Use of Complementary and Alternative Medicine and its Affecting Factors in Women with Breast Cancer (유방암 여성의 대체요법 이용과 영향요인)

  • Purpose: This exploratory study was to identify the use of complementary and alternative medicine(CAM) in women with breast cancer and to analyze which factor influence CAM use. Methods: 114 patients with breast cancer were identified and approached. The instrument used to gather data were the common types, main reasons and effects as well as stress, mood states and fatigue. Results: 42.7% of all participants reported the use of CAM. The most common types of CAM used were Phellinus linteus, elm tree, and vitamin C. The main reasons for CAM use were to boost the immune system and to prevent recurrence. Prayer, massage, Shitake mushroom, and Ganoderma lucidum were, identified as being more effective than others. 93.6% of the respondents informed their physicians of their use of CAM. Logistic regression analysis determined that stress, mood states, and duration after diagnosis were factors significantly associated with CAM use. Conclusion: CAM use is increasing among patients with breast cancer. Health care professionals are in the main position to identify what treatments patients are using and implement CAM therapies that can be helpful to relieve patient symptoms related to treatment and psychological distress.

Intradural Involvement of Multicentric Myxoid Liposarcoma

  • Liposarcomas are malignant tumors of the soft tissue, with myxoid liposarcoma being the second most common subtype, tending to occur in the limbs, particularly in the thighs. Myxoid liposarcomas have an intermediate prognosis between well-differentiated and pleomorphic tumors. Spinal metastasis is usual but intradural involvement is extremely rare. We present an unusual case of a multicentric myxoid liposarcoma with intradural involvement. A 41-year-old woman complained of tingling sensation on her left arm. Radiological evaluation revealed multiple masses in her cervical spine, abdominal wall, liver, heart and right thigh, all of which were resected. She was histologically diagnosed with small round cell myxoid sarcoma and underwent adjuvant chemotherapy. However, magnetic resonance imaging analysis after 1 year revealed a large metastatic mass with bony invasion at the C6-T1 level. This mass consisted of extradural and intradural components causing severe compression of the spinal cord. She underwent resection via a posterior facetectomy of C6-7 and an anterior C7 corpectomy. However, the patient died of multiple metastases 18 months after the first diagnosis.

Robotized Percutaneous Biopsy (로봇을 이용한 경피 생체 검사)

  • Biopsy is a type of histopathological examinations, in which a physician samples cells or tissues from a patient's suspicious lesion. Such a lesion frequently resides deep inside human body, and then a percutaneous biopsy is therefore performed using a thick needle with the assistance of medical imaging such as computed tomography(CT) and magnetic resonance imaging(MRI). Recently modern robotic technology is being introduced to percutaneous biopsy in order to reduce any possible human error and hazard on physicians caused by medical imaging. After medical imaging locates the exact location of lesion, an optimization algorithm plans the path for a biopsy needle. Subsequently, a robot system moves the biopsy needle to the lesion in accurate and safe way with the control of a practitioner or automatically. In this article, we try to look into the state-of-art of percutaneous biopsy using such robotic technology. We classified percutaneous biopsy robots by mechanical characteristics and by imaging technology. Then, advantage and disadvantage of each class type are described as well as the basic description, and a few representative designs for each type are introduced. Current research issues of robotized percutaneous biopsy are subjectively selected for the readers' convenience. We emphasize the basic technology of actuator and sensors compatible with imaging technology to conclude this review.

Clinical Study on a Case of Cervical Myelopathy (경추증성 척수증 환자 1례에 대한 임상적 고찰)

  • Objective : Cervical myelopathy is one of the most severe disease, as well as a common diesaes. However, its clinical study and report about efficiency of oriental treatment are absolutely insufficient. Therefore, we report the treatment about 1 case of Cervical myelopathy like followings. Methods : From November 27th, 2002 to January 11th, 2003, we observed patient hospitalized because of cervical myelopathy, and for its valuation, JOA score(Japanese Orthopedic Association score), Nutric grading, VAS(visual analog scale), and D.I.T.I(Digital Infrared Thermal Imaging)were used. Results : After being hospotalized for 50 days, the patients' JOA score, Nuric grading system, and VAS were improved each from 7 to 13, from grade IV to grade II, and from 8 to 2. Conclusions : We considered that oriental treatment has an useful effect on cervical myelopathy patients' treatment and recovery. But continuous studies, such as comparative studies accompanied by more treatment case.

A Study of Nursing Students' Spiritual Care Experiences (간호 대학생의 영적 간호 경험 분석)

  • The purpose: This study was to describe the spiritual nursing care experiences of nursing students who were in a university, Seoul, Korea. Method: This study is based on descriptive analysis of case studies that were submitted by nursing students. Nursing students were 64 person who were sophomores in a university of the year 2000, 2001. Results: 1. The problems that students report for their subjects were: Loss of self-confidence, body image complex, health status problems, maladaptability, economic problems of home, guilty feeling, problems with dating, loneliness, uncertainty of their future, faith problem, accountability, stress and apart from intimate friends. 2. The methods which students use to solve the subjects's problems were: prayer with subjects, use the Scripture, conversation, meet frequently, listening, frequent phone call, explore problem solving methods together, write letters or E-mails, present with books or music CD etc. 3. After the students have experienced spiritual nursing care they feel that: satisfaction, lack of knowledge of spiritual care, understanding the importance of spiritual nursing care make a new resolution for others, understanding the importance of listening, understanding the power of prayer, the need of Bible study. Conclusion: Maintaining spiritual wellness is a important as maintaining physical fitness and essential for optimal well being. Therefore educating nursing students in developing and maintaining spiritual wellness is essential for the patient to achieve holism.

