This study was conducted longitudinally following the total nitrogen , total lipid, and lactose concentrations and their factors on concentrations of energy-yielding nutrients in human milk from 2-5 days to 12 weeks postpartum of 45 lactating Korean mothers. All samples were from well-defined subjects. And uniform collection procedures were used . Total nitrogen concentration of milk decreased significantly from 385mg/이 at 2-5 days to 201mg/dl at 12 weeks postpartum. Total lipid concentration increased from 1.98g/dl at 2-5 days to 3.09g/dl at 12 weeks postpartum. However, there was a large variation in the concentration of total lipids. Lactose concentration increased signifciantly from 6.53g/dl at 2-5 days to 7.48g/dl at 12 weeks. Total nitrogen concentration in milk was positively related to pregnancy weight gains at 2 and 6 weeks postpartum. Total lipid concentration was positively related to infant birth weight : however, it was negatively related to the maternal weight loss. In addition, total lipid concentration was positively related to parity and maternal percentage body fat at 12 weeks postpartum. Milk lactose concentration was positively related to parity at 2 weeks postpartum.
Objectives: The purpose of this study is to investigate the various postpartum symptoms and the relevant factors. So it can provide fundamental data about postpartum health care. In conclusion, we can prevent and manage the postpartum disease by using this data. Methods: The subject of the present study was 94 women (63 women who completed vaginal delivery and 31 women who delivered by cesarean section) who completed labor between November 1, 2013 and January 31, 2016, at the clinic of OB&GYN. They have been taking good care of their health at postpartum clinic in Andong Woori Women Hospital. We investigated the various symptoms and situation which occurred from the moment of hospital to postnatal admission health care period, and Korean medical doctor examined the patients through the four examination methods. We classified the symptoms by maternal age, the frequency of maternal childbirth, the method of delivery, the delivery season, the term of pregnancy, the body weight of infant, the weight change of mother and the way of feeding. Results: There were no remarkable corelation in the frequency of symptoms according to maternal age, the way of delivery, the delivery season, the change of weight before and after of delivery, gestational weeks at delivery, the manner of feeding and the body weight of infants except for the frequency of maternal childbirth. Conclusion: The symptoms of hemorrhoids, the feeling of coldness and chilliness increased with increasing the frequency of maternal childbirth. Other circumstances, there was no relationship with symptoms of postpartum.
Postpartum depression is one of the most serious problems in maternal health because it affects not only the mother but also her family. Postpartum depression disturbs the maternal-infant interaction and attachment. However, most postpartum depression patients ignore this problem and do not seek treatment. Many clinicians and researchers realiza there is a need to develop a postpartum depression scale. Thus, this study has been designed to development of a postpartum depression scale. Data were collected through a survey over a period of three months. Subjects who participated in the study were 167 Korean mothers in their postpartum period. The author used a convenience sampling method. The analysis of the data was done with SPSS PC/sup +/ for descriptive statistics, item analysis and factor analysis. Initially 62 items were generated from the interview data of eight postpartum depression patients and from a literature review. This preliminary scale was analyzed for reliability and validity. The results of this analysis are as follows. 1. Initially 62 items were analyzed through the Index of Content Validity(CVI) and 48 items were selected. 2. Seven factors were extracted through the principal component analysis, and these contributed 61% of the variance in the total score. Finally 46 items in the scale loaded .41∼ .84 on one of seven factors. 3. Each factor was labeled. Factor 1 was labeled 'emotional phenomena-emotional upset' and included 13 items, factor 2 was labeled' cognitive phenomena-self concept disturbance' and included seven items, factor 3 was labeled 'relationship to baby-negative feeling' and included six items, factor 4 was labeled 'relationship to baby- overload' and included eight items, factor 5 was labeled 'negative maternal identity' and included five items, factor 6 was labeled 'biophysiological phenomena-disturbance of physical functioning' and included four items, and factor 7 was labeled' interpersonal relationship phenomena-blamed others' and included three items. 4. Cronbach Coefficient Alpha for internal consistency was .95 for the total 46 items. Finally, the author suggests that this scale could be adequately applied in assessing the postpartum depression of mothers during the postpartum period. The results of this study can contribute to designing an appropriate postpartum depression prevention strategy.
