양 하지 불편감을 호소하는 교뇌 경색 환자에 대한 한의 치험 1례 (A Case of a Patient with Pontine Infarction Complaining of Discomfort in Both Legs)

  • Objective: This study examined the effectiveness of Korean medical treatment for an 81-year-old female patient with pontine infarction complaining of discomfort in both legs. Methods: The patient was hospitalized for 45 days and treated with herbal medicine (mainly Bohyulanshin-tang [補血安神湯], pharmacopuncture, acupuncture, electroacupuncture, and moxibustion). We used the International Restless Legs Scale (IRLS) and Numerical Rating Scale (NRS) and checked the duration of the symptoms to evaluate the clinical effects of the treatment. Results: After 45 days of hospitalization, the IRLS score decreased from 33 to 19, with the lowest score of 8 recorded on the twenty-seventh day after discharge. The NRS score and the duration of symptoms also improved, and the treatment effect continued following discharge. Conclusions: This study suggests that Korean medical treatment may be effective for the treatment of restless legs.

급성 원발성 교뇌 출혈 환자의 하나반증후군, 현기증, 이상감각에 대한 한양방 협진치료 : 증례보고 (A Case Report of Acute Primary Pontine Hemorrhage with One-and-a-Half Syndrome, Vertigo, and Paresthesia Treated with Eastern-Western Integrative Medicine)

  • Background: The purpose of this study was to report the improvement of a patient with one-and-a-half syndrome, vertigo, and paresthesia caused by acute primary pontine hemorrhage (PPH) after a combination treatment of traditional Korean and Western medicine. Case report: A 51-year-old female with one-and-a-half syndrome, vertigo, and paresthesia after PPH was treated with Korean medicine, including herbal medication, acupuncture, and moxibustion, and Western medicine, including medication and rehabilitation therapy during hospitalization. Her progress was evaluated by checking for changes in symptoms with the extraocular muscle (EOM) function test, numeral rating scale (NRS), and follow-up brain computed tomography scans and magnetic resonance imaging (MRI). After 41 days of treatment, the EOM movement was improved, leaving limited abduction of the left eye. The NRS scores for vertigo and paresthesia decreased from 10 to 5 and from 10 to 3, respectively. Improvement was noted in hematoma in MRI, but a new ischemic lesion was also discovered. Conclusion: This case reports the clinical course of one-and-a-half syndrome and suggests that a combined therapy of traditional Korean and Western medicine can be useful for PPH patients with one-and-a-half syndrome, vertigo, and paresthesia. However, studies of larger populations are required.

묘성증후군 환아의 뇌 자기공명영상 소견: 증례 보고 및 정리 (Brain MRI Findings of the Cri-Du-Chat Syndrome: A Case Report and Summary)

  • 묘성증후군은 드문 유전자 결손 증후군으로, 환아는 후두 및 후두개의 기형과 신경학적 구조적 이상으로 인해 특징적인 높은 톤의 단조로운 울음소리를 내고 반복적인 흡인성 폐렴을 앓는다. 이전 보고된 증례들의 뇌 자기공명영상 소견을 정리한 결과 교뇌 저형성이 가장 뚜렷하였고, 소뇌 저형성이 동반되기도 하여 주로 후두개와의 이상 소견을 보였다. 천막상부 구조물의 위축도 자주 관찰되었는데 이것은 교뇌 저형성에 의한 이차적인 변화로 생각되었다. 본원에서 확진된 3개월 환아 또한 교뇌 저형성이 두드러져 이전 보고된 증례들과 거의 유사하였으나 수초화 양상에서 내섬유막 전완의 수초화 감소가 아니라 전반적인 수초화의 지연이 관찰되었다는 점에서 타 증례와 차이가 있었다. 후두, 교뇌, 소뇌는 비슷한 척삭에서 유래하므로, 묘성증후군에서 생기는 후두 및 뇌의 구조적 이상은 발생 초기의 이상임을 시사한다.

전신성 홍반성 루푸스의 초기 증상으로 나타난 교뇌경색의 증례 보고: 자기공명영상 및 디지털감산 혈관조영술에서의 진단 단서 (A Case Report of Pontine Infarction as an Initial Manifestation of Systemic Lupus Erythematosus: Diagnostic Clues from MRI and Digital Subtraction Angiography)

  • 척추기저동맥의 혈류공급감소로 인한 뇌간 경색은 매우 드문 전신성 루푸스 환자의 초기 증상으로, 경색의 직접적인 원인으로 매우 작은 기저동맥 분지인 교뇌공급혈관의 박리성 동맥류는 보고된 사례가 없다. 이에 저자들은 디지털감산 혈관조영술과 고해상도 혈관벽 자기공명영상를 이용하여 작은 교뇌공급혈관의 박리성 동맥류의 진단과 추적관찰 중 치유된 20세 여성의 사례를 보고하고자 한다. 전신성 루푸스의 진단은 신경학적장애의 유무와 혈액화학검사 결과를 바탕으로 하였다. 추적 고해상도 혈관벽 자기공명영상에서 환자의 교뇌천공지의 박리성동맥류는 폐색되어 있었고 우측 척추동맥의 박리성동맥류는 치유되어 보이지 않았다. 환자는 퇴원 시 수정랭킨척도 점수가 1점으로 증상 개선을 보였으며 3개월과 12개월 추적관찰에서도 증상이 악화되지 않고 1점을 유지하였다.

