Initial Assessment and Care Planning in Palliative Hospice Care: Focus on Assessment Tools (호스피스 완화의료에서의 초기평가와 돌봄 계획의 수립: 평가도구를 중심으로)

  • For hospice palliative care that provides comprehensive and general care, it is necessary to use assessment tools to objectively list issues and detail care plans. The initial assessment is a process of establishing an overall direction of care by identifying the patient's symptoms, social and spiritual issues and palliative care needs on the admission day or within one day of admission. This process is also used to identify the patients' and families' awareness of the illness, prognosis, treatment options and if the Physician Orders for Life-Sustaining Treatment (POLST) has been drafted. Consisting of 13 simple questions regarding the physical, mental, social, and spiritual domains, the Needs at the End-of-Life Screening Tool (NEST) is recommended as an initial assessment tool. Using specific assessment tools, a care plan is established for the issues identified in the initial assessment within three days of admission. A multidisciplinary assessment tool can be helpful in the physical domain. The psychosocial domain evaluates psychological distress, anxiety and depression. The social domain examines an ability to make decisions, understanding of the socioeconomic circumstance, family relationship, and death preparedness. A spiritual evaluation is also important, for which the Functional Assessment of Chronic Illness Therapy-Spiritual WellBeing Scale (FACIT-Sp) or the Spiritual Health Inventory (SHI) can be used. The use of an assessment tool could not only contribute to pain mitigation a better quality of life for patients, but also provide systematic training for a multidisciplinary team; And the process itself could be a stepping stone for the better care provision.

A Narrative Inquiry on Korea Science Academy Physical Education Teachers's Assessment Experiences (한국과학영재학교 체육교사의 체육평가 경험에 대한 내러티브 탐구)

  • This narrative study aims to describe the experience of P.E. assessment that was conducted by P.E. teachers of Korea Science Academy of KAIST, and interpret the educational significance that was found in the process. The study participants were two P.E. teachers who were selected by decisive case sampling method. Data were collected mainly through official interviews with study participants, and through researcher's field notes, informal interviews, various minutes, students' evaluation of teaching, and emails between the researcher and study participants. Data were analyzed through inductive categorization, and to gain veracity of the study, there were integration of diverse materials, advice and suggestions of fellow researchers, continuous confirmation of study texts by study participants. Study participants, while conducting P.E. assessment in Korea Science Academy of KAIST, experienced effectiveness of evaluation such as qualitative development of P,E. classes in accordance with the simplified assessment, freedom from the chores of handling assessment results, students' improved perceptions of P.E. class, realization of safe classes without excessive competition, and the possibility of giving alternative evaluations to pass/fail system but at the same time experienced limitations such as concerns over gaining validity and reliability of P.E. evaluation, the students' attitude who take lightly of P.E. class, and the reality that teachers cannot fail students. The evaluation experiences of the two P.E teachers were educationally interpreted as encounter with good P.E. classes, invitation to P.E. class criticism, and the start of school P.E. culture that is led by students.

Developing a Comprehensive Geriatric Assessment Package for Successful Aging (성공노화를 위한 포괄적 노인평가 패키지 개발)

  • This is a methodological study designed to develop a comprehensive geriatric assessment package for successful aging of the elderly in various health condition. Assessment items and tools were primarily selected and categorized into 9 domains through the review of existing geriatric assessment tools, which have been used in the clinic and community settings, and the investigation of related articles. We have obtained professional advice from three experts and conducted a preliminary survey (n=15). Based on findings, the final version of comprehensive geriatric assessment package were devised. It was constructed of 29 assessment items in 6 domains of basic, psychologic function, physical function, life style, subjective health state, and environment. It also contained concrete implementation guide of each tool.

  • Safety' can be used in a variety of ways and may also have different meanings when used in theoretical field and routinely used. In this paper, the 'safety' means that human injury, fire or physical accident condition does not occur while used by the end-user. The meaning of safety may be different by era and culture. Even in contemporary era, the meaning can be used differently by country, region and culture. As the rights of consumers are increasingly reinforced, we can expect the acceptable risk or safety level can rise higher. In this paper, the R-map of Japan and the European risk assessment guidelines (RAPEX) were reviewed considering domestic incidents database status and its applicability. Because it is difficult to make a model based on a R-map, a revised model was developed mainly based on European Assessment Model with a combination of the important characteristics of Japan model R-map. Also utilizing this revised model, the availability as a new risk assessment model was confirmed by comparing the test results for the same scenarios to the other risk assessment model (RAPEX/RAG).

  • Food borne pathogens are a growing concern for human health and food safety throughout the world. Milk and dairy products are commonly associated with spoilage or contamination from a wide variety of physical, microbial, and chemical hazardous. Microbiological risk analysis consists of three components: risk assessment, risk management, and risk communication, and overall objective of this process is ultimately public health protection. The microbiological risk assessment is useful tool to evaluate food safety as it is based on a scientific approach. In addition risk assessment process includes quantitative estimation of the probability of occurrence of microbial hazards to evaluate more accurate human exposure. The aim of this study is to review the microbiological risk assessment on the prevalence of bacterial foodborne pathogens in milk and dairy products.

