만성 발목 불안정성 환자들에 대한 샌들과 균형 융합 운동이 정적균형 및 고유수용성감각에 미치는 효과 (The effect of Sandal and balance convergence exercise on static balance and proprioception on patient with chronic ankle instability)

  • 본 연구는 샌들을 이용한 균형 훈련이 만성 발목 불안정성 환자에 균형 능력과 고유수용성 감각에 미치는 영향을 알아보고자 하였다. 만성 발목 불안정성으로 진단받은 대상자 36명을 무작위로 샌들운동과 균형훈련, 대조군으로 할당하였으며, 정적균형능력과 고유수용성 감각 평가를 중재 전-후 실시하였다. 12명의 샌들운동군은 균형샌들을 이용하여 실시하였고, 12명의 균형훈련군은 고전적인 균형훈련을 실시하였다. 세 집단은 동일한 시간, 동일한 조건에서 30분 동안 주당 3회를 8주간 실시하였다. 샌들을 이용한 균형훈련 집단은 두 집단보다 균형능력에서 전반적인 유의한 효과를 보였으며(p<.001), 고유수용성 감각은 가쪽번짐에서 유의한 효과를 보였다(p<.001). 따라서 샌들을 이용한 균형훈련은 만성발목 불안정성 환자뿐 아니라 노인의 낙상예방 훈련에도 긍정적인 중재 방법이 될 것이다.

소아 환자에서의 외상성 간 손상 (Traumatic Liver Injury in Pediatric Patients)

  • Purpose: Trauma is an important cause of death in children. In particular, the liver is the second most commonly organ injured by blunt abdominal trauma. Treatment of patients with liver injury is has changed, and non-operative treatment is the major treatment method at present. In this study, we reviewed traumatic liver injury in pediatric patients. Methods: Seventy-seven patients younger than 16 years of age with traumatic liver injury were assessed for 10 years from July 1999 to June 2009 at Wonju Christian hospital. Records of the patients were reviewed retrospectively. Demographic and clinical data were analyzed. Results: The median age was 6 years, and the male-to-female ratio was 1.2 : 1. The most common injury grade was grade I. The majority of injuries were caused by was traffic accidents, and the second most common cause of injuries was falls. Twenty-four patients had liver injuries alone, and the most common accopaning injury was a lung injury. The average hospital stay was 20.7 days, and the average ICU stay was 4.8 days. Four patients died (5.2%). There were 6 patients with under 10 points on the Glasgow coma scale (GCS). Among these patients, three died. All mortality cases had over 16 points on the Injury Severity Score (ISS). Two patients were treated surgically, one of whom died. Of the 75 patients with non-operative management, three died due to associated injuries. Conclusion: Most pediatric patients with liver injury have good results with non-operative management. Associated injuries and hemodynamic instability are predictive of patient outcome, and those with isolated liver injuries can be successfully managed non-operatively.

  • Purpose : This study is to determine effects of cane-shape which influenced on the change of muscle activation, gait component in hemiplegic patients caused by cerebrovascular accident. Methods : Twenty one stroke patients using T-shape cane(TCG) and twenty one stroke patients using I-shape cane(ICG) participated in this study. Surface electromyography(SEMG) of erector spinae, transvers abdominis, tibialis anterior, soleus of both side were measured during walking without cane and walking with cane. Results : The activation of tibialis anterior was significantly increased in affected side of ICG. The activation of tibialis anterior was significantly decreased in affected side of TCG. The activation of soleus was significantly increased in affected side of ICG. The activation of soleus was significantly decreased in affected side of TCG. The activation of erector spinae was significantly increased in nonaffected side of ICG and TCG. The activation of transvers abdominis was significantly increased in nonaffected side of ICG and TCG. Conclusion : From the finding of this study, it was suggested that I-shape cane increase weight bearing of affected side in hemiplegic patient. Therefore, I-shape cane can be applied to improve gait asymmetry of hemiplegic patients.

Etiology and Patterns of Maxillofacial Fractures in 518 patients in Korea

  • Purpose: Different patterns in the causes of maxillofacial injury are thought to correlate with socioeconomic status and regional environment. This study investigated maxillofacial fractures in order to analyze maxillofacial trauma characteristics and the relationship between the causes and injury patterns in Korea. Material and methods: A total of 518 patients with maxillofacial fractures who were treated at the Seoul National University Boramae Hospital between 1996 and 2004 were retrospectively analyzed. Data were obtained from the patients' medical records and radiographs. The male to female ratio in the patient group was 2.78:1, and the mean age was 32.3 years. Results: Midfacial fractures were the most common location of injury (46.1%). The most common etiologic factor was an activity associated with daily life (42.6%) including falls, stumbling, and collisions. The second most common cause was assault (32.4%), followed by traffic accidents (13.7%). In the case of midfacial fractures and mandibular fractures, assault was the most common etiologic factor, whereas in the case of alveolar bone fractures, activities associated with daily life were the most common cause. With regard to age groups, assault was the most common cause for patients between 10 and 39 years old and an activity associated with daily life was the most common cause in those under 10 years and over 40 years. Conclusions: This study concluded that activities associated with daily life and assault causes a large proportion of Korean maxillofacial injuries and that preventive measures should be implemented in order to minimize these risks.

