Purpose: The purposes of the authors' analysis were to assess the values that plastic surgery journals demonstrate in terms of the standardized measures created by the Korea Medical Citation Index, and to assess the relationship between these values and the turnaround time of these journals. Methods: The overall indexes of J Korean Soc Plast Reconstr Surg(JKSPRS), J Korean Soc Aesthetic Plast Surg(JKSAPS), J Korean Cleft Palate-Craniofac Assoc (JKCPRA) were compared with those of journals related with Korean plastic surgery using the following parameters: impact factor, cited half-life, total articles, and the number of journals. Korean plastic surgery journals were compared with journals from relative fields. In addition, an evaluation of all original articles published in 2007, assessing the time intervals from submission to publication was conducted for Korean plastic surgery journals and various journals which were related with plastic surgery. Results: The number of articles for Korean plastic surgery journals for 2006 ranged from 19 for JKCPRA to 149 for JKSPRS. The time interval from submission to publication of an article among Korean plastic surgery journals for 2007 ranged from 73.7 days for JKSAPS to 176.2 days for JKSPRS. The variation in impact factor of JKSPRS for the period from 2002 to 2005 increased from 0.084 in the year 2002 to 0.168 in 2005. But the impact factor in 2006 has fallen to 0.112. Conclusion: JKSPRS demonstrated comparatively high overall index values and a short turnaround time in comparison to relative journals. To improve the status of Korean plastic surgery journals, members of Korean plastic surgeons should quate Korean plastic surgery journals and adjust key word to MeSH. The title written down in Korean should use medical terminology published by Korean medical association.
In this paper, the sensitivity of a plastic-damage-based structural damage index on mesh density is studied. Multiple finite element meshes with increasing density are used to investigate their effect on the damage index values calculated from nonlinear finite element simulations for a reinforced concrete column subjected to cyclic loading. With the simulation results, this paper suggests a correction method for the objective damage index based on nonlinear regression of volumetric tensile damage ratio data. The modified damage index values are presented in the quasi-static cyclic simulation to show the efficacy of the suggested correction method.

Background Fellowship training is becoming more popular in plastic surgery, with over half of residents pursuing advanced training. Here, we investigate how clinical and research fellowship training impacts career trajectory and scholastic achievement in academic plastic surgery. Methods Plastic surgery faculty members, from programs recognized by the American Council of Academic Plastic Surgeons, were identified using institutional Web sites. Data extracted included faculty demographics, training history, academic positions, and research productivity. Continuous and categorical variables were compared using t-tests and chi-square, respectively. Results In total, 949 faculty members were included, with 657 (69%) having completed fellowship training. Integrated program residents were more likely to complete a fellowship when compared with independent residents (p < 0.0001). Fellowship trained faculty were more likely to have graduated from a higher ranked residency program, in terms of both overall and research reputation (p = 0.005 and p = 0.016, respectively). When controlling for years in practice, there was no difference found in number of publications, Hirsch index (h-index), or National Institutes of Health funding between faculty between the two cohorts (p > 0.05). In a subanalysis comparing hand, craniofacial, microsurgery, and research fellowships, those who completed a research fellowship had higher h-indices and were more likely to reach full professor status (p < 0.001 and p = 0.001, respectively). Fellowship training had no effect on being promoted to Chief/Chair of departments (p = 0.16). Conclusion Fellowship training is common among academic plastic surgeons. In this study, both clinical and research fellowships were associated with various aspects of academic success. However, fellowship training alone did not affect attainment of leadership positions.
Pulmonary embolism is a one of the major cause of postoperative death. Surgery predisposes patients to pulmonary embolism, even as late as one month after the operation. The accurate detection of pulmonary embolism remains difficult, and the differential diagnosis is extensive. The prevention of pulmonary embolism is thus of paramount importance. We report a case of pulmonary embolism after coverage of pressure sore on the left ischium on the 8th day after operation. The patient was 60 years old, a severe smoker, in the high quantity of body mass index and had hypertension. The risk factors are 60 years of age or older who were in the highest quantity of body mass index. Heavy cigarette smoking and high blood pressure are also identified as risk factors. Plastic surgeons should keep the probability of pulmonary embolism development after operation in mind. When taking history, the risk factors should be checked certainly. The immobilization may explain the probability of pulmonary embolism development. Therefore absolute bed rest or positional maintenance should be avoided. Until recently, low molecular weight heparin has been used for preoperative prophylaxis. Plastic surgeons should be concerned in low molecular weight heparin for prophylaxis of the pulmonary embolism and study the indications and effectiveness in liposuction or abdominoplasty.
Purpose: In performing breast reconstruction, making symmetrical breast is still a challenge. A precise estimate of the volume of the breast specimen is necessary to reconstruct a symmetrical and aesthetically pleasing breast. This study aims at finding out the relationship with breast tissue density and body mass index designed to apply for breast reconstruction. Methods: By using the Archimedes' principle, the authors calculated the volume of the breast specimen and drew a correlation between the density of breast specimen and BMI. From October 2002 to November 2004, this method was used on 197 patients to predict breast volume for TRAM flap reconstruction. Results: The mean density was 0.9954g/cc and had no correlation with BMI (p-value=0.069). There was no difference between denstiy of breast tissue after skin spared mastectomy and that of breast tissue after nipple spared mastectomy. Conclusion: These data will be helpful to predict the needed volume for breast reconstruction.

