Resistance to Thyroid Hormone Syndrome Mutation in THRB and THRA: A Review

  • Resistance to thyroid hormone syndrome (RTH) is a genetic disease caused by the mutation of either the thyroid hormone receptor-β (THRB) gene or the thyroid hormone receptor-α (THRA) gene. RTH caused by THRB mutations (RTH-β) is characterized by the target tissue's response to thyroid hormone, high levels of triiodothyronine and/or thyroxine, and inappropriate secretion of thyroid-stimulating hormone (TSH). THRA mutation is characterized by hypothyroidism that affects gastrointestinal, neurological, skeletal, and myocardial functions. Most patients do not require treatment, and some patients may benefit from medication therapy. These syndromes are characterized by decreased tissue sensitivity to thyroid hormones, generating various clinical manifestations. Thus, clinical changes of resistance to thyroid hormones must be recognized and differentiated, and an approach to the practice of personalized medicine through an interdisciplinary approach is needed.

  • This review examines the pivotal clinical trials that evaluated the efficacy and safety of semaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist, in the management of obesity. The reported findings underscore significant and sustained weight loss achieved with semaglutide in diverse patient groups, although gastrointestinal disorders occurred frequently, leading to therapy discontinuation. Overall, the studies demonstrated the potential of semaglutide as a therapeutic option not only for type 2 diabetes but also for obesity. The treatment landscape in obesity is evolving, as reflected in changing regulatory approvals and clinical guidelines, suggesting a paradigm shift toward personalized approaches in this chronic disease states to achieve optimal treatment outcomes for patients.

Systems Biology and Emerging Technologies Will Catalyze the Transition from Reactive Medicine to Predictive, Personalized, Preventive and Participatory (P4) Medicine

  • We stand at the brink of a fundamental change in how medicine will be practiced. Over the next 5-20 years medicine will move from being largely reactive to being predictive, personalized, preventive and participatory (P4). Technology and new scientific strategies have always been the drivers of revolutions and this is certainly the case for P4 medicine, where a systems approach to disease, new and emerging technologies and powerful computational tools will open new windows for the investigation of disease. Systems approaches are driving the emergence of fascinating new technologies that will permit billions of measurements on each individual patient. The challenge for health information technology will be how to reduce this enormous amount of data to simple hypotheses about health and disease. We predict that emerging technologies, together with the systems approaches to diagnosis, therapy and prevention will lead to a down turn in the escalating costs of healthcare. In time we will be able to export P4 medicine to the developing world and it will become the foundation of global medicine. The "democratization" of healthcare will come from P4 medicine. Its first real emergence will require the unprecedented integration of biology, medicine, technology and computation. as well as societal issues of major importance: ethical, regulatory, public policy, economic, and others. In order to effectively move the P4 scientific agenda forward new strategic partnerships are now being created with the large-scale integration of complementary skills, technologies, computational tools, patient records and samples and analysis of societal issues. It is evident that the business plans of every sector of the healthcare industry will need to be entirely transformed over the next 10 years.and the extent to which this will be done by existing companies as opposed to newly created companies is a fascinating question.

수레바퀴 암 치료법에 대한 근거중심적 연구 (Evidence Based Approach of Wheel Balance Cancer Therapy: A Review)

  • Background : Integrative cancer treatment is a holistic approach embracing body, mind, and spirit incorporating conventional treatments of surgery, chemotherapy, radiation and personalized complementary treatments. Wheel Balance Therapy (WBT) of East-West Cancer Center(EWCC), Dunsan Oriental Hospital of Daejeon University was developed to balance out all factors involved in cancer care based on the traditional theories of oriental medicine. Objective : This work aims to analytically review literatures on WBT and its related components. Methods : Literatures published from January 1st, 1990 to April 30th, 2011 were reviewed focusing on 4 main components of WBT; herbal medicine, immune activation, anti-cancer diet, and breathing/meditation. Data were retrieved from medical search engines and electronic data bases including Pubmed, Research Information sharing Service (RISS), Korean-studies Information Service System (KISS), China National Knowledge Infrastructure (CNKI), and Korea's National Digital Library (KNDL). Results : In this review, EWCC's most commonly prescribed formulas are explored. The composition of the formulas, their use in clinical settings as well as the background studies and other therapeutic efficacies are explained. Information on incorporating anti-cancer dietary support and breathing and meditation techniques, other therapies practiced as part of the center's integrative cancer care are also covered. Conclusion : WBT based on holistic theories of oriental medicine embracing body, mind, and spirit is expected to further contribute in promotion of cancer patients' quality of life and prolonged survival time.

