Decision and Practice of End-of-Life Care in Lung Disease Patients with Physicians Orders for Life Sustaining Treatment

  • Purpose: The purpose of this study was to analyze end-of-life care practices in lung disease patients with physician orders for life-sustaining treatment (POLSTs). Methods: We retrospectively analyzed data from medical records regarding the end-of-life care practices of POLST decisions for patients with lung disease hospitalized at a tertiary hospital in Seoul, South Korea. Data were collected from January 1 to June 30, 2021. Results: Of 300 total patients, 198 had lung cancer (66.0%) and 102 had non-malignant lung diseases (34.0%). A POLST was written for 187 patients (62.3%), and an advance directive was written for 20 patients (6.7%). Subsequent treatments were hemodialysis in 13 patients (4.3%), surgery in 3 patients (1.0%), and cardiopulmonary cerebral resuscitation in 1 patient (0.3%). Among cancer patients, chemotherapy was performed in 11 patients (3.7%), targeted therapy in 11 patients (3.7%), immunotherapy in 6 patients (2.0%), and radiation therapy in 13 patients (4.3%). Depending on the type of lung disease, types of treatment differed, including hemodialysis, ventilators, bilevel positive airway pressure, high-flow nasal cannulas, nebulizers, enteral nutrition, central line, inotropic agents, and opioids. Conclusion: Although the goals of hospice care are the same whether a patient has lung cancer or a non-malignant lung disease, because the characteristics of the respective diseases differ, end-of-life care practices and hospice approaches must be considered differently.

  • Purpose: The coronavirus disease 2019 (COVID-19) pandemic has brought the world to a standstill and has exposed the lack of preparedness of most nations' health care systems. Even in usual times, palliative care has not received its fair share of recognition as an important component of patient care; instead, the emphasis is often placed on aggressive patient management. Now, with the entire medical community and decision-making committees focussed on intensive patient care, end-of-life care has taken a backseat. Methods: This article is a brief communication. Results: COVID 19 infection has been shown to lead to greater mortality and morbidity in patients with pre-existing illnesses such as hypertension, diabetes, renal failure, and cancer. Patients typically in need of end-of-life care, such as those with late-stage cancer or heart failure, are therefore at a higher risk of both contracting COVID-19 and suffering a more severe disease course. The strict nationwide lockdowns being imposed in most countries have deterred patients from seeking medical attention or hospice care. Every day new research is coming to light regarding COVID 19. This has helped significantly in creating awareness and limiting the spread of disease. However, misinformation is also rampant, leading to discrimination and mistreatment of infected patients. Conclusion: This pandemic has been a terrifying ordeal for all and has exposed our entire population physically, psychologically, emotionally, and financially to unimaginable stresses. In the present scenario, EOL care is as much a necessity as intensive care and should be given at least a fraction of its importance.

말기 암 환자에서 수행능력에 따른 삶의 질 비교 (Comparison of Quality of Life due to Performance Status in Terminal Cancer Patients)

