A Study on the Trends of Korean Medicine in Children and Adolescents - Focused on Deer Antler - (소아청소년 환자의 한약 치료 동향 - 녹용을 중심으로 -)

  • Objectives This study aimed to provide a basis for the research on the trend and safety of Korean herbal medicines among children and adolescents. Methods Patients aged <19 years who received outpatient treatment at the Korean pediatrics department of Kyung Hee University Hospital from January 1, 2017 to October 31, 2022 were included. The current state of annual visits and herbal medicine prescriptions was analyzed. A retrospective chart review was conducted by examining the medical records of subjects who received herbal medicines, including deer antler. Adverse drug reactions in patients who received herbal medicine were also investigated. Results A total of 57,457 people visited the Korean pediatrics department, and 51,768 people (90.1%) were prescribed herbal medicine. Among these, 21,826 patients (42.2%) were prescribed with deer antler. A total of 117 adverse drug reactions were reported among the patients who received herbal medicines. Conclusions This study aimed to determine the trends and safety of herbal medicine treatment, particularly deer antlers, in children and adolescents. Well-designed, long-term, multi-institutional follow-up studies are required to expand the field of Korean pediatrics.

Review of Clinical Researches for Herbal Medicine Treatment in Pediatric Growth - Focusing on recent national studies - (국내 소아성장 한약치료에 대한 고찰)

  • Objectives The purpose of this study is to provide clinical evidences on effectiveness of herbal medicine treatment regarding pediatric growth based on recent clinical studies. Methods Selected 17 clinical studies since January 1998 to June 2018 from the Journal of Pediatrics of Korean Medicine and OASIS Traditional Medicine Knowledge Portal were reviewed. Results Medications that strengthening spleen and stomach (健脾胃藥), digestant (消導藥), supplementing kidneys (補 腎藥), and lungs (補肺藥) were frequently used. Frequently used herbs were Hoelen (茯?), Citri Pericarpium (陳皮), Glycyrrhizae Radix (甘草), Atractylodis Rhizoma Alba (白朮), Dioscoreae Rhizoma (山藥), Angelicae Gigantis Radix (當歸), Amomi Fructus (砂仁), Rehmanniae Radix Preparata (熟地黃). Conclusions This study showed that Korean herb medicine treatment can be effective for pediatric growth.

Current Status of Pediatric Critical Care in Korea: Results of 2015 National Survey

  • Background: The aim of this study was to describe the structure, organization, management, and staffing of pediatric critical care (PCC) in Korea. Methods: We directed a questionnaire survey for all Upper Grade General Hospitals (n = 43) in Korea in 2015. The first questionnaire was mainly about structure, organization, and staffing and responses were obtained from 32 hospitals. The second questionnaire was mainly about patients and management. Responses to second questionnaire were obtained from 18 hospitals. Results: Twelve from 32 Upper Grade General Hospitals had pediatric intensive care units (PICUs) and 11 of them had the PICU which was exclusive for children. Total number of PICU beds in Korea was 113. The ratio of the number of PICU beds to the number of children was 1:77,460 in Korea and this ratio is lower than that of other developed countries. The mean number of beds in the PICUs was $9.4{\pm}9.3$ (range, 2-30). There were 16 medical doctors who were assigned for PCC and only 5 of them were full time pediatric intensivists. In the 18 Upper Grade General Hospitals that responded to the second questionnaire survey, there were 97 patients in the PICUs with an average number of $5.7{\pm}7.2$ (range, 0-22) on the survey day. The mean age of the patients was $3.4{\pm}5.6$ years. The mean length of hospital stay was $82{\pm}271days$. The mean Pediatric Risk of Mortality score III was $9.4{\pm}7.8$ at the time of admission to the PICUs. Conclusion: There is a considerable shortage of PICU beds compared to those in developed countries. In addition, the proportion of PICUs with PCC specialists is much lower than those in the US and European countries.

Case Study of the Effects of Saenghyeoryunbueum on Atopic Dermatitis (생혈윤부음(生血潤膚飮)을 이용한 아토피 피부염 치험례 보고)

  • Objectives The purpose of this study is to report the clinical effects of herbal medicine on atopic dermatitis. Methods This nine clinical studies were done by those people who were visited the Oriental Hospital and all of them were diagnosed as atopic dermatitis. We treated the atopic dermatitis patients with herbal medicine(Saenghyeoryunbueum), and we checked SCORAD score and observed the wounds those patients had. Results The patients' wounds were improved and SCORAD score was decreased. Conclusions Herbal medicine(Saenghyeoryunbueum) is an effective in the treatment of atopic dermatitis, and it helped to improve regenerating the skin in the body. The further studies might be also needed.

