신장이식 환자의 치료지시 이행정도와 교육 요구도에 관한 연구 (A study on the Compliance and Educational Demand of Renal Transplantation Patient)

  • This study was started for the purpose of providing the basic data for continous managment of kidney transplantation patients after discharge. This study was conducted on 180 patients who received renal transplants at three hospital( B, M, P) pusan, korea. The data collection was done for june 1, to August 31, 2002. General characteristics, renal transplantaton characteristics, physical characteristics, the level of compliance and the degree of educational demand were done by the number and percentage, the mean, standard deviation. The level of compliance and educational demand followed by the characteristics of general and kidney transplantation were analyzed by t-test and ANOVA. The result were as fallows; 1. Man was higher than woman such as 60.0%, Mean age was 42.5 years old, Average total duration of after operation was 5.5 years. 2. Cases of systolic blood pressure over 140mmHg were 10.0%, cases of diastolic blood pressure over 90mmHg were 22.8% and obesity factor in BMI was 15.6%. The person who daily water intake amount is 5000cc was 0.6%, the case that daily urine output is below 1000cc was 8.9%, and the case that urine output is zero was 0.6%. 3. The mean score of compliance was 77.47 point, The score in medication part was highest such as 4.67 point, that in stress situation was lowest such as 3.50 point. 4. The average score of educational demand was 154.02 point, and physical state part was 4.36 points highest, activation part was 3.48 points lowest. As a role of nurse Confirmation of compliance is very important encourage to make good through regular hospital visitation, point out the noncompliance part and then increase compliance of renal transplantation patient As well there will be maintain the normal kidney function to satisfy educational demand through continous education.

한국인 치료순응도 향상을 위한 개입 효과에 대한 메타분석 (Meta-Analysis on Effectiveness of Intervention to Improve Patient Compliance in Korean)

  • 본 연구는 최근 20년간의 치료순응도 관련 국내연구를 수집하여 연구 동향을 파악하고, 계량적 메타분석을 통하여 순응도 향상을 위한 개입 효과를 산출하여 제시하고자 하였다. 연구 결과는 다음과 같다. 첫째, 1980년대 이후 20여년간 치료순응도와 관련된 연구는 133편에 달하였다. 이중 순응도 향상을 위한 실험연구가 11.3%, 단면연구 및 전향적 관찰연구가 88.7%에 해당하였으며, 후자의 경우 주로 순응도 향상 관련 요인에 관한 연구들이었다. 둘째, 연구대상의 상병 종류는 고혈압이 전체 연구의 24.1%로 가장 많았고, 다음으로 기타 만성질환, 당뇨병 및 결핵의 순으로 주로 이환기간이 길고 완치가 어려워서 치료순응도에 많은 노력을 기울일 필요가 있는 질환들이었다. 셋째, 133편 논문의 질적 메타분석과정을 거쳐 선별기준에 부합되는 논문은 10편으로 순응도 측정은 고혈압의 경우 혈압 조절과 같은 건강효과를, 당뇨병의 경우 뇨당, 혈당 저하와 같은 직접적인 지표 측정의 방법을 측정하였고, 이외에도 약속 이행과 환자의 주관적 보고, 약물 복용이행과 같은 객관적 지표를 측정하였다. 한편 개입방법으로는 행동변화를 적용한 1편의 연구 이외에 교육적, 행동적 및 정서적 개입의 방법을 모두 병행하여 적용하고 있었다. 넷째, 이들 중 6편의 논문에 대하여 순응도 향상에 대한 개입의 통합효과크기를 산출한 결과, 개입을 하지 않은 군에 비하여 개입이 이루어진 경우에 치료순응도 향상 효과는 4.1192여서 커지는 것으로 나타났다. 이를 다시 순응도 측정방법으로 구분하여 분석한 결과, 치료 순응도를 건강결과(혈압)로 측정한 경우 개입으로 인한 유효크기는 0.4679였고, 직접 측정지표(혈당/뇨당)에 의한 경우 개입 후 유효크기는 0.7753으로 나타나 순응도가 향상되었음을 알 수 있다. 이상의 결과에서 다양한 개입활동들이 환자의 치료순응도를 향상시키는 데 기여할 수 있음을 제시할 수 있다. 그러나 계량적 메타분석 기준에 부합되는 연구의 논문 수가 제한적이라는 한계점이 있으며, 연구에 사용되는 개입방법의 시도가 더욱 다양화될 필요성이 제시되고 있어 향후 이와 관련된 많은 연구들이 활성화되기를 기대한다.