  • 목적: 의학의 발달에도 불구하고 암 환자의 발생과 더불어 사망률은 점점 더 증가하고 있는 실정이다. 그러므로 암의 완치도 중요하지만 환자의 남은 삶에 대한 가치와 의미는 더욱더 중요하게 대두되고 있다. 이에 저자 등은 본 연구를 통해 말기 암 환자들이 수행능력에 따라 삶의 짚 신체적, 정신적 증상에 어떤 차이가 있는지를 보고자 하였다. 방법: 2003년 9월 1일부터 2005년 8월 31일까지 관동의대 명지병원 가정의학과에 내원한 암 환자를 대상으로 수행능력 상태, 인구통계학적 자료, 혈액검사, 삶의 질을 평가하였다. 수행능력을 두 군으로 나누어 혈액검사, 삶의 질에 차이가 있는지를 ANOVA를 통해서 분석하였다. P값이 0.05이하일 때 의미 있는 것으로 하였다. 결과: 대상이 된 환자는 총 104명이고, 수행능력 상태가 $0{\sim}1$인 경우 71명(남자 23명, 여자 48명), $2{\sim}4$인 경우 33명(남자 8명, 여자 25명)이었다. 혈액 검사를 보면 수행능력 상태가 $2{\sim}3$기일수록 림프구, 헤모글로빈, 단백질, 알부민, 나트륨 수치가 의미 있게 낮았다. 삶의 질을 평가 해 보면 수행능력 상태가 $2{\sim}4$일수록 전반적인 건강 상태(P=0.02)가 나빴다. 또한 수행능력 상태가 $2{\sim}4$일수록 기능적인 측면에서는 신체적 점수(P=0.05)와 역할 점수(P=0.01)가 나빴고, 증상적인 측면에서는 식욕부진(P=0.01)이 의미 있게 차이가 났다. 결론: 수행능력이 떨어지면 혈액검사 상 일부 영양과 관련된 수치가 떨어지고, 전반적인 건강상태가 나빠진다. 그러나 증상에서는 식욕부진을 제외한 나머지 것에는 차이가 없게 나왔다. 결론적으로 수행능력이 떨어진 환자에서 식욕을 증진시키는 것이 가장 중요하다고 생각한다.

호스피스 병동 입원 환자의 삶의 질 변화 (Quality of Life Changes in Patients Admitted to the Hospice Unit)

  • 목적 : 본 연구는 강남성모병원 호스피스 병동에 입원한 환자들의 입원 시 삶의 질과 간호서비스를 받는 동안 삶의 질 변화와 호스피스 간호서비스가 말기환자의 삶의 질에 미치는 영향을 알아보고자 시도하였다. 방법 : 1999년 10월부터 2000년 3월 사이에 가톨릭 대학교 강남성모병원 호스피스 병동에 입원한 환자와 가족 100명을 대상으로 연구자가 수정, 보완하여 개발한 설문지를 통해 자료를 수집하였고, 수집된 자료는 repeated measures ANOVA로 분석하였다. 결과 : 1) 간호제공자에 의해 인지된 환자의 삶의 질 평점은 입원시, 입원 1, 2, 3, 4주에 각각 3.31, 3.68, 3.56, 3.73, 3.75로 입원 당시보다 시간이 지남에 따라 유의하게 향상되었고, 입원시점에 따라 항목별로 살펴 볼 때 "신체적 청결"(F=6.50, P=0.0001) "통증조절"(F=18.01., P=0.0001) "대변상태"(F=2.96, P=0.0237) "수면상태"(F=3.99, P=0.0048) "메스꺼움/구토"(F=4.50, P=0.0022) "의료진의 편안한 돌봄"(F=3.95, P=0.0051) "가족들의 돌봄"(F=2.76, P=0.0317) "불안감"(F=3.14, P=0.0177) "마음이 편안함"(F=3.63, P=0.0085) "인간적인 대우"(F=3.32, P=0.0136) "죽음이 끝이 아니라 새로운 시작이라고 생각함"(F=2.54, P=0.0450) 등의 항목에서 유의한 차이가 있었다. 2) 환자 자신에 의해 인지된 삶의 질 평점은 입원 시와 입원 1주에 각각 3.63, 3.83으로 향상되었지만 유의한 차이는 보이지 않았다. 입원시점에 따라 항목별로 살펴볼 때 "통증조절" 항목만이 유의한 변화를 보였다. 3) 사망시점으로부터 삶의 질 평점은 사망 5주전, 4주전, 3주전, 2주전, 1주전에 각각 3.48, 3.51, 3.44, 3.46, 3.50이었으며 유의한 차이는 없었다. 결론 : 본 연구의 결과 호스피스 서비스는 말기 환자의 삶의 질에 긍정적인 영향을 주는 것으로 보여졌다. 따라서 호스피스 서비스의 질 향상을 위해 호스피스 환자들을 지지해 줄 중재방안을 모색해야 하며, 특히 간호의 영적 영역에 초점을 두어야 하겠다. 또한 말기 환자의 주관적인 삶의 질 측정을 위해 도구개발이 이루어져야 하며 대상자를 확대하여 계속적인 연구가 있어야 할 것으로 생각된다.

