  • This paper attempts to analyze the socio-economic determinants of demanders for cultural performance. To take into account zero responses (no expenditure or no experience for cultural performance, this paper employed a two-equation model and tobit model. the results of this study can be obtained the following conclusion. The demands of performing arts tend to be increased to people as they were younger, women, more earned and higher educated. In addition, the income is inelastic to demand. And after a certain age (peak age), the demand is increased.

  • Osteoporosis is the most common metabolic disease of the bone, and constitutes one of the most important major pubulic health problems world wide. Therefore, in order to be helped early diagnosis, treatment and prevention of osteoporosis, measurement of Bone Mineral Density (BMD) is to be needed. Authors has analysed Bone Mineral Density (BMD) of 480 normal adults who visited woo suk unoversity Oriental Meclical Center from April 1998 to July 1998. The aims of this study is to investigate correlation between Bone Mineral Density (BMD) and age distribution, to examine the correlation between Bone Mineral Density (BMD) and Obesity. The results were as follows. 1. In distribution of age, the peak bone density of lumbar spine was noted around 30 years, and the peak bone density of the femoral neck was noted around 20 years. The age related loss of bone density follows soon after peak density. And the signifficant difference was revealed between lumbar spine and femoral neck bone density (p<0.001) 2. In distribution of sex, the bone density in male was signifficantly higher than in female (p<0.001). 3. In the correlation between Bone Mineral Density (BMD) and Obesity, Bone Mineral Density (BMD) in obese group was signifficantly higher than in non-obese group. Especially, in female from 50 up to 69 years, BMD had a positive correlation with Body mass index(BMI).

Clinical Importance of Peak Cough Flow in Dysphagia Evaluation of Patients Diagnosed With Ischemic Stroke

  • Objective To investigate the relationship between peak cough flow (PCF), pulmonary function tests (PFT), and severity of dysphagia in patients with ischemic stroke. Methods This study included patients diagnosed with ischemic stroke, who underwent videofluoroscopic swallowing study (VFSS), PCF and PFT from March 2016 to February 2017. The dysphagia severity was assessed using the videofluoroscopic dysphagia scale (VDS). Correlation analysis of VDS, PFT and PCF was performed. Patients were divided into three groups based on VDS score. One-way ANOVA of VDS was performed to analyze PCF, forced vital capacity (FVC), forced expiratory volume in one second (FEV1), and age among the different groups. Results The correlation coefficients of VDS and PCF, VDS and FVC, and VDS and FEV1 were -0.836, -0.508, and -0.430, respectively, all of which were statistically significant at the level of p<0.001. The one-way ANOVA indicated statistically significant differences in PCF, FVC, FEV1, and age among the VDS groups. Statistically significant differences in VDS and age were observed between aspiration pneumoia and non-aspiration pneumonia groups. Conclusion Coughing is a useful factor in evaluating the risk of aspiration in dysphagia patients. Evaluation of respiratory and coughing function should be conducted during the swallowing assessment of patients with ischemic stroke.

Changes in Liver Enzymes and Metabolic Profile in Adolescents with Fatty Liver following Exercise Interventions

  • Purpose: Nonalcoholic fatty liver disease (NAFLD) is the most frequent cause of chronic liver diseases in both adults and children with obesity. The aim of this study was to compare the changes in liver enzymes and metabolic profile in adolescents with fatty liver following selected school-based exercise (SBE) and high-intensity interval training (HIIT) interventions. Methods: In a semi-experimental study, 34 obese male adolescents with clinically defined NAFLD were divided into the HIIT (n=11, age=12.81±1.02 years, body mass index [BMI]=26.68±2.32 kg/㎡), selected SBE (n=11, age=13.39±0.95 years, BMI=26.47±1.74 kg/㎡), and control (n=12, age=13.14±1.49 years, BMI=26.45±2.21 kg/㎡) groups. The ultrasonography NAFLD grade, peak oxygen uptake (VO2peak), lipid profile, insulin resistance, and alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels of the participants were measured before and after the exercise interventions. Results: The BMI, waist-to-hip ratio, and body fat percentage of the participants decreased, and a significant increase in VO2peak was observed after the intervention; however, the HIIT group showed a significant improvement compared with the SBE group (p<0.01). Significant reductions were observed in the levels of insulin resistance, triglyceride, total cholesterol, ALT, and AST in both groups, although high-density lipoprotein levels decreased only in the HIIT group (p<0.01). Further, a significant reduction in low-density lipoprotein level was observed in the training groups (p<0.01), but this decrease was not significant compared with the control group (p>0.01). Conclusion: HIIT and SBE are equally effective in improving health parameters in obese children and adolescents.

