늑막중피세포종 2례 보고 (Pleural Mesothelioma [Report of 2 cases])

  • Pleural mesothelioma which arise from pleura is relatively uncommon tumor. We are reporting 2 cases of pleural mesothelioma which were treated with surgical resection. First case, benign epithelial mesothelioma was confirmed incidentally after decortication due to localized pleural thickening. The second case, malignant mesothelial mesothelioma was diagnosed by examination of chest radiology, diagnostic pneumogram and pleural biopsy as malignancy which was treated with the resection of the tumor mass. In the first case, postoperative recurrence of tumor growth was found within 1.5 months after resection. In the section malignant case, no evidence of recurrence was noted even after 3 months of resection.

국소형 흉막 중피세포종: 3례 보고 (Localized Pleural Fibrous Mesothelioma - Report of 3 Cases -)

  • Pleural mesothelioma is usually divided into two forms of localized and diffuse type. Localized pleural fibrous mesothelioma is uncommon mesodermal neoplasm, which may occurs in both sexes and at the age of 50 years. This type of mesothelioma is usually asymptomatic and detected on routine chest X-ray and made fibrous tissue and shows of collagen fibers microscopically. Most localized fibrous mesothelioma arises from the visceral pleura and is well encapsulated and pedunculated mass. CT findings included well delineated, often lobulated, non-calcified soft tissue masses in close relation to a pleural space, associated pleural thickening, and absence of chest wall invasion and a peripheral or fissure location. Three cases of localized pleural fibrous mesothelioma diagnosed by resectional surgery were reported with the review of literature.

  • Pleural mesothelioma is the tumor of the cell of mesodermal origin lining the pleura. It is relatively uncommon tumor and its localized form is much rarer than diffuse form. Authors experienced a localized mesothelioma in a patient who was 44 year old male worker at copper plumbing field for 20 years, and admitted due to incidentally found abnormal chest X-ray. Exploratory thoracotomy was done and a 23 x 16 x 8 cm sized solitary mass was resected with adherent right middle lobe. Low grade malignancy of the pleural mesothelioma was confirmed by the pathology. We report the case with a literature review.

개의 악성중피종의 흉강내 Carboplatin 투여 증례 (Intracavitary Carboplatin Chemotherapy in a Malignant Mesothelioma Dog)

  • A 10-year-old castrated male Siba dog was presented for signs referable to pleural effusion associated with neoplasm of the thoracic cavity. The pleural effusion fluid consisted of blood and tumor cells by thoracocentesis. Histopathological examination of the sedimentary tumor cells revealed malignant mesothelioma. Intracavitary carboplatin was administered at 300 mg/$m^2$ every 5 weeks for 3 treatment and pleural effusion was disappeared after 3 treatments. The dog had recurrence of pleural effusion at 515 days but intracavitary carboplatin chemotherapy had no effect. It would be thought that the intracavitary carboplatin treatment was quite a useful method to control a canine malignant mesothelioma with minimal toxicity.

  • Malignant mesothelioma is a common, primary tumor that can invade pleura, and is associated with previous exposure to asbestos. However, it poses considerable difficulties regarding its diagnosis and treatment, and thus, accurate history taking with respect to exposure to asbestos, and radiologic and pathologic examinations are essential. In addition, the involvement of a multidisciplinary team is recommended in order to ensure prompt and appropriate management using a framework based on radiotherapy, chemotherapy, surgery, and symptom palliation with end-of-life care. Because lymphocyte-dominant, exudative pleural effusion can occur in malignant mesothelioma, adenosine deaminase values may be elevated, which could be mistaken for tuberculous pleurisy, and lead to an incorrect diagnosis and suboptimal treatment. The authors describe a case of malignant mesothelioma initially misdiagnosed as tuberculous pleurisy. As evidenced by the described case, malignant mesothelioma should be considered during the differential diagnosis of patients with lymphocyte-dominant, exudative pleural effusion with a pleural lung lesion.