호스피스간호가 말기암환자의 통증에 미치는 효과에 관한 연구 (The Study on the Effects of Hospice Care on the Pain Management of the Terminal Cancer Patients)

  • 목적 : 본 연구는 호스피스 간호가 말기암환자의 통증정도에 미치는 효과를 파악하고 분석함으로써, 말기암환자의 안위를 도모하여 전인적이고 개별화된 간호를 제공하는데 도움을 주기 위하여 시도된 단일군 전후설계(one group pre-post test)이다. 방법 : 2000년 7월부터 11월까지 전주시에 소재하는 1개 종합병원의 호스피스에 의뢰되어 연구참여에 동의한 말기암환자 37명을 대상으로 하였다. 호스피스 간호는 1주일에 $5{\sim}6$회를 조주 동안 제공하였으며, 자료분석은 실수, 백분율, 평균, 표준편차 등의 서술통계와 paired t-test로 분석하였다. 결과 : 1) 통증정도의 중증도는 통증정도가 24시간 동안 가장 심했을 때 통증점수는 중재전이 6.35점, 중재후가 4.76점이었다. 통증으로 인한 지장정도는 중재전에는 인생을 즐김(8.22점), 통상적인 일(7.46점), 보행(7.08점), 일반적인 활동(7.08점)순이었으며, 중재후에는 인생을 즐김(6.62점), 통상적인 일(6.43점), 보행(6.11점), 일반적인 활동(5.78점)순으로 중재전과 후가 동일한 순위를 보였다. 2) 통증의 중증도는 중재전보다 중재후에서 각각 유의하게 감소하였다. 영역별로 분석하였을 때 24시간 동안 가장 심했을 때 통증(t=4.085, P=0.000), 24시간 동안 가장 약했을 때 통증(t=4.020, P=0.000), 평균통증(t=4.254, P=0.000), 조사당시 바로 지금 느끼는 통증(t=5.017, P=0.000)영역에서 중재후 통증이 중재전보다 유의하게 감소하였다. 3) 통증으로 인한 지장정도는 중재전보다 중재후에서 각각 유의하게 감소하였다. 영역별로 분석하였을 때 일반적인 활동(t=3.137, P=0.003), 기분(t=6.713, P=0.000), 보행능력(t=2.027, P=0.050), 통상적인 일(t=20.132, P=0.040), 대인관계(t=4.143, P=0.000), 수면(t=4.071, P=0.000), 인생을 즐김(t=3.881, P=0.000)에서 통계적으로 유의한 차이가 나타났다. 결론 : 호스피스 간호를 제공받은 말기암환자는 제공받지 않은 말기암환자에 비해 통증정도와 삶의 치장 정도에서 모두 낮아짐이 확인되었으며, 유의한 차이를 보였다. 이상과 같이 말기암환자에 대한 호스피스 간호는 통증감소와 통증이 생활에 지장을 주는 정도를 감소하는데 효과적인 것으로 확인되었으므로 앞으로 말기암환자를 위해 적극적인 활용을 권장한다.

골전이로 인한 장골의 병적 골절의 수술적 치료 (Surgical Treatment of Pathologic Fracture of the Long Bone due to Metastatic Tumor)