Analysis of Left Ventricular Functional Parameters in Normal Korean Subjects by ECG Gated Blood Pool Scan (정상 한국인에서 게이트혈액풀스캔을 이용한 좌심실 심기능지표들의 분석)

  • Background : The demand for refinement in noninvasive and quantitative assessment of left ventricular (LV) function is increasing. Purpose : To assess normal values of left ventricular functional parameters during both systole and diastole by scintigraphic method using computerized triple-head gamma camera and to evaluate correlations between these parameters. Methods : ECG gated blood pool scan with $^{99m}Tc$-Human serum albumin was performed in 94 normal Korean subjects. Ejection fraction (EF), systolic parameters [peak emptying rate (PER), average emptying rate (AER), time to peak emptying rate (TPER)], and diastolic parameters [peak filling rate (PFR), average filling rate (AFR), time to peak filling rate (TPFR)] were obtained by analysis of LV time-activity curve, the correlation of these parameters to the age and sex, and the correlation between these parameters were evaluated. Results : 1) Mean value of ejection fraction in study subjects was $59.6{\pm}5.25%$ and showed no significant correlation to age (r=-0.08) and sex but showed most pronounced correlation to PFR (r=0.46, p<0.001), PER (r=0.41, p<0.001), AFR (r=0.34, p<0.001) and AER (r=0.28, p<0.01). 2) Mean values of systolic parameters were as follows: $PER=3.22{\pm}0.50$ end-diastolic volume/sec, $AER=2.22{\pm}0.45$ end-diastolic volume/sec, $TPER=103.5{\pm}29.30$ msec. They showed no significant correlation to age and sex. 3) Mean values of diastolic parameters were as follows: $PFR=2.71{\pm}0.51$ end-diastolic volume/sec, $AFR=1.83{\pm}0.44$ end-diastolic volume/sec, $TPFR=132.1{\pm}33.45$ msec. They showed strong correlation to age (r=0.70, -0.64, 0.37, p<0.001). Conclusions : Left ventricular functional parameters in normal Korean subjects were obtained reliably by computerized scintigraphic method and may be applied to the evaluation of cardiac function in diseased patients.

Effect of Added Mass between Male and Female on The Lower Extremity Joints Angular Velocity, Moment, Absorb Energy During Drop Landing (착지 시 외부 무게 부하에 따른 남성과 여성의 하지 관절 각속도, 모멘트, 에너지 흡수에 미치는 영향)

  • This study aimed to analyze the effects of external load between male and female on angular velocity, moment, and absorbed energy of the lower-extremity joints during drop landing. The study subjects were 9 male($mass=70.82{\pm}4.64kg$, $height=1.71{\pm}0.04m$, $age=24.5{\pm}1.84years$), 9 female($mass=50.14{\pm}4.09kg$, $height=1.61{\pm}0.03m$, $age=23.6{\pm}2.62years$), without any serious musculoskeletal, coordination, balance, or joint/ligament problems for 1 year before the study. The angular velocity, flexion/extension and abduction/adduction moments, and absorbed energy of the lower-extremity joints were compared between the men and women during drop landing under 4 different conditions of external load(0%, 8%, 16%, and 24%) by using two-way repeated ANOVA(p < .05). The women landed with a greater peak angular velocity of the ankle joint, greater peak inversion moment, and lower peak hip-extension moment than the men did, under all 4 conditions. Additionally, the landing characteristics of the women were distinct from those of the men; the women showed a greater peak knee-adduction moment and greater absorbed energy of the knee joint. These differences indicate that anterior cruciate ligament(ACL) strain was greater in the women than in the men and therefore, women may be at a higher potential risk for noncontact injuries of the ACL with an increase in external load.

Effects of 12 Week Regular Aerobic Exercise on ST-segment and QTc Interval in Type 2 Diabetes Mellitus Patients (12주 규칙적인 유산소 운동이 제 2형 당뇨환자의 ST 분절과 QTc 연장에 미치는 영향)

  • The purpose of the present study was to examine effect of 12 week regular aerobic exercise on ST-segment and QTc interval in middle age type 2 diabetes mellitus (T2DM) patients. The subjects consist of 13 type 2 diabetes mellitus (T2DM) patients in middle age men and all of them had no other complications. Subjects participated in aerobic exercise training for 12 weeks. They started to exercise for $20{\sim}60$ min at $60{\sim}80%$ of $HR_{max}$, (exercise intensity has been increased gradually) per day, $3{\sim}5$ times a week. The results were compared before and after. Weight and BMI, % body fat, fasting glucose, HOMA-IR, $_{peak}DBP$ were significantly decreased and $_{peak}HR$, $_{peak}VO_2$, exercisre time were significantly increased after 12 week aerobic exercise. Also, QTc interval and ST-segment were significantly decreased during at rest, peak exercise after 12 week aerobic exercise. Conclusionally, 12 week aerobic exercise may be improvement in decreased cardiovascular mortality factors (ST-segment) and abnormal autonomic dysfunction (QTc interval) and potentially increased exercise capacity.

