A Study on the Burdens and Depressive Reactions on Families who Cared for Patients Suffering from Senile Dementia (치매노인을 돌보는 가족의 부담감과 우울반응에 관한 연구)

  • The purpose of this study was to investigate the relationship between the burden on families who live with an elderly person suffering from senile dementia, and the degree of their depression. There were 400 participants in this study, staying in the Seoul and Kyonggi areas from August 1, 1997 to February 28, 1998. Among the group, 100 participants took care of their patient at home, and another 300 participants left 100 patient at a day-care center, 100 sanatorium for senile dementia(asylum for helpless elderly people), 100 an infirmary for elderly people. Eventually 242 subjects out of the 400 were selected for the data analysis. The Zarit (1980) tool was employed to measure the degree of burden and Zung's(1965) “Self-Rating Depression Scale” was employed for the data analysis. The data was analyzed, and the percentage, t-test, ANOVA and Person's Correlation Coefficient were calculated. The results are as follows : 1. The average degree of burden that care-giving families felt was 49.13, which is somewhat high. 2. The average degree of depression that care -giving families felt was 51.95, which is relatively high. 3. The degree of burden was directly affected by the relation with the patient(F=2.48, P<.05), and the socio-economic status of the family(F=5.17, P<.05). Its also affected by the patient's educational status(F=2.17, P<.05). 4. The degree of depression of the family was significantly dependent on sex(t=-2.05, P<.05), age (F=2.99, P<.05), the relationship with the patient(F=3.65, P<.01), socio-economic status (F=7.74, P<.001), occupation(t=2.82, P<.01), health status(F=4.42, P<.01), and the place of residence(F=4.30, P<.01), The patient characteristics was significantly dependent on his/her educational status(F=3.85, P<.01), the period of suffering from senile dementia(F=2.47, P<.05), and smoking habit(F=6.17, P<.001). 5. The relationship between the degree of burden and that of depression reads r=0.43, which is statistically positive correlation in the high significant level. Upon analyzing the entire summation, most care-givers for elderly patients suffering from senile dementia lack time in caring for themselves. They also experience chronic fatigue and mental discomfort caused by the isolation from society, curtailment of certain activities, a sense of responsibility for their patients, and limits of their endurance in taking care of their patients over time. In conclusion, this study emphasizes the necessity for the following propositions : 1. In order to measure the degree of burden that Korean care-giving families undergo, a new tool must be developed on the basis of Korean culture. 2. An educational program based on the demands that care-giving families undergo must be developed, and its clinical effect also has to be examined.

  • Staphylococcus aureus SA2, which was isolated from patient, is resistant to 10 usual antibiotics. The methanolic extracts of 21 well-known herb materials were combined with 10 antibiotics and applied to cheek inhibitory effects on the resistance of S. aureus SA2. The hexane fractions from methanolic extracts of Acori graminei Rhizoma and Anethi Fructus had most potent activity to inhibit the resistance of the bacteria when combined with ampicilin or chloramphenicol.

  • The effects and necessity of the breast milk are well known, but the breast-feeding rate in our country is very low. One of the reason is due to the separation policy of mother and baby after delivery. The mother is in the inpatient-room and the baby is in the new-born baby room. This isolation operation of hospital get increased according to the decreasing ratio of breast feeding in Korea. Only a few hospital operates and provides the space for a new-born baby in the mother's room. The system of baby and mother in one room is adopted and operated due to the breast feeding campaign. It is very encouraging, but the space for the baby and the mother is not enough in multi-bed rooms. We should reconsider the relationship between the new-born baby unit and the obstetric ward, and design the patient's room for the baby and the mother.

Isolation of Influenza Virus from Patients with Respiratory Disease in Pusan in 1997 (1997년도 부산지역 호흡기환자로부터 인플로엔자 바이러스의 분리)

  • This study was done to detect the causative agent of patient with respiratory disease in Pusan, 1997. Male and female patients with respiratory disease in Pusan, 1997, were 31.9% and 68.1 %, respectively. In the aspect of out-break by month, patients with respiratory disease were mostly concentrated at February, March, April, October, November and December. Fifteen strains of influenza virus were isolated from 1,268 swabbed samples of throat, and thirteen strains and 2 strains among 15 isolates were classified with influenza A and B virus, respectively. One of 13 influenza A virus was confirmed as A/Johannesburg/33/94- like strain, and the other isolates of influenza A virus were confirmed as A/sydney/05/97-like strains. Two isolates of influenza B virus were confirmed as B/Bei-jing/08/93-like strains.

