64 dental students who had brushed their teeth with scrubbing method, using 'W' dentifirice for a long lime, were made to practise either scrubbing or Bass method. using one of the 'W','X','Y','Z' toothpaste instead of only 'W' paste for recent 3 weeks. Dental plaque on the teeth of every students were carefully examined compared the results. The simplified oral hygiene index was applied for this purpose and following conclusions were achieved. 1) The Scrubbing method of tooth-paste 'X', Bass method of tooth-paste 'X', and the Bass method of tooth-paste 'Z' had an obvious effect to inhibition of accumulation of plaque. 2) In the tooth brushing method, the scrubbing and Bass are both effective in reducing the plaque accumulation 3) The tooth paste 'X' has the most effectiveness in reducing the plaque accumulation among four tooth pastes 4) The difference in the value of plaque index between the tooth pastes, 'W' and 'X' was only significant statisticallay
A novel glucanhydrolase from a mutant of Lipomyces starkeyi(KSM 22)has been shown effective in hydrolysis of mutan, reduction of mutan formation by Streptococcus mutans and removal pre-formed sucrose-dependent adherent microbial film and Lipomyces starkeyi KSM 22 dextranase has been strongly bound to hydroxyapatitie. These in vitro properties of Lipomyces starkeyi KSM 22 dextranase are desirable for its application as a dental plaque control agent. This study was performed to determine oral hygiene benefits and safety of dextranase(Lipomyces starkeyi KSM 22 dextranase)-containing mouthwash in human experimental gingivitis. This 3-week clinical trial was placebo-controlled double-blind design evaluating 1U/ml dextranase mouthwash and 0.12% chlorhexidine mouthwash. A total 39 systemically healthy subjects, who had moderate levels of plaque and gingivitis were included. At baseline, 1, 2 and 3 weeks, subjects were scored for plaque(Silness and $L{\ddot{o}e$ plaque index and plaque severity index), gingivitis($L{\ddot{o}e$ and Silness gingival index), and at baseline and 3 weeks of experiment, subjects were scored for plaque(Turesky-Quingley-Hein's plaque index and plaque severity index), tooth stain(Area and severity index system by Lang et al). Additionally, oral mucosal examinations were performed and subjects questioned for adverse symptoms. Two weeks after pre-experiment examinations and a professional prophylaxis, the subjects provided with allocated mousewash and instructed to use 20-ml volumes for 30s twice dailywithout toothbrushing. All the groups showed significant increase in plaque accumulation since 1 week of experiment. During 3 weeks' period, the dextranase group showed the least increase in plaque accumulation of Silness and $L{\ddot{o}e$ plaque index, compared to the chlorhexidine and placebo groups, but chlorhexidine group showed the least increase inplaque accumulation of Turesky-Quingley-Hein's plaque index. As for gingival inflammation, all the groups showed significant increase during 3 weeks of experiment. The dextranase group also showed the least increase in gingival index score, compared to the chlorhexidine as well as the placebo groups. Whereas the tooth stain was increased significantly in the chlorhexidine group, compared to the baseline score and the placebo group since 3 weeks of mouthrinsing. It was significantly increased after 3 weeks in the dextranase group, still less severe than the chlorhexidine group. As for the oral side effect, the dextranase group showed less tongue accumulation, bad taste, compared to the chlorhexidine group. From these results, mouthrinsing with Lipomyces starkeyi KSM 22 dextranase was comparable to 0.12% chlorhexidine mouthwashin inhibition of plaque accumulation and gingival inflammation and local side effects were if anything less frequent and less intense than chlorhexidine, in human experimental gingivitis. All data had provided positive evidence for Lipomyces starkeyi KSM 22 dextranase as an antiplaque agent and suggested that further development of dextranase formulations for plaque control are warranted.