  • Metastatic cancer is the most common tumor of the skeleton. The prevalence of pathologic fracture may increase as patient survival is prolonged by improved cancer therapy. With recent advances in orthopaedic procedure and medical management of terminal cancer patients, it is generally agreed that aggressive treatment should be undertaken for patient with pathologic fracture secondary to metastatic disease, and a team approach should be utilized. The authors have reviewed twenty cases of pathologic fracture of the long bone due to metastatic tumor treated in the Department of Orthopedic Surgery, Yonsei University College of Medicine, from April 1989 to April 1994 and the following results were obtained. 1. The mean age at surgery was 58.4 years (ranged from 24years to 86years) and among 20 cases, 10 cases were male and the others were female. 2. The most frequent site of pathologic fracture in long bone is femur(15 cases, 75%), and followed by humerus(4 cases, 20%), tibia(1 case). 3. The frequently encountered primary tumors that metastases to long bone are those of the lung(7 cases, 35%), breast(4 cases, 20%), and prostate(2 cases, 10%). 4. The operative procedure was performed by resection of the tumor mass extensively, and we used polymethylmetacrylate for filling the dead space after resection, in all cases. 5. The mean survival period after operation is 9.2 months(ranged from 1 month to 4 years and 9 month). 6. The results of postoperative pain relief status were graded as fair to excellent in 17 cases(85%).

암환자를 위한 호스피스 케어에 관한 탐색적 연구 (An Exploratory Study of Hospice Care to Patients with Advanced Cancer)

  • True nursing care means total nursing care which includes physical, emotional and spiritual care. The modern nursing care has tendency to focus toward physical care and needs attention toward emotional and spiritual care. The total nursing care is mandatory for patients with terminal cancer and for this purpose, hospice care became emerged. Hospice case originated from the place or shelter for the travellers to Jerusalem in medieval stage. However, the meaning of modem hospice care became changed to total nursing care for dying patients. Modern hospice care has been developed in England, and spreaded to U.S.A. and Canada for the patients with terminal cancer. Nowaday, it became a part of nursing care and the concept of hospice care extended to the palliative care of the cancer patients. Recently, it was introduced to Korea and received attention as model of total nursing care. This study was attempted to assess the efficacy of hospice care. The purpose of this study was to prove a difference in terms of physical, emotional a d spiritual aspect between the group who received hospice care and who didn't receive hospice care. The subject for this study were 113 patients with advanced cancer who were hospitalized in the S different hospitals. 67 patients received hospice care in 4 different hospitals, and 46 patients didn't receive hospice care in another 4 different hospitals. The method of this study was the questionaire which was made through the descriptive study. The descriptive study was made by individual contact with 102 patients cf advanced cancer for 9 months period. The measurement tool for questionaire was made by author through the descriptive study, and included the personal religious orientation obtained from chung(originated R. Fleck) and 5 emotional stages before dying from Kubler Ross. The content ol questionaire consisted in 67 items which included 11 for general characteristics, 10 for related condition with cancer, 13 for wishes far physical therapy, 13 for emotional reactions and 20 for personal religious orientation. Data for this study was collected from Aug. 25 to Oct. 6 by author and 4 other nurse's who received education and training by author for the collection of data. The collected data were ana lysed using descriptive statistics, $X^2-test$, t-test and pearson correlation coefficient. Results of the study were as follows: "H.C Group" means the group of patient with cancer who received hospice care. "Non H.C Group" means the group of patient with cancer who did not receive hospice care. 1. There is a difference between H.C Group and Non H.C Group in term of the number of physical symptoms, subjective degree of pain sensation and pain control, subjective beliefs in physical cure, emotional reaction, help of present emotional and spiritual care from other personal, needs of emotional and spiritual care in future, selection of treatment method by patients and personal religious orientation. 2. The comparison of H.C Group and Non H.C Group 1) There is no difference in wishes for physical therapy between two groups(p=.522). Among Non H.C Group, a group, who didn't receive traditional therapy and herb medicine was higher than a group who received these in degree of belief that the traditional therapy and herb medicine can cure their disease, and this result was higher in comparison to H.C Group(p=.025, p=.050). 2) Non H.C Group was higher than H.C Group in degree of emotional reaction(p=.050). H.C Group was higher than Non H.C Group in denial and acceptant stage among 5 different emotional stages before dying described by Kubler Ross, especially among the patient who had disease more than 13 months(p=.0069, p=.0198). 3) Non H.C Group was higher than H. C Group in demanding more emotional and spiritual care to doctor, nurse, family and pastor(p=. 010). 4) Non H.C Group was higher than H.C Group in demanding more emotional and spiritual care to each individual of doctor, nurse and family (p=.0110, p=.0029, P=. 0053). 5) H.C Group was higher th2.n Non H.C Group in degree of intrinsic behavior orientation and intrinsic belief orientation of personal religious orientation(p=.034, p=.026). 6) In H.C Group and Non H.C Group, the degree of emotional demanding of christians was significantly higher than non christians to doctor, nurse, family and pastor(p=. 000, p=.035). 7) In H.C Group there were significant positive correlations as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and: the degree of intrinsic behavior orientation in personal religious orientation(r=. 5512, p=.000). (2) Between the degree of emotional demandings to doctor, nurse. family & pastor and the degree of intrinsic belief orientation in personal religious orientation(r=.4795, p=.000). (3) Between the degree of intrinsic behavior orientation and the degree of intrinsic: belief orientation in personal religious orientation(r=.8986, p=.000). (4) Between the degree of extrinsic religious orientation and the degree of consensus religious orientation in personal religious orientation (r=. 2640, p=.015). In H.C. Group there were significant negative correlations as following; (1) Between the degree of intrinsic behavior orientation and extrinsic religious orientation in personal religious orientation (r=-.4218, p=.000). (2) Between the degree or intrinsic behavior orientation and consensus religious orientation in personal religious orientation(r=-. 4597, p=.000). (3) Between the degree of intrinsic belief orientations and the degree of extrinsic religious orientation in personal religious orientation(r=-.4388, p=.000). (4) Between the degree of intrinsic belief orientation and the degree of consensus religious orientation in personal religious orientation(r=-. 5424, p=.000). 8) In Non H.C Group there were significant positive correlation as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of intrinsic behavior orientation in personal religious orientation(r= .3566, p=.007). (2) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of intrinsic belief orientation in personal religious orientation(r=.3430, p=.010). (3) Between the degree of intrinsic behavior orientation and the degree of intrinsic belief orientation in personal religious orientation(r=.9723, p=.000). In Non H.C Group there were significant negative correlation as following; (1) Between the degree of emotional demandings to doctor, nurse, family & pastor and the degree of extrinsic religious orientation in personal religious orientation(r= -.2862, p=.027). (2) Between the degree of intrinsic behavior orientation and the degree of extrinsic religious orientation in personal religious orientation(r=-. 5083, p=.000). (3) Between the degree of intrinsic belief orientation and the degree of extrinsic religious orientation in personal religious orientation(r=-. 5013, p=.000). In conclusion above datas suggest that hospice care provide effective total nursing care for the patients with terminal cancer, and hospice care is mandatory in all medical institutions.