  • New epidemics of infectious diseases often involve health care workers. In this short communication we present a case report of a health care professional who became the first case of influenza H1N1 virus to be notified in the United Arab Emirates. There are several issues related to workplace considerations and general public health, including preventive measures, the need for isolation of the patient, dealing with contacts, return to work, and communication with the workforce.

  • Congenitally corrected transposition of the great arteries is a rare congenital heart anomaly, in isolation, has no hemodynamic consequences. It is usually associated with one or more of a variety of intracardiac lesions, ventricular septal defect, valvular or subvalvular pulmonary stenosis, and deformity of the systemic atrioventricular valve with insufficiency. This report describes a successful two stage operation for congenitally corrected transposition, [SLL] type, with ventricular septal defect, pulmonary atresia, persistent ductus arteriosus, and atrial septal defect. A 9 years old patient underwent modified Blalock-Taussig operation because of severe pulmonary hypoplasia. 2 years later a corrective operation, direct closure of ASD and PDA, VSD closure with Dacron patch, Enlargement of left pulmonary artery with pericardial patch and Relief of ROTO with Rastelli procedure could be successfully performed without complication.

A Case Study on the Ventilation System of the General hospital (종합병원의 환기설비 사례 연구)

  • In many other buildings, hospital is very important because of its function and purpose. A proper building service systems including heating, cooling, ventilation and plumbing systems is required in hospial. Besides, a special systems like medical gas, vacuum cleaning systems are required to prevent contamination as well as cure a patient in hospital. Ventilation is very important method to keep the indoor pressure and clean class. There are some specific rooms to consider clean class and indoor pressure in the general hospital. In this article, specific rooms were set to operating room, laboratory, ICU, isolation unit, and sterilizer room. In this case study, design factors of ventilation system, including air changes of OA and TA, indoor pressure, clean class, filter, and material of system, of specific rooms in hospital were proposed through a literature research and a design examples review.

  • This study is a phenomenological study done to promote understanding of the dying process in patients with terminal cancer who were in an independent hospice center. The purpose of study was to explore and understand indepth information on the dying process in order to provide data for holistic hospice care in nursing and to give insights in to practical applications in the nursing care In-depth interviewing was done from may, through November, 1995 with 11 patient with cancer who were being cared for at K Hospice Care Center. Experiences in the dying process were discussed as they expressed feelings about death including (a) feeling of isolation because family members try to hide the diagnosis of cancer. (b) hopelessness, (c) guilt, anger, and hostility, (d) suffering from pain, (e) fear of death. However, subjects did not deny death itself and were developing peace of mind and acceptance of death through religion.

  • The mattresses and dust in the bed rooms of nine dermatophytes infected patients and nine domestic animals were examined by the KOH method. Microsporum canis species and Trichophyton mentagrophytes were isolated from cats and rabbits, respectively. The sources of infection of three patients were the M. canis infected cats raised by them and the four other patient's sources of infection were not confirmed. The sites of infection of the nine patients were their heads and those of the domestic animals were their heads and bodies. M. canis species were isolated from the infection sites of three cats and specimens collected by hair brush from the nine domestic animals. T. mentagrophytes species were also isolated from the infection sites of two rabbits. The seven patients had mattresses and bed room dust contaminated with M. canis.

Clinical Result of Modified Percutaneous Repair Technique of Ruptured Achilles Tendon (변형된 경피적 술식을 이용한 아킬레스 건 봉합술의 임상적 결과)

  • Purpose: To investigate the results of percutaneous repair technique of Achilles tendon ruptures, and to describe the surgical technique. Materials and Methods: We retrospectively analyzed the outcomes of 73 patients with ruptured Achilles tendon from October 1995 to September 2009. 28 patients were excluded due to short follow up period. 34 patients were male and 11 patients were female. The mean patient age was 37.19 (10~62) years. The location of rupture site was 6.58 cm proximal to the tendon insertion into the calcaneus on average. Mean follow up period was 55 months and All patients were surgically repaired using percutaneous technique with sural nerve isolation. Results: Arner-lindholm score were excellent in 32 (71%), good in 12 (27%), poor in 1 (2%) case. 44 cases (98%) had the score more than good. Mean American Orthopedic Foot and Ankle Society (AOFAS) Ankle-Hindfoot function score was 92.93 (67~100). We had 1 case of superficial infection, 1 case of soft tissue irritation by suture knot. Conclusion: Percutaneous repair with sural nerve isolation in treating ruptured Achilles tendon showed low complication rate and reliable clinical outcome.
