Dual Lingual Thyroid Mimicking Mono-ectopic Thyroid in the Anterior View of Tc-99m Sodium Pertechnetate Thyroid Scan (전면 영상에서 하나로 오인된 두개의 설갑상전종)

  • Ectopic thyroid is not common disease and double ectopic thyroid is rare. Until January of 2008, dual ectopic thyroid has been reported only 23 cases in international literatures. Tc-99m sodium pertechnetate scan is playing an important role in the diagnosis of ectopic thyroid. In most of the cases, dual lingual thyroid tissues could be detected as two radiotracer uptake foci. We report a case of dual lingual thyroid mimicking mono-lingual thyroid in the anterior view of thyroid scan. Lateral view helped in this case to detect the dual lingual thyroid. With lateral view, the anatomical position and relationship could be examined in more detail.

  • [ $^{201}T1/^{99m}Tc$ ] pertechnetate subtraction scintigraphy of the parathyroid gland was performed in a study of 8 patients with clinical and biochemical evidence of hyperparathyroidism for diagnosis and Localization of the lesion prior to sugery. 6 cases of parathyroid adenomas were accurately localized and 2 cases of diffuse hyperplasia associated with chronic renal failure(CRF) and multiple endocrine neoplasia (MEN) type I. $^{201}T1/^{99m}Tc$ pertechnetate scan was useful for patients with hyperparathyroidism, especially in definite localization preoperatively.

Feasibility of Dacryoscintigraphy in Normal Dogs (정상 개에서 누비공신티그라피의 적용)

  • The purpose of this study was to evaluate the value of dacryoscintigraphy with Tc-99 m Pertechnetate, which is useful in functional nasolacrimal duct obstruction diagnosis in human medical science, by applying it to normal dogs. Dacryoscintigraphy was performed on six clinically healthy beagle dogs to confirm normal passage of their lacrimal ducts. The scintigraphic images of both lacrimal system were obtained at 5 min, 10 min, and 20 min after Tc-99 m Pertechnetate administration, respectively. If lacrimal duct does not come into view within 20 min, delayed images at 30min, 45min and 60 min were gained. After drawing ROI of left and right canaliculus, lacrimal sac, and nasolacrimal duct acquired after dacryoscintigraphy, we measured each counting rate and developed a counting rate table for each time interval. Of the total of 12 places, 10 (83.3%) showed patency within 20 min, and 11 (91.6%) showed patency within 30 min. In one dog, a functional obstruction of right lacrimal canal was observed. Dacryoscintigraphy could provide useful information about functional and anatomical lacrimal duct obstruction in veterinary medicine as well as in clinical research.

Comparing of the Administered Activities and the Effective Dose of the Various Pediatric Dose Formulas of Nuclear Medicine (핵의학검사의 방사성의약품 소아투여량 공식 별 투여량 및 유효선량 비교)

  • The purpose of this study is to calculate and compare administered activities(MBq) and effective dose(mSv) of the various pediatric dose formulas of pediatric nuclear medicine and to provide base data for the criteria of the optimal administered activities. This study compares dosages and effective doses of 5 types of pediatric dose formulas(Clark rule, Area rule, Webster rule, Young rule, Solomon(Fried) rule) based on the dosage for adults of 2 types of radiopharmaceuticals($^{99m}Tc$-MDP, $^{99m}Tc$-Pertechnetate). The administered activities in adults, which is the criteria for calculating the Pediatric administered activities, used the value from the 'Nuclear Medicine' written by J-G Jeong & M-Ch Lee. and the administered activities by the radioactivity per effective dose(mSv/MBq) of the radiopharmaceuticals for calculating the effective dose used the value from ICRP 80 and the UNSCEAR 2008 Report. As a result of the study, the output of Young rule is the lowest, and its difference between other formulas is from minimum 1.7 times to maximum 3,4 times. The difference between administered activities of $^{99m}Tc$-MDP is maximum 309.9MBq and the effective dose is 3.76mSv. $^{99m}Tc$-Pertechnetate showed the figure at the maximum 154.9MBq and the effective dose has a difference of 5.50mSv. Since the pediatric dose formulas differ not only in administered activities but also in effective doses, the optimal administered activities have to be developed for optimization of medical radiation.

  • The major complications of Meckel's diverticulum (MD) are bleeding, intestinal obstruction, infection, umbilical fistula and perforation. Although the relative incidences vary between authors, bleeding is the most common complication in children. The aim of our study is to show the symptomatic guideline for the diagnosis of the bleeding MD. Eight cases with bleeding MD which were operated upon at the department of Pediatric Surgery, Yeungnam University Hospital from April 1985 to April 2001 were reviewed. Half of the patients were under 2 years of age and all patient s we re male. All patients were preoperatively diagnosed by previous history of intestinal bleeding (melena, hematochezia) and 99mTc pertechnetate MD scan. Segmental resection and end-to-end anastomosis was performed in 6 patients and diverticulectomy in 2 patients. Heterotropic gastric mucosa was found in 6 patients. Postoperative complication was not observed in any cases. In conclusion, in any male children with obscure intestinal bleeding, especially less than 2 years of age, bleeding MD must be suspected. It seems to us that 99mTc pertechnetate MD scan is a useful tool to diagnose bleeding MD.

  • Multiple endocrine neoplasia type 2 (MEN type 2, Sipple's syndrome) is a rare disorder characterized by the association of medullary carcinoma of the thyroid, parathyroid hyperplasia and can be diagonsed in early stage of the disease by meticulous screening tests of the family. This case report describes the location and categorization of tumors using $^{99m}Tc-pertechnetate,\;^{131}I-NaI,\;^{99m}Tc-pentavalent(V)$, DMSA $^{131}I-MIBG$ scans in two cases of MEN type 2 occurred in a 32-year old women and her 29-year old brother. In MEN type 2, we think, combined use of $^{99m}Tc-(V)-DMSA,\;^{99m}Tc-pertechnetate\;and\;^{131}I-MIBG$ may be useful for the categorization of tumor mass lesions and planning appropriate therapy.