  • 치매 환자나 혼자 힘으로 전혀 움직이지 못하는 노인 환자는 간병 인력 부족으로 낙상 사고 및 욕창 발생 가능성이 매우 크다. 본 논문에서는 이러한 문제를 해결하기 위해 일정 주기마다 감지한 압력 세기를 기준으로 머리, 어깨, 엉덩이 등 주요 신체 부위를 판별하여 환자의 누운 자세를 판별할 수 있는 알고리즘을 제안한다. 신체적 특성에 무관하게 신체 부위 판별이 가능하도록 격자 구조로 압력 센서를 배치한 스마트 매트를 제작하였다. 스마트 매트는 $7{\times}7$ 배열 크기의 2개 모듈을 조합하여 구성하였다. 각 모듈은 모두 49개의 FSR-406 센서로 구성되며 독립적으로 압력을 감지한다. 각 모듈에 대해 필터를 사용한 누적 압력 합 등 압력 분포를 이용해 상체 또는 하체에 해당하는 신체 부위를 순차적으로 판별한다. 제안한 알고리즘은 머리, 어깨, 엉덩이 부위 등 계층-1에 속한 신체 부위간 포함 관계를 조사해 5가지 누운 자세를 판별할 수 있다.

경두개 직류전류자극과 가상현실재활프로그램을 적용한 뇌졸중환자의 균형과 낙상관련 체력 및 낙상효능감에 미치는 영향 (The Effects of Transcranial Direct Current Stimulation on Balance, Fall Efficacy, and Fall-Related Fitness in Stroke Patient's through a Virtual Reality Rehabilitation Program)

  • Purpose : This study assessed the effects of transcranial direct current stimulation (tDCS) on balance, fall efficacy, and fall-related fitness in stroke patients, using a cohort of 30 stroke patients divided into two groups. Methods : The experimental group (was given transcranial direct current stimulation in a virtual reality program) and the control group was given false tDCS in virtual reality. there were 15 patients in each group, receiving appropriate treatment over 30 sessions (30 minutes per session per week over a six-week period). In order to assess the change in balance before and after the intervention, the Berg Balance scale was utilized. Fall efficacy was evaluated using the Korean Falls Efficacy Scale for the Elderly (FES-K), The following exercises were performed by patients to assess fall-related fitness : sitting down in a chair and standing up : walking a 244 cm round= trip, and standing on one foot. Results : After the intervention, the experimental group exhibited significantly increased fall efficacy and fall-related fitness, while the control group exhibited no change. These findings suggest that tDCS has positive effects on balance, fall efficacy, and fall-related fitness in stroke patients. Conclusion : Using tDCS as an intervention would bring positive effects on balance, fall efficacy, and fall-related fitness in stroke patients undergoing rehabilitation.

Incidence and management of mandibular fractures in a low-resource health facility in Ghana

  • Objectives: The mandible and other parts of the maxillofacial region suffer significant morbid injuries following road traffic accidents. Our study gives epidemiological description of mandibular fractures in Ghana and also evaluates the relevance of closed reduction and indirect fixation for managing mandibular fractures in low-resource health facilities in low-income countries like Ghana. Patients and Methods: This is a retrospective study involving 268 patients who reported to the Department of Oral and Maxillofacial Surgery of the Sunyani Regional Hospital with mandibular fractures from January 2010 to December 2019. Patient medical records were assessed for information on age, sex, fracture etiology, anatomic location of fracture, time of day of road traffic accident, and other associated injuries. Results: A total of 268 patients were included in this study (males, 216 [80.6%]; females, 52 [19.4%]). Motor vehicular accident (MVA) was the leading cause of mandibular fractures (202 injuries, 75.4%). Other etiologies included assault (39, 14.6%), gunshot (13, 4.9%), falls (12, 4.5%), and industrial accidents (2, 0.7%). Of the 161 male cases caused by MVA, 121 (75.2%) occurred at night and in the evening while the remaining 40 (24.8%) occurred in the morning and afternoon. Among all managed 222 patients, 212 (79.1%) were treated with closed reduction and indirect fixation technique while 10 (3.7%) were treated with open reduction and direct fixation. Conclusion: Closed reduction with indirect fixation could successfully be used to manage mandibular fractures in low resourced health facilities, especially in low-income countries. The poor lightening system on roads in Ghana is a major contributory factor to motor vehicular accidents.