분자영상의 적용분야 및 전망 (Application and Prospects of Molecular Imaging)

  • 본 논문에서는 분자영상을 분류하고 적용 분야와 미래를 예측해 보고자 하였다. 분자영상은 생체 내에서 분자수준과 세포수준에서 일어나는 변화를 영상화하는 것으로써 분자세포생물학과 첨단영상기술이 발전하여 접목된 새로운 분야이다. 분자영상은 형광, 생물발광, SPECT, PET, MRI, Ultrasound 등의 영상 기법들을 이용하여 유전자 치료 모니터링, 세포추적, 세포 치료 모니터링, 항체영상, 약제 개발, 분자 상호작용 영상, 근적외선 형광 물질을 이용한 암 형광 영상, Bacteria 를 이용한 종양 표적 영상, 치료효과 조기 평가, 치료 효과 예측 등에 적용되고 있다. 분자 영상의 미래는 분자세포 생물학, 유전학, 화학, 약학, 물리학, 전산학, 의공학, 핵의학, 영상의학, 임상의학 등 여러 학문 분야가 융합되어 상호협조와 공동연구를 통하여 발전해 나갈 것이다. 분자영상의 태동으로 미래의 의료의 모습은 질병의 조기진단과 개인 맞춤형 치료가 가능하게 될 것이다.

개인 맞춤형 국소부위 질소 냉각 장비 개발 (Development of Nitrogen Cooling Equipment for Personalized Local Area)

  • 본 논문에서는 개인 맞춤형 국소부위 질소 냉각 장비 개발을 제안한다. 제안하는 장비는 크게 냉기공급모듈과 본체, 그리고 질소분사(건)으로 구성되며, 다음과 같은 특징들을 가진다. 첫 번째로 부피온도 감지센서로 피부온도를 측정한 정보를 활용하여 냉기의 공급량과 시간을 자동으로 제어하므로 완벽한 안전성 확보를 통한 기능상의 경쟁력을 가질 수 있다. 두 번째로 거리측정 센서를 적용함으로써 일정거리 이상 피부에 근접하게 되면, 제어 GUI와 연동하여 냉기를 차단하거나 질소의 배출을 조절하여 보다 높은 냉각요법의 효율을 높이면서도 안전한 관리가 가능하다. 세 번째로 질소의 공급을 조절할 수 있는 제어모듈을 설치하여 질소의 손실을 최소화하여 유지관리 비용을 최소화할 수 있다. 제안된 장비의 성능을 평가하기 위하여 외부시험기관에서 실험한 결과, 온도센서 정확도는 세계 최고 수준(±5%)보다 정확한 ±3.8%의 범위에서 측정이 되었고, 온도범위는 세계 최고 수준과 비슷한 110℃~-160℃의 범위가 측정되었다. 거리 정확도는 세계 최고 수준(±5%)보다 낮은 ±3.0%의 범위에서 측정이 되었고, 무게 정확도는 세계 최고 수준(±5%)보다 정확한 ±0.1%의 범위에서 측정이 되었다. 또한, 토출 제어는 세계 최고 수준(1단계)보다 높은 4단계가 측정되었고, 질소 사용량은 세계 최고 수준(6L/min) 이하인 0.8L/min로 측정되었다. 따라서 본 본문에서 제안한 개인 맞춤형 국소부위 질소 냉각 장비개발의 성능의 그 효용성이 입증되었다.

정신치료와 신경생물학적 연구결과의 관계 (The Relationship between Psychotherapy and Neurobiological Findings)

  • The mechanism of psychotherapy is explained by the recent developments in neuroscience and neuroimaging. The purpose of this study is to understand the nature of psychotherapy and to discuss the future of psychotherapy improvement with the help of advances of the neurobiological findings in psychotherapy. For this study, we investigated a wide range of materials. We searched for various researches on psychotherapy, brain, and neurobiology. In addition to the conventional psychodynamic psychotherapy, we investigated research findings on cognitive behavioral therapy, interpersonal psychotherapy and eye movement desensitization and reprocessing (EMDR). Moreover, based on the actual experiences of treating patients, we speculated the neurobiological mechanisms of the process and results of psychotherapy. With the development of neuroscience, we are now able to understand the personal consciousness, unconsciousness and developmental process. Also subdividing the disease is made possible. Personalized treatment has become available, and we are able to predict the prognosis of patients. Our memories are composed by implicit memory and explicit memory. By psychotherapy, we can consciously remember explicit memory, and it becomes easier to explore implicit memory through free association. Through psychotherapy, we will also be able to learn the effect of acquired environment and experience. Psychotherapy is able to correct human behaviors by modifying the memories. Through the regulation of emotions, it becomes possible to modify the memories and correct the behaviors. In this process, doctor-patient relationship is the main factor which cause positive treatment effects. Furthermore imagination therapy or unconscious, non-verbal stimuli could bring about positive treatment effects. Now psychotherapy could be explained and studied by neuroscientific researches. In this sense, we could provide the direction of future advances in neuroscience by the neurobiological understanding of psychotherapy.

