Mucoepidermoid carcinoma of the upper lobar bronchus -2 cases report- (상엽 기관지내에 발생한 점막표피암 -2례 보고-)

  • Mucoepidermoid carcinoma arising in the tracheobronchial tree is an extremely rare tumor. Usually it remains as locally invasive neoplasm, although malignant change is described. Histologically, it is characterized by an admixture of vacuolated, mucus producing cells and sheets of epithelial cells with a cohesive pattern which resemble squamous cells. Its clinical and histopathological behaviors were reported as varying degrees of benign to extremely malignant. We had experienced two young patients with low-grade mucoepidermoid carcinoma of the right and left upper lobar bronchi. A 15-year-old man who had had intermittent hemoptysis for 1 year underwent right upper lobectomy. And the other 18-year-old man had suffered from obstructive pneumonitis for 6 months underwent left pneumonectomy. The postoperative courses were uneventful, and the bronchoscopy and chest CT which were done at 6 months later revealed no regional recurrence.

  • Mucoepidermoid tumor arising from the bronchial tree as one of the bronchial adenoma is a extremely rare in incidence. And its clinical and histopathologic behavior has been reported as varying degree of benign to extremely malignant. Because symptoms are usually related to the bronchial obstruction or obstructive pneumonitis followed by endobronchial growth of tumor, frequently error is made in diagnosis of this tumor as entity of obstructive lung disease. We present a case of mucoepidermoid tumor with review of relevant literatures arising from the right middle lobe bronchus extending to intermediate bronchus in 47 years old housewife and was surgically removed by middle and lower Iobectomy.

Radioprotective Effect of Cyclo(L-Phenylalanyl-L-Prolyl) on Irradiated Rat Lung

  • In the present study, we investigated the radioprotective effect of cyclo(L-phenylalanyl-L-prolyl) on irradiated rat lungs to determine its potential as a radioprotective agent. We found that early lung damage induced by irradiation was reduced by treatment with 40mg/kg of cyclo(L-phenylalanyl-L-prolyl) in the latent and early pneumonitis phases. Expression of $TNF-{\alpha}\;and\;TGF-{\beta}1$ at 2 and TGF-${\beta}1$ at 8 weeks post-irradiation was decreased in animals that received both radiation and cyclo(L-phenylalanyl-L-prolyl) compared with animals that received radiation alone. Evidence indicated that the proinflammatory cytokine TNF-${\alpha}$ and the fibrogenic cytokine TGF-${\beta}1$ likely play a role in the radioprotective effect of cyclo(L-phenylalanyl-L-prolyl). However, besides TNF-${\alpha}$ and TGF-${\beta}1$ expressions, the precise mechanism by which cyclo(L-phenylalanyl-L-prolyl) ameliorates the induced radiation damage is not clear.

  • Occult bronchial foreign body is that long-standing foreign body lodge in bronchial tree. Occult bronchial foreign bodies arc rare in adults, whereas tracheobronchial aspiration of foreign bodies occurs commonly in children. A 65-year-old man with chronic cough, sputum production, and fever was transferred for treatment of right middle and lower lobc collapse and obstructive pneumonitis as evidenced by imaging studies. The patient was treated with right middle-lower bilobectomy because fiberoptic bronchoscopic removal of the foreign body failed. We report this case with review of literatures.

Multicentric Form of Lymphosarcoma in a Dog (개에서의 다중심형 림프육종의 발생례)

  • A 3-year old male Maltese, which had enlarged and ballooned superficial lymph-nodes, was admitted to the Veterinary Teaching Hospital, College of Veterinary Medicine, Kyung-pook National University. Radiographic findings indicated cranial mediastinal mass, interstitial pneumonitis, conspicuous splenomegaly and liver enlargement. The patient showed anorexia, depression and anemia. It was autopsied following its death 2 days after admittance. There were copious hemoperitoneum, lymphomatous mass in the spleen, torsion of the mesenterium and stenosis of the duodenum. On the microscopic observation, there was the increase of the amount of lymphocytic cytoplasm and numbers of nucleus-devided lymphocytes followed by their diffused infiltration in lymph nodule. Based on the physical, histological and pathological findings, this case was diagnosed as a multicentric form of lymphosarcoma.

A Case of Liver Abscess in A Child (소아에서 발병한 간농양 1예)

  • Liver abscess in children is rare in developed countries; the incidence is 25 per 100,000 admissions in USA. Common complications are pleural effusion, empyema, pneumonitis, hepatopleural or hepatobronchial fistula, intraperitoneal or intrapericardiac rupture, septic shock, cerebral amebiasis, etc. These complications may lead to death if the management is delayed. However, recent management results in a mortality of less than 15%. We report a case of liver abscess in a child. He manifested with fever and abdominal pain in the right upper quadrant. On computerized tomography scans, multiple cystic lesions were seen in both lobes of the liver and were 5 to 55 mm in size. In laboratory findings, neutrophilic leukocytosis, peripheral eosinophila, elevated values of ESR, C-reactive protein, and elevated serum AST, ALT, ALP and GGT were detected. Furthermore, we determined the organisms in the blood culture and serum. Blood culture was positive for Streptococcus spp., and amebic indirect hemagglutination antibody titer was increased to 1:512.

  • Cytomegalovirus (CMV) pneumonitis leading to inflammation and obstruction of the tracheobronchial tree may cause the cystic changes in the lung. We performed segmentectomy of lung under the diagnosis of congenital cystic lung disease in an infant of 2 weeks presenting severe respiratory failure. Histology and serology confirmed congenital CMV bronchiolopneumonitis.

  • On the study of surface activity in excized lung extracts of various pulmonary diseases, following facts were concluded. 1]The minimum surface tension measured in lung extracts of tuberculous tissue surrounding cavitary lesion was 26.3dyne/cm and its stability index was 0.53. 2]Macroscopically almost normal lung tissue at a distance of tuberculous lesion in same lobe revealed 21.3 dyne/cm of minimum surface tension in extracts and its stability index showed 0.66. This low surface activity may be due to the chronic pneumonitis microscopically. 3] In the atelectatic lung which had been collapsed by chronic empyema the extracts revealed much higher minimum surface tension in 27.3 dyne/cm and its stabillry index revealed the least value of 0.47 without correlation of duration of disease. This suggests that the longstanding collapsed lung may be soon collapsed even after mechanical full expansion because of lack of surfactant.

  • P. carinii is a protozoan which induces an often fatal pneumonitis in a variety of compromised patients. The ultrastructure of P. carinii was studied in a male infant with pneumocystitis pneumonia associated with hypogammaglobulinemia. Four principal structural varieties-small trophozoites, large trophozoites, mature cyst and empty cyst were identified. The ultrastructure of these organisms was similar to the cases previously reported. Relevance of the morphologic findings to the functional aspect were discussed.

  • A male Blue-fronted amazon parrot suddenly died after having a history of depression, anorexia and emaciation. At necropsy, numerous well-demarcated yellow to white firm nodules were scattered throughout the left caudal lung lobes and adjacent air sac. Histologically, the wall of air sac was significantly thickened due to necrosis and infiltration of large numbers of fungi and degenerated heterophils. Multifocal necrotizing granulomatous lesions with numerous fungal hypae as well as vasculitis and thrombosis were found in tl1e lung. The fungi had conidiophore, vesicle, phialides and conidia which were characteristic of Aspergillus sp. This is tl1e first report of mycotic air sacculitis and pneumonitis caused by Aspergillus in a parrot in Korea.