Nerve Block Anesthesia for Foot and Ankle Surgery (신경 차단 마취를 이용한 족부 및 족근 관절 수술)

  • We present our experience with the use of nerve block anesthesia in 212 of 484(43.8% ) surgical procedures of the foot and ankle between 1995 and 1997. Nerve block anesthesia was used for surgical procedures of the forefoot, midfoot, hindfoot, and ankle in the setting of elective surgery and trauma. From the viewpoint of the surgeon, nerve block anesthesia was completely successful in 99.5% of the procedures performed. We confirmed that 80% of patients were satisfied with the use of nerve block anesthesia for their operative procedure. There were only one(0.5%) miner complication being toxic neuritis of posterior tibial nerve. With increasing experience, the expanded indications and uses of nerve block anesthesia for foot and ankle surgery are proving to be simple, safe, reliable, and well tolerated by the patient.

A Case of Successful Management of Lung Cancer Pain Using Ultrahigh-dose Fentanyl Patch

  • A 55-year old woman with advanced stage non-small cell lung cancer was admitted to hospital for the management of severe chest pain, which measured 7 out of 10 on a numerical rating scale (NRS). Despite palliative radiation and the application of multiple epidural blocks, she continued to experience severe cancer pain. We gradually increased the dose of transdermal fentanyl patches from $500{\mu}g/hr$ to $3,650{\mu}g/hr$, for 3 months without any significant side effects. Concomitantly, adjuvant therapy with antidepressants and anticonvulsants were added, decreasing the patient's pain to NRS 3~4 down from 7. After being transferred to a hospice clinic, her chest pain was well-controlled below NRS 4 by means of strong opioid medications, including the highest dose of transdermal fentanyl $4,050{\mu}g/hr$ for more than 16 months.

  • Obstructive sleep apnea (OSA) in children is a frequent disease for which optimal diagnostic methods are still being defined. Treatment of OSA in children should include providing space, improving craniofacial growth, resolving all symptoms, and preventing the development of the disease in the adult years. Adenotonsillectomy (T&A) has been the treatment of choice and thought to solve young patient's OSA problem, which is not the case for most adults. Recent reports showed success rates that vary from 27.2% to 82.9%. Children snoring regularly generally have a narrow maxilla compared to children who do not snore. The impairment of nasal breathing with increased nasal resistance has a well-documented negative impact on early childhood maxilla-mandibular development, making the upper airway smaller and might lead to adult OSA. Surgery in young children should be performed as early as possible to prevent the resulting morphologic changes and neurobehavioral, cardiovascular, endocrine, and metabolic complications. Close postoperative follow-up to monitor for residual disease is equally important. As the proportion of obese children has been increasing recently, parents should be informed about the weight gain after T&A. Multidisciplinary evaluation of the anatomic abnormalities in children with OSA leads to better overall treatment outcome.

Life-Threatening Lower Gastrointestinal Hemorrhage in Pediatric Crohn's Disease

  • In Crohn's disease, mild gastrointestinal bleeding often occurs; however massive gastrointestinal hemorrhage, which can have a dramatic effect on a patient's vital sign, is rare. This could result in potentially life-threatening complications, which can lead to death. Massive hemorrhagic Crohn's disease is not well known and for this reason, they are a diagnostic and therapeutic challenge. Various diagnostic and therapeutic methods are currently being developed and used. The surgical method is often used only as a last measure since this approach has the risk of serious complications that may endanger patients. However, if massive bleeding continues even after all therapeutic methods are used, the surgical method must be implemented. In this case, all therapeutic methods were found to be ineffective; therefore, surgery was used as a last option. Ultimately, the surgical method was found to be successfully used to treat life-threatening hemorrhagic Crohn's disease.

A Concept Mapping Study of Good Service Experience among the Elderly Residents of Long-term Care Facilities (장기요양시설노인의 좋은 서비스 경험에 관한 개념도 연구)

  • Purpose: The purpose of this study was to explore the reported good service experiences from the perspective of elderly residents of long-term care facilities. Methods: Of those residents who are 65 years old or older, 14 residents whose length of stay were one month or longer and scores of the K-Mini Mental State Examination were 15 or higher were interviewed. The interview data formed the basis for the empirical statements about the reported nature of patients' experiences as residents of long-term care facilities. These data were used in concept mapping. Results: Through multidimensional scaling analysis and hierarchical cluster analysis, 62 core statements, two dimensions, and six clusters of good service experiences were derived. The two dimensions were classified as 'care centered-participation centered services' and as 'physical-emotional services.' Six cluster themes emerged as good service experiences: 'safety of care and treatment', 'responsible and supportive staff', 'comfort of living environment', 'mental well-being', and 'respect and communication'. Conclusion: The result of the study provides information about what experiences are important to older adults with cognitive impairment. The concept map can be used to develop a patient experience index for the elderly residents of long-term care facilities.
