Gingival crevicular fluid CSF-1 and IL-34 levels in patients with stage III grade C periodontitis and uncontrolled type 2 diabetes mellitus

  • Purpose: Periodontal diseases are inflammatory conditions that alter the host's response to microbial pathogens. Type 2 diabetes mellitus (T2DM) is a complex disease that affects the incidence and severity of periodontal diseases. This study investigated the gingival crevicular fluid (GCF) levels of colony-stimulating factor-1 (CSF-1) and interleukin-34 (IL-34) in patients with stage III grade C periodontitis (SIII-GC-P) and stage III grade C periodontitis with uncontrolled type 2 diabetes (SIII-GC-PD). Methods: In total, 72 individuals, including 24 periodontally healthy (PH), 24 SIII-GC-P, and 24 SIII-GC-PD patients, were recruited for this study. Periodontitis patients (stage III) had interdental attachment loss (AL) of 5 mm or more, probing depth (PD) of 6 mm or more, radiographic bone loss advancing to the middle or apical part of the root, and tooth loss (<5) due to periodontal disease. Radiographic bone loss in the teeth was also evaluated; grade C periodontitis was defined as a ratio of the percentage of root bone loss to age greater than 1.0. The plaque index (PI), gingival index (GI), presence of bleeding on probing (BOP), PD, and clinical AL were used for clinical periodontal assessments. GCF samples were obtained and analyzed using an enzyme-linked immunosorbent assay. Results: All clinical parameters-PD, AL, GI, BOP, and PI-were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups for both the full mouth and each sampling site (P<0.05). The total IL-34 and CSF-1 levels were significantly higher in the SIII-GC-PD group than in the PH and SIII-GC-P groups (P<0.05), and there were significant differences between the periodontitis groups (P<0.05). Conclusions: These findings suggest that IL-34 and CSF-1 expression increases in patients with SIII-GC-PD. CSF-1 was associated with the inflammatory status of periodontal tissues and T2DM, while IL-34 was associated only with T2DM.

골내치주낭에서 탈회냉동건조골 동종이식시 이식효과에 대한 임상적 연구 (CLINICAL STUDY ON THERAPEUTIC EFFECTS OF DECACIFIED PREEZE DRIED BONE ALLOGRAFT IN INTRABONY DEFECTS)

  • The ultimate goal of periodontal therapy is promoting the regeneration of lost periodontal tissue. The purpose of this study is to evaluate the effect of treatment using decalcified freeze dried bone allograft as a bone graft material. 47 intrabony defects from 27 patients with clinical diagnosis of chronic periodontitis were selected among those 24 defects were treated via flap operation only and designated as the control group, the other 23 defects were treated with decalcified freeze dired bone allografting via flap operation and designated as the experimental group. Clinical parameters including probing depth, loss of attachment, probing bone level and gingival recession have been recorded at 6th months, and the significance of the changes has been analyzed. The results are as follows : 1. Probing depths were reduced significantly in both control group($2.75{\pm}0.99mm$) and experimental group($3.69{\pm}0.97mm$) postoperatively(p<0.01). Experimental group showed significantly higher decrease compared to the control group(p]0.01). 2. Loss of attachment showed statistically significant decrease in both control group($1.77{\pm}1.08mm$) and experimental group postoperatively($2.70{\pm}1.55mm$). Experimental group showed significantly higher decrease compared to the control group(p]0.05). 3. Probing bone levels were reduced with statistically significance in both control group($1.08{\pm}0.97mm$) and experimental group($4.00{\pm}1.41mm$) postoperatively(p<0.01). Experimental group showed significantly higher decrease compared to the control group(p<0.01). 4. Gingival recession showed statistically significant increase in the control group($1.21{\pm}0.72mm$) and experimental group($1.00{\pm}1.09mm$) postoperatively(p<0.01). There was no statistical significance between the control group and the experimental group. On the basis of these results, treatment using allogenic decalcified freeze dried bone is effective in reducing probing depth, loss of attachment and probing bone level. Therefore allogenic decalcified freeze dried bone is an effective bone graft material in periodontal regeneration.