PURPOSE. The aim of this study was to evaluate the efficiency of manual polishing over autoglazed and overglazed porcelain and their effect on plaque accumulation. MATERIALS AND METHODS. Thirty-six porcelain discs were fabricated out of which 18 each was subjected for autoglazing and overglazing. Half surface of the discs was left intact; the remaining half was roughened with medium grit diamond bur. Roughened surfaces were repolished by porcelain polishing kits (Shofu, DFS, Eve). All the surfaces were evaluated by the perthometer and SEM. Six discs from each sample were placed in human volunteer's mouth for 72 hours to evaluate the plaque accumulation. Acquired data was subjected to ANOVA comparative evaluation. RESULTS. Roughened surfaces had average roughness value of $2.88{\pm}0.1935{\mu}m$. The repolished surfaces by porcelain correction kits Shofu, DFS and Eve, average roughness value reduced to $0.6250{\pm}0.1036$, $0.9192{\pm}0.0953$, $0.9017{\pm}0.1305$ respectively. Autoglazed and overglazed surfaces showed the mean roughness value (Ra) of $0.4217{\pm}0.0685$, $0.3450{\pm}0.0729$. SEM study showed the improved surfaces when subjected for polishing. Plaque accumulation percentage was the highest on roughened surface ($93.83{\pm}6.2552%$), followed by porcelain discs polished by commercial kits. Autoglazed surfaces found to be the best surfaces with the least plaque accumulation ($0.5237{\pm}0.4209%$). CONCLUSION. All the polishing kits used in the study reduced the average roughness by approximately 77%. Corrected porcelain surfaces should ideally be reglazed, alternatively, polish the surfaces before final cementation.
The short-term effects of plaque control instruction were studied in 42 adolescent orthodontic patients under active fixed treatment. At first visit, all the patients were received plaque control instruction using materials such as oral hygiene education slides, dentiform, disclosing agents, tooth brush and interdental brush. After that, Pateints were asked to perform the tooth brushing according to instructions. Such a procedure was repeated every week lot 3 weeks. Plaque index and bleeding index were scored once a week for 4 weeks and were compared according to ages, sex and duration with fixed appliance. The results were as follows: 1. Plaque control instruction was effective in reducing plaque accumulation and gingival inflammation of the orthodontic patients. 2. The effect of plaque control instruction was continued during 3 weeks among all groups of subjects, and it was prominent at the first week. 3. There was no statistically significant difference in the effects of the plaque control of instruction according to sex, ages, or duration with fixed appliances.

The purpose of this study was to investigate the effect of wearing the removable partial denture on plaque accumulation. Twelve removable partial denture cases were examined. Patients were recalled three consecutive 3 day periods. Plaque index was measured after each 3 day period. (1) not wearing the denture, (2) wearing the denture, (3) wearing the denture after intensive tooth brushing instruction. The results were as follows: 1. Plaque indices of all the remaining teeth were higher following the wearing of removable partial denture than those of teeth not wearing the denture and those of teeth after intensive tooth brushing instruction. 2. Plaque indices showed statistical difference between all the tooth surfaces which were in contact with the denture and those which were not. 3. Plaque indices showed no statistical difference between buccal and lingual surfaces which were in contact with the denture and those which were not. 4. Plaque indices showed statistical difference between proximal surfaces which were in contact with the denture and those which were not.
Purpose: The present study measured changes in arteriolar and venular capillary flow and structure in the gingival tissues during the development of plaque-induced gingival inflammation by combining dynamic optical coherence tomography (OCT), laser perfusion, and capillaroscopic video imaging. Methods: Gingival inflammation was induced in 21 healthy volunteers over a 3-week period. Gingival blood flow and capillary morphology were measured by dynamic OCT, laser perfusion imaging, and capillaroscopy, including a baseline assessment of capillary glycocalyx thickness. Venular capillary flow was estimated by analysis of the perfusion images and mean blood velocity/acceleration in the capillaroscopic images. Readings were recorded at baseline and weekly over the 3 weeks of plaque accumulation and 2 weeks after brushing was resumed. Results: Perfusion imaging demonstrated a significant reduction of gingival blood flow after 1 and 2 weeks of plaque accumulation (P<0.05), but by 3 weeks of plaque accumulation there was a more mixed picture, with reduced flow in some participants and increased flow in others. Participants with reduced flux at 3 weeks also demonstrated venular-type flow as determined by perfusion images and evidence of the development of venular capillaries as assessed by the velocity/acceleration ratio in capillaroscopic images. After brushing resumed, these venular capillaries were broken down and replaced by arteriolar capillaries. Conclusions: After 3 weeks of plaque accumulation, there was wide variation in microvascular reactions between the participants. Reduced capillary flow was associated with the development of venular capillaries in some individuals. This is noteworthy, as an early increase in venous capillaries is a key vascular feature of cardiovascular disease, psoriasis, Sjögren syndrome, and rheumatoid arthritis-diseases with a significant association with the development of severe gingival inflammation, which leads to periodontitis. Future investigations of microvascular changes in gingival inflammation might benefit from accurate capillary flow velocity measurements to assess the development of venular capillaries.
