  • Objectives The purpose of this study is to report a Tic-disorder child with allergic rhinitis. Methods The child was suffered from Tic-disorder and the allergic rhinitis, and there were two symptoms of all can last more than a year. sometimes the sniffling, as a sin of allergic rhinitis had caused tic disorder. I thought It was needed to control of both of the symptoms at the same time, so I applied the herbal medicine, CheongKanSoYoSan, for Tic-disorder, and carried out the Aromatherapy and the Laser acupuncture for allergic rhinitis. Results After this treatments, the symptoms of Tic-disorder were disappeared according to improving the symptoms of allergic rhinitis. Conclusions It was suggested that we tried to take care of the patient with Tic-disorder, had to considered the other symptoms that were capable of causing.

  • Accidental or suicidal fatalities of paraquat(Gramoxon) poisong are occasionally seen in the emergency room or intensive care unit in this country. In most cases, respiratory symptoms and eventual death by respiratory distress occur within several days. The most striking pathologic change is fibrosis of the lung due to widespread proliferation of fibroblastic cell. We experience a 21-year-old woman with huge bulla on left lung and diffuse fibrosis in other site, who ingested paraquat 10 months ago. After thoracoscopic removal of bulla, the patient survive without progression of pulmonary complication till now.

  • A 52-year-old male patient who underwent multiple wedge resections experienced postoperative acute respiratory distress syndrome in both lungs after Viscum album pleurodesis. Despite initial rapid deterioration in clinical condition and rapid progression of bilateral lung infiltration, he exhibited a relatively smooth clinical recovery with marked response to glucocorticoid treatment. Our case report suggests that care must be taken to guard against the development of acute respiratory complications in the use of Viscum album for pleurodesis. However, in view of the clinically benign course, initial aggressive management of complications can prevent suffering and sequelae.

  • A 57-year-old male patient was admitted to our center because of a cystic mass on the lower portion of the right major fissure that was found incidentally by chest X-ray. He did not have a history of trauma or anticoagulant use. The lesion was removed by video-assisted thoracoscopic surgery. Pathological examination revealed an organizing pulmonary hematoma without any complications, and a follow-up chest X-ray after 1 year showed no recurrence.

  • Oral and maxillofacial surgery (OMFS) trauma cases are commonly treated under general anesthesia. The purpose of this case report is to introduce an alternative method of anesthesia in patients who refuse general anesthesia. A combination of dexmedetomidine and ketamine for sedation anesthesia in 3 frequent fracture types in the field of OMFS-Le Fort I fracture, mandibular fracture, and alveolar bone fracture-was used. Dexmedetomidine as the single agent has not shown stable success rates for invasive procedures. To overcome some of the pitfalls with dexmedetomidine, combination sedation using ketamine was performed. Visual analogue scale scores were recorded postoperatively. Dexmedetomidine combined with ketamine administration provided safe and effective sedation and anxiolysis for surgical reduction and internal fixation of OMFS fractures. It showed advantages of decreased admission time, reduced expenses, minimal pain, and reduced anesthetic burden for the patient thus ultimately increasing overall satisfaction.

  • Herein, we report a case of emergency cesarean section after severe maternal drug intoxication in late pregnancy. At a 38-week-gestation, a 32-year-old woman with a 10-year history of bipolar disorder took olanzapine (200 mg), diazepam (20 mg), and zolpidem (200 mg) as part of a suicidal attempt. Given her unconscious state and the evident concern regarding the toxic effects of the drugs on the fetus, a cesarean section was performed immediately. The patient gave birth to a male baby with Apgar scores of 5 at 1 and 8 at 5 minutes. The baby showed dyspnea and decreased activity directly after birth. After supportive care, the condition of both mother and baby improved and both were discharged.

  • Craniovertebral junction (CVJ) tuberculosis is a rare disease, and potentially fatal due to the risk of atlantoaxial dislocation. The disease usually accompanies a retropharyngeal cold abscess, which can cause subsequential otolaryngologic symptoms such asdysphagia, odynophagia, or airway obstruction. Such a patient must be handled with great care to avoid a disaster. The disease is diagnosed with microbiological or other laboratory tests on the pus collected through puncture and aspiration, which will need otolaryngologic skills. For treatment, otolaryngologists play an important role by doing incision and drainage of the retropharyngeal abscess, or by attending the transoral vertebra surgery as partners with spine surgeons who will manage the CVJ lesions and ensure the craniocervical stability.

  • Penetrating chest trauma by stab injury may result in massive hemothorax from damage to single or multiple intrathoracic organs such as heart, aorta, internal mammary artery, intercostal artery or pulmonary parenchyme. Prognosis of massive hemothorax necessitating emergency thoracotomy is fatal especially so if there exists concomitant underlying compromise of cardiopulmonary function. A 56 year old man with destroyed left lung due to old pulmonary tuberculosis was stabbed in right parasternal lesion through third intercostal space. Intubation with cardiopulmonary resuscitation and closed thoracostomy were performed to resuscitate from cardiac asystole from hemorrhagic shock and acute respiratory distress. Midsternotomy was made to expose active bleeding foci in right mammary artery, subclavian vein, intercostal artery and anterior segment of right upper lung showing severe bullous change and pleural adhesion. Postoperative care included ventilator support, inotropic instillation and cautious, balance fluid therapy ; successful extubation was done on third postoperative day and patient was discharged on tenth postoperative day without any complication.

  • A 3-year-old, captive female river cooter was presented with a 4-day history of progressive unilateral swelling of the right side of the head, lethargy, and anorexia. History, physical examination, and radiographic examination revealed an aural abscess. After administration of antibiotics and supportive care, surgical intervention was performed. Swab samples were collected from the tympanic cavity during surgery for cytology and antimicrobial susceptibility testing. Molecular analyses of 16S ribosomal RNA gene sequences identified Citrobacter spp. and Morganella morganii. The patient was treated with ciprofloxacin and meloxicam and recovered after 2 months. This report describes the successful correction of a unilateral aural abscess that responded well to surgical intervention and a properly selected antibiotic.

  • If an urgent situation, such as a stroke, faint, heart attack, happens in a lavatory, it is hard to recognize and perform an immediate emergency treatment unless the patient inside of the lavatory calls for help. If a system that can detect an emergency in a lavatory and report to the relevant personnel is developed, it would be an excellent installation case demonstrating how a train is nicely designed to take care passenger's safety. In this paper, we showed a design of lavatory emergency detection system (LEDS) that detects an urgent situation in a lavatory by using sensors and timers and reports to proper personnel. This system can be installed not only on a train but also in a building where visual monitoring is not possible.
