  • Objective: The purpose of this study is to find the level of physical relaxation of individual subject by monitoring psychophysiological biofeedback to different types of mattresses. And, the study also aims to find a protocol to make a selection of the best mattress based on the measured information. Background: In Korea, there are an increasing number of people using western style bed. However, they are often fastidious in choosing the right mattress for them. In fact, people use their past experience with their old mattress as well as the spontaneous experience they encounter in a show room to finally decide to buy a bed. Method: Total five mattresses were tested in this study. After measuring the elasticity of the mattresses, they were sorted into five different classes. Physiological and psychological variables including Electromyography (EMG), heart rates (HR), oxygen saturations (SaO2) were used. In addition, the peak body pressure concentration rate was used to find uncomfortably pressured body part. Finally, the personal factors and subjective satisfaction were also examined. A protocol was made to select the best mattress for individual subject. The selection rule for the protocol considered all the variables tested in this study. Results: The result revealing psychological comfort range of 0.68 to 0.95, dermal comfort range of 3.15 to 6.07, back muscle relaxation range of 0.25 to 1.64 and personal habit range of 2.0 to 3.4 was drawn in this study. Also a regression model was developed to predict biofeedback with the minimal use of biofeedback devices. Moreover results from the proposed protocol with the regression equation and subjective satisfaction were compared with each other for validation. Ten out of twenty subjects recorded the same level of relaxation, and eight subjects showed one-level difference while two subjects showed two-levels difference. Conclusion: The psychophysiological variables and suitability selection process used in this study seem to be used for selecting and assessing ergonomic products mechanically or emotionally. Application: This regression model can be applied to the mattress industry to estimate back muscle relaxation using dermal, psychophysiology and personal habit values.

  • In order to investigate the vortex body frame interaction around the side mirror of a passenger car, velocity vector fields in the wake, pressure distributions and boundary layer flows over both the mirror surface and the mirror housing, have been measured by several experimental tools. It was resulted that only within an half downstream distance of the mirror span there appears the recirculation zone, and also found that vortex trail towards to the driver side window between A and B pillars, making the acoustic noise and vibration. Wake vortex rolls up after this recirculating zone and makes the trail of the vortex center towards the driver side window, which was also confirmed by measurements of wake velocity vectors in the vertical sections of the trail and visualization over the side mirror surfaces as well. It was also observed that total pressure distribution over the mirror surface has the minimum peak near the lower tip region which can be considered as the origin of the vortex center. It can be concluded that the geometrical modification of the lower tip and the upper root area of the mirror housing is the key to control the wake vortex.

  • A 4-year-old intact male Shih Tzu dog (5.4 kg of body weight) was referred with primary complaints of heart murmur and exercise intolerance. Diagnostic studies found severe valvular pulmonic stenosis (peak velocity of 6.4 m/s, pressure gradient 165 mmHg). The dog was treated with 3 cm (length) ${\times}$ 1.5 cm (diameter) of Palmaz biliary stent. The outcome of stent placement was favorable and remarkably reduced the severity of PS (6.4 m/s to 3.0 m/s of peak velocity). Clinical condition was remarkably improved with the absence of cardiac murmur, although mild pulmonic regurgitation existed from the day of stent placement. To our best knowledge, this case is the first clinical trial for treating PS with intravascular stent in Korea.

  • It is widely recognized that manipulation of body position takes advantage of the influences of gravity for improving oxygenation. The study aims to determine the effects of positioning(supine, prone, right lateral decubitus and left lateral decubitus positions) applied to the mechanically ventilatory acute respiratory failure patients on arterial oxygen partial pressure($PaO_2$), alveolar arterial oxygen tension difference($AaDO_2$), mean aterial pressure, peak inspiratory pressure and plateau pressure. Thirty two acute respiratory failure patients admitted to the medical intensive care unit at Kangnam St. Mary's Hospital, The Catholic University of Korea from March 1997 to January 1998, were divided into three groups by radiographic evidence of unilateral or bilateral lung disease. In group 1 with dominant right lung disease were twelve subjects, group 2 with dominant left lung disease had eight subjects and group 3 had twelve subjects with bilateral lung disease. The variables were measured in 30 minutes after each position of supine, prone, good lung down lateral decubitus and sick lung down lateral decubitus position. The position order was done at random by Latin squre design. The results are as follows; 1) With group 1 patients, the $PaO_2$ in the left lateral decubitus and prone position were $126.8{\pm}30.8$ mmHg and $106.7{\pm}36.8$ mmHg, respectively(p=0.0001). 2) With group 2 patients, the $PaO_2$ in the prone and the right lateral decubitus position were $121.7{\pm}44.7$ mmHg and $118.5{\pm}31.7$ mmHg, respectively (p=0.0018). 3) With group 3 patients, the $PaO_2$ was $143.6{\pm}36.6$ mmHg in the prone position (p=0.0001). 4) With group 1 patients, the $AaDO_2$ in the left lateral decubitus and the right lateral decubitus position were $178.1{\pm}29.7$ mmHg and $233.1{\pm}24.4$ mmHg, respectively(p=0.0001). 5) With group 2 patients, the $AaDO_2$ in the prone and the left lateral decubitus postion were $184.0{\pm}39.5$ mmHg and $231.0{\pm}23.9$ mmHg, respectively(p=0.0019). 6) With group 3 patients, the $AaDO_2$ in the prone and the supine postion were $377.1{\pm}35.6$ mmHg and $435.7{\pm}13.1$ mmHg, respectively (p=0.0001). 7) There were no differences among the mean arterial pressure, peak inspiratory pressure and plateau pressure for each of the supine, prone, left lateral decubitus and right lateral decubitus position. The results suggest that oxygenation may improve in mechanically ventilatory patients with unilateral lung disease when the position is good lung dependent and prone, and patients with bilateral lung disease when the position is prone without any effects on the mean arterial pressure and airway pressure. It is suggested that body positions improve ventilation/perfusion matching and oxygenation need to be specified in patient care plans.

