  • Reduction in competence makes older adults with dementia more sensitive to the influence of the physical environment. The aim of the longitudinal study was to examine whether residents with dementia in long-term facilities with variability in physical environmental characteristics in Vancouver (N= 11), Canada and Stockholm (N=13), Sweden had a difference in their quality of life (QoL). QoL was assessed using Dementia Care Mapping tool three times over one year for the reliability of data. The results of the study demonstrated that the residents with dementia living in a homelike and positive stimulating setting showed less withdrawn behaviors and a higher level of well-being compared to those in a large-scale institutional setting. This study also found that the residents living in a large-scale institutional environment spent more monotonous times than the other groups, which may be to provision of fewer structured activity programs or less social interaction with neighbors or staff members. Residents living in a large-scale institutional setting in Canada showed so far as five times more agitated/ distressed behaviors and twice more withdrawal compared to the ones living in a small-scale homelike setting in Sweden. The study supports that the large-scale institutional environment was considerably associated with levels of lower quality of life among the residents with dementia.

  • The effects of herbal ethanol extract from Gynostemma pentaphyllum (GP extract) on chronic stress-induced anxiety in mice were investigated. The animals were treated with GP extracts (50 and 100 mg/kg/day, p.o.) for 21 days before exposure to electric footshock (EF; duration and interval 10 sec for 3 min, 2 mA) for chronic stress once a day. The ambulatory locomotor activity was reduced by chronic EF stress and it was recovered by 12.9-15.1% in GP extract-treated groups. The grip strength was also significantly decreased by chronic EF stress, however, the EF-stressed groups treated with GP extract increased grip strength from 13.9% to 56.8% compared to EF-stressed groups. In addition, the serum levels of corticosterone were significantly elevated by chronic EF stress to 197% of the control levels, which was reduced to 73.1% by treatment with GP extract (100 mg/kg). In contrast, the brain levels of dopamine and serotonin were reduced to 67.6% and 63.1% by chronic EF stress, which was recovered to 90% of the control levels by treatment with GP extract. These results indicate that GP extract shows the ameliorating effects on chronic EF stress-induced anxiety in mice and it can be developed as the promising anti-anxiety agent.

  • Purpose The purpose of this study was to examine the relationship between physical activities, sleep disturbance, and health related quality of life (HRQOL) in patients with chronic obstructive pulmonary disease (COPD). Methods A descriptive survey design used pretest dataset of COPD symptom management intervention study (N=245). Measures included the international physical activity questionnaire, the COPD and asthma sleep impact scale, and the St. George's respiratory questionnaire of HRQOL. The data were analyzed using descriptive analysis, t-test, ANOVA, Pearson correlation coefficients, and simultaneous multiple regression by the SPSS WIN 18.0 program. Results The mean scores (SD) of HRQOL and sleep disturbance were 36.04 (19.43) and 14.33 (6.20), respectively. About 32% of participants were physically inactive. The multivariate approach showed the patients who have more sleep disturbance (β=.27), lower levels of FEV1 % predicted (β=-.23), lower physical activities (β=-.19), lower household income (β=-.16), and diagnosed longer than 5 years (β=.14) reported lower HRQOL (R2=.34). Conclusion The findings of this study suggest that improving the quality of sleep and physical activities can be efficient strategies for HRQOL in patients with COPD. Future research in enhancing HRQOL through improving sleep quality and physical activities is needed.

  • The present study was undertaken to investigate the influence of eupatilin on vascular smooth muscle contractility and to determine the mechanism involved. Denuded aortic rings from male rats were used and isometric contractions were recorded and combined with molecular experiments. Eupatilin more significantly relaxed fluoride-induced vascular contraction than thromboxane $A_2$ or phorbol ester-induced contraction suggesting as a possible anti-hypertensive on the agonist-induced vascular contraction regardless of endothelial nitric oxide synthesis. Furthermore, eupatilin significantly inhibited fluoride-induced increases in pMYPT1 levels. On the other hand, it didn't significantly inhibit phorbol ester-induced increases in pERK1/2 levels suggesting the mechanism involving the primarily inhibition of Rho-kinase activity and the subsequent phosphorylation of MYPT1. This study provides evidence regarding the mechanism underlying the relaxation effect of eupatilin on agonist-induced vascular contraction regardless of endothelial function.

  • Background: For performing various movements well, cooperation between the muscles around the scapula and shoulder has been emphasized. Taping has been widely used clinically as a helpful adjunct to other physiotherapy methods for shoulder pathology and dysfunction treatment. Previous studies have evaluated the effect of taping techniques using dynamic tapes on shoulder function and pain. However, no study investigated the electromyographic (EMG) changes in the shoulder muscles. Objects: This study aimed to investigate the effect of the upper limb offload taping technique using a dynamic tape on EMG activities of the upper trapezius (UT), lower trapezius, serratus anterior (SA), and middle deltoid (MD) muscles during scaption plane elevation. Methods: A total of 26 healthy subjects (19.85 ± 6.40 years, male = 20) volunteered to participate in this study. The subjects were instructed to perform scaption elevation with and without dynamic taping on the shoulder. Shoulder elevation strength tests were performed at 100%, 75%, 50%, and 25%, for the maximal isometric contraction force. Results: There were statistically significant interaction effects between the taping application and shoulder scaption elevation force in EMG activities in the UT (p < 0.05) and MD (p < 0.05). EMG activities in the UT showed significant increases in 50%RVC (reference voluntary contraction, p < 0.05) and 25%RVC (p < 0.01). Furthermore, the EMG activity of the SA significantly increased in 50%RVC (p < 0.01) and 25%RVC (p < 0.01) after dynamic taping. For the MD, the EMG activity level significantly decreased in 100%RVC (p < 0.05). Conclusion: These results indicated that upper limb offload dynamic taping application affects the muscle activities of some shoulder muscles depending on different scaption elevation strength levels. Therefore, we suggest that the upper limb offload dynamic taping can be applied to the shoulders when patients need middle deltoid inhibition or upper trapezius facilitation, such as patients with shoulder impingement syndrome.