p53 Expression in a Malignant Mesothelioma Patient during Seven-Year Follow-up

  • Malignant mesothelioma (MM) is the aggressive tumor of serosal surfaces. There are crude pathogenetic results regarding the biology of MM. Coordinated upregulations of p53 gene expression are shown in malignancies. We believed that there are changes in the p53 expression with transformation from reactive hyperplasia to MM. A 65-year-old male was admitted the hospital because of left pleuritic chest pains in 2004. Chest computed tomography (CT) results showed left pleural effusions with loculation and pleural thickening. Pathologic findings revealed reactive mesothelial hyperplasia. In 2008, the patient again felt left pleuritic chest pains. Chest CT showed progressive thickening of the left pleura. Pathologic diagnosis was atypical mesothelial hyperplasia. In 2011, chest CT showed progressive thickening of his left pleura. He was diagnosed with well-differentiated papillary mesothelioma. Serial change was analyzed by immunohistochemical staining for p53 of pleural tissues. There were no remarkable changes in p53 expressions during the transformation to MM.

  • Localized mesothelioma is a primary pleural tumor that induces hypoglycemia rarely. We experienced a case of the localized mesothelioma that induced hypoglycemia. The patient was 70 year-old man who was admitted to Severance Hospital because of general weakness and mental confusion in morning before breakfast. He was found to have low level of fasting blood sugar and C-peptide, but had normal serum insulin level. After excision of the tumor, the patient was free from symptoms of hypoglycemia, and fasting blood sugar level was returned to normal range. The most likely speculation of causing hypoglycemia by the mesothelioma was thought to be that the tumor did not secrete insulin itself but the undetectable insulin-like substance and/or antigluconeogenic substances. During the follow up, 5 months after surgical removal of tumor, the patient was in a good condition without symptoms of hypoglycemia.

Malignant Pleural Mesothelioma Diagnosed by Endobronchial Ultrasound- Guided Transbronchial Needle Aspiration

  • A 61-year-old woman came to the hospital with dyspnea and pleural effusion on chest radiography. She underwent repeated thoracentesis, transbronchial lung biopsy, bronchoalveolar lavage, and thoracoscopic pleural biopsy with talc pleurodesis, but diagnosis of her was uncertain. Positron emission tomography showed multiple lymphadenopathies, so she underwent endobronchial ultrasound-guided transbronchial needle aspiration of mediastinal lymph nodes. Here, we report a case of malignant pleural mesothelioma that was eventually diagnosed by endobronchial ultrasound-guided transbronchial needle aspiration. This is an unusual and first case in Korea.

Mesotheliomas in Lebanon: Witnessing a Change in Epidemiology

  • Background: Mesotheliomas are relatively rare tumors in Lebanon. The only previous study goes back to 14 years ago, when we published epidemiological characteristics of mesotheliomas in Lebanon, showing that the pleural location accounted for the vast majority of cases, with clear evidence of asbestos exposure from the Eternit factory of Chekka region. The objective of this current study was to estimate the incidence of mesothelioma in the past decade and to identify its epidemiological, clinical and therapeutic characteristics, making comparisons with our first study published in 2001. Materials and Methods: Between 2002 and 2014, patients diagnosed with malignant mesothelioma at Hotel-Dieu de France University Hospital were investigated. Epidemiological data focusing on asbestos exposure history were collected from medical records and interviews with the families. Results: A total of 26 patients were diagnosed with mesothelioma, 21 of which were successfully investigated. The mean age of these 21 patients is 62.5 (19-82). Only 3 (14.29%) are women. 18 (85.71%) were smokers. Among the 21 available mesotheliomas, 15 (71.4%) are pleural, while 5 (23.8%) are peritoneal and 1 (4.8%) pericardial. Only 60% of patients with pleural mesothelioma and 50% of those with an obvious exposure to asbestos lived and/or worked in Chekka region. The mean time of asbestos exposure in patients with mesothelioma is 24.5 (1-50) years and the mean latency is 37.4 (4-61) years. Of the 21 patients, 10 (47.6%) underwent surgery during their treatment, 16 (76.2%) received chemotherapy and 3 (14.3%) received best supportive care. Conclusions: Compared to the previous study (1991-2000), substantial changes in the epidemiology of mesothelioma in Lebanon were observed, such as an increase in peritoneal localizations and a lower correlation with Chekka region asbestos contamination.