말기 암 환자에서 호중구-림프구 비가 예후인자로서 생존기간에 미치는 영향 (Neutrophil-Lymphocyte Ratio as a Prognostic Factor in Terminally Ill Cancer Patients)

  • 목적: 말기 암 환자에게 있어서 정확한 여명 예측은 환자의 효율적인 치료 계획을 세우고 환자의 삶의 질을 높이는데 있어서 중요하다. 본 연구에서는 말기 암 환자에서 호중구-림프구 비가 생존기간 예측을 위한 예후 인자로서 유용한가를 알아보고자 한다. 방법: 2004년 1월부터 2007년 6월까지 말기 암 환자로 완화 치료를 목적으로 영동세브란스병원 가정의학과에 입원 혹은 전입되어 치료를 받는 중 사망한 67명의 환자를 대상으로 하였다. 호중구-림프구 비에 따라서 3개의 군으로 나누어 과거 병력, 신체 계측, 임상 증상, 혈액검사 소견, 생존기간을 분석하였다. 결과: 호중구-림프구 비가 가장 높은 군(${\geq}12.5$)에서 환자의 생존기간이 단변량 분석에서 통계적으로 유의하게 짧았으며(hazard ratio (HR)=3.270, P=0.001)), 저하된 활동도, 호흡 곤란 증상을 보정한 다변량 분석에서도 통계적 유의성을 보였다(HR=2.907, P=0.007). 완화 치료를 위해 입원 혹은 전입된 시점에 비하여 사망이 임박한 시점에서 호중구-림프구 비는 의미 있는 증가를 보였다(P=0.001). 결론: 호중구-림프구 비는 말기 암환자에서 생존기간 예측을 위한 독립적인 예후 인자로 확인 되었다.

