광역동요법을 병행한 두피 모낭염 한방치험 1례 (A case of scalp folliculitis treated by Korean Medicine with photodynamic therapy(PDT))

  • Objectives : The purpose of this study is to report the effect of combination therapy of photodynamic therapy (PDT) with Korean medical treatments on scalp folliculitis, which has been difficult to treat by using antibiotic.Methods : A 32-year-old male patient with scalp folliculitis, received treatment with acupuncture, herbal medicine and PDT and so on. We used Visual Anologue Scale(VAS) to evaluate recovery from erythema, pruritus and pain. Also, the photos of patient were taken before and after treatments.Results : Visible symptoms to the naked eye and VAS scale effectively decreased after Korean medicine treatments for two months.Conclusions : It is expected that Korean medical treatments and PDT were thouht to be effective for scalp folliculitis.

A Strategy Using Photodynamic Therapy and Clofibric Acid to Treat Peritoneal Dissemination of Ovarian Cancer

  • Background: The current study examined the effectiveness of concurrent therapy using photodynamic therapy (PDT) and clofibric acid (CA) to treat peritoneal carcinomatosis resulting from ovarian cancer. Materials and Methods: Nude rats were used to create a model of peritoneal carcinomatosis resulting from ovarian cancer and the effectiveness of PDT with 5-aminolevulinic acid methyl ester hydrochloride (methyl-ALA-PDT) was determined. The survival time of rats receiving that therapy was compared to the survival time of a control group. Rats with peritoneal carcinomatosis resulting from ovarian cancer were divided into 3 groups: a group that received debulking surgery (DS) alone, a group that received DS+methyl-ALA-PDT, and a group that received DS+methyl-ALA-PDT+CA. The survival time of the 3 groups was compared. Protoporphyrin, a metabolite of methyl-ALA, produces a photochemical action when activated by light. The level of protoporphyrin (the concentration) that reached organs in the abdomen was measured with HPLC. Results: Rats receiving methyl-ALA-PDT had a significantly longer survival time compared to the controls. Rats with peritoneal carcinomatosis that received DS+methyl-ALA-PDT+CA had a significantly longer survival time compared to the rats that received DS alone. Some of the rats that received concurrent therapy survived for a prolonged period. Protoporphyrin was highly concentrated in peritoneal metastases, but only small amounts reached major organs in the abdomen. PDT was not found to result in necrosis in the intestines. Conclusions: The results indicated that concurrent therapy consisting of PDT with methyl-ALA and CA is effective at treating peritoneal carcinomatosis resulting from ovarian cancer without damaging organs.

Photodynamically induced endothelial cell injury and neutrophil-like HL-60 adhesion

  • Photodynamic therapy (PDT) is a treatment modality based on photochemical reaction and the resultant cytotoxic reactive oxygen species. The platelet thrombus formation leading to stasis observed in vivo during PDT is called vascular shut down (VSD) effect. To investigate the mechanism of the VSD effect, we observed Human Umblical Vein Endothelial Cell (HUVEC) injury induced by photochemical reaction. We observed cell retraction and blebbing after PDT. It seems that the injury was not fetal and only morphological change. Then, the cytoplasm was stained by Calcein-AM and subendothelial area was evaluated from fluorescence microscopy. The rate of subendothelial area after PDT increased significantly. Second, we investigated interaction between neutrophils and HUVEC. Human promyelocytic leukemia cells (HL-60) were differentiated into neutrophil by incubation with all-trans retinoic acid. Calcein-AM labeled neutrophil adhesion to HUVEC was evaluated from fluorescence microscopy. PDT-induced neutrophil adhesion to HUVEC depended more on the exposure of subendothlial area than on neutrophil activation. This result suggests that there is a certain interaction between neutrophil and HUVEC during PDT.

  • 본 논문은 광역학적 암치료 효율을 높이기 위한 레이저 시스템의 최적화 구현에 관한 연구이다. 레이저 시스템의 최적화를 위해서는 레이저 출력의 출력 변동율과 파장대의 편이가 적어야 한다. 임상에서 암치료의 효율을 높이기 위해서는 악성종양부위에 조사되는 방사방식이 다양하게 구현되어야 한다. 기존에 사용되고 있는 C.W 방식은 정상조직에도 손상을 입힐 수 있는 생체조직의 열적효과를 가져올 수 있다. 이에 새로운 방사모드를 제안하여 생체조직에 대한 열적 안전성과 치료의 효율을 높였다. 실험을 통하여 광파장의 안정성 및 광 출력과 출력의 안정성을 확인 하였고, 피부조직의 열적 손상 방지를 위한 방사모드의 구현은 생체조직에 대한 실험을 통해서 본 연구의 타당성을 검증하였다.

  • Photodynamic therapy (PDT) is a medical treatment using laser and photosensitizing drug taken up to destroy cancer cells. Singlet oxygen ($^1$0$_2$) generation is strongly related to this treatment. We have built a direct detection system monitoring feeble luminescence, in the near IR region, from $^1$0$_2$, We have comparatively studied the photophysical and photochemical properties in solution of a newly developed drug ATX-S10 and Photofrin already investigated clinically. We demonstrdted that ATX-S10 was capable of efficiently yielding $^1$0$_2$, which may lead to highly efficient PDT treatment. Successive laser excitation photobleached ATX -S10 readily in a dose-dependent manner. This result shows that ATX -S10 is useful in setting up suitable medical treatment conditions to minimize side effects.