Objective : This research was designed to study the characteristics of heart rate variability in postpartum women compared with normal women. Method : HRV data of postpartum women were gathered from 255 women who was hospitalized for oriental postpartum treatments(Age : 25-35). HRV data of comparison group were gathered from 327 women who visited hospital to check up their health(Age : 25-35). The SPSS 12.0 for windows was used to analyze the date and the independent samples t-test was used to verify the result. Result : 1. Mean-RR and SDNN of postpartum women group significantly decreased compared with that of normal women group. But, Heart Rate of postpartum women group significantly increased compared with that of normal women group. 2. HRV-Index, RMSSD and SDSD of postpartum women group significantly decreased compared with that of normal women group. pNN50 of postpartum women group significantly increased compared with that of normal women group. 3. Ln(TP), Ln(VLF), Ln(LF) and Ln(HF) of postpartum women group significantly increased compared with that of normal women group. 4. There were no significant differences in Normal LF, Normal HF and LF/DF Ratio between postpartum women group and normal women group. Conclusion : The result suggest that the function of heart of postpartum women group significantly decreased compared with that of normal women group. Futhermore although the ANS maintained the balance in the range of normality, the sympathetic nervous system frequently revitalized which caused increasing the heart of pulsation.
The Korean government has a new system for charging patient care for patients in hospital, on hold for the present(9 / 1995) but to start implementation in certain areas of patient care next year. From the latter half of next year the Ministry of Health and Welfare would like to start demonstration projects for hospitals who want to start using DRGs for frequently seen medical diagnosis and for patients with a course that is predictable and for whom non-insurance costs are minimal : such as the patient who has a delivery, cesarean deliveries, cataract surgery, tonsillectomy or an appendectomy, and apply the DRG system of payment for hospital care for these patients. The purpose of this study was to establish a recording system to give effective home health care to postpartum women and their newborns. Recently the government announced a DRG system to apply to postpartum women for pilot purposes starting next year. This gives impetus to the need to develop home care records that will allow for systematic recording and provide continuity and consistency in care across all health professionals and with in-depth communication between the professions to assure high quality care. There has been a rise in medical costs and a shortage of patient bed space in hospitals, particularly since the introduction of national medical insurance. The study focused on developing client selection criteria, a primary assessment tool, progress notes and nursing diagnoses applicable to postpartum and newborn clients. Selection criteria for home health care, assessment tool content, nurses progress notes and diagnoses were developed through a review of the literature, advice from professionals who are expert in home health care and actual practice in the use of recording tools through workshops. The recommendations based on the research results are as follows : 1) Replication and application of these tools is needed to test the validity of the tools 2) In order to have systematic nursing records standardization of records has to be done after nurses have had experience using them. 3) Reliability and validity of the tools has to be established through applicability to actual care situation.
The purpose of this study was to collect basic data the health care of postpartum women. Three hundred seventy five women who were 3 days postpartum were enrolled at two university hospitals. Data were collected from April 1999 to November, 1999 using a questionnaire titled 'Symptom Table on Fatigue Perception' designed by the Fatigue Research Committee of Japan. The collected data were scored by the use of mean and standard deviation according to the subjective symptoms of fatigue and each item was analyzed independent variable by the t-test and the ANOVA test. The results were as follows: 1. The average fatigue score of subjects was 1.61. Physical fatigue had the highest value with a mean of 1.82, followed by neuro-sensory fatigue with a mean score of 1.54, psychological fatigue was rated lowest with a mean of 1.45. 2. With the respect to the general characteristics of subjects, there were statistically significant differences in difficulty of labor (t=2.335. p=.020), sleeping time (t=2.340. p=.020) and desirability of pregnancy (t=2.409. p=.018).
Purpose: The purpose of this study was to determine the factors that may affect quality of life of mothers who delivered premature infants. Methods: With survey design, data were collected from 145 mothers of premature infants with corrected age of 2 months to 12 months from January 9 to February 2, 2017. Quality of life was assessed with two measures of direct survey in the selected hospital and online survey. A self-report questionnaire was administered regarding personality of the mothers and the infants, postpartum depression, parenting stress, social support, and the quality of life. Data were analyzed using t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results: The quality of life of the mothers of the premature infants was influenced by postpartum depression, parenting stress, parents-infant dysfunctional interactions, and social support accounted for 65% of the variance. Conclusion: These results indicate that early screening and continuous management of postpartum depression during postpartum period are important to improve the quality of life of the mothers of the premature infants. Education program and information and social support systems need to be developed to monitor mother-infant interaction and their role development.
