Calcaneal Fractures-Extended Lateral Approach (종골 골절의 치료-광범위 외측 접근법)

  • Calcaneus is largest tarsal bone and the fracture of calcaneus is most common tarsal fractures. Calcaneal fractures are divided into extra-articular and intra-articular fractures. Intra-articular calcaneal fractures could be classified as tongue type and joint depression type using simple lateral radiograph (Essex-Lopresti classification), but Sanders suggested new classification according to involving the posterior facet of calcaneus using computed tomography. The involvement of posterior facet was revealed as more complicated than Essex-Lopresti classification. The principle purpose of treatment of calcaneal fractures are restoration of calcaneal height (B$\ddot{o}$hler angle), width, axis, anatomical reduction of joint and restoration of function through the stable fixation. Good visualization of joint and anatomical reduction could be achieved by extended lateral approach. But, skin problem could be occurred after of extended lateral approach.

Change of the Pelvic Tilt Angle on Bobath Approach in Hemiplegic Patients (편마비 환자에 있어서 Bobath Approach가 골반경사각도 변화에 미치는 영향)

  • The purpose of this study was to compare a pelvic tilt angle between sound side and affected side in hemiplegic patients and the changing affected pelvic posterior tilt angle was measured at intervals of 3, 6, 9 weeks after Bobath approach. The subject for the study were 10 hemiplegic patients(mean age of 54.1 years)without orthopedic disability on pelvic bone. The data were analyzed by t-test, one-way ANOVA. The results of this study were as follows. There was a significant difference in the pelvic tilt angle between sound side and affected side in hemiplegic patients. There was a significant difference in affected pelvic posterior tilt angle between pre-treatment and post-treatment(9 weeks).

  • In Bayesian analysis of randomized response models, the likelihood function does not combine tractably with typical priors for the parameters of interest, causing computational difficulties in posterior analysis of the parameters of interest. In this article, the difficulties are solved by introducing appropriate latent variables to the model and using the Gibbs sampling algorithm.

  • The modern approach for wavelets imposes a Bayesian prior model on the wavelet coefficients to capture the sparseness of the wavelet expansion. The idea is to build flexible probability models for the marginal posterior densities of the wavelet coefficients. In this note, we derive exact expressions for a popular model for the marginal posterior density.

Posterior Cervical Fixation with Nitinol Shape Memory Loop in the Anterior-Posterior Combined Approach for the Patients with Three Column Injury of the Cervical Spine: Preliminary Report

  • Objective: The authors reviewed clinical and radiological outcomes in patients with three column injury of the cervical spine who had undergone posterior cervical fixation using Nitinol shape memory alloy loop in the anterior-posterior combined approach. Materials: Nine patients were surgically treated with anterior cervical fusion using an iliac bone graft and dynamic plate-screw system, and the posterior cervical fixation using Nitinol shape memory loop ($Davydov^{TM}$) at the same time. A retrospective review was performed. Clinical outcomes were assessed using the Frankel grading method. We reviewed the radiological parameters such as bony fusion rate, height of iliac bone graft strut, graft subsidence, cervical lordotic angle, and instrument related complication. Results: Single-level fusion was performed in five patients, and two-level fusion in four. Solid bone fusion was presented in all cases after surgery. The mean height of graft strut was significantly decreased from $20.46{\pm}9.97mm$ at immediate postoperative state to $18.87{\pm}8.60mm$ at the final follow-up period (p<0.05). The mean cervical lordotic angle decreased from $13.83{\pm}11.84^{\circ}$ to $11.37{\pm}6.03^{\circ}$ at the immediate postoperative state but then, increased to $24.39{\pm}9.83^{\circ}$ at the final follow-up period (p<0.05). There were no instrument related complications. Conclusion: We suggest that the posterior cervical fixation using Nitinol shape memory alloy loop may be a simple and useful method, and be one of treatment options in anterior-posterior combined approach for the patients with the three column injury of the cervical spine.

Interval Estimation for a Binomial Proportion Based on Weighted Polya Posterior (이항 비율의 가중 POLYA POSTERIOR 구간추정)

  • Recently the interval estimation of a binomial proportion is revisited in various literatures. This is mainly due to the erratic behavior of the coverage probability of the will-known Wald confidence interval. Various alternatives have been proposed. Among them, Agresti-Coull confidence interval has been recommended by Brown et al. (2001) with other confidence intervals for large sample, say n $\ge$ 40. On the other hand, a noninformative Bayesian approach called Polya posterior often produces statistics with good frequentist's properties. In this note, an interval estimator is developed using weighted Polya posterior. The resulting interval estimator is essentially the Agresti-Coull confidence interval with some improved features. It is shown that the weighted Polys posterior produce an effective interval estimator for small sample size and a severely skewed binomial distribution.

The endoscopic transnasal approach to the lesions of the craniocervical junction: two case reports

  • The endoscopic endonasal approach (EEA) to the craniovertebral junction (CVJ) has recently been considered a safer alternative and less invasive approach than the traditional transoral approach because the complications associated with the latter are avoided or minimized. Here, we present two challenging cases of CVJ pathologies. The first case involved os odontoideum associated with anterior displacement of the occipitocervical junction where the EEA was used, followed by C0-C1-C2 fusion using a posterior approach to decompress the CVJ, and was complicated by rhinorrhea and Candida albicans meningitis. The second case involved basilar invagination with syringomyelia previously treated using a posterior approach, where aggravation of neuropathic symptoms required combined treatment with EEA and occipitocervical fusion of C0-C2-C3-C4, with the postoperative course challenged by operative site infection requiring drainage with debridement and antibiotic therapy. The EEA is an alternative approach for accessing the CVJ in well-selected patients. Knowledge of EEA complications is crucial for the optimal care of patients.

Implant placement simultaneously sinus augmentation using crestal approach in severely atrophic maxilla; minimally invasive approach (골 흡수가 심한 상악 구치부에서 치조정 접근법을 이용하여 상악동 골이식술 동시 임플란트 식립)

  • The atrophy of edentulous ridge and pneumatization of the maxillary sinus often limit the volume of bone available for implant placement on maxillary posterior teeth. Most clinicians suffer difficulties from poor bone quality and quantity on maxillary posterior site. Thus, the success of maxillary posterior implant surgery depends on the increase of the available bone and obtaining a good initial stability of the implant after maxillary sinus reconstruction. The maxillary sinus augmentation methods include a crestal approach and a lateral approach. Less morbidity and complications after operation is major advantage to sinus augmentation using crestal approach than lateral approach. However, when the residual ridge height is ${\geq}6mm$, it is known that crestal approach is appropriate. Also delayed implantation after sinus augmentation is recommended in severely atrophic ridge. We present the three cases of implant placement simultaneously sinus augmentation using crestal approach in posterior maxilla site with ${\leq}3mm$ of residual alveolar bone.

Confidence Intervals for a Linear Function of Binomial Proportions Based on a Bayesian Approach (베이지안 접근에 의한 모비율 선형함수의 신뢰구간)

  • It is known that Agresti-Coull approach is an effective tool for the construction of confidence intervals for various problems related to binomial proportions. However, the Agrest-Coull approach often produces a conservative confidence interval. In this note, confidence intervals based on a Bayesian approach are proposed for a linear function of independent binomial proportions. It is shown that the Bayesian confidence interval slightly outperforms the confidence interval based on Agresti-Coull approach in average sense.
