비스포스포네이트 제제 관련 관절통 및 근육통에 대한 지역 약국 약사 및 환자 능동적 모니터링 (Arthralgia and Myalgia Associated with the Use of Bisphosphonate: An Active Monitoring Study)

  • Objective: We investigated arthralgia and myalgia associated with the use of oral bisphosphonate (BP) by conducting a survey of patient. Methods: The pharmacists conducted a survey between 1 Oct 2019 and 30 Sep 2020 among patients who were dispensed BP in community pharmacies to assess their demographic and medical characteristics, and their experiences with, and process for pain. Logistic regression analyses were performed to find the risk factors associated with the pain, and the odds ratios (OR) and 95% confidence intervals (CI) were determined. Results: A total of 160 patients who used BP participated in the survey (74 [46.3%] used risedronate; 61 [38.1%] used alendronate; 23 [14.4%] used ibandronate), and 20 (12.5%) of them experienced pain. Significant statistical differences of the characteristics between patients who experienced pain or not were observed regarding menopause, and parity. Compared with women who had one or two parity, women who had more than three parity were associated with the decreased risk of pain (OR, 0.24; 95% CI, 0.04-0.98). Moreover, steady exercise was associated with the decreased risk of pain compared to less exercise (OR, 0.37; 95% CI, 0.14-0.98). Conclusion: Our findings suggest that the risk of pain in BP-treated patients might be different regarding the different ingredients of BP, and dosing frequency. This survey highlights a need for a further safety research to understand the factors influencing the pain associated with the BP treatment.

아세트아미노펜 중독 청소년에서의 프로칼시토닌 상승 (Elevation of Procalcitonin in an Adolescent Acetaminophen Intoxication)

  • Procalcitonin (PCT) is commonly employed in medical practice as a diagnostic biomarker of bacterial infection and also as a monitoring biomarker for antimicrobial therapy. There have been a few published reports concerning elevated PCT levels in people with acute liver injury caused by an overdose of acetaminophen. We report here on a case of PCT elevation in an adolescent with acute acetaminophen poisoning without any bacterial infection or liver injury. A 15-year-old girl had deliberately ingested 20 tablets of 650 mg acetaminophen (13 g) and she presented to our emergency department. The PCT level on admission was elevated to 65.64 ng/mL (reference range: 0-0.5 ng/mL). Her PCT level on the second day peaked up to 100 ng/mL and then it gradually decreased. There was no evidence of liver injury or infection on the computed tomography examination and other lab tests. The patient regained her good health and was discharged on the sixth day of hospitalization.

일부 보건소 모자보건실 연계 비외과적 치주처치 중심의 치위생관리 프로그램 운영 사례연구 (Case study on Dental Hygiene Care Program with the Focus on Non-surgical Periodontal Treatments with the Maternal and Child Health Center)

  • 본 연구는 제천시보건소 모자보건실을 이용하는 임산부를 대상으로 비외과적 치주처치 중심의 치위생관리 프로그램을 시범운영하고, 시범운영의 구조평가, 과정평가, 결과평가를 통하여 보건소에서 치과위생사가 지역사회 임산부를 대상으로 구강건강증진 프로그램을 운영하는데 기초자료로 제공하기 위해 수행되었다. 총 3회의 치위생관리 프로그램에 모두 참여한 9명의 임산부를 대상으로 하였다. 치위생관리 프로그램 결과 4mm 이상 치주낭 보유율은 40.4%에서 14.0%로(p<0.05), 치은출혈 치아 보유율은 4.6%에서 2.7%로(p=0.180), 치석형성 치아 보유율은 23.7%에서 0%로(p<0.05) 감소하였다. PHP Index(Patient Hygiene Performance index)는 2기 임산부의 경우 1차 1.7점에서 3차 1.4점으로 개선되었고, 3기 임산부의 경우 1차 2.4점에서 1.3점으로 개선되었다(p<0.05). 본 연구 결과 비외과적 치주처치 중심의 치위생관리 프로그램을 보건소 구강보건실에서 적용 및 확대할 수 있는 가능성이 높다는 것에 의의가 있으며, 향후 본 프로그램의 장기적인 운영과 지속적인 모니터링이 필요할 것으로 사료된다.

  • 임상검사실에서의 혈소판 수 계산은 지혈이상의 진단과 치료에 필수적이며, 혈소판 수가 적은 경우 혈소판 수혈이 필요하고 항암치료 후 혈소판 수 경과를 모니터링하는데 매우 중요하다. 정도관리는 환자결과를 내보내기 전에 검사실에서 오류를 줄이고 교정하는 과정이며 분절된 적혈구는 혈소판과 크기가 비슷하여 혈소판 수 계산에 영향을 미친다. 검사실에서 내부정도관리low QC물질이 2SD를 벗어난 것을 경험하였고, 지금까지 용혈과 혈소판 지표들과의 관계에 대해 밝혀진 것이 충분하지 않아 연구를 시작하였다. 이에 본 연구에서는 XN CHECK low level QC물질을 이용해 PLT-I, PLT-O, PLT-F 방법간의 혈소판 수치를 비교하였으며, 용혈검체를 만들어 5가지 혈소판지표들에 대해 비교분석 하였다. 그 결과PLT-F방법에서 CV값이 가장 적게 나타났으며, 용혈검체에서 P-LCR 수치가 18.4%에서 31.9%로 증가함을 보였다. 이 연구를 통해 혈소판 수치가 낮은 경우는 PLT-F방법으로 하는 것이 더 정확하며, 검체가 용혈이나 변질이 의심되는 경우 이를 평가할 때 P-LCR을 새로운 지표로 제시하고 있으며, 사람 혈액검체를 이용한 추후 연구가 더 필요할 것이라고 사료된다.