Objectives: This study analyzed an online survey targeting women and Korean medicine doctors who participated in the Yeosu City Postpartum Health Center Support Project. Through this, we would like to discuss the direction of support for postpartum health care support projects and new policies to encourage childbirth. Methods: In this study, we examined the results of an online survey from 73 women and 29 Korean medicine doctors. We examined 39 items, including basic information, childbirth-related items, Korean medicine treatment-related items, business satisfaction, and policies, answered by 73 women and 29 Korean medicine doctors who responded to the online survey. Results: The average age of women was 33.62±4.19 years, and the average age of Korean medicine doctors was 49.82±8.60 years. Musculoskeletal pain appeared most commonly in both maternal and Korean medicine doctors' online surveys. The visit date from the date of delivery was 36.87±27.06 days. Gungguijohyeol-um and Boheo-tang were widely used. The survey score of women whose main symptoms improved after taking herbal medicine was 3.04±1.15 points, and the survey score of Korean medical doctors was 4.13±0.74 points. The positive response to the policy was 73.7% for satisfaction with the overall policy to encourage childbirth, 54.8% for perceived support, and 65.7% for reduction of economic burden. Conclusions: Women who participated in the postpartum health care support project and received Korean medicine treatment and Korean medicine doctors who performed Korean medicine treatment were very satisfied. The most frequent postpartum symptom was musculoskeletal pain. The degree of improvement in main symptoms after taking herbal medicine was higher among Korean medicine doctors than among women. Among childbirth policies, the areas that should be prioritized are childbirth support projects and postpartum care projects.
This study was designed to verify the effect of Doula-type-delivery nursing care on plasma $\beta$-endorphin, serum cortisol, related to delivery stress during labor, and postpartum anxiety of primipara by a quasi experiment(nonequivalent control group pretest-posttest design), from December, 1999 to August, 2000. The subjects of this experiment consisted of sixty eight primipara, with single gestation, full term, uncomplicated pregnancies, thirty three for the experimental group and thirty five for the control group. Their mean age was 26.1 years for the experimental group and 25.5 years for the control group. Their mean gestation period was 39.7 weeks for the experimental group and 40.1 weeks for the control group. As treatment, Doula-type-delivery nursing care was given for the experimental group. Data assessed plasma $\beta$-endorphin, serum cortisol during labor, and anxiety during postpartum. Plasma $\beta$-endorphin, serum cortisol were measured in the latent phase before treatment(pre-test) and the transition phase after treatment(posttest). Also, anxiety was measured in the latent phase before treatment(pre-test) and 24 hours postpartum after treatment(posttest). Data was analyzed by t-test, $x^2$-test, Repeated measures ANOVA with SAS Program. The results of this study were as follows; 1. Plasma $\beta$-endorphin was significantly elevated in the experimental group who were cared for with Doula-type-delivery nursing care during labor(P=.0463). 2. No significant group effects were found, but significant time effects were found for serum cortisol. 3. The postpartum anxiety of the experimental group was significantly lower than the control group(P=.0110). In conclusion, these findings indicate that Doula-type-delivery nursing care during labor could be effective in increasing maternal plasma $\beta$-endorphin and decreasing postpartum anxiety. Doula-type-delivery nursing care during labor could be applied as an effective nursing treatment for primipara.
The purpose of this study was to find the effects of an educational program for primiparas on maternal confidence and continuity of breastfeeding. The subjects of this study were the healthy mothers, and the healthy infants whose weight was more than 2.500gm and gestational age was more than 37 weeks. The final targets were thirty seven mothers-19 of intervention group and 18 of control group. Data were collected from 15th of March to 3rd of September, 1999. To the intervention group, education on infant care and breast feeding were provided before discharge. And that, telephone advice was provided within one week after discharge, and at 2 months postpartum. And that, reeducation and counsel were provided at one month and three months postpartum by home-visiting care for the intervention group. For the control group. home visiting was also conducted only for data collection. Data were analyzed by chi-square test and t-test. The results were as follows: 1. The maternal confidence of the experimental group was higher than that of control group at one month postpartum. but the difference of confidence of these two groups were not significant at three months postpartum. 2. For both of experimental and control groups, maternal confidence significantly increased at three months postpartum than one month postpartum. 3. The rate of breastfeeding of the experimental group was higher than that of control group at one month and two months postpartum (p=.050, p=.049). But the difference was not significant at three months, although experimental group continued breastfeeding more(47.4%) than control group(27.8%). In conclusion, educational program for primiparas of this study was effective in the promotion of maternal confidence and breastfeeding.