흡연량과 흡연 기간에 따른 치주 상태 (EFFECT OF CIGARETTE SMOKING ON PERIODONTAL STATUS)

  • The purposes of this study were to investigate the relationship between cigarette smoking levels and periodontal status. 199 subjects, consisting of male 127 and female 72, classified by smoking levels and duration were selected: Patients who had smoked for more than 20 cigarettes/day were considered heavy smokers, 10 to 19, moderate smokers; 1-9, light smokers:0, non-smoker. And smokers were divided into ones who had smoked for more than 20 years, 10-19 years, less than 10 years and non-smokers. Heavy smokers and moderate smokers showed significantly greater pocket depths, less clinical attachment levels, more molar furcation involvements, greater alveolar bone loss and higher scores of plaque index than light smokers and non-smokers,(p < 0.05). When the duration of smoking was considered as a factor, scores of clinical parameters were worse and alveolar bone loss were significantly greater in long-term smokers, who had smoked for more than 20 years. In conclusion, cigarette smoking is associated with periodontal status.

치주질환치료에서 국소약물 송달제재의 임상 및 세균학적 효과 (THE CLINICAL AND MICROBIOLOGICAL EFFECTS OF MINOCYCLINE-LOADED POLYCAPROLACTONE STRIP ON PERIODONTAL DISEASE)

  • The purpose of this study was to evaluate the clinical and microbiological effects of minocycline-loaded polycaprolactone strip on periodontal disease. Ten patients with probing depth (deeper than 5mm) were tested. Of the two periodontal pockets selected from each patient, one randomly selected pocket was treated by the insertion of 30% minocycline-loaded polycaprolactone sttip as the experimental group and the other with a minocycline-free polycaprolactone strip as the control group. All groups were examined by clinical and microbiological methods. 1. Plaque index scores, gingival index scores and sulcular bleeding index scores in both group were significantly reduced from the baseline to 2 weeks. Plaque index scores of experimental group and sulcular bleeding index scores of control group tended to be progressively reduced in all experimental periods. 2. Probing depth amounts in both group were significantly reduced from 4 weeks to 8 weeks. 3. Attachment loss amounts in both group shows no singnificant differences in time. Attachment loss amounts in experimental group tended to be less than those in control group. 4. The number of cocci in both groups were significantly increased in all experimental periods, but that of the non-motile rods were significantly reduced from 2 weeks in experimental group, and from 4 weeks in control group, that of the motile rods were reduced from 4 weeks in experimental group and from 1 week in control group. The number of spirochetes were reduced from I week during all experimental period in experimental group, but there was no changes in control group.

Outcomes of nonsurgical periodontal therapy in severe generalized aggressive periodontitis

  • Purpose: Aggressive periodontitis, especially in its severe form, was traditionally considered to have an unfavourable prognosis. It required a complex treatment and its stabilization was often achieved by surgical therapy. The aim of this study was to investigate the results of nonsurgical periodontal treatment in severe generalized forms of aggressive periodontitis. Methods: Patients with advanced generalized aggressive periodontitis were included in the study. Probing depth (PD) of pockets ${\geq}7mm$ and clinical attachment level (CAL) of sites with attachment loss ${\geq}5mm$ were measured at baseline before nonsurgical periodontal treatment, at re-evaluation, and after treatment. The following other parameters were recorded: resolution of inflammation and bone fill. We compared the baseline values with re-evaluation and posttreatment values using the Friedman test. The Wilcoxon test with the Bonferroni correction was used for both re-evaluation and posttreatment values. Results: Seven patients with 266 periodontal sites were examined. A significant difference was found between values, reported as medians with interquartile ranges, for PD at baseline (7.94 [7.33-8.19] mm) and both re-evaluation (4.33 [3.63-5.08] mm) and posttreatment (3.54 [3.33-4.11] mm) values (P=0.002). A significant difference was also found between values for CAL at baseline (9.02 [7.5-9.2] mm) and both re-evaluation (6.55 [6.30-6.87] mm) and posttreatment (6.45 [5.70-6.61] mm) (P=0.002). Inflammation was resolved and angular bone defects were repaired in all cases. Conclusions: These therapeutic results suggest that this form of periodontitis could have positive outcomes after nonsurgical periodontal treatment. The reparative potential of tissue affected by severe aggressive periodontitis should encourage clinicians to save apparently hopeless teeth in cases of this form of periodontitis.

