A Survey of Actual Guides for Children to Take the Oriental Medicine in the Oriental Clinics & the Oriental Hospitals (한의원과 한방병원에서 행해지고 있는 소아에 대한 한약복약지도 현황 조사 연구)

  • Objectives Children's unique taste bud often gives them difficult time to take oriental medicines. To overcome, survey was conducted focusing on useful and practical tips on how to take them. Methods The survey was carried out by posting mail questionnaire. Target clinics for the survey were selected by the internet search engine, Google, Daum, and Naver, and typing in "oriental treatment + pediatrics", "pediatrics + oriental medicine", "pediatric oriental clinic". 24 clinics were searched, but they were further narrowed down to the specialized pediatric oriental clinics. Also, total of 38 inquiry leaflets were distributed to 14 University Oriental Hospitals. Results If children refuse to take the medicine, mixture with sweeteners can be prescribed for them. Among them, oligosaccharide is the most recommended followed by sugar and honey. To avoid rejection to medication, use of different formulation, such as distillation medicine, soft X-Section, granulated medicine, and character printing can be used. Conclusions The most effective method is to investigate the mixture first, and then the alternative formulations.

An Unusual Case of Acute Pyelonephritis Caused by Shigella dysenteri in a Child (소아에서 Shigella dysenteri에 의한 신우신염 1례)

  • Shigella infection usually produces gastrointestinal symptoms but rarely causes urinary tract infection. A 7-year-old girl was admitted for fever, chills, right flank pain, and dysuria. She had no vomiting or diarrhea. There was mild tenderness in her right lower abdomen, and right CVA tenderness was also noted. Acute pyelonephritis was diagnosed by abdominal CT. She showed improvement with intravenous administration of antibiotics. The first urine culture grew $1{\times}10^5$ CFU/mL Shigella dysenteri. Although urinary tract infections due to Shigella species are extremely rare, Shigella species should be considered as a possible cause of pediatric urinary tract infection. We report the first case of urinary tract infection caused by S. dysenteri, which presented as acute pyelonephritis without gastrointestinal symptoms in a child.

A Novel VPS33B Variant Identified by Exome Sequencing in a Patient with Arthrogryposis-Renal Dysfunction-Cholestasis Syndrome

  • Arthrogryposis-renal dysfunction-cholestasis (ARC) syndrome is a rare autosomal recessive multisystemic disease that is associated with the liver, kidney, skin, and central nervous and musculoskeletal systems. ARC occurs as a result of mutations in the VPS33B (Vacuolar protein sorting 33 homolog B) or VIPAR (VPS33B interacting protein, apical-basolateral polarity regulator) genes. A female infant presented with neonatal cholestasis with a severe clinical outcome. She was diagnosed with ARC syndrome using targeted exome sequencing (TES). Exome sequencing revealed compound heterozygous mutations, c.707A>T and c.239+5G>A, in VPS33B, where c.707A>T was a novel variant; the resultant functional protein defects were predicted via in silico analysis. c.239+5G>A, a pathogenic mutation that affects splicing, is found in less than 0.1% of the general population. Invasive techniques, such as liver biopsies, did not contribute to a differential diagnosis of ARC syndrome; thus, early TES together with clinical presentations constituted an apparently accurate diagnostic procedure.

  • A 3-year-old girl with acute respiratory distress syndrome due to a H1N1 2009 influenza virus infection was complicated by necrotizing pneumonia was successfully treated with extracorporeal membrane oxygenation (ECMO). This is the first reported case in which a pediatric patient was rescued with ECMO during the H1N1 influenza epidemic in Korea in 2009.

  • The Korean Society of Pediatric Infectious Diseases and the Korea Centers for Disease Control and Prevention issued the guidelines about coronavirus disease 2019 (COVID-19) for children and adolescents. Case definitions and management of COVID-19 in neonates, infants, children and adolescents are presented in this guideline. In addition, guidelines for caregiver management are also provided. In this review, we introduce the contents of the current guidelines for COVID-19 in children and adolescents in Korea.

The Clinical Status of Adolescent Patients with Dysmenorrhea Visiting Korean Medicine Hospital (한방병원에 내원한 청소년기 월경통 환자의 임상현황 )

  • Objectives We aimed to analyze menstrual pain clinical patterns and treatment characteristics in pediatric and adolescent patients under 18 years old treated for menstrual pain at a Korean medicine hospital. Methods We analyzed the medical records of pediatric and adolescent patients under 18, treated for menstrual pain from January 1, 2018, to December 31, 2023. Results A total of 88 participants were included in the study, with a mean age of 15.3 ± 1.5 years. The most common diagnosis was unspecified dysmenorrhea. Among the 42 participants with recorded pain scores, the mean Numeric Rating Scale (NRS) for dysmenorrhea was 7.4 ± 1.0. Patients who received two or more treatments had an average of 6.3 ± 5.0 clinic visits. Treatment methods included herbal medicine, acupuncture, moxibustion, pharmacopuncture, and electroacupuncture, in descending order of frequency. Conclusions It is necessary to analyze the clinical characteristics and status of adolescent dysmenorrhea patients in more Korean medical institutions. Additionally, research and promotion of Korean medicine treatments for adolescent dysmenorrhea are needed.

  • Objectives The purpose of this study is to report the nocturnal enuresis in children using Sensitiv $Imago^{TM}$. Methods Twenty nocturnal enuresis children were tested using Sensitiv $Imago^{TM}$. Results 10 cases showed the lowest score at filter $#5^*$ and 7 cases showed the lowest score at filter $6^*$ in {Express Monitoring} of {Review of System Disorders of Homeostasis}. Conclusions Further study with more samples is necessary to establish the accuracy of Sensitiv $Imago^{TM}$.

A Retrospective Study of Autism Spectrum Disorder in Children Treated by Korean Medicine (한의학적 치료를 받은 자폐 스펙트럼 장애 환아 63례에 대한 후향적 차트리뷰)

  • Objectives This study aimed to analyze the clinical characteristics of 63 patients with autism spectrum disorder (ASD) who received Korean medicine treatment. Methods This study identified and retrospectively reviewed the first visit records and clinical characteristics of all of the patients who visited the Kyung Hee University Korean Medicine Hospital and Kyung Hee University Korean Medicine Hospital at Gangdong in South Korea with ASD as their chief complaints. The herbal medicines that were prescribed according to the patient's symptoms were analyzed by separating them into categories. Results The data of 83 children with ASD were collected. Among them, the primary medical record charts of 63 patients were reviewed. Additional analysis was performed for 56 patients who were treated with herbal medicine. Through a retrospective chart review, the patients' clinical characteristics and the tendency of the prescribed herbal medicine was identified. Conclusions This study was conducted to provide preliminary data that proposes Korean medicine as a new alternative medical treatment. Our study is meaningful as it used the demographic and clinical records of 63 patients with ASD in Korea who visited an Korean medicine hospital in the last 11 years to identify tendencies and analyzed the prescriptions of two or more Korean medicine pediatricians.
