The Effects of Snail Secretion Filtrate on the damaged skin barrier's recovery of the Atopic dermatitis (달팽이 점액이 아토피 피부염 환자의 피부장벽 회복에 미치는 영향)

  • Purposes : The object of present study is to detect the Effects of Snail Secretion Filtrate and Hyaluronic acid on the skin barrier's recovery of the Atopic dermatitis. Methods : A total of 20 patients who visited Semyung Hanny Oriental Medical Center from september 1st, 2009 to August 31th, 2010 were included in this study. In this study, they were treated with Snail Secretion Filtrate(experimental group) and Hyaluronic acid(control group). For 4 weeks gross examination, hematological examination and instrumentation through skin-ANBT equipment were made before and after the experiments to see how well the products for experimental group act against those for control group in recovering the damaged skin barriers by Atopic dermatitis. Results : 1. In the primary endpoint, SCORAD Index showed a statistically significant decline in both the control group and the experimental group. However, the experimental group showed greater statistical significance than the control group. 2. In the secondary endpoint index of skin hydration, both the control group and the experimental group did not show a statistically significant increase. However, the degree of skin hydration in the experimental group is greater than in the control group. 3. In global assessment of efficacy, it was higher in the experimental group than in the control group for both the subjects and the researchers. 4. To evaluate the safety of the products for the human body, hematological examination and hematological biochemical examination were conducted; both the control group and the experimental group showed no abnormal level. Therefore, the safety of the products, if used for so long a time, proved to be safe for the human body. 5. Product satisfaction was higher in the experimental group than in the control group. Conclusions : According to above experiments, Snail Secretion Filtrate was effective on the Atopic dermatitis.

Do Knots Matter in Superior Labrum Anterior to Posterior Lesions Repair?

  • Background: In general, the outcomes of arthroscopic repair for superior labrum anterior to posterior lesions (SLAP) are favorable, however, persistent pain and limitation of motion are not rare complications. One of the possible cause is a "knot-ache". This study evaluated the results of reoperation of symptomatic recurrent SLAP lesions and asked whether the knot is associated with postoperative complications. Methods: Between 2005 and 2015, a total of 11 patients who had undergone arthroscopic SLAP repair were reoperated for recurrent symptomatic SLAP lesion. By retrospective chart review, operative findings, the visual analogue scale for pain (pVAS), the range of motion (ROM), and functional scores were analyzed. Results: The mean age of the study participants was 38.3 years, and the mean follow-up period was 42.5 months. In the primary operation, there were nine cases of repairs with conventional knot-tying anchors and three cases with knotless anchors. Impingement of the knots during abduction and external rotation of the shoulder was observed in the all cases with knot-tying anchors. The mean pVAS, ROM, and functional scores significantly improved with reoperation. At the final follow-up, the mean satisfaction VAS was 8.3. Conclusions: The knots of suture anchor maybe a possible etiology of the pain, which we termed a "knot-ache". Considering that reoperation is performed due to pain after primary repair, the use of knotless suture anchor may have benefits of eliminating one of possible cause, "knot-ache". Therefore, authors suggest the use of knotless anchors during reoperation for recurrent or recalcitrant pain after primary SLAP repair.

Clinical Experience in Treatment of Diabetic Foot Ulcers Using Platelet Concentrates from Blood Bank (혈액은행 제조의 혈소판 농축액을 이용한 당뇨족부궤양의 치료 경험)

  • Many clinical trials have shown the effectiveness of the platelet releasate or the platelet gel on chronic wounds. However, the patient's own blood had to be aspirated and processed to make the platelet releasate or a platelet gel. The purpose of this study was to assess the effects of platelet concentrates from the blood bank for the treatment of diabetic foot ulcers. To obtain the basic data of the PDGF-BB content in platelet concentrates supplied from the blood bank, enzyme-linked immunosorbent assay quantification was performed. On average, 8.5 pg of the PDGF-BB was released per 1 million platelets. Sixteen patients with diabetic foot ulcers ranging from 1.0 to $18.0cm^2$(mean, $6.1cm^2$) in size were treated. The platelet concentrates was centrifuged and the precipitantte was mixed with 1 ml of fibrinogen. The platelets and fibrinogen mixture was dispersed on to the ulcer lesions. The liquid platelet and fibrinogen mixture was then sealed using 0.3-1.0 ml of thrombin and moisture dressing was performed. The procedure was repeated every one or two weeks until wound closure. Time required for complete healing ranged from 3 to 12 weeks after treatment (mean, 7.3 weeks). Patient satisfaction was also very positive. In this study, the use of platelet concentrates from the blood bank was found to be effective in treating diabetic foot ulcers.

