뇌졸중 환자의 낙상 예측을 위한 Four Square Step Test의 활용에 관한 연구 (A Study of the Clinical Utility of the Four Square Step Test for Predicting Falls in Stroke Patients)

  • Purpose: This study was designed to evaluate the clinical utility of the Four square step test (FSST) for predicting falls in stroke patients, to compare the ability of the FSST test to discriminate between subgroups of fallers, and to determine if the test has any predictive value in identifying stroke patients who will fall. Methods: Stroke patients (N=37) who could walk at least 50 m with minimal assistance were recruited consecutively when attending a physical therapy session during their rehabilitation. Dynamic standing balance was measured using the FSST. The main outcome measures were FSST time and fall number. Numbers of falls were compared with FSST scores. Differences between the groups in FSST scores were examined using a t-test and 1-way analysis of variance. Post hoc analysis using the Tukey B procedure was used to identify specific group differences. Alpha was set at 0.05. Results: A total of 15 participants (40.5%) reported falls 6 had recurrent falls (2 falls) and 9 fell once. The mean FSST time differed significantly between groups with zero and multiple falls. A cutoff score of greater than 17 seconds on the FSST was associated with a sensitivity of 83% and a specificity of 84% for the identification of subjects with multiple risk factors for falls. Conclusion: The FSST is a feasible and valid clinical test of dynamic standing balance and can predict falls in post-stroke patients.

사회적 지지의 특성에 따른 뇌졸중 환자의 우울 (The Relationship between the Characteristics of Social Support and Post-Stroke Depression)

  • The effect s of social support on post-stroke depression were investigated. Social support was classified into three categories : primary caregiver's, significant other's, and professional support. And primary caregiver's and significant other's support were divided into their relation, living state (which means they living together or not), frequency which they met at, and perceived satisfaction about the support, respectively. Professional support was divided into number of professionals who take care of, frequency they met at, and perceived satisfaction. The subjects were 254 stroke patients who had been discharged and were taken follow-up care at the out patient department. The depression was measured using CES-D. Out of the primary caregiver's support, only relation and perceived satisfaction were identified to affect the post-stroke depression. The patients who primarily their spouse takes care of were less depressed than those who their adult children take care of. None of the significant other's support affects the post-stroke depression. Only perceived satisfaction of the professional support was found to affect the post-stroke depression. So it was found that the quality, not the amount, of care was important to post-stroke depression. These results also support the claim that the facilities and teaching programs for primary caregivers are necessary.

  • Dysphagia generally has a good prognosis after stroke involving cerebral hemisphere or braibstem, but it could have serious consquence with dehydration leading to haemoconcentration, renal failure and aspiration leading to pneumonia. This preliminary report was written for main report. The main report will be written to give an objective guide post of management and treatment in stroke patient with dysphagia. The objective guide posts were follows as, 1. Close examination for relationship between dysphagia in stroke patients and other neurological deficits 2. Influence on the prognosis of dysphagia in stroke patients by the grade of dysphagia at admission time. 3. The realtionship between the site and size of brain damage and the grade and prognosis of dysphagia 4. The frequency and prognosis of dysphagia in stroke patients according to sex and age. 5. The rate and speed of recovery with dysphagia after stroke. 6. Frequency and process of complications, aspiration pneumonia, weight loss, sore, in stroke patient group with dysphagia.

Effects of Regularly Performed Walking on the Bilateral Limb Compositions of Post-Stroke Korean Men

  • The purpose of this study was to examine the effects of hemispheric damage in body composition of male adults with stroke experiences. The Fourth and Fifth Korea National Health and Nutrition Examination Surveys (KNHANES) with body composition results obtained from the DXA (dual-energy X-ray absorptiometry) assessments were used for this study. Survey data of 18 post-stroke men and 28 healthy controls were obtained. Both the lean and fat masses of the upper and lower limbs were utilized to compare for the compositions between the limbs in post-stroke subjects. In addition, the effect of exercise habit was also observed for the influence of physical activity in body composition. Mixed results in left and right limb compositions were shown between the groups. When the subjects were further divided based on walking days per week, sedentary (walk ${\leq}2d/wk$) post-stroke group showed significantly greater fat mass and less lean mass than the physically active people (walk ${\geq}3d/wk$). In comparison to the healthy sedentary and physically active controls, two post-stroke groups showed greater variations. The results indicate that physical activity maintains or improve the quality of both the upper and lower limb composition in patients with post-stroke men.

건측 위주의 양측자침이 뇌졸중 환자의 NIH Scale상의 기능 회복에 미치는 영향 (Effects of Contralateral Both Side Acupuncture on NIH Scale in Stroke Patients)

  • Objective : In order to study the effect of contralateral both side acupuncture on recovery of motor disorders in stroke patients, a clinical study was performed. Methods : Thirteen patients with post-stroke hemiplegia were randomized into two groups. Six patients (test group) were treated with contralateral both side acupuncture. The other seven patients (control group) were treated with ipsilateral both side acupuncture. The activity of daily living was measured with a National Institutes of Health (NIH) scale. The therapy was performed once a day for 3 weeks. Results : In terms of score of NIH, the test group showed statistically meaningful increase after I weeks treatment, while the control group showed statistically meaningful increase after 2 weeks (P<0.05). The results showed no statistically meaningful difference after 3 weeks treatment between the groups. Conclusions : These results support that contralateral both side acupuncture therapy has almost the same effectiveness compared with ipsilateral both side acupuncture therapy in improvement of the activity of daily living of post-stroke hemiplegic patients.

