In this article, we review the statistical methods and theory for dose finding in early phase clinical trials, where the primary objective is to identify an acceptable dose for further clinical investigation. The dose finding literature is initially motivated by applications in phase I clinical trials, in which dose finding is often formulated as a percentile estimation problem. We will present some important phase I methods and give an update on new theoretical developments since a recent review by Cheung (2010), with an aim to cover a broader class of dose finding problems and to illustrate how the general dose finding theory may be applied to evaluate and improve a method. Specifically, we will illustrate theoretical techniques with some numerical results in the context of a phase I/II study that uses trinary toxicity/efficacy outcomes as basis of dose finding.

Two experiments were conducted to determine the effects of dietary lysine and energy level on performance and apparent total tract digestibility (ATTD) of nutrients in weanling pigs. In Exp. 1, a total of 288 weaned pigs (initial BW $5.77{\pm}0.85\;kg$) were randomly allotted to 4 treatments (4 replicates per treatment with 18 pigs in each replicate). Experimental diets were fed in the 3 phases: phase I (d 0 to 7), phase II (d 8 to 14) and phase III (d 15 to 28). Isocalorific diets (3,450 kcal/kg) with incremental lysine levels (Phase I: 1.51, 1.61, 1.71 and 1.81; Phase II: 1.35, 1.46, 1.56 and 1.66; Phase III: 1.18, 1.28, 1.39 and 1.49% lysine respectively for T1, T2, T3 and T4) were used as treatments. An increase in the dietary lysine levels linearly improved (p<0.05) the ADG and G:F during phases I, II and III as well as overall study period. The ATTD of DM (d 7 and 28) and CP (d 7, 14 and 28) were linearly improved (p<0.05) with increasing dietary lysine levels. ATTD of ash, Ca and P were not affected by dietary lysine level. In Exp. 2, 64 weanling pigs (initial BW $4.79{\pm}0.79\;kg$) were randomly allotted to 4 treatments (4 replicates per treatment with 4 pigs in each replicate) in a $2{\times}2$ factorial arrangement on the basis of BW. Effects of two levels of energy (high, 3,450 or low, 3,350 kcal/kg) and lysine (high or low; 1.70 or 1.50, 1.55 or 1.35 and 1.40 or 1.20% in phase I, II and III diets, respectively) on performance and ATTD of nutrients were investigated. High energy and lysine diets improved ADG (p<0.05) in pigs during phase I, II and III and overall period (p<0.001), while G:F increased (p<0.05) during phase I and overall period. Pigs fed high lysine diets consumed more (p<0.05) feed during phase III and overall period. Additionally, pigs fed high energy diets had greater (p<0.05) ATTD of GE (d 7 and 14), CP (d 7 and 28) and DM (d 28); whereas, pigs fed high lysine diets had greater (p<0.05) ATTD of GE and CP during d 7, 14 and 28. ATTD of ash, Ca and P remained unaffected (p>0.05) by dietary energy and lysine level. However, there was no energy${\times}$lysine interaction for any of the measured variables. Results obtained in present study suggested that high energy and lysine level improve the growth performance and ATTD of nutrients in weanling pigs.
Purposes: This study examined uncertainty and nursing need according to illness phases(phase I, II, III) in cancer patients, and investigated relationships between uncertainty and nursing need. Methods: A cross-sectional descriptive study was conducted with a sample of 121 adult cancer patients treated in two hospitals. Results: As for the total score, the uncertainty was not significantly different across the subgroups by illness phase. As for the subscale score, however, two sub-scales of the uncertainty were different across the subgroups. 'The unpredictability' was highest in illness phase I and II groups, while 'the lack of information' was highest in the illness phase III group. Nursing need as the total score was not significantly different across the subgroups by illness phase. However, all subscale scores of the nursing need were significantly different across the subgroups. Educational need was highest in the illness phase I group; physical and emotional needs were highest in the illness phase III group. Uncertainty and nursing need were not related to each other at any illness phase. Conclusion: The results suggest that nursing need and uncertainty may change across illness phases. Clinicians need to consider this pattern in caring for cancer patients.
Currently an office in information society is advanced to a digitalized VDT office. The VDT office has a natural luminous system prioritizing visual perception and so importance of lighting design by artificial lighting is emphasized in the VDT office. The purpose of the study is to analyze Illumination and Luminance for lighting design by the standard of ergonomics in the VDT office. The study analyzed Illumination and Luminance in the main control room of an nuclear power plant needing a design of ergonomics ACR(Advanced Control Room) type Ling Ao phase II NPP MCR in Guangdong, China. The study examined the relativity of ACR characteristics to its operator's duties and set up an outline of lighting design, the standard of ergonomics and input data for the analysis. Thus, the study examined appropriateness with the standard of ergonomics setting up the analyzed Illumination and Luminance and drew a conclusion.
The core purpose of the study is to develop and validate an ADDIE model based instructional model for English Language Teaching (ELT) in early childhood classroom in Bangladesh as an aid to teachers to reconstruct their knowledge and experience more strategically, and for them to design and implement their instruction more structurally. This study is developmental in nature which has been divided in five phases as follows. Phase I: Existing methods and instructional strategy review, Phase II: Instructional model development, Phase III: Delphi 1st round, Phase IV: Delphi 2nd round and Phase V: Model validation. After reviewing relevant literature and existing strategy in phase I, the 1st version of instructional model is made phase II. Next in phase III and phase IV, two rounds of Delphi have been conducted where experts related to different concerning areas of this study reviewed the 1st version and gradually the final version of the instructional model is made. Finally, the instructional model for English teachers of early childhood classroom in Bangladesh got validated by the same Delphi panelists in Phase V. In respect with each phases of ADDIE, the instructional model elaborates the 1) representative key points, 2) instructors' activities prescribed for the instructors, 3) supporting strategies. Both the conceptual and procedural models are included in this study for clearer identification of the whole process. Lastly the study provides some recommendations for instructors and practitioners on choosing the instructional model like doing prior need analysis, incorporating teacher training programs, training students, keeping on researching for finding effective teaching technique and tools and being open to changes etc. In addition, the study also acknowledges its limitations like not being able to consider the psychological factors due to time limitation. Finally, at the end the study points out the areas that welcome further research.
Early attempts to identify plausible conformations of a linear octapeptide hormone, angiotensin-II (Asp-Arg-Val-Tyr-Ile-His-Pro-Phe), using various theoretical and experimental methods, have led to various conformational models. So far, no consensus has been made about the solution phase structure and the receptor binding structure of angiotensin-II. The ultimate goal for the conformation study of the peptide hormone is to develop a new potent drug. Therefore, we have devised a strategy for designing the pharmacophore by studying thermodynamically possible conformations of various kinds of angiotensin-II antagonists and angiotensin-II.