경기지역 여대생이 지각한 유방 자가검진의 지식, 자기효능감 및 이행 간의 관계 (Knowledge, Self-Efficacy and Compliance on Breast Self Examination among Female College Students in Gyeonggi Province)

  • Objectives: This study was conducted to identify the relationship among knowledge, self-efficacy and compliance behavior on breast self examination(BSE) of female college students'. Methods: This descriptive study was conducted with a convenient sample of 499 female college student. Surveyed data collected from May to August 2008 were analyzed using descriptive statistics, t-test, pearson correlation coefficients and stepwise multiple regression. Results: The mean score of knowledge and self-efficacy concerning BSE was low, and compliance was high. The levels of knowledge, self-efficacy and compliance were differed significantly by experience of thinking about breast cancer, perceived susceptibility of breast cancer, experience of meeting with breast cancer patient, and educational experience of BSE. Significant correlations were found between knowledge and compliance, knowledge and compliance, knowledge and self-efficacy. Knowledge and self-efficacy were a powerful predictor of compliance. Conclusion: Focus on perceived knowledge, self-efficacy about breast self examination(BSE) of female college students may be beneficial to improve compliance, and should be reflected in the development of effective BSE education program.

관상동맥질환자의 D유형 성격과 치료지시이행의 관계에 미치는 극복력의 매개효과 (The Mediating Effect of the Resilience on the Relationship between Type D Personality and Compliance in Patients with Coronary Artery Disease)

  • Purpose: This study aimed to identify the mediating effect of the resilience on the relationship between type D personality and compliance of Coronary Artery Disease (CAD) patients. Methods: The subjects were 236 CAD patients who visited the cardiology outpatient clinics of D University Medical Center in B City in Korea. The type D personality, resilience, and compliance of subjects were measured using structured questionnaires from July to August, 2014. The mediating effect was analyzed by a multiple hierarchical regression. Results: The subjects with type D personality accounted for 30.9% of all study subjects. The resilience and compliance of type D subjects were significantly lower than those of non type D subjects. There was a significant correlation between resilience and compliance. The direct effect of type D personality on compliance was not significant (${\beta}=-.07$, p=.243), while the indirect effect of type D personality on compliance via resilience was significant (${\beta}=.23$, p<.001). Conclusion: On the basis of above results, it can be concluded that the resilience had completely mediating effect on the relation between type D personality and compliance of CAD patients. Further studies need to be done to develop the intervention enhancing resilience of CAD patients with Type D personality.

혈액투석 환자의 역할행위이행 영향요인 (Factors Influencing Sick Role Behavior Compliance in Patients on Hemodialysis)

  • Purpose: This study was a descriptive survey research to identify the factors that influence sick role behavior compliance in patients on hemodialysis. Methods: Structured surveys were used to collect data from 170 patients who are on hemodialysis three times a week through outpatient care at a university hospital located in G City using tools measuring resilience, family support, and sick role behavior compliance. Data were analyzed using descriptive statistics, t-test, one-way ANOVA, Pearson's correlation coefficient, and Stepwise multiple linear regression. Results: The results showed that factors that influenced sick role behavior compliance in the subjects were in the order of family support (${\beta}=.27$, p<.001), age (${\beta}=.27$, p<.001), and resilience (${\beta}=.23$, p=.003). Resilience, family support, and sick role behavior compliance were positively correlated. Factors influencing hemodialysis patients' sick role behavior compliance included family support, age, and resilience. These variables explained 30.2% of the variance in sick role behavior compliance. Conclusion: Based on these results, care intervention and the development of a training program that involves family in the treatment plan and process to support and encourage patients are needed to increase the sick role behavior compliance in patients on hemodialysis.

고혈압·당뇨병 신규 환자 발견 이후 지역사회 협력을 통한 등록관리가 치료순응도 및 혈압 및 혈당 조절에 미치는 영향 (Impact of Registration Program after Hypertensive or Diabetic Patient Detection through Community Partnership on Compliance and Blood Pressure or Blood Sugar Control)

  • = ABSTRACT = Objectives : Incheon metropolitan city has set a model program of community partnership for hypertensive or diabetic patients detection and follow-up since 2005. This study examines the effects of the model on the compliance and the control of blood pressure or blood sugar level Methods : Telephone-surveys were done for 140 persons selected from 408 patients who were detected newly as hypertensive or diabetic patients at Gangwha-Gun and Seo-Gu in Incheon between January 1st and August 31th in 2006, and finally completed in 110 patients(78.6%). Survey questionnaires included socio-demographic(age, gender, and educational level), health behavioral (smoking, drinking, physical activity, and diet), and therapy-related (registration state, compliance, blood pressure or blood sugar control) variables. Odds ratio and 95% CI were derived from logistic regression model. Results : Registered group exhibited high compliance and well managed blood pressure or blood sugar level. The odds ratio of registration were 5.55(95% CI:1.83~16.89) for compliance and 3.78(95% CI:1.43~9.99) for blood pressure or blood sugar control after adjusting for age, gender, disease, and area Conclusions : It is independently related to compliance and blood pressure or blood sugar control whether the patient is registered or not. To control hypertension or diabetes mellitus in terms of compliance and control state, a community-based registration program through community partnership could be a powerful tool.