단안 외전신경마비를 보이는 뇌경색 환자 치험 1례 (A Case of Monocular Abducens Nerve Palsy in a Patient with Pontine Infarction)

  • ■ Objective This is a case report about monocular abducens nerve palsy in a patient with pontine infarction. ■ Method At the time of onset, the patient had eye movement impairment in right eye and diplopia. Brain MRI image showed that there was an infarction in the right pons where the nucleus of abducens nerve is located. There was no obvious improvement in the eye movement before he received Korean medicine treatment. He recieved Korean medicine including acupuncture, electroacupuncture, pharmacoacupuncture and herb medicines for fifty three days. ■ Result After treatment, the movement of right eye was improved to a normal range. The difference in distance from 'center of the pupil' to 'external canthus of the eye' at maximum abducent in both eye changed 0.9cm to 0.1cm at the discharge. In addition, Diplopia was improved at the discharge. ■ Conclusion This case report demonstrated that the Korean medicine treatment is effective to diplopia and eye movement impairment from abducens nerve palsy caused by pontine infarction.

뇌교 경색에 의한 일측성핵간마비로 유발된 외사시 환자에 대한 증례보고 (A Clinical Study on One Case of Exotropia Caused by Unilateral-internuclear- ophthalmoplegia due to Pontine Infarction)

  • Objectives : This study is to report one case of the patient who has exotropia caused by unilateral internuclear ophthalmoplegia due to pontine infarction, which is a rare disease so there has few research about the effect of acupuncture therapy on it, treated with the SAAM(舍巖) Acupuncture Sojangjeonggyeok(小湯正洛) Methods & Results : In the point of differentiation of Syndrome, these subjects were diagnosed as deficiency of Yin and blood(陰血不足). We treated her with SAAM Acupuncture Sojangjeonggyeok(小湯正洛), herb medication(淸肝湯, 六味地黃湯), and the symptoms were resolved clearly by above treatment during about a week. Conclusion : This case shows proper oriental medical treatment based on the exact differentiation of symptoms has good effect on this disease as one of conservative therapies.

Intraoperative Neurophysiological Monitoring in Cerebello Pontine Angle Tumor

  • Intraoperative Neurophysiological Monitoring (INM) inspection has a very important role. While preserving the patient's neurological function be sure to safe surgery, neurological examination should thank. Cerebello pontine angle tumor surgery, especially in the nervous system is more important to the meaning of INM. In cochlear nerve, facial nerve, trigeminal nerve, which are intricate brain surgery, doctors are only human eye and brain to the brain that it is virtually impossible to distinguish the nervous system. They receives a lot of help from INM. In this paper, we examined six kinds broadly. First, the methods of spontaneous EMG and Free-running EMG, which can instantly detect a damage inflicted on a nerve during surgery. Second, methods of triggered EMG and direct nerve electrical stimulation, which directly stimulate a nerve using electricity to distinguish between nerves and brain tumors. Third, the method of knowing a more accurate neurologic status by informing neurological surgeons about Free-running EMG wave forms that are segmetalized into four. Fourth, three ways of knowing when a patient will be awaken from intraoperative anesthesia, which happens due to a weak anesthetic. Fifth, a method of understanding the structures of a brain tumor and a facial nerve as five dividend segments. Sixth, comparisons between cases normal facial nerve recovery and occurrence of a facial nerve paralysis during the postoperative course.

  • Anterior inferior cerebellar artery(AICA) is a branch of the basilar artery which supplies the bloods for ventrolateral cerebellum and the lateral tegmentum of the lower two-thirds of pons. AICA occlusion can cause such symptoms as acute-onset unilateral deafness, vertigo, facial weakness and ataxia. A case of sudden hearing loss, facial palsy and vertigo caused by AICA territory infarction recently presented itself. Korean Oriental and Western medical therapy were applied in combination. Facial palsy and vertigo improved, but hearing loss did not improve. This case is reported with a brief overview of related literatures.

뇌의 REM 수면 발생기전 (Brain Mechanisms Generating REM Sleep)

  • The author reviews current knowledge about what REM sleep is and where and how it is generated. REM sleep is the state in which our most vivid dreams occur. REM sleep is identified by the simultaneous presence of a desynchronized cortical EEG, an absence of activity in the antigravity muscles(atonia), and periodic bursts of rapid eye movements. Another characteristic phenomena of REM sleep are the highly synchronized hippocampal EEG of theta frequency and the ponto-geniculo-occipital(PGO) spike. All these phenomena can be explained in terms of changes in neuronal activity. Transection studies have determined that the pons is sufficient for generating REM sleep. Lesion studies have identified a small region in the lateral pontine tegmentum corresponding to lateral portions of the nucleus reticularis pontis oralis(RPO) and the region immediately ventral to the locus coeruleus, which is required for REM sleep. Unit recording studies have found a population of cells within this region that is selectively active in REM sleep. Cholinergic neurons of the giant cell field of pontine tegmentum(ETG), which is 'REM a sleep-on cells', has shown to be critically involved in the generation of REM sleep. Noradrenergic neurons of the locus coeruleus and serotonergic neurons of the dorsal raphe, which are called 'REM sleep-off cells', appear to act in a reciprocal manner to the cholinergic neurons. It is proposed that the periodic cessations of discharge of 'REM sleep-off cells' during REM sleep might be significant for the prevention of the desensitization of receptors of these neurons.