Surgical Treatment of Funnel Chest (누두흉에 대한 외과적 치험)

  • Background: Funnel chest is one of the most common anomaly of chest wall, which is manifested by depression of sternum and costal cartilage. Popular operative methods were Ravitch operation and Wada operation. Material and Method: From 1983 to 1996, 21 cases of funnel chest were corrected surgically in the department of thoracic surgery, National Medical Center. Investigated age and sex distribution, combined anomaly,clinical symptom, degree of correction and complication, postoperative satisfaction. We used 2 different surgical methods, one was Wada & its variants(17 cases), the other was Ravitch and it variants(4 cases). Most of operative indications were cosmetic problems. Result: The pre-operative Welch index was 4.188, but this index decreased to 3.46 after the operations.(p=0.046) The degree of correction was higher in Wada & it variant operation than the modified Ravitch operation.(p=0.54) Their results were satisfactory in 20 patients, while unsatisfactory in 1 patient because of a k-wire fracture. There was no recurrence of chest wall depression or postoperative death during the OPD follow up period. Conclusion: We recommend Wada operation in symmetric and small degree of depressive chest wall deformity in preand post school age.

  • This study was developed as a mobile serious game app (Roly Poly 160 App) based on a prototype model and then used in a public health center for diabetes patients. The mobile serious game application upgraded the mobile version of Roly Poly 160, which is a content for diabetes self-management of the existing PC version, by requesting to charge service of G city. The study period was from February 19, 2018 to May 11, 2018, and users were allowed to access the Roly Poly 160 app at least once a day for a total of 8 to 12 weeks after the preliminary investigation. As a result, HbA1C was significantly decreased(p=0.04), and diabetic knowledge was significantly higher than that before the follow-up(p=0.01). After applying Roly Poly 160 App, Satisfaction score was 3.74 out of 5 points

  • Background: Since the breast is strongly relevant to sexual desire, and physical and sexual attractiveness, the high prevalence of breast cancer (BC) in Iran and long-term survival of patients experiencing side effects means that measures to identify associated sexual problems are necessary. Therefore, this study was conducted to assess sexual function and affecting factors in women with BC. Materials and Methods: This cross-sectional study was performed on 94 women with BC, referred to Imam Reza (AS) Hospital, Mashhad, Iran, in 2014. The data were collected through demographic and clinical questionnaires and also a sexual function questionnaire and analyzed using SPSS version 16. Results: The total score of women's sexual function was about $24.3{\pm}4.41$. Of the total, 63 (71.3%) reported sexual dysfunction, for example reduced satisfaction or more pain. Age was the only significantly related factor. Conclusions: Breast cancer can adversely affect women's sexual function and decrease quality of life. Thus, taking measures to overcome women's sexual problems are necessary.

Treatment of the Septic Shoulder after Arthroscopic Rotator Cuff Repair in Diabetes Mellitus - A Case Report - (당뇨병 환자에서 관절경적 회전근 개 봉합술 후 발생한 화농성 견관절염의 치료 - 증례 보고 -)

  • Two diabetes mellitus patients treated by arthroscopy and associated procedure for an infection after arthroscopic rotator cuff repair were involved. The time interval from rotator cuff repair to symptom development was 18 days in average and arthroscopic debridement and associated procedure for infection applied in average 65 days after symptom development. Patient evaluation was done according to the KSS, ASES, UCLA and Constant Score. We used antibiotics for average 22.5 days after arthroscopic debridement and associated procedure, the infection was treated in average 4 months. At final follow-up, the mean KSS score was 82 points, the mean UCLA score was 33 points, the mean ASES score was 91 points, the mean Constant score was 71 points. All infections following arthroscopic rotator cuff repair were cured by arthroscopy and associated procedure. ROM and functional results were much improved, the pain and satisfaction were also much improved.