This study was undertaken to determine the effects of diet supplementation with complex probiotics (CPB), on growth performance, nutrient digestibility, blood metabolites, noxious gas, and fecal microflora in weaning pigs. On the basis of body weight, a total of 234 weaned pigs (Landrace×Yorkshire×Duroc, 6.14±0.78kg) were randomly allotted to 3 treatments and 6 replicates (13 pigs per pen). The experimental diets were fed in a meal form for 28 days (days 0-14, PhaseI, and days 15-28, PhaseII). The dietary treatment groups were as follows: T1 (basal diet), T2 (T1+0.13% CPB) and T3 (T1+0.25% CPB). The CPB supplement contained Bacillus subtilis 1.0×106 CFU/g, Enterococcus faecium 1.0×106 CFU/g, Saccharomyces cerevisiae 1.0×106 CFU/g, Bacillus licheniformis 3.0×108 CFU/g, and Bacillus polyfermenticus 3.0×108 CFU/g. Pigs fed the T3 diet showed an increase (p<0.05) in the overall average daily gain and average daily feed intake, increased (p<0.05) crude protein digestibility in PhaseI, and greater (p<0.05) dry matter and gross energy digestibility in PhaseII. Supplementation of CPB had no effect on the blood profile. Furthermore, pigs fed the T3 diet had lower (p<0.05) NH3 emission and overall count of fecal Clostridium spp. In conclusion, we believe that CPB supplementation has a beneficial effect on growth performance, nutrient digestibility, noxious gas, and fecal microflora in weaning pigs.

Simon's two-stage designs are frequently used in phase II single-arm trials for efficacy studies. A concern of safety studies is too many patients who experience an adverse event. We show that Simon's two-stage designs for efficacy studies can be similarly used to design a two-stage safety study by modifying some of the design parameters. Given the type I and II error rates and the proportion of adverse events experienced in the first stage cohort, we prescribe a procedure whether to terminate the trial or proceed with a stage 2 trial by recruiting additional patients. We study the relationship between a two-stage design with a safety endpoint and an efficacy endpoint as well as use simulation studies to ascertain their properties. We provide a real-life application and a free R package gen2stage to facilitate direct use of two-stage designs in a safety study.
Purpose: This study aimed to examine the effects of forward head posture on the flexion-relaxation ratio (FRR) and muscle activity during sustained neck flexion and to investigate the correlation between craniovertebral angle and FRR. Methods: Nineteen subjects participated in this study and were allocated to a forward head posture (FHP) group or a non-forward head posture (NFHP) group. Craniovertebral angle (CVA) and FRR were measured in all subjects, and all participants performed a standardized cervical flexion-extension movement in two phases: Phase I, sustained cervical full flexion for 5s; and Phase II, cervical extension with the return to the starting position for 5s. The value of CVA has calculated three times, and the value of FRR was measured three times in order to obtain the mean value. Results: FRR values in the FHP and NFHP group were significantly different (p<0.05). Phase I was significantly different, but the Phase II was not significantly different between the FHP and NFHP group (p>0.05). There was a significant correlation between the muscle activity of Phase I and CVA (p<0.05). However, FRR and the muscle activity of the Phase II were not a significant correlation with the CVA (p>0.05). Conclusion: FHP increases the muscle activity of the cervical erector spinae during sustained neck flexion and reduces FRR, which can cause fatigue in the cervical erector spinae. In addition, for those with a smaller CVA, muscle activity of the cervical erector spinae is increased during sustained neck flexion, which can increase neck muscle tension.