딥러닝 기반 항생제 내성균 감염 예측 (Antibiotics-Resistant Bacteria Infection Prediction Based on Deep Learning)

  • 세계보건기구(WHO)를 비롯해 세계 각국의 정부기관은 항생제 오남용에 따른 항생제 내성균 감염에 대해 심각하게 경고하며 이를 예방하기 위한 관리와 감시를 강화하고 있다. 하지만 감염을 확인하기 위한 감염균 배양에 수일의 시간이 소요되면서 격리와 접촉주의를 통한 감염확산 방지 효과가 떨어져 선제적 조치를 위한 신속하고 정확한 예측 및 추정방법이 요구되고 있다. 본 연구는 Electronic Health Records에 포함된 질병 진단내역과 항생제 처방내역을 neural embedding model과 matrix factorization을 통해 embedding 하였고, 이를 활용한 딥러닝 기반분류 예측 모형을 제안하였다. 항생제 내성균 감염의 주요 원인인 질병과 항생제 정보를 embedding하여 환자의 기본정보와 병원이용 정보에 추가했을 때 딥러닝 예측 모형의 f1-score는 0.525에서 0.617로 상승하였고, 딥러닝 모형은 Super Learner와 같은 기존 기계학습 모형보다 더 나은 성능을 보여주었다. 항생제 내성균 감염환자의 특성을 분석한 결과, 감염환자는 동일한 질병을 진단받은 비감염환자에 비교해 J01 계열 항생제 사용이 많았고 WHO 권고기준(DDD)을 크게 벗어나는 오남용 청구사례가 6.3배 이상 높게 나타났으며 항생제 오남용과 항생제 내성균 감염간의 높은 연관성이 발견되었다.

Analysis of the risk factors of acute kidney injury after total hip or knee replacement surgery

  • Background: Postoperative acute kidney injury (AKI), which increases the risk of postoperative morbidity and mortality, poses a major concern to surgeons. We conducted this study to analyze the risk factors associated with the occurrence of AKI after orthopedic surgery. Methods: This was a retrospective study that included 351 patients who underwent total hip or knee replacement surgery at Inje University Haeundae Paik Hospital between January 2012 and December 2016. Results: AKI occurred in 13 (3.7%) of the 351 patients. The patients' preoperative estimated glomerular filtration rate (eGFR) was 66.66 ±34.02 mL/min/1.73 m2 in the AKI group and 78.07±21.23 mL/min/1.73 m2 in the non-AKI group. The hemoglobin levels were 11.21±1.65 g/dL in the AKI group and 12.39±1.52 g/dL in the non-AKI group. Hemoglobin level was related to increased risk of AKI (odds ratio [OR], 0.13; 95% confidence interval [CI], 0.02-0.68; p=0.016). Administration of crystalloid or colloid fluid alone and the perioperative amount of fluid did not show any significant relationship with AKI. Further analysis of the changes in eGFR was performed using a cutoff value of 7.54. The changes in eGFR were significantly related to decreased risk of AKI (OR, 0.74; 95% CI, 0.61-0.89; p=0.002). Conclusion: Renal function should be monitored closely after orthopedic surgery if patients have chronic kidney disease and low hemoglobin level. Predicting the likelihood of AKI occurrence, early treatment of high-risk patients, and monitoring perioperative laboratory test results, including eGFR, will help improve patient prognosis.

Programmed-release intraosseus anesthesia as an alternative to lower alveolar nerve block in lower third molar extraction: a randomized clinical trial

  • Background: Intraosseous anesthesia is the process by which an anesthetic solution, after penetration of the cortical bone, is directly injected into the spongiosa of the alveolar bone supporting the tooth. This study aimed to compare the effectiveness of the traditional inferior alveolar nerve block (IANB) and computerized intraosseous anesthesia in the surgical extraction of impacted lower third molars, compare their side effects systemically by monitoring heart rate, and assess patients' a posteriori preference of one technique over the other. Methods: Thirty-nine patients with bilaterally impacted third molars participated in this study. Each patient in the sample was both a case and control, where the conventional technique was randomly assigned to one side (group 1) and the alternative method to the contralateral side (group 2). Results: The traditional technique was faster in execution than anesthesia delivered via electronic syringe, which took 3 min to be administered. However, it was necessary to wait for an average of 6 ± 4 min from the execution to achieve the onset of IANB, while the latency of intraosseous anesthesia was zero. Vincent's sign and lingual nerve anesthesia occurred in 100% of cases in group 1. In group 2, Vincent's sign was recorded in 13% of cases and lingual anesthesia in four cases. The average duration of the perceived anesthetic effect was 192 ± 68 min in group 1 and 127 ± 75 min in group 2 (P < 0.001). The difference between the heart rate of group 1 and group 2 was statistically significant. During infiltration in group 1, heartbeat frequency increased by 5 ± 13 beats per minute, while in group 2, it increased by 22 ± 10 beats per minute (P < 0.001). No postoperative complications were reported for either technique. Patients showed a preference of 67% for the alternative technique and 20% for the traditional, and 13% of patients were indifferent. Conclusion: The results identified intraosseous anesthesia as a valid alternative to conventional anesthesia in impacted lower third molar extraction.