Photodynamic Therapy for Methicillin-resistant Staphylococcus aureus Strain using Photogem and 630 nm LED

  • Methicillin-resistant Staphylococcus aureus (MRSA) is one of the most common nosocomial pathogens. It is associated with hospitals is now being isolated in the community. The aim of this study was to evaluate the antibacterial effect of photodynamic therapy using Photogem and 630 nm LED on MRSA and methicillin-sensitive Staphylococcus aureus (MSSA). The broth cultured MRSA and MSSA incubated with various concentrations of Photogem (500,50,5 and $0.5{\mu}g/mL$) for 4 h. Then 630 nm LED was given at $9\;J/cm^2$, $20{\mu}l$ of the exposed bacteria solution was inoculated onto agar plate. Plates were incubated for 24 hand colonies were counted. The PDT group was effective in killing MRSA and MSSA at the Photogem dose of $50{\mu}g/mL$. But MSSA is more sensitive than MRSA in photodynamic effect. Other groups (light only, sensitizer only, or no treatment) observed no bacterial cell killing. These results raise the possibility of using PDT with or without antimicrobial drugs to eradicate MRSA and MSSA. In order to confirm this result, we need to further study bacterial death mechanism and in vivo study.

Effectiveness of methylene blue photosensitizers compared to that of corticosteroids in the management of oral lichen planus: a systematic review and meta-analysis

  • This study aimed to systematically review the effectiveness of methylene blue (MB) photosensitizers in the management of symptomatic oral lichen planus (OLP). Electronic online databases and manual searches were performed for randomized controlled trials (RCTs) published in English between January 2010 and February 2022. RCTs comparing photodynamic therapy (PDT) and corticosteroid therapy at baseline and follow-up period were identified. The Cochrane risk of bias tool was used to assess the quality of the included studies. A meta-analysis was performed regarding visual analog scale (VAS) scores, Thongprasom sign scores, lesion size, response to treatment, and exacerbation of lesions after therapy. The clinical severity was analyzed qualitatively. Five RCTs consisting of 180 samples fulfilled the inclusion and exclusion criteria. All parameters of VAS score, Thongprasom sign score, lesion size, and response to treatment were statistically non-significant. Our results indicate that both MB-PDT and corticosteroid therapy are effective for the management of OLP. Moreover, MB-PDT is an effective alternative treatment option for OLP when corticosteroids are contraindicated. However, conclusive evidence cannot be ascertained owing to the heterogeneity among the studies.

AMC-HN3 인체 두경부 암세포에서 genistein과 photofrin PDT의 병행처리에 의한 세포 독성능의 증가 (The Anticancer Effect of Combination of Genistein and Photofrin PDT in Human AMC-HN3 Head and Neck Cancer Cell Lines)

  • 본 연구에서는 두경부 암세포 주에서 genistein의 증식억제 효과를 알아보고, genistein을 광역학 치료와 병행 치료 시 나타나는 세포 독성능 및 세포고사 유발에 대해 연구 하였다. 두경부 암세포 주에서 genistein의 세포독성능을 확인한 결과, 농도 및 시간 의존적으로 세포 독성능이 나타났다. 이에 genistein을 photofrin을 이용한 광역학 치료와 병행 치료 시 더 많은 세포 독성능이 나타났다. Genistein과 광역학 치료의 병행 치료 시 나타나는 형태적 변화는 광역학 치료시에는 주로 세포괴사의 형태가 관찰 되었으나, genistein과 병행 치료 시에는 세포고사가 관찰 되었다. 시간별로 관찰하였을 때, 광역학 치료를 $IC_{80}$으로 처리하였을 때 3시간 때에 apoptotic body가 약 3배정도 증가하였다. 세포 내 ROS를 확인한 결과 genistein과 광역학 치료를 단독으로 처리하였을 때 세포 내 존재하는 ROS가 각각 증가 하였으며, 병행 치료하였을 때는 더 많은 ROS가 형성되는 것으로 나타났다. 즉, genistein이 세포 내 ROS를 유발하여 mitochondria가 손상되고, 또한 photofrin에 의한 광역학 치료도 mitochondria를 손상시켜 세포고사를 유도하는 것으로 추측되어 진다.

폐암에서 광역동치료술의 효과 (Effect of Photodynamic Therapy in Lung Cancer)

  • 연구배경 : 폐암에서 광역동치료는 미세침습성 비소세포폐암 및 기관지폐쇄를 일으키는 악성종양에 대한 기관지내 치료를 미국식품의약국(FDA)에서 승인한 상태이다. 국내에서는 폐암세포주에 대한 보고 외에 폐암에 대한 광역동치료 연구가 많지 않아 임상 성적에 대한 보고가 없어 이에 저자들은 폐암에서 시행된 광역동치료에 대한 결과를 문헌고찰과 함께 보고하고자 한다. 방 법 : 2002년 8월부터 2003년 5월까지 조선대학교 병원에서 기관지 내시경을 통해 조직학적으로 진단된 폐암환자 중 10명을 대상으로 하여 광역동치료 48시간 전에 광과민제(Photogem$^{(R)}$, Lomonosov institute of Fine Chemical, Russia)를 2.0mg/Kg을 정맥 주사한 후 48시간, 72시간에 Diode LASER system(Biolitec Inc., Germany, wavelength; 633nm)을 사용하여 광역 동치료를 시행하였다. 결 과 : 10명 중 9예에서 부분관해와 함께 기관지 개통을 보였으며, 1예에서는 변화를 보이지 않았다. 결 론 : 저자들은 광역동치료를 통해 기관지폐쇄로 인한 호흡곤란 및 기관지 폐쇄와 관련된 폐렴이 개선됨을 확인 하였으며, 합병증이 적어 안전한 기관지내 치료법으로 생각된다. 앞으로 광역동 치료에 대한 장기적인 결과 및 적절한 치료 적응증 및 조기 폐암에서 치료효과 등에 대한 연구가 필요할 것으로 생각된다.