  • The possible mechanisms of ginseng saponins on the inhibition of development of morphine tolerance and physical dependence were investigated in the aspects of morphine metabolism by morphine 6-dehydrogenase. Administration of morphine causes a reduction of non-protein sulfhydryl contents in liver, because morphinone is metabolized from morphine by morphine 6-dehydrogenase conjugates with sulfhydryl compounds. However, ginseng saponins inhibited the activity of morphine 6-dehydrogenase which catalized the production of morphinone from morphine. In addition, ginseng' saponins inhibited the reduction of non-protein sulfhydryl levels by Increasing the level of hepatic glutathione. These results suggest that the dual action of the above plays an important role in the inhibition of development of morphine tolerance and physical dependence. On the other hand, it was observed that less polar components of ginseng saponins with parent structures were more active components in vitro.

The mean exercise duration per once of men (69.7 minutes) was significantly higher than that of women (52.4minutes) (p < 0.01). The mean exercise duration per day was 36.0 minutes for men and 29.9 minutes for women. 67.9% for men and 78.0% for women often skipped meals and 68.4% for men and 69.4% for women skipped breakfast in the main. The mean energy intake of men was 2067.2 kcal and that of women was 1783.1 kcal comprised of 87.2% and 92.1% of the Estimated Energy Requirements (EER). The mineral intakes of men and women were over Recommended Intake (RI) and Adequate Intake (AI) except calcium and potassium. The mean calcium intake was 88.3% for men and 84.0% for women of RI. The mean potassium intake was 63.3% for men and 59.2% for women of AI. The mean vitamin intakes of men and women were over RI and AI except vitamin C and folic acid. The mean vitamin C intake was 92.5% for men and 85.6% for women of RI. The mean folic acid intake was 76.6% for men and 70.0% for women of RI. The mean energy, protein, sodium and zinc intakes of men were significantly higher than those of women (p < 0.01, p < 0.05, p < 0.05, p < 0.05). The mean vitamin $B_6$, vitamin C and folic acid intakes of men in the 20-29 years group were significantly lower than those of men in the 30-49 and 50-59 years group (p < 0.01, p < 0.05, p < 0.05). For energy, proportions of subjects with intake levels less than 90% EER were 64.3% for men and 56.0% for women. For calcium, proportions of subjects with intake levels less than Estimated Average Requirement (EAR) were 52.4% for men and 59.6% for women. For folic acid, proportions of subjects with intake levels less than EAR were 78.6% for men and 83.5% for women. For iron and phosphorus, proportions of women (36.7%, 14.7%)with intake levels less than EAR were significantly higher than those of men (6.0%, 1.2%) (p < 0.01, p < 0.001). For men, age was positively correlated with intakes of potassium, vitamin $B_6$, vitamin C and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.01). For men, weight showed significantly negative correlations with intakes of carbohydrate, phosphorus, potassium, zinc, vitamin $B_6$ and folic acid (p < 0.01, p < 0.05, p < 0.05, p < 0.05, p < 0.01) and BMI showed significantly negative correlations with protein, lipid, phosphorus, potassium, zinc, vitamin E and folic acid (p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05, p < 0.05) For men, exercise duration per once showed significantly positive correlations with intakes of calcium, phosphorus, potassium, zinc, vitamin $B_1$, vitamin $B_2$, niacin, vitamin C and folic acid (p < 0.01, p < 0.05, p < 0.01, p < 0.05, p < 0.05, p < 0.01, p < 0.01, p < 0.01, p < 0.05). Therefore, nutritional education for adult health management is needed by gender and age groups.

  • This study used an unstable platform to change the support surface type and position of both lower limbs in order to determine changes in weight distribution and muscle including the vastus medialis, tibialis anterior, lateral hamstring, and lateral gastrocnemius of both lower limbs were evaluated during knee joint flexing and extending in a semi-squat movement in 32 hemiplegic patients. The support surface conditions applied to the lower limbs were divided into four categories: condition 1 had a stable platform for both lower limbs; condition 2 had an unstable platform for the non-hemiplegic side and a stable platform for the hemiplegic side; condition 3 had a stable platform for the non-hemiplegic side and an unstable platform for the hemiplegic side; and condition 4 had an unstable platform for both sides. The normalized EMG activity levels of muscles and weight bearing ratio of both sides in the four surface conditions were compared using repeated measures ANOVA. A significant increase was found in the weight support distribution for the hemiplegic side in flexing and extending sessions in condition 2 compared to the other conditions (p<.05). A statistically significant decrease in significant decrease in asymmetrical weight bearing in flexing and extending sessions was observed for condition 2 compared to the other conditions (p<.05). A similar significant decrease was found in differences in muscular activity for both lower limbs in condition 2 (p<.05). The muscular activity of the hemiplegic side, based on the support surface for each muscle showed a significantly greater increase in condition 2 (p<.05). An unstable platform for the non-hemiplegic side and a stable platform for the hemiplegic side therefore increased symmetry in terms of the weight support distribution rate and muscle activity of lower limbs in hemiplegic patients. The problem of postural control due to asymmetry in hemiplegic patients should be further studied with the aim of developing continuous effects of functional training based on the type and position of the support surfaces and functional improvement.