Three Cases Report of Molluscum Contagiosum Children treated by Gwakhyangjeonggisangamibang (곽향정기산가미방(藿香正氣散加味方)으로 치료한 소아 전염성 연속종 3례)

  • Objectives : Molluscum Contagiosum is a contagious disease most often seen in children. It is caused by molluscum contagiosum virus(MCV-1,2). The purpose of this study is to report three cases of Molluscum Contagiosum children treated by oriental medicine. Methods : The subjects are in regard to children diagnosed as Molluscum Contagiosum. We treated three children with herbal medicine(Gwakhyangjeonggisangamibang). Results : After treatment, the symptoms(papule on the whole body and pruritus) of Molluscum Contagiosum were improved. Two children of them were cured completely and the one child still appealed a little itching. But, the one child had atopic dermatitis, and suffered from itching primarily, so after treatment the itching sign decreased rather than before an attack of Molluscum Contagiosum. Conclusions : This study shows that oriental medicine can be an effective treatment for Molluscum Contagiosum. And the further study is needed with more cases and longer duration.

Association between cord blood 25-hydroxyvitamin D concentrations and respiratory tract infections in the first 6 months of age in a Korean population: a birth cohort study (COCOA)

  • Purpose: Previous studies suggest that the concentration of 25-hydroxyvitamin D [25(OH)D] in cord blood may show an inverse association with respiratory tract infections (RTI) during childhood. The aim of the present study was to examine the influence of 25(OH)D concentrations in cord blood on infant RTI in a Korean birth cohort. Methods: The levels of 25(OH)D in cord blood obtained from 525 Korean newborns in the prospective COhort for Childhood Origin of Asthma and allergic diseases were examined. The primary outcome variable of interest was the prevalence of RTI at 6-month follow-up, as diagnosed by pediatricians and pediatric allergy and pulmonology specialists. RTI included acute nasopharyngitis, rhinosinusitis, otitis media, croup, tracheobronchitis, bronchiolitis, and pneumonia. Results: The median concentration of 25(OH)D in cord blood was 32.0 nmol/L (interquartile range, 21.4 to 53.2). One hundred and eighty neonates (34.3%) showed 25(OH)D concentrations less than 25.0 nmol/L, 292 (55.6%) showed 25(OH)D concentrations of 25.0-74.9 nmol/L, and 53 (10.1%) showed concentrations of ${\geq}75.0$ nmol/L. Adjusting for the season of birth, multivitamin intake during pregnancy, and exposure to passive smoking during pregnancy, 25(OH)D concentrations showed an inverse association with the risk of acquiring acute nasopharyngitis by 6 months of age (P for trend=0.0004). Conclusion: The results show that 89.9% of healthy newborns in Korea are born with vitamin D insufficiency or deficiency (55.6% and 34.3%, respectively). Cord blood vitamin D insufficiency or deficiency in healthy neonates is associated with an increased risk of acute nasopharyngitis by 6 months of age. More time spent outdoors and more intensified vitamin D supplementation for pregnant women may be needed to prevent the onset of acute nasopharyngitis in infants.

A Case Report of Mesenteric Lymphadenitis with Diarrhea (설사를 동반한 장간막 림프절염 환아 증례 보고)

  • Objectives : The purpose of this study is to report a case of nine-year-old female pediatric patient who had diagnosed as mesenteric lymphadenitis with abdominal pain. Methods : The patient was treated by acupunture (acupoint: Hegu(LI4), Zusanli(S36), Zhongwan(CV12), Tianshu (S25)), indirect moxibustion and herb medicine (Koakhyangjeonggisan-kamibang, Jakyakgamchotang Ex) for one week. We observed clinical progress. Result and conclusions : After oriental medical treatment, symptoms of mesenteric lymphadenitis (day and night abdominal pain, diarrhea) were improved. This study showed that oriental medicine can be a meaningful treatment for mesenteric lymphadenitis. For more accurate studies, further treatment cases would be needed.

Identification of rare coding variants associated with Kawasaki disease by whole exome sequencing

  • Kawasaki disease (KD) is an acute pediatric vasculitis that affects genetically susceptible infants and children. To identify coding variants that influence susceptibility to KD, we conducted whole exome sequencing of 159 patients with KD and 902 controls, and performed a replication study in an independent 586 cases and 732 controls. We identified five rare coding variants in five genes (FCRLA, PTGER4, IL17F, CARD11, and SIGLEC10) associated with KD (odds ratio [OR], 1.18 to 4.41; p = 0.0027-0.031). We also performed association analysis in 26 KD patients with coronary artery aneurysms (CAAs; diameter > 5 mm) and 124 patients without CAAs (diameter < 3 mm), and identified another five rare coding variants in five genes (FGFR4, IL31RA, FNDC1, MMP8, and FOXN1), which may be associated with CAA (OR, 3.89 to 37.3; p = 0.0058- 0.0261). These results provide insights into new candidate genes and genetic variants potentially involved in the development of KD and CAA.