폐암환자의 치료순응도와 관련요인 (Therapeutic Compliance and Its Related Factors of Lung Cancer Patients)

  • Objectives : To investigate the therapeutic compliance and its related factors in lung cancer patients. Methods : The subjects of this study comprised 277 patients first diagnosed with lung cancer at Kyungpook National University Hospital between Jan 1999 and Sept 1999. Of these, 141(50.9%) participated in the study by properly replying to structured questionnaires. The data was analyzed using a simplified Health Decision Model. This model includes categories of variables covering therapeutic compliance, health beliefs, patient preferences, knowledge and experience, social interaction, sociodemographic and clinical characteristics. Results : The therapeutic compliance rate of the 141 study subjects was 78.0%. An analysis of health beliefs and patient preferences revealed health concern (p<0.05), dependency on medicine (p<0.05), perceived susceptibility and severity (p<0.05) as well as preferred treatment (p<0.01) as factors related to therapeutic compliance. Factors from the sociodemographic characteristics and clinical factors that were related to therapeutic compliance were age (p<0.01), monthly income (p<0.05), histological type (p<0.05) and clinical stage (p<0.05) of cancer. Conclusions : In order to improve therapeutic compliance in lung cancer patients it is necessary to educate the aged, low-income patients, or patients who have small cell lung cancer or lune cancer of an advanced stage for which surgery is not indicated. Additionally, it is essential for medical personnel to have a deep concern about patients who have poor lifestyles, a low dependency on medicine, or a high perceived susceptibility and severity. Practically, early diagnosis of lung cancer and thoughtful considerations of low-income patients are important. By means of population-based education in a community, we may promote attention to health and enhance the early diagnosis of lung cancer.

Four months of rifampicin monotherapy for latent tuberculosis infection in children

  • Diagnosing and treating latent tuberculosis infection (LTBI) is an important part of efforts to combat tuberculosis (TB). The Korean guidelines for TB published in 2020 recommend 2 LTBI regimens for children and adolescents: 9 months of daily isoniazid (9H) and 3 months of daily isoniazid plus rifampicin. Isoniazid for 6-12 months has been used to effectively treat LTBI in children for over 50 years. However, a long treatment period results in poor patient compliance. This review summarizes pediatric data on the treatment completion rate, safety, and efficacy of 4 months of daily rifampicin (4R) and evaluates the pharmacokinetics and pharmacodynamics of rifampicin in children. The 4R regimen has a higher treatment completion rate than the 9H regimen and equivalent safety in children. The efficacy of preventing TB is also consistent with that of 9H when summarizing reports published to date. A shorter treatment period could increase patient compliance and, therefore, prevent TB in more patients. By using an effective, safe, and highly compliant regimen for the treatment of children with LTBI, we would become one step closer to our goal of eradicating TB.

아토피피부염 아동 어머니의 지식, 치료지시이행 및 삶의 질 (Knowledge, Compliance and Quality of Life of Mothers of Children with Atopic Dermatitis)

  • Purpose: This study was conducted to survey knowledge, compliance, and quality of life of mothers who have children with atopic dermatitis, and to determine whether above mentioned factors influence each other. Methods: The participants in this study were 165 mothers of children with atopic dermatitis recruited from one out-patient clinic in a hospital in Seoul. The data were collected between March 8, and April 16, 2010 using a structured questionnaire. Results: The mean score for participants' knowledge was 4.17/5, for compliance, 4.27/5 and for quality of life, 3.42/5. Among the five domains of quality of life, emotional status had the lowest score (3.20/5). Knowledge was significantly correlated with compliance (r=.215, p<.005). However, quality of life was not correlated with knowledge (r=-.069) and compliance (r=-.077). Conclusion: Accurate knowledge affects compliance of mothers of children with atopic dermatitis. Therefore, to increase the level of compliance, it is necessary to enhance the knowledge of the mothers. Results also suggest that interventions are needed to improve poor quality of life, particularly the emotional condition of mothers of children with atopic dermatitis.