Remifentanil-Propofol Sedation as an Ambulatory Anesthesia for Carpal Tunnel Release

  • Objective : This prospective study evaluated the use of continuous sedation using propofol and remifentanil when carpal tunnel release (CTR) was performed under local anesthesia. Methods : We sedated 60 patients undergoing CTR using local anesthesia with remifentanil at loading and continuous doses of $0.5\;{\mu}g\;kg^{-1}$ and $0.05\;{\mu}g\;kg^{-1}min^{-1}$, respectively, and propofol, using a target controlled infusion (TCI) pump set to a target of $2\;{\mu}g\;mL^{-1}$ (group A), or with the same drug doses except that the continuous remifentanil dose was $0.07\;{\mu}g\;kg^{-1}min^{-1}$ (group B) or $0.1\;{\mu}g\;kg^{-1}min^{-1}$ (group C). Results : In group B, the levels of pain when local anesthetics were administered (p = 0.001), intraoperative pain (p < 0.001) and anxiety (p = 0.001) were significantly lower than those of group A. Furthermore, the incidence of adverse events, including desaturation (p < 0.001) and vomiting (p = 0.043), was significantly lower in group B than in group C. Conclusion : Continuous sedation using an appropriate dose of remifentanil and propofol can be used as safe, efficacious ambulatory anesthesia in cases of CTR under local anesthesia, performed using only 2 mL of local anesthetic, with a high degree of patient satisfaction.

Results of Operative Treatment for Large Osteochondral Lesion of Medial Talar Dome (내측 거골 체에 발생한 비교적 큰 골연골 병변에 대한 수술적 치료 결과)

  • Purpose: The purpose of this study is to assess the results of the autologous osteochondral grafting harvested from medial side of talus for relatively large osteochondral lesion of the medial talar dome. Materials and Methods: From October 2004 to September 2005, 12 patients with osteochondral lesion measured more than 10 mm in axial MRI who were followed up more than 1 year after operation were analyzed. We evaluated postoperative symptoms by Mann and Reynolds scale, morbidity of donor site, and compared the range of both ankle motion. We also evaluated the union at the medial malleolar osteotomy site, trabecular connection between the grafted osteochondral mass and talus, irregularity of the articular surface in lesion. Results: Clinical results were rated as excellent in 4, good in 7, fair in 1. The mean angle of the total range of motion in affected ankle was decreased by 3 degrees compared to that in unaffected ankle. We did not observe abnormal findings at donor site. The osteotomized bone was united at mean 9 weeks (range, 8-12 weeks). We observed trabecular connection between grafted osteochondral mass and talus at mean 14 weeks (range, 12-16 weeks). We also observed irregular articular surface in osteochondral lesions in 6, smooth articular surface in 6. Conclusion: The local autologous osteochondral graft for relatively large osteochondral lesion of the medial talar dome is useful operative method with advantages of wide operative field, low morbidity of donor site, and high satisfaction rate.

Iatrogenic Hallux varus deformity after Hallux valgus surgery (무지 외반증 수술 후 발생한 의인성 무지 내반증)

  • Purpose: To evaluate and analyze the incidence, clinical features, cause and surgical outcomes of iatrogenic hallux varus deformity after hallux valgus surgery. Materials and Methods: Twenty-six Hallux varus deformities after hallux valgus surgery were evaluated. Clinical tolerability, patient's satisfaction and the main causative factor for varus deformity were evaluated. Radiologically, we measured the 1st intermetatarsal angle and hallux valgus angle on pre- & postoperatively. Results: 10 cases of 26 varus deformities were clinically intolerable. The patients complaint of mainly cosmetic and shoe fitting problems rather than pain and the main cause of deformities were over-correction of 1 st intermetatarsal angle. Radiologically, the average 1st intermetatarsal angle was 2.4 degrees and the hallux valgus angle was -9.2 degrees. After varus correction surgery, the average follow up were 17 months and the average 1st intermetatarsal angle was 2.3 degrees and the hallux valgus angle was 2.7 degrees. The average score of AOFAS Hallux Metatarsophalangeal -Interphalangeal Scale was 91 points. Conclusion: The hallux varus deformity after hallux valgus surgery came from mainly overcorrection of 1 st intermetatarsal angle. The management composed of just observation, tendon transfer and fusion, and each method could get satifactory results with appropriate indication.