  • Due to top-coal and immediate roof as cushion layer connecting with support and overlying strata, it can make significant influence on strata behaviors in fully mechanical top-coal caving working face (TCCWF). Taking Qingdong 828 working face as engineering background, $FLAC^{3D}$ and $UDEC^{2D}$ were adopted to explore the influence of top-coal thickness (TCT), immediate roof thickness (IRT), top-coal elastic modulus (TCEM) and immediate roof elastic modulus (IREM) on the vertical stress and vertical subsidence of roof, caving distance, and support resistance. The results show that the maximum roof subsidence increases with the increase of TCT and IRT as well as the decrease of TCEM and IREM, which is totally opposite to vertical stress in roof-control distance. Moreover, although the increase of TCEM and IREM leading to the increase of peak value of abutment pressure, the position and distribution range have no significant change. Under the condition of initial weighting occurrence, support resistance has negative and positive relationship with physical parameters (e.g., TCT and IRT) and mechanical properties (e.g., TCEM and IREM), respectively.

  • The characteristics of amplitudes and power spectra of X axial, Y axial, and RZ axial (i.e., body axis) wind forces on a 492 m high-rise building with a section varying along height in typical wind directions are studied via a rigid model wind tunnel test of pressure measurement. Then the corresponding mathematical expressions of power spectra of X axial (across-wind), Y axial (along-wind) and torsional wind forces in $315^{\circ}$ wind directions are proposed. The investigation shows that the mathematical expressions of wind force spectra of the main structure in across-wind and torsional directions can be constructed by the superimposition of an modified wind spectrum function and a peak function caused by turbulent flow and vortex shedding, respectively. While that in along-wind direction can only be constructed by the former and is similar to wind spectrum. Moreover, the fitted parameters of the wind load spectra of each measurement level of altitude are summarized, and the unified parametric results are obtained. The comparisons of the first three order generalized force spectra show that the proposed mathematical expressions accord with the experimental results well.

  • To investigate the characteristics of the combined wind field produced by the natural wind field and the train-induced wind field on the bridge, the aerodynamic models of train and bridge are established and the overset mesh technology is applied to simulate the movement of high-speed train. Based on ten study cases with various crosswind velocities of 0~20 m/s and train speeds of 200~350 km/h, the distributions of combined wind velocities at monitoring points around the train and the pressure on the car-body surface are analyzed. Meanwhile, the difference between the train-induced wind fields calculated by static train model and moving train model is compared. The results show that under non-crosswind condition, the train-induced wind velocity increases with the train speed while decreases with the distance to the train. Under the crosswind, the combined wind velocity is mainly controlled by the crosswind, and slightly increases with the train speed. In the combined wind field, the peak pressure zone on the headstock surface moves from the nose area to the windward side with the increase of wind velocity. The moving train model ismore applicable in analyzing the train induced wind field.

  • A 6-month-old intact male Maltese dog (2.5 kg of body weight) was presented with the primary complaint of heart murmur and exercise intolerance. Diagnostic studies revealed severe pulmonic stenosis. Since the pressure gradient in stenotic pulmonic valve area was higher than 200 mmHg (maximum flow velocity = 7.1 m/s), the dog was underwent balloon valvuloplasty. With this procedure, the peak pulmonic velocity was reduced to 3.0 m/s (PG = 36 mmHg). Periodic re-evaluations performed in monthly interval revealed no further deterioration even after 4 month of procedure. This is the first case report of pulmonic stenosis fixed by non-surgical balloon valvuloplasty in Korea.