  • Objectives : The purpose of this study is to evaluate the effect of A-Shi point acupuncture on the post-stroke hand edema. Methods : The subjects in this study were 8 patiens with post-stroke hand edema. The patients were treated with acupuncture on A-Shi point in hand. We treated them once a day, 7 days. The effectiveness was assessed by hand volumeter and mesuring tape. And Wilcoxon signed rank test was performed to evaluate the effect. Results : A significant change was observed after 7 times acupuncture treatment. Both volume and circumference of hand were decreased after treatment. Conclusions : This study suggests that A-Shi point acupuncture is effective for reducing post-stroke hand edema, although further study would be necessary.

퇴원후 뇌졸중환자의 재적응과 사회적 지지와의 관계분석 (Readjustment and Social Support of the Post Hospitalized Stroke Patients)

  • An explanatory design was employed to identify the relationship of physical, emotional & social readjustment and social support of post hospitalized stroke patients and their caregivers. A convenient sample of 254 patients who given follow-up care at the outpatient department after discharge and 225 caregivers were recruited. Mental Status Questionnaire (MSQ), Social Support Inventory Stroke Survivors (SSISS), Illness intrusiveness(II), Instument Activity of Daily Living(IADL), Center of Epidemilogic Studies-Depression(CES-D), social activity and caregiver burden were used for measurement in this study. Results showed patient's physical level measured by IADL and psychological level measured by depression were high. But social activity was low. Cognitive function, depression & social activity were not significantly different by the posthospitalized period, but IADL was. The source of professional support was mostly the physician at the outpatient department. The family support was found significantly related to patient's depression & social activity and caregiver's subjective burden. Professional support was found significantly related to patient's IADL & depression. Illness intrusiveness as a mediating variable was a sig nificantly predicting power on patient's IADL & depression. The path analysis was used to identify the variables to predict the physical, emotional, and social status of patients. As a result, patient's age, cognitive function, illness intursiveness and professional support significantly predicted the level of IADL ; patient's cognitive function, illness intrusiveness and family support significantly predicted the level of depression ; and patient's age and family support significantly predicted the level of social activity of posthospitalized stroke patients. Based upon these results, the rehabilitation programs to reduce the illness intrusiveness and improve cognitive funtion were recommended for the readjustment of the stroke patients. This model of the readjustment of the posthospitalized stroke patients is recommended as the framework for care of the stroke patients.

The effect of a water exercise on gait characteristics in the elderly post stroke patients

  • Objective: To investigate the effect of long-term water exercise on gait parameters in the elderly post stroke patients. Design: One group pretest-posttest design. Methods: Twenty elderly post stroke patients (age: $56.5{\pm}10.1$ years, height: $168.6{\pm}5.8cm$, body mass: $69.3{\pm}6.9kg$, handicapped level: $2.5{\pm}0.8$) participated in this study. Participants took part in gait training of 1 hour per day, three days per week, for three months. Participants' gaits were analyzed to find the effect of 3 months water exercise by a 3-D motion analysis with 8 infrared camera and 1 force plate (sampling frequency: 100 and 1,000 Hz, respectively). A paired t-test was used to find the significance with significant level as 0.05. Results: Gait velocity, step length & step width significantly increased, and asymmetric index for gait parameters significantly decreased after the water exercise (p<0.05). Conclusions: The water exercise effectively affect on patients' gait ability and electromyography analysis will be needed further study.

  • Objectives: The aim of this study was to analyze the change of lateral shift of Center of Pressure (CoP) according to the gait improvement in post-stroke hemiplegic patients and to investigate relationship between the change of motor grade and lateral shift of CoP. Methods: We measured the lateral shift of CoP and motor grade of eight post-stroke hemiplegic patients at the beginning of dependent gait and independent gait. Results: We found that CoP tended to be shifted to the non-affected side when patients started to walk independently. Furthermore, there was no relationship between the change of motor grade and lateral shift of CoP. Conclusions: This result may suggest it is more important to control the non-affected side than the affected side, at least until the beginning independent gait.

  • Objectives: This study was performed to evaluate the efficacy of Korean Medicine on post-stroke patients with upper limb pain due to complex regional pain syndrome diagnosed by three-phase bone scan, digital infrared thermal imaging, and International Association for the Study of Pain diagnostic criteria Methods: To evaluate the effectiveness of the treatments, visual analogue scale, medical research council grade, Brunnstrom stage of motor recovery, modified Ashworth scale, and Korean modified Barthel index were used. Results: After Korean medical treatments like acupuncture, herb medication, bee-venom therapy and rehabilitation therapy for 4 weeks, upper limb pain was considerably less and function of upper limbs was improved. Conclusions: Korean medical treatments have some good effects on post-stroke patients with complex regional pain syndrome.