휠체어 몸통 훈련이 급성기 뇌졸중 환자의 낙상 효능감, 낙상 위험도, 일상생활활동에 미치는 영향: 무작위 대조군 예비 연구 (Effect of trunk training in wheelchair on fall efficacy, fall risk, activities of daily living in acute stroke patients: Randomized controlled pilot study)

  • Background: The aim of this randomized controlled pilot study was to determine the effect of trunk training in wheelchair on fall efficacy, fall risk and activities of daily living in acute stroke patients. Design: Randomized controlled pilot trial. Methods: The study included 18 patients with acute stroke who were randomly allocated to an experimental group (EG) (n=9) and a control group (CG) (n=9). Patients in the EG group received general rehabilitation therapy combined with trunk training in wheelchair for 20min, whereas CG group received general rehabilitation therapy combined with bicycle training for 20min. Both groups performed the exercise 5 times a week for 3 weeks. Outcomes were assessed using Korean-Fall Efficacy Scale (K-FES), Fullerton Advanced Balance Scale (FAB) and Korean-Modified Barthel Index (K-MBI). Results: After 3 weeks of training, both groups showed significantly improved K-FES, FAB and K-MBI (p<.05 in both groups). However, the K-FES, FAB and K-MBI in the experimental group was significantly improving than in the control group (p<.05). Conclusion: These findings indicate that trunk training in wheelchair may be effective at decreasing fall risk and improving activities of daily living in acute stroke patients. Therefore, trunk training in wheelchair may be recommended as an intervention in reducing the incidence of fall risk in acute stroke patient.

Nonfatal injuries in Korean children and adolescents, 2007-2018

  • Background: Injury is the leading cause of death or disability in children and adolescents. Rates of deaths from injuries have recently declined, but studies of the occurrence of nonfatal injuries are lacking. Purpose: This study aimed to investigate nonfatal injuries in children and adolescents younger than 20 years based on data from the Korean National Health and Nutrition Survey, 2007-2018. Methods: A questionnaire survey was conducted to determine whether children and adolescents had experienced an injury requiring a hospital visit in the previous year. We investigated each injury's risk factors and characteristics. Results: Of a total of 21,598 children and adolescents, 1,748 (weighted percentage, 8.1%) experienced one or more injuries in the previous year. There was no yearly difference in the proportion of injuries experienced. Among the male subjects, 10.0% had an injury experience; among the female participants, 6.1% had an injury experience (P<0.001). The highest rate was 9.0% in children aged 1-4 years. In multivariate logistic regression analysis, male sex; having an urban residence; having restricted activity due to visual, hearing, or developmental impairment; and attention deficit/hyperactivity disorder were significant risk factors for injury experience. The characteristics of up to 3 injuries per patient were investigated, and 1,951 injuries were analyzed. Falls and slips accounted for 34.9%, collisions for 34.1%, and motor vehicle accidents for 11.3% of the total injuries. Ninety-six percent of injuries were unintentional, 20% caused school absences, and 10% required hospitalization. Conclusion: Among Korean children and adolescents, 8.1% experienced injuries at least once a year with no significant differences in incidence over the past 12 years. Greater attention and effort to prevent injuries are needed.

Injury trends among foreign and domestic tourists in Jeju from 2008 to 2018

  • Jeju is the largest island in Korea and one of its key tourist attractions. As the number of foreign tourists steadily increases, so does the number of injuries incurred there. Accordingly, this study aimed to describe and compare the characteristics of injuries suffered by foreign tourists versus those of domestic tourists. As a cross-sectional study of a retrospective medical record survey, the study was conducted with reference to the Jeju Injury Surveillance System from the 11-year period of January 2008 to December 2018. The following factors were investigated: demographic data, mechanism of injury, place of occurrence, activity when injured, patient outcome, and mortality. A total of 92,095 injured Jeju Island visitors was recorded during this time, a number that included 88,050 Koreans and 4,045 foreigners. The gender ratio showed similar patterns between the two groups and there were no significant age differences. In both groups, the most common mechanism of injury was collisions/cuts. Also, more foreigners experienced falls than Koreans. Regarding the location, Koreans had the most road accidents, while foreigners were most likely to be injured at outdoor locations, such as seas and rivers. Furthermore, more foreigners experienced severe injuries requiring hospitalization. Notably, this study showed the differences in injury between foreign and Korean tourists visiting Jeju Island and its findings lend support to targeted safety promotion programs.