This paper reports a side-coupled asymmetric $1{\times}2$ plastic optical fiber coupler for an optical sensor system. The dependence of the optical power coupling ratio on the coupling angle and refractive index of the adhesion layer in both the forward and backward directions was examined based on the geometrical optics. It was confirmed experimentally that the coupling ratios can be optimized by controlling the coupling angle and refractive index of the adhesion layer. A maximum forward coupling efficiency > 93% was achieved.

Purpose: The nose of most Koreans is characterized as a low nasal dorsum, retracted columella, and an acute columella-labial angle. For the surgical correction of the tip and columella, a modified septal extension graft, along with augmentation rhinoplasty has been developed. Methods: With this technique, a septal extension graft is fixed on the entire caudal margin of the septum (the cephalic-caudal axis) and at the same time, it is placed above the anterior nasal spine, in the membranous portion of the septum and at the base of the columella(the anterior-posterior axis). The present report describes the results obtained in 13 patients and offers an analysis of the results as judged by the columella-labial angle and 4 proportional indices(nose height index, nasal bridge length index, nasal tip projection index, columella length index), measured by photogrammetry. Results: The postoperative values obtained in these 5 categories increased significantly compared to the preoperative ones, thus confirming that the projection of tip was augmented, the nose was lengthened, and the columella was advanced caudally and lengthened. Moreover, these positive outcomes were still maintained during the follow-up period, and no side effects, such as saddle nose deformity, were reported. Conclusion: A modified septal extension graft can be considered as an effective method for the surgical correction of the nasal tip and columella in Koreans.
Pachydermodactyly is a rare distinct form of fibromatosis characterized by acquired, asymptomatic dense fibrosis and fibroblastic proliferation around one or more proximal interphalangeal joints. Intralesional triamcinolone injection may be helpful in improving the clinical appearance. A 20-year-old unemployed female patient gave a four years history of progressive swelling of the fingers affecting predominantly the proximal interphalangeal joints of index fingers of left hand and index, middle fingers of right hand. There was no pain and tenderness. Physical examination, complete blood count, antinuclear antibody, rheumatoid factor were unremarkable and radiograph of both hands showed soft tissue swelling only without bony abnormality. We excised the redundant soft tissue from the both sides of proximal interphalangeal joint with longitudinal elliptical fashion. After operation, the fingers of the patient showed marked improvement cosmetically. There were no motor or sensory changes. Pachydermodactyly was first reported in 1996 in Korea. Histologic features include an increased dermal accumulation of collagen and occasionally an increased number of fibroblast. We report for one patient with pachydermodactyly treated with longitudinal elliptical excision and obtained an improvement of clinical appearance.

Background Implant-based breast reconstruction has evolved tremendously in the last decades, mainly due to the development of new products and techniques that make the procedure safer and more reliable. The purpose of this study was to compare the outcomes in immediate one-stage breast reconstruction between acellular dermal matrix (ADM) and inferior dermal flap (IDF). Methods We conducted a retrospective comparative study of patients submitted to immediate breast reconstructions with an anatomical implant and ADM or IDF in a single center between 2016 and 2018. Outcomes evaluated included major complications, early complications, reinterventions, readmissions, and reconstruction failure. Simple descriptive statistics and univariate analysis were performed. Results A total of 118 breast reconstructions (85 patients) were included in the analysis. Patients in the IDF group had a higher body mass index (median = 27.0) than patients in the ADM group (median = 24). There were no statistically significant differences among both groups regarding immediate major complication, early complications, readmissions, and reinterventions. Conclusion There are no significant differences in complications between the ADM and IDF approach to immediate implant breast reconstruction. In patients with higher body mass index and large, ptotic breasts, we recommend an immediate implant reconstruction with IDF.