암환자의 퇴원후 계속 간호 프로그램 개발 (The Development of a Continuing Nursing Care Program for Cancer Patients after Discharge)

  • The purpose of this study was to develop a continuing nursing care program for cancer patients and the caregivers staying at their home and to test its effects on patients' symptom distress, caregiver burden, and satisfaction of life. The continuing nursing care program was based on the homecare needs of cancer patients, and was provided by three clinical nurses who took care of the patients while they were hospitalized. The program consisted of discharge education about selfcare using education materials(book and handout) and provisions of direct care. These were counselling and education during which visiting patients' home one week after discharge and telephone counseling two weeks after discharge. Counseling by telephone was always available during the research period. A quasi-experimental research design was used to test the effects of the program. Subjects for this study were 53 cancer patients discharged from the hospital and caregivers. These subjects were assigned to an experimental group (n=23) receiving continuous nursing care, or to a control group (n=30) not receiving continuous nursing care. Data from control group was collected first to protect from contamination. Data collection was done from October of 1998 to February of 2000. The collected data was analysed using mean, t-test, and chi-square test computed by SPSS software. The summary of results was as follows: 1) The symptom distress was a little decreased at posttest, but there were no significant differences between the experimental and the control group in symptom distress. 2) The score of caregiver burden was significantly decreased in experimental group at posttest, but no differences in control group. 3) There were no significant differences between the experimental and control group in the satisfaction of life. 4) The score of satisfaction of continuing nursing care program in experimental group at posttest was 2.321 of 3. In conclusion, even though this study did not obtain evidence of effectiveness of continuing nursing care program on patients, such as. It is still expected to be effective by a more improved program. Therefore we want to give some suggestions for further studies. 1) It is needed to make a communication channel with the patient's doctor to response promptly and appropriately to patient's conditions. 2) The research is necessary on patients in terminal stage or early stage of cancer diagnosis who have many nursing needs. 3) It is needed to readjust the roles and job assignment of clinical nurse to implement effectively as a program provider.

Epidural Infusion of Morphine and Levobupivacaine through a Subcutaneous Port for Cancer Pain Management

  • Background: To manage intractable cancer pain, an alternative to systemic analgesics is neuraxial analgesia. In long-term treatment, intrathecal administration could provide a more satisfactory pain relief with lower doses of analgesics and fewer side-effects than that of epidural administration. However, implantable drug delivery systems using intrathecal pumps in Korea are very expensive. Considering cost-effectiveness, we performed epidural analgesia as an alternative to intrathecal analgesia. Methods: We retrospectively investigated the efficacy, side effects, and complications of epidural morphine and local anesthetic administration through epidural catheters connected to a subcutaneous injection port in 29 Korean terminal cancer patients. Patient demographic data, the duration of epidural administration, preoperative numerical pain rating scales (NRS), side effects and complications related to the epidural catheterization and the drugs, and the numerical pain rating scales on the 1st, 3rd, 7th and 30th postoperative days were determined from the medical records. Results: The average score for the numerical pain rating scales for the 29 patients decreased from $7{\pm}1.0$ at baseline to $3.6{\pm}1.4$ on postoperative day 1 (P < 0.001). A similar decrease in pain intensity was maintained for 30 days (P < 0.001). Nausea and vomiting were the most frequently reported side effects of the epidural analgesia and two patients (6.9%) experienced paresthesia. Conclusions: Epidural morphine and local anesthetic infusion with a subcutaneous pump seems to have an acceptable risk-benefit ratio and allows a high degree of autonomy to patients with cancer pain.
