on the clipped area and vector-borne pathogens. The test result was negative. The dog in this case was treated with ivermectin (300 mcg/kg SC) one time. Also, treatments with amoxicillin clavulanate (20 mg/kg PO, BID) was established to prevent secondary bacterial infections. Then, myiasis resolved with 2 weeks and the affected area was healed.

Ultrasound Guided Shoulder Joint Injection through Rotator Cuff Interval (초음파를 활용한 회전근개 간격으로 접근한 견관절 주사법)

  • Background: Shoulder joint injection is currently performed under fluoroscopic or computed tomography scan guidance. We performed this study to determine if an ultrasound guided shoulder joint injection through rotator cuff interval would have clinical usefulness. Methods: A total of 17 volunteers [12 women, 5 men; mean age 28 yr (23-32 yr)] received shoulder joint injection under multilinear ultrasound (5-10 MHz). Volunteers were positioned supinely on a table with their arm in a neutral position. The anterior shoulder region of the patient was sterilized using povidone iodine. A 24 gauge needle was introduced and directly visualized in real time as it passed obliquely from the skin surface to the inferior space of the biceps tendon. If there was little or no resistance to the injection, a contrast media (omnipaque) was injected and checked fluoroscopically. Results: Ultrasound guided shoulder joint injection through rotator cuff interval was successful in all cases. The average time taken for the procedure was $27.5{\pm}16.5sec$. The vertical distance from skin to the inferior space of the biceps tendon was $1.6{\pm}0.4cm$ and the distance of needle from the skin to the inferior space of biceps tendon was $2.8{\pm}0.6cm$. The procedure was well tolerated by all volunteers. Conclusions: Ultrasound guided shoulder joint injection through rotator cuff interval is an effective, rapid, and easy-to-perform injection technique. Ultrasound guided injection enables exact needle placement and avoids the use of both ionizing radiation and iodinated contrast material.

EXPERIMENTAL STUDY ON THE EFFECTS OF VARIOUS DISINFECTANTS TO ORAL MICROFLORA (각종 소독제가 구강균총에 미치는 영향에 관한 실험적 연구)

  • Though oral microgranisms were among the first to be observed by humans, the interest in oral microbiology lagged. When it became apparent that the oral microflora did influence systemic disease of the body, interest was aroused in the nature and kinds of the microgranisms. The risk of infection in dental procedures is due to the abundant flora of the mouth. This hazard can be reduced to some extent by the use of a local disinfectant. The present studies were undertaken to evaluate and compare the various disinfectants which are commonly used in clinics and hospitals. The results were as follows. 1. The bactericidal activity of the disinfectants mainly depends upon the kinds of the agents, not upon the kinds of the microorganisms. 2. In H₂O₂(3%), the bactericidal activity was greatly related to the contact time. So, at least 4 minitues of contract time was required to use it as an oral antiseptic. 3. In ethyl alcohol (70%), Pseudomonas aeruginosa and Streptococcus salivarius surived a little after 15 seconds of contact time, but, no other colony was discovered after more than 15 seconds of contact time in any kins of microorganisms. 4. Merthiolate (0.1%) showed low antibacterial activity, more in Gram-positive organisms and less in Gram-negative organisms. 5. Benzalkonium chloride (0.1%) and povidone-iodine (10%) showed the most excellent results, revealing no surviving organisms only after 14 seconds of contact time.
