Statement of problem : Use of fiber composite technology as well as development of nonmetal implant prosthesis solved many problems due to metal alloy substructure such as corrosion. toxicity, difficult casting, expensiveness and esthetic limit. After clinical and laboratory test, we could find out that fiber-reinforced composite prostheses have good mechanical properties and FRC can make metal-free implant prostheses successful. Purpose : The purpose of this study is to evaluate the flexural strength of implant fixed prosthesis using fiber reinforced composite. Material and methods : 2-implant fixture were placed in second premolar and second molar area in edentulous mandibular model, and their abutments were placed, and bridge prostheses using gold, PFG, Tescera, and Targis Vectris were fabricated. Tescera was made in 5 different designs with different supplements. Group I was composed by 3 bars with diameter 1.0mm and 5 meshes, 2 bars and 5 meshes for Group II, 1 bar and 5 meshes for Group III, and only 5 meshes were used for Group IV. And Group V is composed by only 3 bars. Resin (Tescera) facing was made to buccal part of pontic of gold bridge. All of gold and PFG bridges were made on one model, 5 Targis Vectris bridges were also made on one model, and 25 Tescera bridges were. made on 3 models. Each bridge was attached to the test model by temporary cement and shallow depression was formed near central fossa of the bridge pontic to let 5 mm metal ball not move. Flexual strength was marked in graph by INSTRON. Results : The results of the study are as follows. The initial crack strength was the highest on PFG. and in order of gold bridge Tescera I, Tescera II, Targis vectris, Tescera IV, Tescera III, and Tescera V. The maximum strength was the highest on gold bridge, and in order of PFG, Tescera I, Tescera IV Tescera II, Targis vectris, Tescera III, and Tescera V. Conculsions : The following conclusions were drawn from the results of this study. 1. Flextural strength of implant prosthesis using fiber reinforced composite was higher than average posterior occlusal force. 2. In initial crack strength, Tescera I was stronger than Tescera V, and weaker than PFG. 3. Kinds and number of auxillary components had an effect on maximum strength, and maximum strength was increased as number of auxillary components increased. 4 Maximum strength of Tescera I was higher than Targis vectris, and lower than PFG.
To enhance the esthetic appearance, the maxillary anterior area is important. It is possible to improve the esthetic appearance through the treatment of maxillary anterior area, which includes altering the color, form, and arrangement of teeth. When planning these treatments, clinicians should individualize personal demands, by using the information obtained from facial, dento-labial, dental, and gingival analysis. It is essential to properly prepare the gingival structure, which includes the height of gingival margin, the location of zenith, reconstruction of the interdental papillae, emergence profile, and symmetry. Clinicians often face unfavorable condition of the gingiva and the edentulous ridge, and appropriate management of the gingival structure is needed. In this case report, the patients were treated to improve the gingival conditions surrounding maxillary anterior teeth. By using conservative treatment without surgical intervention, such as application of pink porcelain, subgingival contour modelling and modification of pontic base, satisfactory esthetic results were gained.
Purpose: The aim of this study was to know whether there is significant difference of peri-implant bone density according to the state of antagonist region. Materials and methods: 51 patients who had implant operation in Daejeon Dental Hospital of Wonkwang University participated in this study and total of 51 implants were analyzed. Implants were classified depending on opposing antagonist region, gender, age and location of jaw. The opposing antagonist region was divided into four groups; natural tooth, implant, pontic and edentulous region. Fractal analysis was performed using two periapical radiographs; one after implant placement and the other after 10 weeks following prosthetic restoration. The analysis was done by Image J. The data was statistically analyzed using one-way ANOVA and Tukey multiple comparison test. Results: The mean value of fractal difference was $0.009{\pm}0.048$ with opposing natural tooth, $0.026{\pm}0.080$ with opposing implant, $0.025{\pm}0.068$ with opposing pontic and $0.093{\pm}0.171$ with opposing edentulous area. There was a statistically significant difference in fractal value between opposing implant and opposing edentulous state. And there was no statistically significant difference according to age, gender and location of jaw. Conclusion: There was no statistically significant difference between 3 groups except opposing edentulous region and there was a statistically significant difference between opposing implant and edentulous region. And there was no statistically significant difference according to age, gender and location of jaw.

Purpose: To evaluate the effects of load direction, number of implants, and alignment of implant position on stress distribution in implant, prosthesis, and bone tissue. Materials and Methods: Four 3D models were made to simulate posterior mandible bone block: two implants and 3-unit fixed dental prosthesis (FDP) with a pontic in the center (model M1), two implants and 3-unit FDP with a cantilever pontic at one end (model M2), FDP supported by three implants with straight line placement (model M3) and FDP supported by three implants with staggered implant configuration (model M4). The applied force was 120 N axially or 120 N obliquely. Results: Peak von Mises stresses caused by oblique occlusal force were 3.4 to 5.1 times higher in the implant and 3.5 to 8.3 times higher in the alveolar bone than those stresses caused by axial occlusal force. In model M2, the connector area of the distal cantilever in the prosthesis generated the highest von Mises stresses among all models. With the design of a large number of implants, low stresses were generated. When three implants were placed, there were no significant differences in the magnitude of stress between staggered arrangement and straight arrangement. Conclusion: The effect of staggering alignment on implant stress was negligible. However, the number of implants had a significant effect on stress magnitude.