Improvement of Quantitative Condition Assessment Criteria for Reservoir Embankment Safety Inspection Considering Characteristics of Small Reservoirs in Korea (소규모 저수지의 특성을 고려한 제체 안전진단의 정량적 상태평가 기준 개선)

  • The physical condition assessment criteria of fill dam safety inspection are now weakly regulated and inappropriate for small agricultural reservoirs since these criteria have fundamental backgrounds suitable for large-scale dams. This study proposes the degree (critical values) of defects for the quantitative condition assessment of the embankment in order to prepare the condition assessment criteria for a small reservoir with a storage capacity of less than one (1) million cubic meters. The critical values of defects were calculated by applying the method that considers the size ratios based on the dimensional data of reservoirs, and the method of statistical analysis on the measured values of the defect degree which extracted from comprehensive annual reports on reservoir safety inspection. In comparison with the current criteria, the newly proposed critical values for each condition assessment item of the reservoir embankment are presented in paragraphs 4 and 6 of the conclusion. In addition, this study presents a method of displaying geometric figures to clarify the rating classification for condition assessment items with the two defect indicators.

The Knowledge and Health Education Need of Physical Education College Student about First Aid (체대생의 응급처치 지식수준과 교육요구도)

  • This study examined the knowledge and health education about First Aid of Physical Education College Students. The survey had been done from September to November, 2007. Total subjects in this study were 193. The collected data were analyzed by SPSS. The results of the study were as follows : 1. Among total 193, 74.6% were male, 25.4% were female, 1st grade were 38.9%, 2nd grade were 61.1%. 58% of the subjects had not been trained first aid education. 2. For needs assessment about first aid education contents, the needs for first aid was high(2.36) of the lecture, and assessment of emergency situation(2.65) was high of the practice. Legal problems of first aid was low of the lecture and practice. 3. An average the knowledge score of the subjects was 55.1088(${\pm}19.09$) when the knowledge score of subjects converted 100 points. 4. There were significant difference in the knowledge of first aid according to subject's gender(t=2.907, p= .004), grade(t=-7.167, p= .000), and experience of first aid education(t=5.174, p= .000). These results will provide the base data for the development of first aid education program for physical education college students.

Correlation of the 3D-Surface Topography and Cobb's Angle in Scoliotic Patient's (측만증 환자에서 3차원 체표면 영상분석과 Cobb 각의 상관관계)

  • The purpose of study correlation between 3D-surface topography and Cobb angle in scoliotic patients. It would be recognizing possibility of clinical application with 3D-surface topography with scoliosis assessment and correlation analysis of obtained Cobb angle through measured results of surface topography and took X-ray of subjects with scoliosis, which used to 3D-surface topography of laser scan method. A scoliosis subjects 16(males 4, females 12) agreed for study's purport volunteer our study that diagnose the scoliosis through 3D-surface topography and X-ray. The results were as follow: It was shown that the relation of Cobb angle of scoliosis and itemized 3D-surface topography measurement in horizontal view(p<0.01). top difference of cervicothoracic and lumbar in anterior-posterior view(p<0.05), the top difference of thoracic, thoracolumbar(p<0.01). These results suggest that an itemized 3D-surface topography measurement was highly correlated with Cobb angle. It would be a useful diagnosis method and assessment of physical therapy plan.

  • The purpose of this thesis is to suggest objective basic data for the environmental design through the quantitative analysis of the visual quality included in the physical environment of Basemsagol valley sequence landscape. For this, visual volumes of physical elements have been evaluated by using the mesh analysis, spatial images structure of physical elements have been analyzed by factor analysis algorithm, and degree of visual quality have been measured mainly by questionnaires. Also, this study aims to understand semiotics and to grope the possibility of application to the sequence landscape assessment. A semiological approach suggests a new dimension in sequence landscape assessment, which is a contrast to the existing scientific evaluation methods. Result of this thesis can be summarized as follows. Visual volumes of the immediate vegetation, rock, bridge, road and distant vegetation are found to be the main factor determining the visual quality. Factors covering the spatial image of natural park sequence landscape have been found to be the overall synthetic evaluation, potentiality, natural quality, spatial, appeal and dignity. By using the control method for the number of factors, T.V. has been obtained as 40.22%. The characteristics of the semiological approach is qualitative, open, holistic, and experiential, whereas that of the scientific approach is quantitative, closed, reductive, and experimental.

Comparative Study of Text Entry Speed and Accuracy Using the Three Different Keyboard Type in Students with Cerebral Palsy: Case Study (키보드 유형에 따른 뇌성마비 학생의 문자입력 속도 및 정확도 비교: 사례연구)

  • PURPOSE: People with physical disabilities such as cerebral palsy usually experience obstacles when interacting with computer through conventional keyboard because of their motor disabilities. The purpose of this study is empirically compare of text entry(alphabet and word) speed and accuracy using the three different keyboard type on four students(male 2 and female 2) with cerebral palsy. METHODS: This research design used a replicated single-case experimental approach to compare the individual performance. An alternating treatments design was used to examine the effectiveness of standard QWERTY keyboard and alternative keyboard(mini and big keyboard) on computer access for students with cerebral palsy. To avoid changes in posture that influence a keyboard character entry training and evaluation was carried out using his sitting in a wheelchair. Compass software program used in this study as an assessment tool to measure speed and accuracy when performance of text entry(alphabet and word). This was repeated until the stable status of reaction time. RESULTS: As a result, the alternative keyboard seems to be the most effective device for students with cerebral palsy to perform text entry. But various factors such as peculiarity of motor disabilities, experience and preferences of the user are heavily related. CONCLUSION: Thus, we must perform the objective and systematic assessment for computer access and if sustained training is accomplished, it could to improve speed and accuracy of text entry(alphabet and word).