  • The known risk factors for atherosclerosis include plasma low density lipoproteins (LDL)or Cholesterol, low PUFA in the diet, hypertension, and high Na intake, obesity, diabetes, lack of exercise, cigarette smoking, sugar, low fiber and nicotinic acid in the diet, sources of Protein foods, and Psychological factors. Among various dietary factors, fat is known as the most serious causative agent for atherosclerosis. The genetic factor is a18o known as an important one but is out of scope in this paper. Since atherosclerosis is a progressive disease which may develop for many years before showing any definitive symptoms, it is very important to develop preventive programs especially in the country like Korea that is not quite overdevelopted as some western countries. In this paper all the factors mentioned above were reviewed and the dietary suggestion were made on the basis of the content of polyunsaturated and saturated fat in the diet to prevent or/and to cure this disease. Most of the available data on diet therapy with emphasis. on P/S ratio were tabulated together. after tile patient's habitual dietary intake is analized the guidelines of personalized fat-controlled diet can be recommended. It is of utmost importance to develop Korean diets for beth prevention and cure of atherosclerosis emphasizing individual eating habit in the near future which can be more Practicaly used both at home and in the hospital.

우울증의 약물유전체학 (Pharmacogenomics of Depressive Disorders)

  • The pharmacotherapy of depression has reduced morbidity and improved outcome for many depressive patients. A wide range of classical and new antidepressants are available for their treatment. However, 30-40% of all patients do not respond sufficiently to the initial treatment and present adverse effects. Pharmacogenetics studies the genetic basis of an individual's ability to respond to pharmacotherapy. Recently, some reports on serotonin transporter gene polymorphisms and their influence on the response to antidepressive therapy provide an interesting diagnostic tool in assessing the chances of response to antidepressants. We also investigated the relationship between serotonin transprter polymorphisms(5-HTTLPR) and the long-term effect of the antidepressant treatment. 128 depressive patients were enrolled into 2nd year study. The therapeutic response of each subset was not different at 8th, 16th week, but the subset with homozygote(l/l) of long variant showed a better therapeutic response to antidepressant than the heterozygote(l/s) of long and short variant, which showed a better therapeutic response than the subset with homozygote (s/s) of short variant at 1st year and 2nd year after the antidepressant treatment. This result shows that the serotonin transporter polymorphisms may be related to the long-term effect of antidepressant treatment. The potential for pharmacogenomics, the use of genetic information to guide pharmacotherapy and improve outcome by providing individualized treatment decisions, has gained increasing attention. pharmacogenomics will contribute to individualize drug choice by using genotype to predict positive clinical outcomes, adverse reactions, and levels of drug metabolism. Personalized medicine, the use of marker-assisted diagnosis and targeted therapies derived from an individual molecular profile, will impact the antidepressant therapy and this approach will replace the traditional trial-and-error practice of medicine.

고혈압 진단자의 혈압 관리를 위한 외래 방문 영향요인 (Factors Related to the Outpatient Visits for Blood Pressure Management in Patients diagnosed with Hypertension)

  • Background: Regular doctor visits are vital for hypertension patients, especially for who have never received hypertension medication or non-pharmacologic therapy. This study purposed to study factors affecting outpatient visits for patients diagnosed with hypertension. Methods: This study included 59,009 respondents with hypertension over 30 from 2019 Community Health Survey data. Outpatient visits were defined by having hypertension treatments such as medication or non-pharmacologic therapy. Logistic regression was used to examine the factors affecting outpatient visits using SAS ver. 9.3. Results: 57,081(96.73%) patients with hypertension were identified as those having a outpatient visit for hypertension treatments, whereas 1,928(3.27%) patients did not have visits. Patient's characteristics such as gender, age, periods of hypertension, education level, perception of the blood pressure, hypertension management education, place of living, body mass index, depression and diabetes were found to have statistically significant relationship with the outpatient visits. Practical Implications: There is a need to select patients with high blood pressure who are unlikely to visit for hypertension treatments based on the study results. For those, establishing a personalized management plan such as health education and counseling programs will be helpful for the successful implementation of national chronic disease management program.
