외톨이 교감신경절 차단을 이용한 직장암성 통증의 치험 -증례 보고- (Blockade of the Ganglion Impar in Rectal Cancer Pain -A case report-)

  • Pain arising from pelvic viscera and perineum is frequently associated with discomfort and disability and caused by local trauma, inflammation and malignancy within the pelvis. Although various interventions have been proposed for the management of intractable perineal pain, their efficacy and applications are limited. The ganglion impar is a solitary retroperitoneal structure located at the level of the sacrococcygeal junction that marks the terminations of sympathetic chains. We propose that blockade of the ganglion impar is an effective method in the management of patient suffered from perianal pain due to advanced rectal cancer.

  • Lysis of epidural adhesion has been done in patients with refractory lumbar radiculopathies. Cauda equina syndrome is a rare complication of epidural block. We report on a case of cauda equina syndrome following epidural adhesiolysis in a patient with spinal stenosis. The patient complained of numbness of the perineum, weakness of the left leg, an inability to void and fecal incontinence. She was treated with medication, bladder training and physical therapy, and finally recovered 1 month after the procedure without any sequelae. We suggest that the causative factors are osmotic damage produced by the subarachnoid injection of large doses of hypertonic saline and temporary neural compression due to spinal stenosis.

  • 만성폐쇄성 폐질환을 동반한 노인환자에서 척추마취하에서 치골상부 전립선 절제술을 시행하는 도중 척추마취와는 무관한 것으로 사료되는 원인 불명의 경련 발작 및 혈압강하가 초래된 환자가 발생하였기에 그 원인을 규명하고 문헌적 고찰과 함께 보고하는 바이다.

  • 대상포진 후 신경통은 통증치료실에서 당면하는 가장 치료하기 어려운 질환중의 하나로서 현재까지 교감신경 차단, 경피적 전기자극, 약물요법 등 여러 방법이 소개되어 있으나 어느것이나 만족스럽지 못하다. 저자는 상기의 모든 방법을 동원하여 통증의 제거에 실패한 60세의 여자환자에 지주막하로 45 ml의 냉식염수를 주입하여 폐부종, cauda equina syndrome 등을 포함한 심한 합병증을 경험하였기에 문헌과 함께 고찰해 보고자 한다.

  • Pain arising from disorders of the viscera and somatic structures within the pelvis and perineum is a frequent cause of discomfort and disability, especially among women. Recently, blockade of the ganglion impar(ganglion of Walther) has been introduced as an alternative means of managing intractable perineal pain of sympathetic origin. The first report of interruption of the ganglion impar for relief of perineal pain appeared in 1990. We successfully treated a patient who had suffered from intermittent severe coccygodynia with ganglion impar block.

  • The perineum is comprised of diverse anatomic structures with mixed sympathetic and somatic innervation. The coccyx is innervated by the coccygeal nerves and branches of the fifth sacral root. Recently, ganglion impar block has been introduced as an alternative means of managing intractable pain of sympathetic origin, coccygodynia by trauma, tenesmus and perineal hyperhydrosis. We managed a 59-year-old female patient who had suffered from perineal pain by metastasis of cervical cancer. Approach to impar ganglion through the anococcygeal ligament was impossible because her coccyx was hyperflexed anteriorly by old fracture. But we could perform ganglion impar block successfully by approach through the separation of sacrum and coccyx.

  • We observed the external genitalia and uterus of a 24-month-old freemartin Hanwoo. The vulva was smaller than observed in a normal female Hanwoo, while the clitoris was larger in the freemartin. The angle between the external genitalia and the perineum also varied. Upon internal genital examination, the uterus of the freemartin was a thin tube approximately 18 cm in size and had not differentiated into a normal uterus and uterine horns.

  • This study comprised 13 cases having canine perianal adenoma at Veterinary Medical T~aching Hospital, Seoul National University from January 2000 to October 2003. All referred dogs were intact male and mean age of dogs was 11.5 years (ranged from 7 to 15 years). The most prevalent breed was Shih Tzu. The clinical signs were presence of 1 to 5 cm in diameter firm mass, perianal swelling, hemorrhage, pain and dyschezia. The masses were located either in the perineum (12 cases) or tail (5 cases). Five dogs had masses with ulceration. Concurrent prostatomegaly and seminoma was found in 5 and 1, respectively. Castration was performed in 5 cases. In 8 cases, surgical excision of the tumor was performed with castration. There was no recurrence in all cases after removal. In all cases with castration, perianal adenomas were regressed within 1-2 months. In one case among them, excision of the tumor was also performed because of persistent ulceration. According to these clinical results, perianal adenoma excellently responded to castration with or without surgical excision of the tumor.

  • Necrotizing fasciitis (NF) is a rapidly progressive necrosis of the subcutaneous tissue and fascia, caused by bacterial infection. Usually presenting in the extremities, trunk, or perineum, it is uncommon in the craniofacial or cervical area. Cervicofacial NF is a potentially fatal infection, which should be managed with early detection and intervention. Most cases have a primary odontogenic source of infection, especially when the masticator space is involved. We report a case of masticator space NF that developed without odontogenic origin in a 78-year old female who was treated with prompt surgical drainage and intravenous antibiotics.

Infiltrative Lipoma Invading the Anal Sac in a Dog: A Rare Case Report

  • A 6-year-old female Chihuahua was presented to the Animal Medical Center for surgical resection of a perineal mass that had recently increased in size. Ultrasonography revealed a large, homogeneous fatty mass with irregular margins between the surrounding muscle layers due to infiltrating fatty tissues. Cytological findings from fine-needle aspirates revealed numerous sheets and clusters of adipocytes, which was consistent with the fatty mass. Based on ultrasonographic and cytological findings, an infiltrative lipoma was suspected. During the surgery, the perineal mass was found to be non-encapsulated, irregularly marginated, and extensively distributed into the surrounding muscles. The mass extended inside the pelvic cavity and left anal sac. The perineal mass was surgically removed and submitted for histopathologic examination. Histopathology confirmed that the mass was an infiltrative lipoma invading into the anal sac and surrounding muscles. The present report was an unusual presentation of infiltrative lipoma that invaded the anal sac in the perineum.