Periodontal disease is caused by accumulation of bacterial plaque. For the reason, plaque control is essential to control and prevent periodontal disease. Among the plaque control methods, mechanical plaque removal, tooth brushing is common and reliable. But it depends on individual habituation and their manner. To catch up the gap of effectiveness, lots of oral hygiene appliances have been developing. Powered toothbrush is the most interesting field and is showing on the market with various motion type. This study was performed to compare clinical effects of plaque removal between powered toothbrush and manual toothbrush. The results were as follows : 1. Plaque index was decreased statistically after the brushing than before the brushing in every group.(p<0.05) 2. Comparing plaque index among the three groups before and after brushing, there were no statistically significant differences. 3. Interproximal plaque index was decreased statistically after the brushing than before the brushing in every group.(p<0.05) 4. Comparing interproximal plaque index among the three groups before and after brushing, there were no statistically significant differences.
This double-blind controlled clinical and microbiological study was carried out to determine the effects of mouthwash preparation containing the mixture of herbal extract on developing plaque and gingivitis in the experimental gingivitis model. Following a 2-week normalization period, 34 dental students were distributed randomly into 1 of 3 treatment groups. They rinsed, under supervision, two times daily for 3 weeks with either normal saline(CT), 0.1% chlorhexidine(CH), or the mixture of herbal extract (HT), but refrained from any oral hygiene measures. The Plaque Index(PlI), the Gingival Index(GI), and the amount of Gingival Crevicular Fluid(GCF) were measured at week 0,1,2, and 3 of the experimental period, while the assessment of total wet weight of plaque and the phase contrast microscopic examination of plaque were performed at the end of experimental period(3 weeks). Subjects using mouthrinse preparation containing the mixture of herbal extract demonstrated negligible, if any, changes in the accumulation and microbial composition of plaque compared to those using normal saline, while the reduction of gingival inflammation by this mixture was highly significant and comparable to that of chlorhexidine. The results of this study indicate that the preparation containing the mixture of herbal extract do not provide any antiplaque benefits but is very effective in inhibiting the development of and in reducing existing experimental gingivitis when used as mouthrinse. Further research is needed to determine whether a significant reduction of gingival inflammation without a concomitant decrease in plaque accumulation is of clinical importance.
A novel glucanhydrolase from Lipomyces starkeyi KSM 22 has been suggested as a promising anti-plaque agent because it has been shown to have additional amylase activity and mutanase activity besides dextranase activity and to strongly bind to hydroxyapatite. Mouthrinsing with Lipomyces starkeyi KSM 22 glucanhydrolase solution was comparable to 0.12% chlorhexidine mouthwash in inhibition of plaque accumulation and gingival inflammation and local side effects were less frequent and less intense in human experimental gingivitis. In this study, Lipomyces starkeyi KSM 22 glucanhydrolase mouthrinses (1 and 2 unit/ml) were compared with a control mouthrinse (commercial 0.01% benzethonium chloride mouthrinse, $Caregargle^{(R)}$, Hanmi Pharmaceuticals) in the ability to inhibit plaque formation. A 3-replicate clinical trial using 4-day plaque regrowth model was used. Fifteen volunteers were rendered plaque-free on the 1st day of each study period, ceased toothcleansing, and rinsed 2X daily with allocated mouthrinse thereafter. On day 5, plaque accumulation was scored and the washout periods was 9 days for the next trial. Lipomyces starkeyi KSM22 glucanhydrolase(1 unit and 2 unit)- containing mouthrinse resulted in Significantly lower plaque formation in plaque area and thickness, compared to the control mouthrinse. There was no significant difference in plaque inhibition between enzyme-mouthrinses at 2 different concentrations used. This glucanhydrolase- containing mouthwash resulted in significantly lower plaque area severity index score and tended to have lower plaque thickness severity index score than those of control mouthrinse. But there was no significant difference according to the enzyme concentration. From these results, Lipomyces starkeyi KSM 22 glucanhydrolase-containing benzethonium chloride mouthrinse has greater anti-plaque effect than the commercial mouthrinse alone. Therefore this glucanhydrolase preparation is a promising agent for new mouthwash formulation in the near future.