비우식성 치경부 병소와 5급 와동 수복물이 치주조직에 미치는 영향 (Effects of non-carious cervical lesions and class V restorations on periodontal conditions)

  • Purpose: The non-carious cervical lesion(NCCL) is a loss of tooth structure at the neck of affected teeth that is unrelated to tooth caries. The reported prevalence of NCCL varies from 5% to 85%. Prevalence and severity of lesions have been found to increase with age. They are becoming more significant as people live longer and become more aware of the importance of oral health. The purposes of this study were first, to examine the periodontal conditions associated with NCCL, and second, to investigate the clinical effects of class V restorations of NCCL on periodontal tissues. Materials and methods: The sample size was 982 teeth of 50 subjects(25 male, mean age $52{\pm}7$) who were seen at the Department of Periodontology, Pusan National University Hospital. At the baseline examination, clinical periodontal parameters were measured. After the initial examination, 24 patients who were absent from hypersensitivity were selected. The teeth with NCCL were randomly divided into the test and control groups. The teeth in the test group were restored with flowable resin; the control teeth were not restored. Six months later, the clinical examinations were repeated. The data were analyzed using the SPSS program. Results: The results were as follows: 1) NCCL occurred on 45.8% of examined teeth. The percentage of affected teeth was higher in maxillary and premolar teeth. 2) The shallow saucer type was the most common. 3) Teeth with NCCL had more gingival recession, lower attachment level, and higher incidences of bleeding on probing(BOP) and plaque than NCCL-free teeth. 4) Six months later, gingival recession, attachment level, the percentages of BOP and plaque in the test group were lower than in the control group(p<0.05). Conclusion: NCCLs were more found in maxillary teeth, especially in premolar teeth. The results suggest that the restoration of NCCL could affect some periodontal parameters favorably.

중도 치주염에 이환된 치아의 보존을 위한 의도적 재식술을 통한 치료: 증례보고 (Treatment of a tooth with severe periodontal involvement using intentional replantation: case report)

  • 의도적 재식술은 흔히 근관 치료가 실패한 경우 사용되는 치료법이나, 중도의 치주 질환에 이환된 치아에서는 대개 추천되지 않는다. 그러나, 일부 연구들은 의도적 재식술을 이용하여 치주 질환에 이환된 치아를 성공적으로 치료한 것을 보고한 바가 있다. 본 연구에서는 중도 치주염에 이환된 치아를 의도적 재식술을 이용하여 치료한 증례를 보고하고자 한다. 해당 치아는 근관-치주 복합 병소로 진단 후 근관치료를 선행하였으나 광범위한 골소실을 나타냈으며, 이에 의도적으로 발치 후 치근 표면의 국소 인자를 완전히 제거한 후 재식하였다. 의도적 재식술을 이용하여 본 증례에서 중도 치주염에 이환된 치아를 발치하지 않고 3년간의 경과 관찰 기간 동안 보존할 수 있었다.

Intentional passive eruption combined with scaling and root planing of teeth with moderate chronic periodontitis and traumatic occlusion

  • Purpose: The aim of this clinical trial was to explore the impact of intentional passive eruption (IPE) using occlusal reduction on the periodontal parameters of teeth with moderate chronic periodontitis and traumatic occlusion. Methods: This study was performed on 40 teeth from 16 subjects. At baseline, 4 weeks after initial periodontal treatment, and 6 months after IPE, clinical and radiographic examinations were performed. The 20 teeth in the test group underwent IPE using occlusal reduction, whereas the 20 control teeth did not undergone occlusal reduction. Results: All the periodontal parameters were improved by the initial periodontal treatment. The teeth in the test group showed a significantly greater decrease in pocket depth, tooth mobility, and marginal bone loss than did the control group (P<0.05), but there were no significant changes in the attachment level. Significantly greater improvements in all the parameters were observed in the test group after 6 months of IPE compared to 4 weeks after the initial periodontal treatment (P<0.05). Conclusions: Within the limits of this study, performing initial periodontal therapy combined with IPE using occlusal reduction was shown to be very simple and effective. Moreover, IPE would be helpful in improving periodontal parameters.