  • In Yugoslavia, hemorrhagic fever with renal syndrome (HFRS) is one of the important national health problem, but no vaccine has been used to prevent HFRS. Since first HFRS case in 1952, sporadic cases of HFRS occurred every year and over 4,000 registered cases with $1{\sim}16%$ mortality so far. We performed a prospective, randomized double-blind placebo-controlled trial to evaluate the effectiveness of $Hantavax^{TM}$ against HFRS in 3,900 healthy adults living in the endemic areas of Yugoslavia. 1,900 people were given 0.5 ml of Hantavax subcutaneously twice at one month interval and a booster shot at one year after. For controls other 2,000 healthy people were given 0.5 ml of physiolosical saline as a placebo. We investigated HFRS cases in both the vaccinated and nonvaccinated groups by monitoring the program for patient registration in the areas from 1996 to 1998, and the effect of vaccine was analyzed epidemiologically. No confirmed case of HFRS was observed among 1,900 Hantavax vaccinees, while 20 confirmed cases were observed among 2,000 nonvaccinated control group. There were no remarkable side effects among the vaccinees either locally or in general after inoculation of the vaccine. The Hantavax vaccine showed statistically significant protective efficacy against HFRS among Yugoslavian people.

임상에서 사용중인 체외충격파쇄석기의 음향 출력 분포 (Acoustic outputs from clinical extracorporeal shock wave lithotripsy devices)

  • 식약처에서 허가된 33개 체외 충격파 쇄석기 중 기술 문서가 공개된 10개(12개의 충격파 음장)에 대해 치료 효과 및 안전을 좌우하는 충격파의 음향 출력 분포를 조사했다. 조사 결과 핵심 충격파 음향 출력(P+, P-, efd, E)의 최대값은 제품별로(각각 최대 2.08배, 3.72배, 3.89배, 15.98배) 크게 차이가 나고 있다. 음향 출력 변수들의 값은 기술 문서에 충실하게 포함되지 않고 있으며, 일부 데이터는(예. efd) 통상적인 범위를 벗어나는 비정상적인 값을 가지는 것으로 나타났다. 제품 별로 큰 차이를 보이는 충격파 음향 출력은 동일한 적응증으로 허가 받은 ESWL 장비의 동등성을 훼손할 가능성이 높다는 것을 시사한다. 허가된 Extracorporeal Shock Wave Lithotripsy(ESWL) 제품의 기술 문서에 기재된 자료의 신뢰성을 검증하고, 임상에서 사용되는 제품의 성능이 기술 문서와 동일한지 확인하기 위해 공인된 시험 검사 기관의 확보가 필요하며, 노화로 인해 충격파 출력이 저하되는 ESWL 제품의 성능 유지 및 관리를 위한 규제 기관 주도의 사후 관리가 제안된다.

  • The creativity of thesis is that the significance of cyber security challenges in blockchain. The variety of enterprises, including those in the medical market, are the targets of cyberattacks. Hospitals and clinics are only two examples of medical facilities that are easy targets for cybercriminals, along with IoT-based medical devices like pacemakers. Cyberattacks in the medical field not only put patients' lives in danger but also have the potential to expose private and sensitive information. Reviewing and looking at the present and historical flaws and vulnerabilities in the blockchain-based IoT and medical institutions' equipment is crucial as they are sensitive, relevant, and of a medical character. This study aims to investigate recent and current weaknesses in medical equipment, of blockchain-based IoT, and institutions. Medical security systems are becoming increasingly crucial in blockchain-based IoT medical devices and digital adoption more broadly. It is gaining importance as a standalone medical device. Currently the use of software in medical market is growing exponentially and many countries have already set guidelines for quality control. The achievements of the thesis are medical equipment of blockchain-based IoT no longer exist in a vacuum, thanks to technical improvements and the emergence of electronic health records (EHRs). Increased EHR use among providers, as well as the demand for integration and connection technologies to improve clinical workflow, patient care solutions, and overall hospital operations, will fuel significant growth in the blockchain-based IoT market for linked medical devices. The need for blockchain technology and IoT-based medical device to enhance their health IT infrastructure and design and development techniques will only get louder in the future. Blockchain technology will be essential in the future of cybersecurity, because blockchain technology can be significantly improved with the cybersecurity adoption of IoT devices, i.e., via remote monitoring, reducing waiting time for emergency rooms, track assets, etc. This paper sheds the light on the benefits of the blockchain-based IoT market.