Comparison of the Isokinetic Strength of the Knee and Ankle and Isometric Strength of the Lumbar Extensor in Female Collegiate Dancers and Controls (무용전공 여대생들과 일반 여대생들의 무릎과 발목 등속성 최대우력과 허리폄근 등척성 최대우력 비교)

  • In dancers, intact muscular coordination is a well balanced antagonist, which could be a decisive factor in protection against injury as dancers often have hypermobile joints and their ankle joints often bear their full body weight in extreme positions. The purposes of this study were to identify the isokinetic strength to the knee and ankle and the isometric strength of the trunk in female collegiate dancers and controls. Furthermore, the study aimed to investigate the peak torque ratio of knee extension to flexion, ankle plantarflexion (PF) to dorsiflexion (DF), and dominant legs to nondominant. Twenty-one female collegiate dancers (20.0 years of age) and twenty-one female collegiate students (19.3 years of age) performed isokinetic maximum efforts of the knee extensors and flexors at $60^{\circ}/sec$ and $120^{\circ}/sec$, the ankle plantarflexors and dorsiflexors at $30^{\circ}/sec$ and $120^{\circ}/sec$ and isometric maximum efforts of the lumbar extensors at $0^{\circ}$, $12^{\circ}$, $24^{\circ}$, $36^{\circ}$, $48^{\circ}$, $60^{\circ}$, and $72^{\circ}$. The results were as follows: The isokinetic peak torque of the knee extensors and the ratio of knee extensors to flexors of dancers were significantly higher than those of controls (p<.01). However, the isometric peak torque of the back extensors (p<.01) and isokinetic peak torque of the ankle plantarflexors and dorsiflexors (p<.05) of dancers were significantly lower than those of controls. Further studies are needed to identify the difference in proprioception of the joints between dancers and controls.

The Influence of Maximal Aerobic Capacity on the Two Years Cardiac Related Re-Hospitalization in Patients with Heart Failure with Reduced Ejection Fraction in Korean Society

  • Purpose: This study examined the influence of the maximal aerobic capacity on the two-year cardiac-related re-hospitalization in patients with heart failure with a reduced ejection fraction (HFrEF) in Korean society. Methods: The maximal aerobic capacity of the study population (n=95, male 63%) was evaluated using a cardiopulmonary exercise (CPX) testing system. Each patient was followed up for two years to divide the HFrEF patients into two groups according to cardiac-related re-hospitalization: re-hospitalization (RH) group (n=29, 30%) and no re-hospitalization (NRH) group (n=66, 70%). Results: The relative peak $VO_2$ (mL/kg/min, p<0.001), exercise duration (p<0.001), respiratory exchange ratio ($VCO_2/VO_2$, p=0.001), systolic blood pressure (SBP) reserve (p=0.004), heart rate (HR) reserve (p=0.007), SBP max (p=0.02), and HR max (p=0.039) were significantly lower in the RH group than the NRH group during the CPX test. On the other hand, the ventilatory efficiency (VE/VCO2 slope, p=0.02) and age (p=0.022) were significantly higher in the RH group than in the NRH group. In binary logistic regression analysis, the relative peak $VO_2$ (p=0.001, Wald Chi-square 10.137) was the strongest predictive factor on cardiac-related re-hospitalization, which was followed by $VCO_2/VO_2$ (p=0.019, Wald Chi-square 5.54). On the other hand, age (p=0.063, Wald Chi-square 3.445) did not have a significant influence on cardiac related re-hospitalization. Conclusion: The maximal aerobic capacity, especially the relative peak $VO_2$, is the strongest factor on cardiac-related re-hospitalization within two years in patients with HFrEF in Korean society.

Which Factors Related to the Renal Cortical Defects in Infants Under 3 Months of Age with Urinary Tract Infections?

  • Purpose: We used technetium-99m dimercaptosuccinic acid (DMSA) scintigraphy to identify factors predictive of renal cortical defects in infants <3 months of age with urinary tract infections (UTIs). Methods: We retrospectively reviewed data on infants <3 months of age with culture-proven UTIs treated at a single center from March 2010 to February 2016. Blood samples were obtained for laboratory evaluation prior to commencement of antibiotic therapy. The therapeutic delay time (TDT) and therapeutic response time (TRT) were recorded. All patients were divided into two groups depending on features of their DMSA scans. We compared the demographic, clinical, and laboratory characteristics of the two groups. Results: A total of 119 infants (94 males and 25 females; mean age, $56.9{\pm}21.3days$) were included. Cortical defects were evident in the DMSA scans of 47 cases (39.5%). In infants with such defects, the peak temperatures ($38.9{\pm}0.57^{\circ}C$ vs. $38.4{\pm}0.81^{\circ}C$, P=0.001), the absolute neutrophil counts ($8,920{\pm}4,460/mm$ vs. $7,290{\pm}4,090/mm$, P=0.043), and the C-reactive protein (CRP) levels ($6.49{\pm}4.33mg/dL$ vs. $3.21{\pm}2.81mg/dL$, P=0.001) were significantly higher than those in infants without cortical defects. The TDT was also longer in those with cortical defects (P=0.037). Conclusion: We found that a TDT ${\geq}8.5hr$ (odds ratio [OR] 5.81), a peak temperature ${\geq}38.3^{\circ}C$ (OR 6.19), and a CRP level ${\geq}4.96mg/dL$ (OR 7.26) predicted abnormal DMSA scan results in infants <3 months of age with UTIs.