Purpose: This in vitro study aimed to compare the trueness of 3-unit fixed dental provisional prostheses (FDPs) fabricated by three different additive manufacturing and subtractive manufacturing procedures. Methods: A reference model with a maxillary left second premolar and the second molar prepped and the first molar missing was scanned for the fabrication of 3-unit FDPs. An anatomically shaped 3-unit FDP was designed on computer-aided design software. 10 FDPs were fabricated by subtractive (MI group) and additive manufacturing (stereolithography: SL group, digital light processing: DL group, liquid crystal displays: LC group) methods, respectively (N=40). All FDPs were scanned and exported to the standard triangulated language file. A three-dimensional analysis program measured the discrepancy of the internal, margin, and pontic base area. As for the comparison among manufacturing procedures, the Kruskal-Wallis test and the Mann-Whitney test with Bonferroni correction were evaluated statistically. Results: Regarding the internal area, the root mean square (RMS) value of the 3-unit FDPs was the lowest in the MI group (31.79±6.39 ㎛) and the highest in the SL group (69.34±29.88 ㎛; p=0.001). In the marginal area, those of the 3-unit FDPs were the lowest in the LC group (25.39±4.36 ㎛) and the highest in the SL group (48.94±18.98 ㎛; p=0.001). In the pontic base area, those of the 3-unit FDPs were the lowest in the LC group (8.72±2.74 ㎛) and the highest in the DL group (20.75±2.03 ㎛; p=0.001). Conclusion: A statistically significant difference was observed in the RMS mean values of all the groups. However, in comparison to the subtractive manufacturing method, all measurement areas of 3-unit FDPs fabricated by three different additive manufacturing methods are within a clinically acceptable range.
A depressed alvelolar ridge and a collapsed interdental papilla in the anterior region may interfere with esthetic prosthodontic repair. The resultant sequelas will cause overcontoured prosthesis or dark shadow under pontic which may not suffice our patients' esthetic demands. So, surgical periodontal treatment has been needed for esthetic repair in deformed ridges and extensive interproximal spacing. Our treatment option alters Dr.Abram's roll technique to make palatally extended connective tissue secondary pedicle flap to obtain additonal connective tissue. Blood supply maintenance, least trauma, and no secondary wound is advantages in this modification.
Resin-bonded bridge has been an alternative to conventional bridge, since resin-bonded bridge has many attractive advantages such as minimal tooth preparation, short chair time and low cost over conventional bridge. Unfortunately, however, it was reported that resin-bonded bridge showed high failure rate from debonding of retainer in spite of consecutive advances in preparation and materials. And it was shown that multiple abutments were more likely to fail. The majority of debonding failure was considered due to the mobility of the abutment during function. In this view, recently, modification in resin-bonded bridge design was tried. Single retainer, single pontic. 2-unit cantilevered resin-bonded bridge was applied to clinical performance and was shown as retentive or more retentive than fixed-fixed type resin-bonded bridge. This was consistent with the results of studies in 2-unit cantilevered resin-bonded bridges made with all ceramic, In-ceram. The purpose of this article was to overview principles of design and to analyze clinical results of 2-unit cantilevered resin-bonded bridge in comparison with the reports of fixed-fixed resin-bonded bridge.

A fiber-reinforced composite (FRC) fixed prosthesis is an innovative alternative to a traditional metal restoration, as it is a conservative treatment method. This case report demonstrates a detailed procedure for restoring a missing anterior tooth with an FRC. A 44-year-old woman visited our department with an avulsed tooth that had fallen out on the previous day and was completely dry. This tooth was replanted, but it failed after one year. A semi-direct technique was used to fabricate a FRC fixed partial prosthesis for its replacement. The FRC framework and the pontic were fabricated using a duplicated cast model and nanofilled composite resin. Later on, interproximal contact, tooth shape, and shade were adjusted at chairside. This technique not only enables the clinician to replace a missing tooth immediately after extraction for minimizing esthetic problems, but it also decreases both tooth reduction and cost.
The purpose of this study was to determine the displacement of prosthesis & abutment and the stress distribution patterns induced in the periodontium by applying force to the fixed prosthesis. Two levels of periodontal support were compared using two-dimensional finite element stress analysis after placement of 3unit or 4 unit fixed partial denture(FPD) in case of missing of the lower first molar. Concentrated vertical load was delivered at the cusp tip of the second bicuspid or the central fossa of the pontic. The following results were obtained : 1. The greater the loss of alveolar bone in abutment teeth area, the greater the displacement of FPD and the stress concentration in alveolar bone around abutment. 2. The amount and direction of displacement and distribution of stress in the 4-unit FPD was better than those in the 3-unit FPD. 3. Multiple abutments reduced the amount of mesial and downward displacement of the weaked abutments and more uniformly distributed the stresses.
All ceramic restorations except In-Ceram Alumina system gave a good esthetics and an exellent marginal fidelity. The flexural strength of them had about 150MPa, so the indication is only single crown. By using In-ceram Alumina System(450Mpa), it is thought to be possible to construct bridge for its high flexural strength. But the prognosis is unclear, The purposes of this study are to clear short term prognosis of In-Ceram bridge restorations, to elucidate its clinical significance. Among 22 In-Ceram Bridge restored in our department, 11 In-Ceram bridges with follow up were used. The period of placement is from 1 to 18 months. The results were as follows : 1. Among follow up 11 bridges, 2 bridges were fractured. One is 4 unit in maxillary lateral incisors, the other is 3 unit bridge in maxillary canine and premolar. Including 11 bridge without follow up, failure rate is very low(2/22). 2. The fracture sites are connector areas between abutment and pontic. To maintain In-Ceram bridge for long term period, it is needed to remove the nonphysiologic occlusal force and to have sufficient thickness of alumina core. For estabilishing clinical use of In-Ceram bridges, it is thought to need clinical research during long term period.
